Abstract
Es wird ein Hüftgelenk-Endoprothesensystem beschrieben, das aus einer Pfanne (1), einem Inlay (2), einem Kopf (3), wahlweise einem separatem Hals (4) sowie einem Schaft (5) besteht. Das Inlay (2) kann eine oder mehrere Überhöhungen (25) aufweisen, so daß eine Luxation des Kopfes (3) verhindert wird. Außerdem soll das Inlay (2) in der Pfanne (1) beweglich ausgebildet sein, wobei eine Luxation des Inlays (2) durch Führungen/ Anschläge (12) im Zusammenspiel mit Nuten (22) verhindert wird. Die Anordnung nach Figur a verhindert eine Luxation bei verbesserter Range of Motion in allen Bewegungsrichtungen, dargestellt durch den halben Gesamtwinkel (a<SUB>g</SUB>). Zusätzlich oder alternativ kann durch das Vorhandensein von Inlayaussparungen (24) und/oder Pfannenaussparungen (14) die Range of Motion (ROM) in Hauptbewegungsrichtung (Extension/Flexion) vergrößert werden. Die Anordnung nach Figur b zeigt die ROM in Ab-/Adduktionsrichtung, dargestellt durch den Abduktionswinkel (ß<SUB>ab</SUB>). Im zugehörigen Schnitt nach Figur c wird die die ROM in der Extensions-Mexionsebene nach dieser Anordnung verdeutlicht, dargestellt durch den Winkel (a<SUB>fl</SUB>). Zudem werden einzelne Komponenten sowie die erforderliche Operationstechnik durch entsprechende Anpassung im Schaft-/Halsbereich angeglichen, sowie die Schäfte (5) als solche, ihre intraoperative Ausrichtung und die möglicherweise später erforderliche Entfernbarkeit weiter optimiert.

Beschreibung
Hüftgelenk-Endoprothesensystem Die Erfindung betrifft ein Hüftgelenk-Endoprothesensystem, das aus einer Pfanne, einem May, einem Kopf, wahlweise einem separaten Hals sowie einem Schaft besteht. Durch entsprechende Ausführungsformen wird die Bewegungsmöglichkeit (Range Of Motion, ROM) erhöht, so daß ein Anschlagen des Halses an der Pfanne bzw. dem Irilay (Impingement) verhindert wird. Zudem kann durch weitere Ausfuhrungsformen ein Auskugeln des Hüftkopfes (Luxation) verhindert werden.
Gängige Hüftprothesen, bestehend aus einer Pfanne, einem Inlay, einem Kopf und einem Schaft, zeigen häufig postoperativ die Probleme der Luxation und des Impingements. Eine Luxationsproblematik tritt vor allem früh postoperativ auf, die Probleme des Impingements hingegen können im Verlauf zu vorzeitigem Verschleiß bzw. zu vorzeitiger aseptischer Lockerung der Prothesenkomponenten führen. Beide Komplikationen sind nicht völlig unabhängig voneinander, denn durch ein Impingement des Halses am Pfannenrand kann der Kopf aus der Pfanne herausgehebelt werden. Dieses Problem zeigen vor allem auch so genannte „Schnapppfannen", bei denen das Inlay den Kopfüber die Halbkugelform hinaus umgreift.
Grundfaktoren, welche die ROM beeinflussen, sind das Verhältnis von Kopf- zu Halsdurchmesser sowie die Ausrichtung der Komponenten mit Anteversion und Inklination der Pfannen-/Inlaykomponente im Zusammenspiel mit der Anteversion und des CCD- Winkels des Schaftes. Weiterführend zeigt sich die theoretisch optimale Ausrichtung und Dimensionierung der Komponenten zueinander nicht unbedingt als die günstigste Eigenschaft für den Kraftfluß und den Abrieb. Die Tribologie selbst ist neben dem Kopfdurchmesser natürlich auch von den verwendeten Materialen abhängig. Zunehmend verwendete Metall-Metall-Paarungen oder Keramik-Keramik-Paarungen von Kopf zu Inlay/Pfanne sind durch den „harten" Anschlag deutlich anfälliger und problematischer, wenn ein Impingement auftritt, als z.B. Metall-Polyethylen- oder Keramik-Polyethylen- Paarungen mit einem „weicheren" Anschlag.
Weitere Einschränkungen der ROM einer Hüftendoprothese entstehen durch nicht optimale Ausrichtungen der Komponenten bei der Implantation, so daß Reserven des Bewegungsumfanges vorteilhaft wären. Der natürliche Bewegungsumfang des Hüftgelenkes beträgt beim Gesunden etwa 150° (Extension/Flexion), wobei in der Ab/- Adduktionsebene nur etwa 70° Bewegungsumfang erreicht werden. Eine herkömmliche Hüftendoprothese zeigt bauartbedingt eine ROM von etwa 120° in alle Richtungen. Demnach liegt für die Extension/Flexion ein Defizit, für die Ab-/Adduktion ein Überschuß an Beweglichkeit vor. Dieser Verlust an Beweglichkeit wird vor allem bei jüngeren/sportlicheren Patienten zum Problem, besonders dann, wenn hier „harte" Gleitpaarungen verwendet werden. Auch bei älteren Patienten mit geringerer Restbeweglichkeit oder bei Revisionsoperationen kann schon bei kleinen Fehlausrichtungen der Bewegungsspielraum aufgebraucht sein, so daß es zu Luxationen und Impingementfolgen kommen kann.
Ein Lösungsansatz im Hinblick auf Verringerung der Luxationstendenz für Hüftgelenk- Endoprothesensysteme wird z.B. in DEl 0304102 aufgezeigt.
Hier wird eine bipolare Pfanne verwendet, wobei ein Kugelkopf in einer Bipolarschale und diese Bipolarschale wiederum in der Pfanne rotiert. Ein Auskugeln des Kopfes aus der Bipolarschale kann mit Hilfe eines Sicherungsringes verhindert werden.
