本発明の、または本発明に使用されるアデノシンA2A受容体拮抗作用を有する化合物の酸付加塩としては、例えば塩酸塩、硫酸塩、臭化水素酸塩、硝酸塩、リン酸塩などの無機酸塩、酢酸塩、メシル酸塩、コハク酸塩、マレイン酸塩、フマル酸塩、クエン酸塩、酒石酸塩などの有機酸塩があげられ、薬学的に許容される金属塩としては、例えばナトリウム塩、カリウム塩などのアルカリ金属塩、マグネシウム塩、カルシウム塩などのアルカリ土類金属塩、アルミニウム塩、亜鉛塩などがあげられ、薬学的に許容されるアンモニウム塩としては、例えばアンモニウム塩、テトラメチルアンモニウム塩などの塩があげられ、薬学的に許容される有機アミン付加塩としては、例えばモルホリン、ピペリジンなどの付加塩があげられ、薬学的に許容されるアミノ酸付加塩としては、例えばグリシン、フェニルアラニン、リジン、アスパラギン酸、グルタミン酸などの付加塩があげられる。
また、本発明の、または本発明に使用されるアデノシンA2A受容体拮抗作用を有する化合物またはその薬学的に許容される塩は、水または各種溶媒との付加物の形で存在することもあるが、これらの付加物も本発明のオピオイドの望ましくない作用を抑制する剤、疼痛の治療および/または予防剤、キット、医薬組成物、オピオイドの望ましくない作用を抑制する方法、疼痛の治療および/または予防方法、オピオイドの望ましくない作用を抑制する剤の製造のための使用、疼痛の治療および/または予防剤の製造のための使用、ならびに組み合わせに使用することができる。
本発明のオピオイドの望ましくない作用を抑制する剤およびアデノシンA2A受容体拮抗作用を有する化合物は、例えば上記のオピオイドと共に使用することができ、また、本発明のオピオイドの望ましくない作用を抑制する剤、キット、医薬組成物、オピオイドの望ましくない作用を抑制する方法ならびに組み合わせは、例えば疼痛の治療および/または予防に使用することができる。本発明のオピオイドの望ましくない作用を抑制する剤、疼痛の治療および/または予防剤、キット、医薬組成物、オピオイドの望ましくない作用を抑制する方法、疼痛の治療および/または予防方法、アデノシンA2A受容体拮抗作用を有する化合物、ならびに組み合わせが使用できる疼痛としては、特に限定されないが、例えば治療および/または予防のために従来よりオピオイドが用いられている疼痛(例えば、慢性疼痛など)があげられ、具体的には侵害受容性疼痛、神経因性疼痛などがあげられる。より具体的には、例えば侵害受容性疼痛、癌性疼痛、腰背部痛、外傷性頸部症候群に伴う痛み、外症痛、術後痛、熱傷痛、分娩痛、帯状疱疹痛、頭痛、片頭痛、骨関節痛、腰背部痛、リウマチ性関節痛、骨関節炎に伴う痛み、線維筋痛症、筋筋膜痛、内臓痛、炎症性疼痛、神経因性疼痛、絞扼性神経障害、幻肢痛、術後の遷延性疼痛、外傷後の遷延性疼痛、帯状疱疹後神経痛、糖尿病性疼痛、神経性腰痛、エイズウイルス感染後の疼痛、脳卒中後の視床痛、脊髄損傷後の疼痛、三叉神経痛、舌咽神経痛などがあげられ、好ましくは、例えば侵害受容性疼痛、癌性疼痛、腰背部痛、術後痛、帯状疱疹痛、骨関節痛、腰背部痛、リウマチ性関節痛、骨関節炎に伴う痛み、線維筋痛症、筋筋膜痛、内臓痛、炎症性疼痛、神経因性疼痛、絞扼性神経障害、帯状疱疹後神経痛、糖尿病性疼痛、神経性腰痛、エイズウイルス感染後の疼痛、脊髄損傷後の疼痛、三叉神経痛などがあげられる。
投与形態としては、例えば錠剤、注射剤、坐剤、パッチ剤などがあげられる。
経口投与に適当な、例えば錠剤などは、乳糖などの賦形剤、澱粉などの崩壊剤、ステアリン酸マグネシウムなどの滑沢剤、ヒドロキシプロピルセルロースなどの結合剤などを用いて製造できる。
アデノシンA2A受容体拮抗作用を有する化合物またはその薬学的に許容される塩の投与量および投与回数は、その投与形態、使用するオピオイドの種類、効力、投与量および/または投与形態、患者の年齢、体重、治療すべき症状の性質もしくは重篤度などにより異なるが、通常経口の場合、成人一人あたり、0.01~1000 mg、好ましくは0.05~100 mgの範囲で、1日1回ないし数回投与する。静脈内投与などの非経口投与の場合、通常成人一人あたり0.001~1000 mg、好ましくは0.01~100 mgを1日1回ないし数回投与する。しかしながら、これら投与量および投与回数に関しては、前述の種々の条件により変動する。
