本明細書において、脂肪性肝疾患(fatty liver disease)とは、背景技術の項でも記載したとおり、脂肪肝とも呼ばれ、肝細胞に脂肪が異常蓄積することに起因して肝障害をきたす疾患を意味する。また、脂肪性肝疾患は、アルコール性肝疾患と非アルコール性脂肪性肝疾患に分類することができる。本明細書において、脂肪性肝疾患に含まれる疾患を以下にまとめる。
(1)アルコール性肝疾患:アルコール性肝障害とも呼ばれ、アルコール摂取を原因とした肝細胞への脂肪蓄積に起因する疾患であり、アルコール性の単純性脂肪肝、アルコール性肝炎(alcoholic steatohepatitis:ASH)、アルコール性肝線維症、アルコール性肝硬変等の疾患を含む。なお、アルコール性肝炎とは、アルコール性脂肪肝炎とも呼ばれ、アルコール性肝線維症を含む。
(2)非アルコール性脂肪性肝疾患:非アルコール性脂肪性肝疾患は肝障害を惹起するほどのアルコール摂取歴がなく、ウィルス性肝炎や自己免疫性肝炎など原因の明らかなものを排除した肝への脂肪沈着を認める疾患であり、非アルコール性単純性脂肪肝、非アルコール性脂肪肝炎(nonalcoholic steatohepatitis:NASH)、非アルコール性の肝線維症、非アルコール性の肝硬変等の疾患を含む。
(2-1)非アルコール性単純性脂肪肝:肝細胞への脂肪沈着のみを認める疾患。
(2-2)非アルコール性脂肪肝炎(nonalcoholic steatohepatitis:NASH):脂肪化に、アルコール性肝炎に類似した炎症、肝細胞壊死、風船様変化、線維化を伴う疾患であり、非アルコール性の肝線維症も含む。
(2-2-1)非アルコール性の肝線維症:肝組織内の繊維化が進み、コラーゲンおよび他の細胞外基質の構成分が過剰に産生・蓄積された疾患。
(2-3)非アルコール性の肝硬変:進展した線維化により肝小葉構造が改築された疾患。
「(1S)-1,5-アンヒドロ-1-[5-(アズレン-2-イルメチル)-2-ヒドロキシフェニル]-D-グルシトール又はその製薬学的に許容される塩」については、別の態様として、前述の特許文献4に記載の、(1S)-1,5-アンヒドロ-1-[5-(アズレン-2-イルメチル)-2-ヒドロキシフェニル]-D-グルシトール コリン塩が挙げられる。
(2)本発明の、式(I)の化合物又はその製薬学的に許容される塩を含有する医薬組成物、式(I)の化合物又はその製薬学的に許容される塩の有効量を患者に投与することを含む治療方法、又は医薬組成物を製造するための式(I)の化合物又はその製薬学的に許容される塩の使用のうち、式(I)の化合物又はその製薬学的に許容される塩が、(1S)-1,5-アンヒドロ-1-[5-(アズレン-2-イルメチル)-2-ヒドロキシフェニル]-D-グルシトール又はその製薬学的に許容される塩、別の態様としては、(1S)-1,5-アンヒドロ-1-[5-(アズレン-2-イルメチル)-2-ヒドロキシフェニル]-D-グルシトール コリン塩、さらに別の態様としては、(1S)-1,5-アンヒドロ-1-[3-(1-ベンゾチオフェン-2-イルメチル)-4-フルオロフェニル]-D-グルシトール又はその製薬学的に許容される塩、さらに別の態様としては、(1S)-1,5-アンヒドロ-1-[3-(1-ベンゾチオフェン-2-イルメチル)-4-フルオロフェニル]-D-グルシトール、さらに別の態様としては、(1S)-1,5-アンヒドロ-1-[3-(1-ベンゾチオフェン-2-イルメチル)-4-フルオロフェニル]-D-グルシトールとL-プロリンとの共結晶(モル比1:1)、さらに別の態様としては、(1S)-1,5-アンヒドロ-1-[4-クロロ-3-(4-エトキシベンジル)フェニル]-D-グルシトール又はその製薬学的に許容される塩、さらに別の態様としては、(1S)-1,5-アンヒドロ-1-[4-クロロ-3-(4-エトキシベンジル)フェニル]-D-グルシトール、さらに別の態様としては、(1S)-1,5-アンヒドロ-1-[4-クロロ-3-(4-エトキシベンジル)フェニル]-D-グルシトールと、L-プロリンとの共結晶(モル比1:1)、さらに別の態様としては、(1S)-1,5-アンヒドロ-1-[4-クロロ-3-(4-エトキシベンジル)フェニル]-D-グルシトールと、L-プロリンとの共結晶(モル比1:2)、さらに別の態様としては、(1S)-1,5-アンヒドロ-1-[4-クロロ-3-(4-エトキシベンジル)フェニル]-D-グルシトールのプロピレングリコール水和物である、医薬組成物、治療方法、又は使用。
(3)本発明の、脂肪性肝疾患の治療用医薬組成物、脂肪性肝疾患の治療方法、又は脂肪性肝疾患の治療用医薬組成物を製造するための使用のうち、脂肪性肝疾患が、非アルコール性脂肪性肝疾患、別の態様としては、非アルコール性単純性脂肪肝、さらに別の態様としては、非アルコール性脂肪肝炎、さらに別の態様としては、非アルコール性の肝線維症、さらに別の態様としては、非アルコール性の肝硬変である、医薬組成物、治療方法、又は使用。
(4)上記(1)~(3)のうち二以上の組み合わせである医薬組成物、治療方法、又は使用。
非経口投与のための注射剤としては、無菌の水性又は非水性の溶液剤、懸濁剤、乳濁剤を含有する。水性の溶液剤、懸濁剤としては、例えば注射用蒸留水及び生理食塩水が含まれる。非水性の溶液剤、懸濁剤としては、例えばプロピレングリコール、ポリエチレングリコール、オリーブ油等の植物油、EtOH等のアルコール類、ポリソルベート80等がある。このような組成物は、さらに防腐剤、湿潤剤、乳化剤、分散剤、安定剤、溶解補助剤等の補助剤を含んでいてもよい。これらは例えばバクテリア保留フィルターを通す濾過、殺菌剤の配合又は照射によって無菌化される。これらはまた無菌の固体組成物を製造し、使用前に無菌水又は無菌の注射用溶媒に溶解して使用することもできる。
非アルコール性単純性脂肪肝モデル(KK-Ayマウス)に対する効果(1)
KK-Ayマウス(雌、日本クレア株式会社より購入)を用いた。餌はCMF(特殊繁殖用、オリエンタル酵母工業株式会社より購入)を自由に摂食させた。14週齢時に、体重、血糖値、血漿インスリン値、血漿トリグリセライド値及び血漿アラニンアミノトランスフェラーゼ(ALT)を測定し、これらの項目が均一になるように2群に群分けを行った(各群8匹)。第一群にはvehicle(0.5%メチルセルロース)10 mL/kg、第二群には(1S)-1,5-アンヒドロ-1-[5-(アズレン-2-イルメチル)-2-ヒドロキシフェニル]-D-グルシトール コリン塩(化合物Aのコリン塩)を、3 mg/kg(化合物A換算)の用量で、1日1回で二週間経口投与した。最終投与の翌日にエーテル麻酔下で肝臓を採取し、液体窒素で凍結後に-80℃に保存した。
1.-80℃に凍結保存しておいた肝臓の一部(50~150 mg)をアシストチューブへ分取した。
2.メタノール2 mLを添加し、POLYTRON(KINEMATICA社)で破砕した。
3.そこへクロロホルムを4 mL添加し、室温で10分間激しく攪拌した。
4.さらにmilliQ水を1 mL添加し激しく攪拌した。
5.低速遠心機(株式会社日立製作所)で遠心分離した(2,500 rpm,5 min,室温)。
6.下層(全量4.5 mL)の一部をエッペンチューブへ分取し、遠心エバポレーター(株式会社佐久間製作所)を用いて溶媒を除去した。
