Description
皮膚を穿刺するマイクロニードルの材質には、金属や各種の樹脂等が用いられているが、皮膚に対する障害を抑制するために、生体内分解性の材質が多用される傾向にある。しかし、マイクロニードルの材質が生体内分解性の樹脂の場合、皮膚に対する穿刺の角度が垂直から少しずれると、マイクロニードルの微小針が折れたり曲がり易く、適切な皮膚への穿刺が困難であった(特許文献1)。
しかし、皮膚にテープで接着固定できるようなマイクロニードル貼付剤を作製して、実際に使用してみると、貼付剤の補助器具を除去する際、マイクロニードルが皮膚から浮き上がったりはずれたりして、充分皮膚に接着固定しないことが明らかになった。
また,テープを粘着剤にてアプリケータに固定すると、皮膚への乗り移りの際にうまく移行できない場合もあることが明らかとなった。そのためにアプリケータとテープの間の粘着剤を弱い粘着にする方法もあるが結果的に流通等に耐えられなくなるというジレンマがあった。
さらに,皮膚への針の挿入が確実に行われない場合があった。皮膚に針が挿入されるかどうかはマイクロニードルの皮膚への応力が一定の値を超えるかどうかの問題と考えられるが、従来の指で加圧するだけの方法では応力を設定することができない問題があった。
以上のように、マイクロニードルの貼付剤化を行い、実用的な製品を作製するためには、更にマイクロニードル貼付剤の保持器具やテープの検討が求められていた。

図1は、本発明で使用するアプリケータの一例を示す断面図である。図1は、補助具を設置する前のアプリケータであり、マイクロニードルを設置する硬質樹脂平板と、2つの硬質樹脂製の側板で構成されている。
本発明の「硬質樹脂平板」は、指で押圧してもあまり変形しない硬質の部材を使用することが望ましく、硬質の部材としては、公知の種々の材質のものを使用することができる。例えば一定以上の厚みの合成樹脂であれば、特に限定されるものではなく、材質としては例えばポリエチレン樹脂、ポリプロピレン樹脂、ポリ塩化ビニル樹脂、アクリル樹脂、ポリエチレンテレフタレート樹脂、ポリスチレン樹脂、アクリロニトル・ブタジエン・スチレン共重合体、ポリカーボネート樹脂、ポリアミド樹脂、フッ素樹脂、ポリブチレンテレフタレート樹脂等の硬質樹脂を使用することが挙げられる。
上記硬質樹脂平板の形状は、特に限定が無く、マイクロニードルを設置収納することが出来れば十分であり、マイクロニードルのサイズに合わせて適宜調整できる。例えば、それぞれ縦横が3~7cmの範囲の四角形や直径が3~7cmの円形のものを使用することができる。例えば、図1に示されるような3×4cmの四角形や図4に示される円形の凹部を持つ一辺が約3cmの正方形の形状のものが使用できる。
なお、効果を示すために必要な量の薬剤が必要な場合、薬剤の水溶液等にマイクロニードルの微小針を浸漬し、乾燥することを繰り返して必要量を担持することが出来る。
本発明の「側板」とは、上記「側壁」の一部のことであり、例えば、図1や図8、図9に示される一対の角柱部材や板状部材のことを言う。板状部材は、直方体状のものでもよく、湾曲して円弧の一部を形成していてもよい。
なお、本発明の側壁や側板は、図13等で示されるように、補助具と共に一体成形されていてもよい。
また、円錐台の直径断面や多角錐の中心断面に見られる台形においても、上記と同様の角度に関する好ましい範囲を挙げることができる。
また,本発明の補助具が設置されたアプリケータは押し込み幅を設定することによってマイクロニードルの皮膚に対する応力を設定することができる。例えば押し込み幅の目印をつけておけば応力を設定することが可能となる。さらに上記の様なテープの剥離によるアプリケータから皮膚への移行を用いるとある一定の幅以上押し込まないとテープのアプリケータからの剥離はおこらず,皮膚に移行しない。すなわち押し込み幅の設定がテープの皮膚への移行によって設定でき,マイクロニードルの皮膚への応力の設定も可能となる。