Eine Luxation der Bipolarschale aus der Pfanne heraus ist jedoch weiterhin möglich, vor
allem, wenn es zum Impingement kommt.
Die Aufgabe der Erfindung ist es, ein Hüftgelenk-Endoprothesensystem zu schaffen, welches den Anforderungen an eine Hüftendoprothese nach dem Stand der Technik in möglichst hohem Maße genügt. Hierzu zählen die Verringerung der postoperativen Luxationstendenz und des Impingements.
Darüber hinaus sollen einzelne Komponenten sowie die zugehörige Operationstechnik durch entsprechende Anpassung im SchafWHalsbereich angeglichen werden. Auch sollen die Schäfte als solche, ihre intraoperative Ausrichtung und die möglicherweise später erforderliche Entfernbarkeit weiter optimiert werden. Diese Aufgabe wird erfindungsgemäß durch ein Hüftgelenk-Endoprothesensystem gelöst, das aus einer Pfanne, einem Inlay, einem Kopf, wahlweise einem separaten Hals sowie einem Schaft besteht. Durch entsprechende Ausführungsformen wird die Bewegungsmöglichkeit (Range Of Motion, ROM) erhöht, so daß ein Anschlagen des Halses an der Pfanne bzw. dem Inlay (Impingement) verhindert wird. Zudem kann durch weitere Ausführungsformen ein Auskugeln des Hüftkopfes (Luxation) verhindert werden.
Der Grundgedanke besteht darin, daß die Beweglichkeit in der Hauptbewegungsrichtung (Extension/Flexion) durch eine oder mehrere etwa gegenüberliegende Aussparungen vergrößert wird. Hierdurch kann ein Anschlagen des Halses am Inlay bzw. an der Pfanne in dieser Ebene vermieden werden. Im Gegenzug dazu und zur Verringerung der Luxationstendenz können in der Ab-/Adduktions-Bewegungsrichtung eine oder mehrere Überhöhungen vorhanden sein, welche auch aus resorbierbarem Material bestehen können. Alternativ kann auch ein in der Pfanne beweglich gelagertes Inlay verwendet werden, wobei die Luxation durch unterschiedliche Kugelzentren von Pfanne, Inlay und Kopf zu einer Selbstzentrierung beitragen können, komplette Fehlstellung und Luxation jedoch durch Führungen und Anschläge vermieden werden. Eine Kombination beider Ansätze ist auch möglich. Diese Systeme aus Pfanne, Inlay und Kopf sind prinzipiell bei entsprechender Ausbildung der Halsaufnahme des Kopfes mit den gängigen Schäften kombinierbar.
Je nach Ausführung ist jedoch eine Änderung des Operationsverfahrens nötig, da eine Reposition bei größeren Überhöhungen nach herkömmlicher Implantationstechnik nicht immer möglich ist. Daher kann die Verwendung von kürzeren Schäften im Halsbereich in Verbindung mit modularen Halseinheiten vorteilhaft sein. Mit Hilfe dieser modularen Hälse kann auf einfache Weise die Beinlänge durch unterschiedlich verfügbare Gesamtlängen nach Bedarf variiert werden. Diese Verfahrensweise ist vor allem dann zu bevorzugen, wenn die Pfanne zusammen mit Inlay und Kopf als Einheit implantiert werden soll. Zusätzlich können bei entsprechender Ausfuhrung durch den modularen Hals verschiedene CCD- Winkel angeboten sowie auch eine Ante-/Retrotorsion des Schaftes im Femur ausgeglichen oder verstärkt werden.
Bei den Schäften wird weiter zwischen verschiedenen Operationsverfahren unterschieden: zementiert oder nicht-zementiert. Während ggf. angebrachte Rippen bei zementierter Operationstechnik eher weich bzw. bogenförmig/rund verlaufen sollen, um ein Umfließen des Zementes bei der Implantation zu gewährleisten, können die Rippen bei nicht- zementierter Operationstechnik eher schärfer gestaltet werden. Neben der Implantation und einer möglichst langen Standzeit ist auch immer die Entfernbarkeit von Implantaten zu berücksichtigen. Daher sollen die Schäfte möglichst einfache geometrische Formen haben, welche mit einfachen Werkzeugen, wie z.B. geraden Meißeln, im Falle eines Revisionsbedarfs aus dem Schaft herausgelöst werden können. Aus diesem Grund wird eine konstante Verjüngung nach distal bevorzugt, wobei lediglich proximal medial eine Kurvation zur Angleichung an die Femuranatomie vorzusehen ist.
Die Grundform der Schäfte ist demnach keilförmig, wobei der Querschnitt oval bis rechteckig ausgeprägt sein kann.
Besonderes Augenmerk gilt der Schaftspitze bei nicht-zementierter Verfahrensweise, hier soll ein intramedullärer Kontakt mit der Kortikalis zur Vermeidung eines postoperativen Oberschenkelschmerzes durch stärkere Verjüngung und entsprechend kleiner Schaftspitze oder alternativ durch Verwendung eines Kurzschaftes mit rein proximalem metaphysärem Verankerungsprinzip zum Einsatz kommen.