7.エッペンチューブへエタノールを10 μL加えて再溶解させた。
8.そこへトリグリセライドE-テストワコー試薬(和光純薬工業株式会社)を1 mL加え、トリグリセライドを定量した。
9.以上より、肝臓1 g当たりのトリグリセライド含量を算出した。データは平均値±標準誤差で示した。
結果は表1の通りであった。正常マウスの肝臓中トリグリセライド含量は5~10(mg/g liver)であるので、KK-Ayマウスの肝臓中トリグリセライド含量は高値であり、脂肪肝を呈していると言える。KK-Ayマウスへ化合物Aを投与することにより、肝臓中トリグリセライド含量が有意に改善された。本結果は、化合物Aが非アルコール性単純性脂肪肝の治療薬として有用であることを示すものである。
非アルコール性単純性脂肪肝モデル(KK-Ayマウス)に対する効果(2)
実施例1と同様に試験を実施した。ただし、本試験では、3群に群分け(各群8匹)を行い、第一群にはvehicle(0.5%メチルセルロース)10 mL/kg、第二群には(1S)-1,5-アンヒドロ-1-[3-(1-ベンゾチオフェン-2-イルメチル)-4-フルオロフェニル]-D-グルシトール(化合物B)とL-プロリンの共結晶(モル比1:1)を3 mg/kg(化合物B換算)、第三群には比較化合物として前述の特許文献1に開示された2-(4-メトキシベンジル)フェニル6-O-エトキシカルボニル-β-D-グルコピラノシド(以下、化合物Xということがある。構造式を下記に示す)を36 mg/kgの用量で、1日1回で二週間経口投与した。そして、実施例1と同様に肝臓のトリグリセライド含量を測定した。
結果は表2の通りであった。KK-Ayマウスにおいては、実施例1と同様に脂肪肝を呈していたが、化合物Bの投与により、肝臓中トリグリセライド含量が有意に改善された。本結果は、化合物Bが非アルコール性単純性脂肪肝の治療薬として有用であることを示すものである。
一方、化合物Xは化合物Bの10倍以上の用量を投与しているにもかかわらず、有意な作用を示さなかった。
非アルコール性単純性脂肪肝モデル(KK-Ayマウス)に対する効果(3)
実施例1と同様に試験を実施した。ただし、本試験では、3群に群分け(各群8匹)を行い、第一群にはvehicle(0.5%メチルセルロース)10 mL/kg、第二群には化合物BとL-プロリンの共結晶(モル比1:1)を3 mg/kg(化合物B換算)、第三群には比較化合物として前述の特許文献2に開示されたT-1095、即ち、3-(ベンゾ[b]フラン-5-イル)-2',6'-ジヒドロキシ-4'-メチルプロピオフェノン 2'-O-(6-O-メトキシカルボニル)-β-D-グリコピラノシド(以下、化合物Yということがある。構造式を下記に示す)を34 mg/kgの用量で、1日1回で二週間経口投与した。そして、実施例1と同様に肝臓のトリグリセライド含量を測定した。
結果は表3の通りであった。KK-Ayマウスにおいては、実施例1と同様に脂肪肝を呈していたが、化合物Bの投与により、肝臓中トリグリセライド含量が有意に改善された。
一方、化合物Xは化合物Bの10倍以上の用量を投与しているにもかかわらず、有意な作用を示さなかった。
非アルコール性脂肪肝炎モデル(メチオニン・コリン欠乏食(MCD食)負荷ラット)に対する効果(1)