従って、本発明のアプリケータに設置されたマイクロニードル含有の貼付剤は、予防接種のような皮内又は局所処置における皮膚に対して、薬剤(任意の薬理作用のある薬剤を含む)を放出するのに使用することができる。使用される薬剤が高分子量の場合に、本発明のマイクロニードル含有の貼付剤が有用である。このような高分子量の薬剤の例としては、タンパク質、ペプチド、ヌクレオチド配列、モノクローナル抗体、DNAワクチン、ヘパリンのようなポリサッカライド、セフトリアキソンのような抗体を挙げることできる。
更に、本発明のマイクロニードルを含有する貼付剤は、他の方法では経皮吸収させることが困難又は不可能な低分子量薬剤を経皮的に吸収させることができる。このような低分子量薬剤の例としては、塩形態;ビスホスホネート、好ましくはアレンドロン酸ナトリウム又はパムドロン酸ナトリウムのようなイオン分子等を挙げることができる。
本発明には、補助具や補助具が設置されたアプリケータ、更には、マイクロニードルやマイクロニードルを含有する貼付剤が設置されたアプリケータ等のいくつかの実施形態があるが、本発明に従って、様々な種類のアプリケータあるいは様々なマイクロニードルを含有するアプリケータを使用することが可能である。
(1)ヒト前腕皮膚モデル(A)
SIS30%と流動パラフィン70%を加熱溶解させて成形した6mm厚のシートを設置し、更にSIS15%と流動パラフィン85%を加熱溶解させて成形した9mm厚のシートを重ねて2層とする。この基盤の上に、ウィスターラット(雄性、5週)の腹部皮膚を設置して、ヒト前腕皮膚モデルとした。
(2)機材
島津製作所製小型卓上試験機(Eztest)を使用し、そのロードセルにプランジャー(φ5mm)を設置し、更にその先端にφ12mmのポリプロピレン(PP)板(0.8mm厚)を取り付けた。そのPP板の先端にマイクロニードルを設置した。
(3)剣山型マイクロニードル
ポリ乳酸(PLA)やポリグリコール酸(PGA)の樹脂シート円板(φ10mm、2mm厚)を使用し、公知方法(WO2008/093679等)に準じて剣山型マイクロニードルを作製した。
(4)評価方法
島津製作所製小型卓上試験機(Eztest)のロードセルにプランジャー(φ5mm)を設置し、その先端にφ12mmPP板(0.8mm厚)を取り付ける。更にその先にPGA製マイクロニードル(φ10mm、2mm厚、98本)を設置した。上記ヒト前腕皮膚モデル(A)の皮膚表面に上記マイクロニードルの先端を接触させる。小型卓上試験機で一定の距離だけ上記マイクロニードルを押し込み穿刺する。その際の上記マイクロニードルに掛かる応力を測定した。
また、穿刺後、ヒト前腕皮膚モデル(A)表面のラット皮膚(wister 5w 雄性 腹部)を取り出し、1%ゲンチアナバイオレットで染色した。穿刺されている個所は紫色で着色されるので、着色の個数を計算し、マイクロニードルの穿刺率を評価した。
(5)評価結果
上記マイクロニードルを押し込み、穿刺した際の応力と穿刺率を以下の表1に示した。
穿刺率を100%とするためには、マイクロニードルへの応力が4N以上であることが必要であることが分かった。
上記試験例1では、マイクロニードルを皮膚モデル(A)に直接押し込むタイプの試験であったが、これまでの知見から断面がコの字型である図1のようなアプリケータ(下駄型アプリケータ)が良好な穿刺効果をあげることから、このアプリケータ(30×40mm、高さは各種)を用いて剣山型マイクロニードルの穿刺性がどのように変化するかを検討した。
図1に示されるアプリケータを使用して皮膚に押圧した場合、押圧されていない皮膚の部分が垂直に盛り上がってくるので、マイクロニードルが皮膚に垂直に刺さることができ、非常に効果的に穿刺できている。