Auch bei zementierten Schäften ist die Ausrichtung der Schaftachse und der Spitze im Femur nicht immer sicher zu gewährleisten. Es gibt verschiedene Lösungsansätze, z.B. mit Zentrierspitzen. Durch eine Ausführungsform mit axialer Bohrung der Schäfte kann eine geführte Implantation mit Hilfe eines Führungsstabes erfolgen. Bei geeigneter Ausbildung kann dieser Führungsstab zusammen mit der Markraumsperre bereits vor dem Raspeln eingebracht werden. Mit ebenfalls aufgebohrten Raspeln kann sowohl die Lage des Schaftes, als auch die Lage der Markraumsperre im Schaft vorgegeben werden. Zur Vermeidung der seltenen, aber schwerwiegenden Komplikation, der Fett-
/Knochenembolie, kann dieses Führungssystem aus Markraumsperre und Führungsstab zusätzlich zentral aufgebohrt sein und distal der Schaftspitze mit Entlüftungslöchern zur Reduzierung des intramedullären Druckes beim Raspeln und beim Einbringen des Schaftes beitragen. Mit dieser Maßnahme kann auf das von manchen Operateuren bevorzugte Entlüftungsloch im Femur verzichtet werden, wodurch Komplikationen wie Zementaustritt oder periprothetische Frakturen durch Kerbwirkung vermieden werden können. Dieses zentral geführte Operationsverfahren ist bei entsprechender Ausführung mit axialer Bohrung auch bei den nicht-zementierten Schäften anwendbar, wobei hier die Markraumsperre durch ein einfaches Zentrierstück ersetzt werden kann.
Nachfolgend wird die Erfindung anhand von Ausführungsbeispielen beschrieben.

Die Figur 1 zeigt schematisch im Schnitt eine herkömmliche hemisphärische Pfanne (1) mit einem leicht überhöhten Inlay (2). In diesem Inlay (2) rotiert ein Kopf (3) welcher bei der Implantation durch entsprechend erhöhte Kraft im Inlay (2) einschnappt. Durch solche Ausführungen soll eine postoperative Luxation erschwert werden. Das Fixationselement (19) dient lediglich zur Veranschaulichung, daß das Inlay (2) in der Pfanne (1) nach den herkömmlichen Methoden fixiert ist und nicht rotiert.
Der Kopf steckt nach üblicher Technik mittels konischer Verbindung auf einem Hals (4). Durch den Extensions-/Flexionswinkelwinkel (αg) wird der maximale Bewegungsumfang des Hüftgelenk-Endoprothesensystems dargestellt. Die ROM ist in diesem Fall in alle Richtungen gleich.
In der Figur 2(a, b) wird demgegenüber schematisch der Bewegungsumfang gemäß einer Ausführungsform nach der Erfindung dargestellt. Auch hier besteht das System aus einer etwa hemisphärischen Pfanne (1), einem Inlay (2), und einem Kopf (3), welcher wiederum auf einem Hals (4) angebracht ist. Zur sicheren Vermeidung einer Luxation des Kopfes (3) aus dem Inlay (2) wurde hier eine noch größere Überhöhung im Vergleich zum Beispiel aus Figur 1 gewählt. Daher ist der Bewegungsumfang zwischen Kopf (3) und Inlay (2), hier nur in einer Richtung als /4αk/i dargestellt, geringer. Zum Ausgleich kann die Bewegung jedoch nicht nur zwischen Kopfaußenfläche (33) und Inlayinnenfläche (23) erfolgen, sondern auch das Inlay (2) kann in der Pfanne (1) rotieren. Dazu werden die Inlayaußenfläche (21) und die Pfanneninnenfläche (11) ebenso als Gelenkflächen ausgebildet. Um eine Luxation des Inlays (2) aus der Pfanne (1) zu verhindern, besitzt in dieser Ausführungsform nach der Erfindung das Inlay (2) zusätzlich eine Nut (22), hier kappenföraiig ausgeprägt, in welche eine Führung (12) greift. Dabei kann die Führung als Anschlag, als Teil der Gelenkfläche oder auch als untergreifendes Element ausgebildet sein. Weiter kann die Führung (12) als festes Element der Pfanne (1) ausgebildet sein, aber auch mittels einer Schraube (7) oder anderen Verbindungselementen in einer Bohrung/ Aufnahme (13) der Pfanne (1) fixiert werden. Die Kombination von einer oder mehreren Führungen (12) mit längeren Schrauben (7) ermöglicht eine sicher Primäre Fixierung der Pfanne (1) im Knochenstock (Figur 9). Der aus dieser Anordnung resultierende Gesamtbewegungsumfang (Range of Motion, ROM) entspricht etwa herkömmlichen Pfannen-/Kopf-/Inlaysystemen ohne Überhöhung. Dieser wird in der Figur 2(b) durch den Winkel 1Aa^ verdeutlicht, welcher sich aus der Kombination der Bewegung zwischen Kopf (3) und Inlay (2), sowie Inlay (2) und Pfanne (1) zusammensetzt und auch in dieser Ausfuhrungsvariante in alle Richtungen gleich groß ist. Als Material für die Elemente eignen sich alle zugelassenen Werkstoffe, vorzugsweise Metalllegierungen, Keramik oder Polyethylen, wobei die Kombinationen je nach
tribologischen Gesichtspunkten und gängigen Verfahren gewählt werden können.