本試験は、文献(J Hepatol., 2003, 39, 756-764.)を参考に試験を実施した。Wistarラット(雄、日本チャールス・リバー株式会社より購入)を用いた。餌はMCD飼料(メチオニン・コリン欠乏食(METHIONINE / CHOLINE DEFICIENT DIET、MPbiochemicals社製))又は正常コントロール飼料(METHIONINE / CHOLINE CONTROL DIET、MPbiochemicals社製)を自由に摂食させた。9週齢時に、体重を測定し、均一になるように群分けを行い試験を開始した(各群10匹)。第一群には正常コントロール食を与え、vehicle(0.5%メチルセルロース)5 mL/kgを投与した。第二群及び第三群にはMCD食を与え、第二群にはvehicle(0.5%メチルセルロース)5 mL/kgを投与し、第三群には化合物BとL-プロリンの共結晶(モル比1:1)を3 mg/kg(化合物B換算)で投与した。薬剤は1日1回で16週間経口投与した。最終投与の翌日にエーテル麻酔下で肝臓を採取し、肝臓の一部を10%中性緩衝ホルマリンにて固定した。定法によりパラフィン切片(3 μm)を作製し、HE染色とワンギーソン(van Gieson)染色を施した。HE染色標本を用いて炎症巣を、ワンギーソン染色標本を用いて線維化をそれぞれ評価した。評価は、炎症巣に関してはNASH activity score(NAS)を、線維化に関してはBrunt分類をそれぞれ参考(日本肝臓学会 編、NASH・NAFLDの診療ガイド、2006年)に、それぞれ0、1、2、3、4の五段階(表4参照)で評価した。なお、表4において、「200倍の拡大」での視野は、「100倍の拡大」での視野の4分の1の面積となる。
結果は図1及び図2の通りであった。MCD食を与えたラットでは、正常コントロール食を与えたラットに比べて、炎症性細胞浸潤病理スコア及び肝線維化病理スコアが有意に上昇し、非アルコール性脂肪肝炎の病態を呈していた。本モデルへ化合物Bを投与することにより、炎症性細胞浸潤病理スコア及び肝線維化病理スコアが有意に改善された。本結果は、化合物Bが非アルコール性脂肪肝炎の治療薬として有用であることを示すものである。
非アルコール性脂肪肝炎モデル(コリン欠乏アミノ酸置換食(CDAA食)負荷ラット)に対する効果(1)
本試験は、文献(Biochem Biophys Res Commun., 2004, 315(1), 187-195.)を参考に試験を実施した。Wistarラット(雄、日本チャールス・リバー株式会社より購入)を用いた。餌はCDAA飼料(コリン欠乏アミノ酸置換食(Choline Deficient and Iron Supplemented L-Amino Acid Defined Rat Diet、Dyets社製))又は正常コントロール飼料(Choline and Iron Supplemented L-Amino Acid Defined Rat Diet、Dyets社製)を自由に摂食させた。9週齢時に、体重を測定し、均一になるように群分けを行い試験を開始した(各群10匹)。第一群には正常コントロール食を与え、vehicle(0.5%メチルセルロース)5 mL/kgを投与した。第二群および第三群にはCDAA食を与え、第二群にはvehicle(0.5%メチルセルロース)5 mL/kgを投与し、第三群には化合物BとL-プロリンの共結晶(モル比1:1)を3 mg/kg(化合物B換算)で投与した。薬剤は1日1回で5週間経口投与した。最終投与の翌日にエーテル麻酔下で肝臓を採取した。肝臓の一部を10%中性緩衝ホルマリンにて固定後、定法によりパラフィン切片(3 μm)を作製し、ワンギーソン(van Gieson)染色を施した。線維化の評価はBrunt分類を参考(日本肝臓学会 編、NASH・NAFLDの診療ガイド、2006年)に、0、1、2、3、4の五段階(表4参照)で評価した。