そこで、アプリケータを用いた場合のマイクロニードルの穿刺性を評価するため、図2に示される冶具を作製して測定を行なった。まず、島津製作所製小型卓上試験機(Eztest)のロードセルにプランジャー(φ5mm)を設置し、その先端にφ12mmPP板(0.8mm厚)を取り付ける。更にその先にPGA製剣山型マイクロニードル(φ10mm、2mm厚、98本)を設置した。剣山型マイクロニードルの針先にかかる応力を測定できるように、剣山型マイクロニードルとアプリケータが同調して押し込むことができる図2の装置をヒト前腕皮膚モデル(B)の上に設置した。なお、アプリケータとしては、補助具の設置されていないものを用いた。上記ヒト前腕皮膚モデル(B)の表面とマイクロニードルの先端の距離が約0.5mmと約1.2mmになるように図2の装置を設定した。
小型卓上試験機で図2の冶具に力を加えて一定の距離だけ動かし、ヒト前腕皮膚モデル(B)に押し込み、PGA製の剣山型マイクロニードルで皮膚を穿刺した。穿刺の際の押し込み距離と応力を小型卓上試験機で測定した。穿刺後、ヒト前腕皮膚モデル(B)表面のラット皮膚(wister 5w 雄性 腹部)を取り出し、1%ゲンチアナバイオレットで染色した。穿刺されている個所は紫色で着色されるので、着色の個数を計算し、マイクロニードルの穿刺率を評価した。
(1)ヒト前腕皮膚モデル(B)
メラミン樹脂平板上に、SIS15%と流動パラフィン85%を加熱溶解させて成形した19mm厚のシートを設置し、更に同じ材質の3mm厚のシートを重ねる。その上に、SIS100%の0.49mm厚のシートを重ねて3層とする。この基盤の上に、ウィスターラット(雄性、5週)の腹部皮膚を設置して、ヒト前腕皮膚モデルとした。これをヒト前腕皮膚モデル(B)とした。
(2)評価結果
上記PGA製マイクロニードル(98本)で穿刺した結果を以下の表2と図3に示した。
以上のように、アプリケータの使用の有無にかかわらず、マイクロニードルが皮膚に押し込まれる距離でマイクロニードルにかかる応力と穿刺率が決まることが明らかになった。
そこで、これまで使用した剣山型マイクロニードルに代えて、マイクロニードルの針先が少し鈍いものを使用して、その穿刺性を評価した。その結果を以下の表3に示す。
試験例1と2で、マイクロニードルの穿刺性は、マイクロニードルにかかる応力の強さ(皮膚への押し込み距離)に影響されることが示された。そこで、同じ押し込み距離であっても、アプリケータの形状の違いによって、どのような応力差が生じるかを評価した。
評価方法としては、以下のヒト前腕皮膚モデル(C)を使用し、図2の冶具を使用したが、マイクロニードルに代えて、φ12mmのPP板(0.8mm厚)を設置し、皮膚モデルの表面との距離が1.0mmとなるように調整した。
A)評価試験方法の検証
(1)ヒト前腕皮膚モデル(C)
メラミン樹脂平板上に、SIS15%と流動パラフィン85%を加熱溶解させて成形した19mm厚のシートを設置し、更に同じ材質の3mm厚のシートを重ねる。その上に、SIS100%の0.49mm厚のシートを重ねて3層とする。これをヒト前腕皮膚モデル(C)とした。
(2)機材
島津製作所製小型卓上試験機(Eztest)を使用し、そのロードセルにプランジャー(φ5mm)を設置し、更にその先端にφ12mmのPP板(0.8mm厚)を2枚重ねて取り付けた。
(3)評価方法
上記ヒト前腕皮膚モデル(C)に上記冶具と上記小型卓上試験機を図2のように設置した。上記小型卓上試験機でアプリケータを上記ヒト前腕皮膚モデル(B)に押し込み、応力を測定した。
(4)評価結果
上記マイクロニードルのダミーのPP板にかかる応力は以下の表4に示されるものとなった。
(1)アプリケータの選択
前項A)では、皮膚を2個所で押圧する下駄型アプリケータが使用された。そこで、外周壁を持つ、図4のワッペン型アプリケータ(30×30mm、窪み部分φ25mm)を作製した。
(2)評価方法