Die Figur 3(a) zeigt schematisch im Schnitt eine weitere Ausruhrungsform nach der Erfindung mit einer Pfanne (1) und einem wiederum leicht überhöhten Inlay (2). Hier ist das Inlay (2) wie in Figur 1 in der Pfanne (1) fixiert, symbolisch durch das Fixationselement (19) dargestellt. Beide Elemente besitzen eine Aussparung, Pfannenaussparung (14) sowie Inlayaussparung (24). Zur realistischeren Darstellung des Bewegungsumfanges in der Hauptbewegungsrichtung (Extension/Flexion) wird ein schematischer Schnitt von lateral unter einem Anteversionswinkel (γ) gezeigt. Im Inlay (2) rotiert der Kopf (3) mit einem Hals (4). Bei entsprechender Ausführung der Überhöhung über einen Halbkreis um den Kopf (3) herum hinaus ist auch hier eine Luxation erschwert (Schnappmechanismus). Durch diese Anordnung kann eine größere Flexion, dargestellt durch den Flexionswinkel (otf) in der Figur 3(b), ermöglicht werden. Der Extensionswinkel (αe) in der Figur 3(c) hingegen entspricht in etwa dem halben Extensions-/Flexionswinkelwinkel (Viag) aus Figur 1 (abhängig vom CCD- Winkel des Halses und der Torsion des Schaftes, s.o.) und ist gleich dem Ab-/Adduktionswinkel. Die ROM ist in diesem Fall zu Gunsten der Flexion erweitert. Nachfolgend (Figur 4-10) werden Ausfuhrungsformen für Kombinationen aus beiden Beispielen (Figur 2 und 3) aufgezeigt. In der Figur 4 wird schematisch eine weitere Ausführungsform nach der Erfindung abgebildet, Figur 4(a) zeigt ein Hüftgelenk-Endoprothesensystem mit Pfanne (1), Inlay (2), Kopf (3) und Hals (4) von ventral, der zugehörige Schnitt von lateral wird in Figur 4(b) dargestellt. Pfanne (1) und Inlay (2) weisen zwei etwa gegenüberliegende Aussparungen (14, 24) auf. Versetzt dazu ist das Inlay (2) mit ebenfalls zwei Inlayüberhöhungen (25) versehen. Diese beiden Inlayüberhöhungen (25) sind vorzugsweise so ausgebildet, daß eine Luxation des Kopfes (3) erschwert bzw. verhindert wird. Ein Teil der Bewegung erfolgt zwischen Kopfaußenfläche (33) und der Inlayinnenfläche (23), ein weiterer zwischen der Inlayaußenfläche (21) und der Pfanneninnenfläche (11), wobei hier die Bewegung durch eine Führung (12) mit entsprechender Nut (22) eingeschränkt wird. Die Führung (12) ist mittels einer Schraube (7) im Pfannenzentrum in einer Bohrung/ Aufnahme (13) befestigt. Der Kopf (3) ist nach üblicher Technik mit einer Konusaufnahme (31) versehen, welche bei entsprechender Ausführung die Kombination mit gängigen Schäften/Halssystemen (4) ermöglicht. Eine Einkerbung (42) kann dabei die ROM erweitern. Die weitere Beschreibung der Bewegungsmöglichkeiten dieser Ausfuhrungsform nach der Erfindung erfolgt bei den Beschreibungen zu Figur 7 und 8.
Figur 5 zeigt die Ansicht von unten der Ausführung nach Figur 4 ohne Halselement (4). Man erkennt, daß die Inlayüberhöhungen (25) über den Äquator des Kopfes (3) reichen und so einer Luxation entgegen wirken. Die Nut (22) ist etwa senkrecht zur Achse der Inlay aussparungen (24) des Inlay s (2) und Pfannenaussparungen (14) der Pfanne (1) ausgerichtet. Die geführte Bewegung zwischen Nut (22) und Führung/Anschlag (12) soll dabei etwa der Ab-/Adduktionsrichtung entsprechen, die Aussparungen (14, 24) hingegen sollen die ROM in der Extensions-/Flexionsrichtung vergrößern. Bei zu strenger Führung des Inlays (2) über Inlaynut (22) und Führung/Anschlag (12) kommt es bei extremen Bewegungen möglicherweise zum Verklemmen oder Sperren. Daher kann es vorteilhaft sein, wenn die Führung (12) durch entsprechende Ausbildung (z.B. durch Abrundung der Führung in dieser Ebene oder durch ein Rotationslager) zumindest eine eingeschränkte axiale Rotation des Inlays zulässt Eine völlig freie Rotation hingegen sollte vermieden werden, da sonst eine völlige Fehlstellung resultieren könnte.
Figur 6 stellt eine Ansicht von unten einer weiteren Ausfuhrung nach der Erfindung dar. Hier ist die Nut (22) im zentralen Bereich erweitert sowie Führung/Anschlag (12) länglicher und abgerundeter ausgebildet. Es kann eine gewisse axiale Rotation ermöglicht werden, so daß z.B. nach maximaler Flexion eine zusätzliche Abduktionsbewegung ohne Verklemmung oder Sperren erfolgen kann. Wenn die Länge von Führung/ Anschlag (12) die maximale Weite der Nut (12) überschreitet, wird eine totale axiale Rotation verhindert.
In der Figur 7(a-c) wird in einer Ansicht von ventral schematisch der Bewegungsumfang in der Ab-/Adduktionsebene gemäß der Ausführungsform nach Figur 4 gezeigt. Zur realistischeren Illustration wird die Pfanne (1) unter einem Inklinationswinkel (δ) dargestellt. In der Figur 7(a) rotiert zunächst der Kopf (3) mit einem Hals (4) im Inlay (2), wodurch ein relativ kleiner Kopf-Inlay-Ab/Adduktionswinkel (ßk/i) erreicht wird. Dieser ist zwar für alltägliche Bewegungen genügend, kann jedoch schon bei nicht völlig korrekter Ausrichtung der Pfanne im Becken unzureichend sein. Figur 7(b) zeigt den maximal möglichen Abduktionswinkel (ßab) durch zusätzliche Ausnutzung der Rotation des Inlays (2) in der Pfanne (1) im Rahmen der geführten Bewegung über Nut (22) und Führung/Anschlag (12). Analog dazu wird in Figur 7(c) der maximal mögliche Adduktionswinkel (ßad) abgebildet. Die ROM dieser Anordnung entspricht in dieser Bewegungsebene der ROM üblicher Hüftprothesen und ist im Vergleich zum physiologischen Bewegungsumfang größer (siehe oben).