結果は図3の通りであった。CDAA食を与えたラットでは、正常コントロール食を与えたラットに比べて肝線維化病理スコアが有意に上昇し、非アルコール性脂肪肝炎の病態を呈していた。本モデルへ化合物Bを投与することにより、肝線維化病理スコアが有意に改善された。本結果は、化合物Bが非アルコール性脂肪肝炎の治療薬として有用であることを示すものである。
非アルコール性単純性脂肪肝モデル(KK-Ayマウス)に対する効果(4)
実施例1と同様に試験を実施した。第一群にはvehicle(0.5%メチルセルロース)10 mL/kg、第二群には(1S)-1,5-アンヒドロ-1-[4-クロロ-3-(4-エトキシベンジル)フェニル]-D-グルシトール(化合物C)を3 mg/kgの用量で、1日1回で二週間経口投与した。そして、実施例1と同様に肝臓のトリグリセライド含量を測定した。
結果は表5の通りであった。KK-Ayマウスにおいては、実施例1と同様に脂肪肝を呈していたが、化合物Cの投与により、肝臓中トリグリセライド含量が有意に改善された。本結果は、化合物Cが非アルコール性単純性脂肪肝の治療薬として有用であることを示すものである。

本発明の医薬組成物の有効成分である(1S)-1,5-アンヒドロ-1-[4-クロロ-3-(4-エトキシベンジル)フェニル]-D-グルシトール又はその製薬学的に許容される塩についても、実施例5と同様に試験を実施し、効果を確認することができる。
非アルコール性脂肪肝炎モデル(メチオニン・コリン欠乏食(MCD食)負荷ラット)に対する効果(3)
本発明の医薬組成物の有効成分である(1S)-1,5-アンヒドロ-1-[5-(アズレン-2-イルメチル)-2-ヒドロキシフェニル]-D-グルシトール又はその製薬学的に許容される塩についても、実施例4と同様に試験を実施し、効果を確認することができる。

- (1S)-1,5-アンヒドロ-1-[3-(1-ベンゾチオフェン-2-イルメチル)-4-フルオロフェニル]-D-グルシトールが、モル比1:1の(1S)-1,5-アンヒドロ-1-[3-(1-ベンゾチオフェン-2-イルメチル)-4-フルオロフェニル]-D-グルシトールとL-プロリンとの共結晶である、請求項17に記載の治療方法。

- (1S)-1,5-アンヒドロ-1-[4-クロロ-3-(4-エトキシベンジル)フェニル]-D-グルシトールが、モル比1:1若しくは1:2の(1S)-1,5-アンヒドロ-1-[4-クロロ-3-(4-エトキシベンジル)フェニル]-D-グルシトールとL-プロリンとの共結晶、又は(1S)-1,5-アンヒドロ-1-[4-クロロ-3-(4-エトキシベンジル)フェニル]-D-グルシトールのプロピレングリコール水和物である、請求項20に記載の治療方法。
- 脂肪性肝疾患が、非アルコール性脂肪性肝疾患である、請求項13~21のいずれかに記載の治療方法。
- 脂肪性肝疾患が、非アルコール性単純性脂肪肝である、請求項13~21のいずれかに記載の治療方法。
- 脂肪性肝疾患が、非アルコール性脂肪肝炎である、請求項13~21のいずれかに記載の治療方法。
- 式(I)の化合物又はその製薬学的に許容される塩が、(1S)-1,5-アンヒドロ-1-[5-(アズレン-2-イルメチル)-2-ヒドロキシフェニル]-D-グルシトール又はその製薬学的に許容される塩である、請求項25に記載の使用。
- (1S)-1,5-アンヒドロ-1-[5-(アズレン-2-イルメチル)-2-ヒドロキシフェニル]-D-グルシトール又はその製薬学的に許容される塩が、(1S)-1,5-アンヒドロ-1-[5-(アズレン-2-イルメチル)-2-ヒドロキシフェニル]-D-グルシトール コリン塩である、請求項26に記載の使用。