前項A)で使用された図2の治具に設置し、前項A)と同様にして、ヒト前腕皮膚モデル(C)にマイクロニードル(ダミー)を押し込み、押し込み距離と応力を測定した。
(3)評価結果
上記マイクロニードルのダミーのPP板にかかる応力は以下の表5に示されるものとなった。
上記試験例3のように下駄型アプリケータの方が押圧時に皮膚表面が盛り上がり易く、マイクロニードルが皮膚表面から深く押し込まれ易いことが分かった。また、下駄型アプリケータの中にマイクロニードルの支持台(補助具)が突出していると、アプリケータの押圧に伴い皮膚が盛り上がる際に、更にマイクロニードルを皮膚表面に押し込み易くなると考えられ、マイクロニードルにかかる応力が増大すると考えられた。そこで、図5の治具を作製し、マイクロニードルの支持台(補助具)の高さと応力の関係を評価した。なお、皮膚表面とマイクロニードルの先端部分との間は、試験例3と同じく1mmになるように設定した。図5のhとは、マイクロニードルの先端部分とアプリケータの底面との距離を表わすものである。マイクロニードルの針の長さが約0.5mm、基盤の厚みが1mmの場合、補助具の高さはh-1.5mmで示されることになる。その結果を以下の表6に示す。
試験例4の結果から、マイクロニードルの先端部がアプリケータの底部より、ある程度突出している方が応力が高く、マイクロニードルの穿刺性が高いことが示された。そこで、図6の治具を作製し、アプリケータの側壁の影響を排除した。この治具を使用すれば、マイクロニードルを設置する補助具が、アプリケータの底部からどの程度突出していれば好適な穿刺率を示すことができるのかを直接評価できることになる。
そこで、図6の治具を用いて、試験例4と同様にしてマイクロニードルにかかる応力を測定評価した。なお、補助具の高さは、試験例4の場合と同様にマイクロニードル先端部の高さ(h)-1.5mmで表わすことができる。その結果を以下の表7に示す。
以上の知見から、マイクロニードルの補助具(支持台)が設置されたアプリケータを以下の実施例1~4で作製し、その機能を評価することにした。
断面が台形(上辺の長さが10mm、下辺の長さが30mm、台形の高さが4mm、上辺と斜辺の交角が158°)であり、長さが30mmであるインゴット状のポリプロピレン製の補助具を作製した。
上記補助具に底板(3×30×40mm)、側板(5×6×30mm)を図7に示されるように接着剤で接着した。その結果、このポリプロピレン製補助具を組み込んだポリプロピレン製アプリケータが図8のように製造できた。
(1)補助器具(マイクロニードルの支持台)の製造
実施例1と同様のサイズ(30×30mm)で断面の台形の高さのみが異なるインゴット状のポリプロピレン製補助具を以下の表8に示すように作製した。
a)上記表8に記載のインゴット状の補助具を30mmの長さに切断し、実施例1と同様にして以下の表9に示されるアプリケータを作製した。なお、使用する側板の高さは、補助具の高さより常に2mm長くなるよう設定した。その結果、図8のアプリケータと同様の形状でアプリケータの高さの異なるものを作製した。
更に、アプリケータNo.14を用いて、図12に示されるように1mm厚の樹脂板や樹脂の半球を付加した形状のアプリケータを上記と同様にポリプロピレン樹脂で一体成型することによって、図14に記載のアプリケータが得られた。
実施例2で得られたアプリケータ(No.1~17)を用いて、図11のように両面テープ(ニチバン社製ナイスダックNWR、15×30mm)を貼付した。
更に、図18に示されるように、上記両面テープの上に重ねて、サージカルテープ(3M社製トランスポア1527SP-1、15×30mm)の粘着面が表面に出るように貼付した。上記サージカルテープの粘着面の中央部(補助具の峰の部分)に、剣山型マイクロニードル又はダミーとしてのポリプロピレン板(直径12mm、0.8厚)を設置する。あるいは、図13に示されるように、サージカルテープ上に、ダミーとしてのポリプロピレン板を設置し、これを上記両面テープに設置して、図19に示される剣山型マイクロニードルが設置されたアプリケータを作製した。