Figur 8(a-c) veranschaulicht schematisch den Bewegungsumfang in der Extensions- /Flexionsebene im Schnitt in einer seitlichen Ansicht der Ausfuhrungsform nach Figur 4. Wie in Figur 3 ist die Pfanne (1) auch unter einem Anteversionswinkel (γ) abgebildet. In der Figur 8(a) sind die Pfannenaussparungen (14) der Pfanne (1) und die Inlayaussparungen (24) des Inlays (2) zu erkennen, welche den Bewegungsumfang des Halses (4) bei Rotation des Kopfes (3) in dieser Ebene erweitern. Figur 8(b) zeigt den maximal möglichen Flexionswinkel (αg). Analog dazu wird in Figur 8(c) der maximal mögliche Extensionswinkel (αex) abgebildet. Die ROM dieser Anordnung ist in dieser Bewegungsebene gegenüber der ROM üblicher Hüftprothesen deutlich vergrößert, selbst im Vergleich zum physiologischen Bewegungsumfang (siehe oben). Dadurch kann ein Impingement (auch bei leichter Fehlausrichtung der Pfanne (1) im Knochenstock) nahezu ausgeschlossen werden. In der Figur 9(a-c) wird in einer ventralen Ansicht schematisch eine mögliche
Implantationsreihenfolge gemäß einer Ausfuhrungsform nach der Erfindung abgebildet. Figur 9(a) zeigt eine in das Becken eingeschlagene Pfanne (1) mit Führung/Anschlag (12) und Pfannenaussparungen (14). In diesem Beispiel ist das FührungsVAnschlagselement (12) mit einer in den Knochenstock ragenden Schraube (7) kombiniert, welche die Primärverankerung der Pfanne erhöhen soll. Je nach Ausführung können auch mehrere Schrauben (7) verwendet werden. Als weiterer Implantationsschritt erfolg in Figur 9(b) das Eindrehen des Inlays (2) mit der Nut (22) über Führung/Anschlag (12). Anschließend kann der Kopf (3), welcher auf Hals (4) steckt, im Inlay (2) bei der Reposition einschnappen, wie in Figur 9(c) skizziert. Gegebenenfalls kann mit Hilfe eines Zusatzwerkzeuges ein Verdrehen des Inlays verhindert werden.
Figur lθ(a-c) zeigt weitere Implantationsmöglichkeiten gemäß weiterer Ausführungsformen nach der Erfindung. In Figur 10(a) wird eine Pfanne (1) mit einem Inlayhauptteil (28) gezeigt, welche auf übliche Weise implantiert werden können. Auch die Reposition von Kopf (3) mit Hals (4) erfolgt wie bei anderen Pfannensystemen. Das Inlayhauptteil (28) weist zusätzlich eine formschlüssige Nase (281) auf. In der Figur 10(b) wird dargestellt, wie
über diese foπnschlüssige Nase (281) ein Verschlußteil (29) mit entsprechend formschlüssiger Nut (291) geführt und bei entsprechender Ausführung eingerastet werden kann. Hier sind verschiedene formschlüssige Verbindungen nach dem Stand der Technik denkbar. Eine weitere Lösung ist in der Figur 10(c) abgebildet. Auch hier kann die Implantation von Pfanne (1), Inlayhauptteil (28), Kopf (3) und Hals (4) auf bekanntem Weg erfolgen. Anschließend werden einzelne Verschlußteile (29) z.B. mit Hilfe von Verschlußschrauben (292), welche in Verschlußgewinde (282) greifen, angebracht. Auch ein kompletter Ring, welcher vor der Reposition und noch vor Anbringen des Kopfes über den Hals gestreift wird, kann eine mögliche Lösung bieten, wenn er so ausgebildet ist, daß die Hauptbewegungsrichtung nicht wesentlich eingeschränkt wird. Prinzipiell ist darauf zu achten, daß die Grenzen der Inlaybestandteile möglichst nicht im Hauptbelastungsbereich liegen.
In der Figur 1 l(a-d) werden schematisch modulare Halselemente (4) gemäß weiterer Ausführungsformen nach der Erfindung dargestellt. Figur 11 (a) zeigt einen Hals (4) mit einem üblichen Konus (41) zur Kopfaufhahme und einer Einkerbung (42) zur Erweiterung der ROM. Dieser Hals (4) weist eine Schaftverbindung (43) auf, welche so ausgebildet ist, daß sie auf einen Konus eines Schaftes paßt. Durch dieses einfache Bauteil kann auf günstige Art und Weise durch entsprechend unterschiedliche Halslängen (Ih) die Beinlänge des Patienten beeinflußt werden. Dieses Verfahren kann vor allem dann zum Einsatz kommen, wenn nach einer Ausführungsform nach der Erfindung eine komplette Pfannen- /mlay-/Kopfeinheit implantiert wird und daher ein Ausgleich über die Kopf- /Halsverbindung nicht möglich ist. Eine stufenlose Einstellung der Beinlänge ist nach Ausführung des Halses (4) gemäß Figur ll(b) erzielbar, wenn der Hals (4) ein Gewinde als Schaftverbindung (43) aufweist, welches in ein entsprechendes Gegenstück des Schaftes greift (Figur 15). Durch einfache Zusatzmaßnahmen kann der CCD- Winkel des Schaftes über einen CCD-Korrektur- Winkel (ε) verändert werden, Figur 1 l(d). Hier ist dann eine Rotationssicherung der Hals-/Schaftverbindung notwendig, z.B. durch eine oder mehrere Rotationsaufhahmen (44) in welche entsprechende Stifte des Schaftes greifen. Auch kann so die Torsion des Schaftes im gewissen Maße korrigiert werden. Es sind diverse
Rotationssicherungen nach dem Stand der Technik denkbar, z.B. Führungen und Nuten im Bereich der Koni. Bei Ausführung nach Figur 1 l(b) und zusätzlichem CCD-Korrektur- Winkel (ε) kann auf eine Rotationssicherung verzichtet werden. Figur 12(a) bildet schematisch einen Schaft (5) für das zementierte Operationsverfahren mit einem Konus zur Kopf-/Halsaufnahme (51) ab, welcher durch einen CCD- Winkel (ζ) abgewinkelt ist. Um möglichst vielen anatomischen Gegebenheiten oder Bedürfhissen gerecht werden zu können, sollte eine Auswahl verschiedner Schäfte unterschiedlichen Durchmessers und unterschiedlicher CCD- Winkel (ζ) intraoperativ zur Verfügung stehen. Die Seitenflächen sind vorwiegend gerade unter einem Verjüngungswinkel (π) nach distal zur Spitze (58) ausgebildet. Lediglich proximal medial ist ein Bogen (57) zur Anpassung an die Femuranatornie vorgesehen. Da die Verwendung eines modularen Halses für die Verwendung der oben beschriebenen Pfannen-/Inlay-/Kopfkombinationen je nach Operationsverfahren vorteilhaft sein kann, ist für diese Fälle zur Verkürzung eine Adaptationsfläche (52) proximal medial abgeschrägt. Auf diese Weise kann ausreichend Platz für das modulare Halselement und dessen Verbindung erreicht werden. Für herkömmliche Operationsverfahren und Pfannen-/Inlay-/Halskombinationen kann auf diese Abschrägung verzichtet werden und ein Konus über einen Hals mit dem Schaft direkt verbunden sein. Mit Hilfe von einem oder mehreren Antirotationsstiften (54) kann eine Kombination mit Hälsen nach Figur 1 l(d) erfolgen, welche einen CCD-Korrektur- Winkel (ε) aufweisen.