経口投与に適当な、例えば錠剤などは、乳糖などの賦形剤、澱粉などの崩壊剤、ステアリン酸マグネシウムなどの滑沢剤、ヒドロキシプロピルセルロースなどの結合剤などを用いて製造できる。
上記の目的で、(a)アデノシンA2A受容体拮抗作用を有する化合物またはその薬学的に許容される塩と(b)オピオイドを複数の製剤の組み合わせとして使用または投与する場合には、それぞれの用量および投与回数はそれぞれの有効成分の効力、投与形態、患者の年齢、体重、症状などにより異なるが、通常一日当たり、(a)アデノシンA2A受容体拮抗作用を有する化合物またはその薬学的に許容される塩と(b)オピオイドを、それぞれ以下の用量で投与するのが好ましい。
次に、アデノシンA2A受容体拮抗作用を有する化合物またはその薬学的に許容される塩によるオピオイドの望ましくない作用の抑制効果、および(a)アデノシンA2A受容体拮抗作用を有する化合物またはその薬学的に許容される塩と(b)オピオイドを組み合わせて投与することによる効果について試験例により具体的に説明する。
試験例1 化合物(I)によるモルヒネ鎮痛耐性抑制作用
実験材料および方法
1. 使用動物
体重19~25 gの雄性ddY系マウス(3~4週齢、日本エスエルシー社)を用い、実験に供するまで、室温19~25℃、湿度30~70%、12時間明暗サイクル (明期 午前7時~午後7時、暗期午後7時~午前7時) で飼育した。餌および水は共に自由に摂取させた。
2. 使用薬剤および調製法
以下、モルヒネの使用にあたっては、すべて麻薬研究者の指導のもと、麻薬取り扱い規則に準じ、試験を行った。モルヒネは0.6 mg/mLの濃度で生理食塩液に溶解し、6 mg/kgの用量で皮下投与した。化合物(IA)は1 mg/mLの濃度で0.5%メチルセルロース(0.5%MC)に懸濁し、10 mg/kgの用量で経口投与した。
3. 鎮痛作用の測定
侵害性疼痛はホットプレート法にて測定した。マウスを53℃に設定したホットプレート装置(35100、Ugo Basile、Comerio、VA、Italy)上に置き、逃避行動(足を舐める、噛む、振る、跳躍する)を示すまでの時間(潜時)を疼痛閾値とした。実験には、投薬開始前の反応潜時が6~16秒の動物を用いた。また、刺激部位への損傷を最小限にするために、カットオフタイムは45秒とした。鎮痛作用は、薬物反復投与最終日(7日目)、薬物投与後、30、60および120分に反応潜時を測定することにより評価した。
4. 統計処理
実験結果は平均値±標準誤差で示した。統計学的解析は統計解析ソフトSAS(SAS Institute Inc、Cary、NC、USA)を用いて行った。2群間の比較は、Wilcoxn rank sum testを行った。P<0.05を有意差ありとした。
5. 実験結果
結果を図1に示した。モルヒネを7日間反復投与した群(モルヒネ反復投与群;1日2回、7日目のみ1回投与)では、モルヒネ単回投与群(生理食塩液を1日2回6日間反復投与し、7日目のみモルヒネ投与)に比べて、反応潜時は有意かつ著しく減弱し、鎮痛耐性の形成が認められた。モルヒネ投与30分前に化合物(IA)を経口投与した併用反復投与群では、モルヒネ単回投与群に比べて、鎮痛作用の減弱は認められず、鎮痛耐性の形成が抑制された。なお、化合物(IA)を単独で反復投与した群(化合物(IA)反復投与群)では反応潜時への影響は認められなかった。
試験例2 化合物(I)によるモルヒネ誘発便秘抑制作用
実験材料および方法
1. 使用動物
体重23~27 gの雄性ddYマウス(5週齢、日本エスエルシー社)を用い、実験に供するまで、室温19~25℃、湿度30~70%、12時間明暗サイクル(明期 午前7時~午後7時、暗期午後7時~午前7時)で飼育した。餌および水は共に自由に摂取させた。採便の際には、絶食・絶水とした。
2. 使用薬剤および調製法