実施例2で得られたアプリケータNo.5を用いて、図20のように両面テープ(ニチバン社製ナイスダックNW、15×30mm)を貼付した。
更に、上記両面テープの上に重ねて、円形の((株)共和製ポアテープ MD-025-N,直径25mm)を貼付した。上記サージカルテープの粘着面の中央部(補助具の峰の部分)に、剣山型マイクロニードル又はダミーとしてのポリプロピレン板(直径12mm、0.8厚)を設置することによって、剣山型マイクロニードルが設置されたアプリケータを作製した。
実施例3~4で作製されたアプリケータが皮膚に圧接されると、マイクロニードルが皮膚を穿刺すると共に、テープの粘着面が皮膚に接触し付着する。そこで、アプリケータをどれだけ押し込むと、マイクロニードルと共に、テープが皮膚に張り付くかを検討した。テープが皮膚に接触して張り付くか否かは、皮膚への押し込み距離とアプリケータの補助具の形状(角度)が大きく影響すると考えられる。そこで以下の方法により、補助具の形状の評価を行なった。
(1)評価試験方法
試験例3に記載のヒト前腕皮膚モデル(B)を使用した。
(2)アプリケータ
実施例2のアプリケータを用い、図21のように補助具に両面テープ(ニチバン製ナイスダックNW)を15×30mm貼付した。
(3)機材
島津製作所製小型卓上試験機(Eztest) を使用し、そのロードセルにプランジャー(φ5mm)を設置し、更にその先端にφ12mmのPP板(0.8mm厚)を取り付けた。
(3)評価方法
評価方法は、図22に示される方法を用いた。まず、上記ヒト前腕皮膚モデル(B)に油性インク(ゼブラ製マッキー)を塗布した。その上に上記アプリケータと上記小型卓上試験機を設置した。上記小型卓上試験機でアプリケータを上記ヒト前腕皮膚モデル(B)に押し込み、アプリケータに貼付した両面テープに付着したインクの面積を測定した。
(4)評価結果
a)補助具のインゴット状の峰が側板と直交するアプリケータ:
アプリケータとして、以下の表13に示されるアプリケータを使用して皮膚モデルへの押し込み距離とテープに付着したインクの面積の関係を評価した。なお、インクの付着面積はテープの幅が共通しているため、補助具のインゴット状の斜面部分におけるインク付着距離(mm)で評価した。その斜面付着距離(左右の斜面の合計)の結果を以下に示す。
なお、皮膚への押し込み距離は、皮膚の厚さに起因する上限があり、約15mm前後が無理のない範囲と考えられる。従って、補助具の高さは10mm以下が適切であると考えられた。
次に、インクが塗布されていないヒト前腕皮膚モデル(B)を使用し、上記と同様にしてアプリケータを押し込み、両面テープがどの時点で補助具から剥離し、皮膚モデルに付着し貼付されるかを評価した。その結果を以下の表14に示す。
マイクロニードルを含有するテープでも、上記と同じ効果が得られており、更には、テープの形状に係らす、図19のようなマイクロニードルを含有する円形のテープを用いた場合でも、皮膚に綺麗に貼付することができた。
次に、インクが塗布されていないヒト前腕皮膚モデル(C)を使用し、上記と同様にしてアプリケータを押し込み、両面テープがどの時点で補助具から剥離し、皮膚モデルに付着し貼付されるかを評価した。その結果を以下の表16に示す。
上記a)とは異なるアプリケータとして、No.16、17を使用し、図21に示されるようにテープを横に貼付したものを使用した。また、No.1のアプリケータを使用し、図21と同様にテープを横に貼付したものを作製した。そして上記表13と同様に皮膚に対する補助具上のテープの接触面積を評価した。その結果を以下の表17に示す。