Die zugehörigen Querschnitte des Schaftes (5) auf den Höhen A, B und C werden in Figur 12(b) dargestellt. In dieser Ausfuhrungsform nach der Erfindung handelt es sich um einen rechteckigen abgerundeten bis ovalen Grundquerschnitt. Zur Rotationssicherung des Schaftes im Zementköcher können zusätzlich eine oder mehrere Rippen oder Rillen vorhanden sein, welche vorzugsweise parallel zur axialen Implantationsrichtung verlaufen (Figur 13(a, b)).
Die Oberfläche sollte bei Verwendung des heutzutage üblichen Zementes (Polymethylrnetacrylat) feingestrahlt sein und der Werkstoff vorzugsweise aus Stahl- oder CoCr-Legierungen bestehen.
Figur 13(a, b) zeigt schematisch einen Schaft (5) für zementierte Operationstechnik gemäß einer weiteren Ausfuhrungsform nach der Erfindung, welcher eine Halsverbindung (51) aufweist, die eine konische Schaftverbindung eines modularen Halses aufnehmen kann. Hierdurch kann modularer Hals nach Figur 1 l(c) verwendet werden. Zur Rotationssicherung des Schaftes (5) im Zementköcher ist diese Ausfuhrungsform mit drei Rippen (53) parallel zur axialen Implantationsachse ausgebildet.
Analog zu Figur 12(a) werden die Querschnitte A, B und C in Figur 13(b) gezeigt. Um den Zementfluß bei Implantation im Bereich der Rippen (53) und Zwischenräume nicht zu behindern, sollten die Rippen (53) vorzugsweise abgerundet ausgebildet werden.
In der Figur 14(a) wird schematisch ein weiterer Schaft (5) für zementierte Operationstechnik gemäß einer weiteren Ausfuhrungsform nach der Erfindung dargestellt. Neben den vorangehend aufgezeigten Merkmalen nach Figur 12/13 weist dieser Schaft (5) eine Schulterabflachung (59) auf, was zum besseren Erhalt des Knochenstocks im Bereich des Trochanter majors dienen soll. Die zugehörige Seitenansicht zeigt Figur 14(b). Hier wird zudem deutlich, daß der Verjüngungswinkel (p) in der Sagittalebene unterschiedlich zum Verjüngungswinkel (π) in der Transversalebene (Figur 12(a)) ausgebildet sein kann.
In der Figur 15 wird eine Kombinationsmöglichkeit einer weiteren Ausführungsform eines weiteren Schaftes (5) nach der Erfindung in Kombination mit einem modularen Hals (4) nach Figur 1 l(b) aufgezeigt. Halsverbindung (51) und Schaftverbindung (43) sind als Schraubenverbindung ausgebildet, wodurch eine stufenlose Einstellung der Beinlänge (in Abhängigkeit vom Off-Set, bzw. umgekehrt) ermöglicht wird. Eine Arretierung der gewünschten Einstellung kann z.B. mittels einer Kontermutter (6) oder durch andere gängige Schraubensicherungen nach dem Stand der Technik erfolgen.