モルヒネは0.3 mg/mLの濃度で生理食塩液に溶解し、3 mg/kgの用量で皮下投与した。化合物(IA)は1 mg/mLの濃度で0.5%MCに懸濁し、10 mg/kgの用量で経口投与した。
3. 便秘抑制作用の測定
試験当日、個体識別用のためマウスの尾に番号付けし体重を測定した。測定した体重をもとに、0.5%MCまたは化合物(IA)を経口投与した。0.5%MCまたは化合物(IA)投与30分後に生理食塩液またはモルヒネを皮下投与し、採便を開始した。モルヒネ投与3時間後に便を採集した。排便量は、個数を計測することにより評価した。
4. 統計処理
実験結果は平均値±標準誤差で示した。統計学的解析は統計解析ソフトSAS(SAS Institute Inc、Cary、NC、USA)を用いて行った。2群間の比較は、Wilcoxn rank sum testで行った。P<0.05を有意差ありとした。

結果を図2に示した。モルヒネ投与群では、正常群に比べて排便量が有意に減少し、便秘が認められた。モルヒネ投与30分前に化合物(IA)を経口投与した併用投与群では、モルヒネ投与群に比べて、有意に排便量が増加し、便秘抑制作用が認められた。 また、化合物(IA)のみを投与した化合物(IA)投与群およびモルヒネと化合物(IA)を投与した併用投与群において、下痢は誘発されなかった。
試験例3 化合物(III)によるモルヒネ鎮痛耐性抑制作用
実験材料および方法
1. 使用動物

以下、オキシコドンの使用にあたっては、すべて麻薬研究者の指導のもと、麻薬取り扱い規則に準じ、試験を行った。オキシコドンは2 mg/mLの濃度で注射用蒸留水を用いて調製し、20 mg/kgの用量で経口投与した。化合物(IA)は1 mg/mLの濃度で0.5%MCに懸濁し、10 mg/kgの用量で経口投与した。

侵害性疼痛はホットプレート法にて測定した。マウスを53℃に設定したホットプレート装置(MK-350B、室町機械)上に置き、逃避行動(足を舐める、噛む、振る、跳躍する)を示すまでの時間(潜時)を疼痛閾値とした。実験には、投薬開始前の反応潜時が6~16秒の動物を用いた。また、刺激部位への損傷を最小限にするために、カットオフタイムは45秒とした。鎮痛作用は、薬物反復投与最終日(4日目)、薬物投与後、30、60および120分に反応潜時を測定することにより評価した。

化合物(IA)は強力で選択的なアデノシンA2A受容体拮抗作用を有することが知られている(Eur. J. Pharmacol., 1994, 267(3), 335)。また、化合物(I)~(VIII)がアデノシンA2A受容体拮抗作用を有することもよく知られている(例えば、Nature Reviews 2006, 5, 247、J. Med. Chem., 1993, 36, 1333、US5587378、WO00/17201、WO2005/063743、Bioorg. Med. Chem. Lett. 2007, 17, 1376、WO2002/055524、WO2003/011864、WO2006/032273、WO2002/055083など)。
また、上記試験例2より、アデノシンA2A受容体拮抗作用を有する化合物(例えば、化合物(I)~(VIII))またはその薬学的に許容される塩は、モルヒネなどのオピオイドが誘発する便秘を改善すると考えられ、アデノシンA2A受容体拮抗作用を有する化合物(例えば、化合物(I)~(VIII))またはその薬学的に許容される塩とオピオイドを組み合わせて使うことにより、オピオイドの投与により生ずる便秘を改善できると考えられた。
常法により、次の組成からなる錠剤を調製する。化合物(IA)40 g、乳糖286.8 gおよび馬鈴薯澱粉60 gを混合し、これにヒドロキシプロピルセルロースの10%水溶液120 gを加える。この混合物を常法により練合し、造粒して乾燥させた後、整粒し打錠用顆粒とする。これにステアリン酸マグネシウム1.2 gを加えて混合し、径8 mmの杵をもった打錠機(菊水社製RT-15型)で打錠を行って、錠剤(1錠あたり有効成分20 mgを含有する)を得る。