次に、インクが塗布されていないヒト前腕皮膚モデル(C)を使用し、上記と同様にしてアプリケータを押し込み、両面テープがどの時点で補助具から剥離し、皮膚モデルに付着し貼付されるかを評価した。その結果を以下の表18に示す。
以上のように図11のa)とb)のアプリケータを使用した皮膚との接触評価試験の結果から、図8に示されるアプリケータを使用すれば、押し込み距離に応じた皮膚への充分な接触面積が得られることが示された。
ヒト前腕皮膚モデル(C)が実際のヒト前腕皮膚のモデルとして妥当か否かを検証することを行なった。3種のアプリケータ(No.4、5、14)を用いて、実施例3と同様にして図18で示されるようなポリプロピレンの円板(マイクロニードルのダミー)を設置したテープを持ったアプリケータを作製した。前記a)と同様に小型卓上試験機(Eztest)を用いて、3種のアプリケータをヒト前腕皮膚モデル(C)と実際のヒト前腕皮膚に押圧した。その際、ヒトの前腕は、こぶしを握って力が入った状態で測定した。押圧解除後に、皮膚に密着したテープが両端から剥離して、補助具から離れ皮膚に貼付される。その際の最小の押し込み距離を比較検証した。その結果を以下の表19に示す。
Claims (19)
- マイクロニードルを皮膚に穿刺するためのマイクロニードル用アプリケータであって、
マイクロニードルとテープを設置するための支持台としての補助具を有することを特徴とするアプリケータ。 - 上記補助具の高さが0.5~10mmであることを特徴とする、請求項1に記載のアプリケータ。
- 上記補助具の形状が、インゴット状、円錐台状、多角錐台状である、請求項1または2に記載のアプリケータ。
- 上記補助具がインゴット状であり、断面の伏角が130~175°である、請求項1~3のいずれかに記載のアプリケータ。
- 上記補助具を囲む周囲に側壁を有することを特徴とする、請求項1~4のいずれかに記載のアプリケータ。
- 補助具を囲む上記側壁が、円筒状、桝状、一対の側板のいずれかである、請求項1~5のいずれかに記載のアプリケータ。
- 上記一対の側板が、一対の平板である、請求項1~6のいずれかに記載のアプリケータ。
- 補助具の高さが、側壁の高さより低く、マイクロニードルを設置しても、マイクロニードルの先端部が側壁を越えないことを特徴とする、請求項1~7のいずれかに記載のアプリケータ。
- 上記補助具が設置されたアプリケータの基盤の形状が、円形または四角形である、請求項1~8のいずれかに記載のアプリケータ。
- マイクロニードルとテープが設置されている、請求項1~9のいずれかに記載のアプリケータ。
- 上記アプリケータが、
a)マイクロニードルとテープを設置するための補助具を有し、
b)補助具を設置する樹脂平板と
c)一対の側板を有し、側板の高さが補助具よりも高くなっている、
ことを特徴とする、上記(1)記載のアプリケータ。 - 上記補助具の上に、テープとその粘着面に担持したマイクロニードルとを設置した、請求項11に記載のアプリケータ。
- 上記の樹脂平板が、3~7cmの四角形の形状であることを特徴とする、請求項11又は12に記載のアプリケータ。
- 上記補助具がインゴット状の形状のものである、請求項11~13のいずれかに記載のアプリケータ。
- 上記側板の高さが2~15mmであることを特徴とする、請求項11~14のいずれかに記載のアプリケータ。
- 上記補助具の高さが2~7mmであり、側板の高さが補助具より2~3mm高いことを特徴とする、請求項11~15のいずれかに記載のアプリケータ。
- 上記インゴット状補助具の断面の台形伏角が175~145°の範囲であることを特徴とする、請求項11~16のいずれかに記載のアプリケータ。
- 上記インゴット状補助具の末端が、上記樹脂平板の末端に重なっていることを特徴とする、請求項11~17のいずれかに記載のアプリケータ。
- 上記補助具と樹脂平板、側板が樹脂で一体成型されていることを特徴とする、請求項11~18のいずれかに記載のアプリケータ。