Figur 16 zeigt schematisch mögliche Implantationsverfahren eines weiteren Schaftes für zementierte Operationstechnik gemäß einer weiteren Ausführungsform nach der Erfindung. Es wird ein Schaft (5) mit einer axialen Bohrung (56) in einem proximalen Femurumriß gezeigt. Für das Ein- bzw. Herausbringen des Schaftes (5) kann dieser zudem mit einem Ein-/ Ausschlagadapter (55) versehen sein. Der Schaft wurde bei der Implantation über einen Führungsstab (8) eingebracht, welcher zum Ein-ZAusbringen einen Flansch (81) aufweist. Distal besitzt der Führungsstab (8) eine Sperrenverbindung (82) worüber er mit einer Markraumsperre (9) verbunden werden kann. Wahlweise können Markraumsperre (9) und Führungsstab (8) bereits vor oder mit dem Raspeln in der Markhöhle platziert werden. So kann bereits ein geführtes Raspeln erfolgen, was zu einer korrekten Ausrichtung und Zentrierung des Schaftes (5) beitragen soll, juber geeignete Abstandshalter oder Ausbildungen der Raspelspitzen kann der Abstand von Markraumsperre zur Schaftspitze definiert werden. Es besteht die Möglichkeit nach dem Raspeln unter Verwendung eines stabilen Führungsstabes (8, z.B. aus Metalllegierungen) diesen gegen einen
Einmalführungsstab (z.B. aus Kunststoff) auszutauschen, welcher beim Zementieren und
bei der Implantation verbleibt. Wenn der Führungsstab (8) bei noch nicht ausgehärtetem Zement entfernt werden soll, kann es hilfreich sein, wenn proximal Markierungen angebracht sind. Diese können dazu dienen, daß der Schaft (5)zunächst nur bis zu einer Vormarkierung eingebracht wird, dann der Führungsstab (8) entfernt wird und anschließend der Schaft bis zur definitiven Markierung nachgedrückt wird. Der Abstand dieser beiden Markierungen sollte dann so gewählt sein, daß das verbleibende Leervolumen des Zementköchers distal der Schaftspitze durch das Nachdrücken ausgefüllt wird. Die Ausschnittvergrößerung im Bereich der Markraumsperre zeigt eine weitere Ausfuhrungsform von Führungsstab (8) und Markraumsperre (9), welche eine gleichzeitige Entlüftung beim Raspeln und/oder Zementieren ermöglichen soll. Der Führungsstab (8) gemäß dieser Ausfuhrungsform nach der Erfindung ist mit einer zentralen Bohrung (83) versehen. Die Verbindung (82/91) zwischen Führungsstab (8) und Markraumsperre (9) ist so ausgebildet, daß die zentrale Bohrung (83) des Führungsstabes (8) bis in den Markräum distal der Markraumsperre (9) reicht. Hierdurch können Druckspitzen beim Einbringen der Markraumsperre (9) und beim Raspeln vermieden werden. Zusätzliche Entlüftungslöcher (84) direkt proximal der Markraumsperre (9) dienen zur Minderung des Druckes in diesem Bereich. Auch bei diesem Verfahren kann der Führungsstab (8) wahlweise zum Zementieren gegen einen Einmalführungsstab gewechselt werden. Dieser kann massiv ausgebildet sein oder gegebenenfalls distal verschlossen sein, damit der Zement nicht über die Markraumsperre hinaus in die Markhöhle gelangt. Eine partielle zentrale Bohrung (83) in Verbindung mit Entlüftungslöchern (84) kann beim Zementieren zum Entweichen der Luft und somit zur Verhinderung von Einschlüssen dienen.
Figur 17(a) zeigt schematisch einen Schaft (5) für nicht-zementierte Operationstechnik gemäß einer Ausführungsform nach der Erfindung. Dieser Schaft besitzt analog zum zementierten Schaft (5) nach Figur 12 bzw. 13 einen Konus als Halsverbindung (51), eine Adaptationsfläche (12), eine oder mehrere Rippen (53) bzw. Rillen und vorzugsweise gerade Seitenflächen. Insgesamt soll der nicht-zementierte Schaft (5) jedoch rein proximal metaphysär verankert werden, wodurch die Schaftlänge (ls) deutlich kürzer gewählt ist, als bei den Versionen für das zementierte Verfahren nach Figur (12-16). Dieses bedeutet, daß sich der Bogen (57) komplett oder fast komplett über die Schaftlänge (ls) zieht. Die Spitze (58) ist im Vergleich breiter ausgeprägt, der Verjüngungswinkel (π) in der Transversalebene eher steiler. Die zugehörigen Querschnitte A und B werden in Figur 17(b) gezeigt. Hier wird deutlich, daß die Rippen (53) vorzugsweise schärfer ausgebildet werden. Auch der Verjüngungswinkel (p) in der Seitenansicht nach Figur 17(c) ist im Vergleich eher etwas steiler gewählt. Die Oberfläche der nicht-zementierten Schäfte kann nach üblichen Verfahren zur besseren Knochenintegration aufbereitet sein, als Werkstoff eignen sich besonders Titanlegierungen. In der Figur 18 wird alternativ zur Ausführung nach Figur 17(a-c) ein längerer Schaft (5) für das nicht-zementierte Operationsverfahren dargestellt. Die Schaftlänge (ls) entspricht etwa der, der nicht-zementierten Schäfte, jedoch ist der Verjüngungswinkel (π) in der Transversalebene ähnlich steil, wie bei dem Kurzschaft nach Figur 17(a-c), wodurch eine kleinere Schaftspitze (58) im Vergleich zum zementierten Schaft (5) erreicht wird. Dieses soll zur Vermeidung des Anliegens der Schaftspitze (58) bei nicht ganz korrekter
Zentrierung in der Markhöhle beitragen. Zudem kann auch hier eine geführte Implantation über Führungsstäbe erfolgen, wenn der Schaft (5) mit einer zentralen Bohrung (56) versehen ist. Eine Markraumsperre (9) ist bei nicht-zementierter Operationstechnik nicht erforderlich, so daß eine einfachere Zentriervorrichtung ausreichend ist.
Claims
1. Hüftgelenk-Endoprothesensystem bestehend aus einer Pfanne (1), einem Inlay (2), einem Kopf (3), sowie einem Schaft (5), dadurch gekennzeichnet, daß die Pfanne (1) mit einer Aussparung (14) oder zwei etwa gegenüberliegenden Aussparungen (14) versehen ist, welche die Pfanne (1) in diesem Bereich über den hemisphärischen Horizont hinaus verkleinert. Die Aussparungen (14) sollen so angebracht sein, daß der Bewegungsumfang des Halses (4) in der Extensions-ZFlexionsebene vergrößert und ein hartes Impingement vermieden wird.
2. Hüftgelenk-Endoprothesensystem nach Anspruch 1 , dadurch gekennzeichnet, daß das in der Pfanne (1) fixierte Inlay (2) ebenfalls mit einer Aussparung (24) oder zwei etwa gegenüberliegenden Aussparungen (24) versehen ist, welche das Inlay (2) in diesem Bereich über den hemisphärischen Horizont hinaus verkleinert. Die Aussparungen (24) sollen so angebracht sein, daß der Bewegungsumfang des Halses (4) in der Extensions- /Flexionsebene vergrößert wird.
3. Hüftgelenk-Endoprothesensystem nach einem der Ansprüche 1 bis 2, dadurch gekennzeichnet, daß das in der Pfanne (1) fixierte Inlay (2) mit einer Inlayüberhöhung (25) oder mehreren Inlayüberhöhungen (25) über die Hemisphäre hinaus vergrößert versehen ist, welche etwa in der Ab-/Adduktionsebene angebracht sind.
4. Hüftgelenk-Endoprothesensystem nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß neben der gelenkigen Verbindung zwischen Inlayinnenfläche (23) und Kopfaußenfläche (33) auch die Pfanneninnenfläche (11) und die Inlayaußenfläche (21) als Gleitpaarung ausgebildet werden.
5. Hüftgelenk-Endoprothesensystem nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet, daß die Bewegung zwischen Pfanne (1) und Inlay (2) durch eine oder mehrere Führungen/Anschläge (12), welche in eine oder mehrere Nuten (22) formschlüssig und überschneidend greifen, so auf eine Achse und Rotation beschränkt wird, dass das Inlay (2) innerhalb des physiologischen Bewegungsumpfanges nicht mehr aus der Pfanne (1) luxieren kann.
Im Umkehrschluss können die Führungen (12) auch am Inlay (2) und die Nuten (22) an der Pfanne (1) angebracht sein. Auch eine entsprechende Verlagerung der geführten Bewegung von Pfanne (1) und Inlay (2) auf die Bewegung zwischen Inlay (2) und Kopf (3) ist möglich.
6. Hüftgelenk-Endoprothesensystem nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, daß die Bewegung zwischen Pfanne (1) und Inlay (2) bzw. zwischen Kopf (3) und Inlay (2) durch eine oder mehrere Führungen/Anschläge (12), welche in eine oder mehrere Nuten (22) nur bei Extremausschlägen greifen, so beschränkt wird, dass das Inlay (2) bei Endanschlägen nicht mehr aus der Pfanne ( 1 ) luxieren kann, j edoch innerhalb des physiologischen Bewegungsumpfanges mehrere Achsen und Rotationen frei sind, eine komplette Verdrehung jedoch vermieden wird.
Dabei können die Kugelzentren von Pfanne (1), Inlay (2) und Kopf (3) so ausgebildet werden, daß das Inlay (2) sich selbst zentriert. Bei entsprechend freigegebenem Bewegungsausmaß können die Inlayüberhöhungen (25) auch radial umschließend ausgebildet sein.
7. Hüftgelenk-Endoprothesensystem nach einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, die Pfanne (1) eine oder mehrere Bohrungen/Aufnahmen (13) aufweist, welche zur primären Fixierung mittels Schrauben (7) oder zur Befestigung der Führungen/Anschläge (12) dienen. Eine Kombination der Schrauben (7) mit Führungen/Anschlägen (12) ist ebenfalls möglich. Die Führungen/Anschläge (12) können dabei eine (Teil-)Rotation um die eigene Achse zulassen. Außerdem können am Pfannenrand zusätzliche Laschen oder Bohrungen zur weiteren Knochenverankerung angebracht sein.
8. Hüftgelenk-Endoprothesensystem nach einem der Ansprüche 1 bis 7, dadurch gekennzeichnet, daß das Inlay (2) aus einer Inlaygrundeinheit (28) und einem oder mehreren
Verschlußteilen (29) besteht, welche z.B. über formschlüssige Verbindungen mittels formschlüssiger Nase (281) und formschlüssiger Nut (291) oder mittels Gewinden (282) und Schrauben (292) nach intraoperativer Reposition an der Inlaygrundeinheit (28) fixiert werden. Dabei können die Verschlussteile (29) bei entsprechender Ausführung auch aus resorbierbarem Material gefertigt sein.
9. Hüftgelenk-Endoprothesensystem bestehend aus einer Pfanne (1), einem Inlay (2), einem Kopf (3), einem separatem Hals (4) sowie einem Schaft (5), dadurch gekennzeichnet, daß der Hals (4) proximal mit einem herkömmlichen Konus (41) analog zur Konusaufhahme (31) des Kopfes (3) und distal mit einer Schaftverbindung (43) ausgebildet wird, welche entweder als Konusaumahme oder Gewindeverbindung oder andere formschlüssige Verbindung ausgebildet wird. Dabei kann der Hals (4) einen CCD-
05 Korrekturwinkel (ε) aufweisen und die Rotation des Halses (4) auf dem Schaft (5) durch eine axiale Antkotationsaufhahrne (44) in Verbindung mit einem Antirotationsstift (54) verhindert werden. Zum Ausgleich der Ante-/Retrotorsion des Schaftes (5) können auch mehrere Antirotationsaufhahmen (44) bzw. Antirotationsstifte (54) oder andere Verbindungselemente vorgesehen werden. Bei Ausbildung der Schaftverbindung (43) als
10 Gewindeverbindung, kann die Rotation mit Hilfe einer Kontermutter (6) oder anderen Schraubensicherungen unterbunden werden.
10. Hüftgelenk-Endoprothesensystem nach Anspruch 9, 15 dadurch gekennzeichnet, daß der Schaft (5) medial proximal eine Adaptionsfläche (52) aufweist, welche auch zur
Verkürzung des Schaftes (5) in diesem Bereich dienen kann, was die Verwendung eines 20 modularen Halses (4) ermöglicht. Die Grundform des Schaftes soll im Wesentlichen keilförmig unter einem Verjüngungswinkel (π) in der Transversalebene und einem
Verjüngungswinkel (p) in der Sagittalebene bis zur Spitze (58) zulaufen, wobei proximal medial ein Bogen (57) vorhanden sein kann.
Die Halsverbindung (51) wird analog zur Schaftverbindung (43) nach Anspruch 9 25 ausgebildet. Ebenso können analog zu Anspruch 9 ein oder mehrere Antirotationsstifte (54) oder Antirotationsaumahmen vorhanden sein.
11. Hüftgelenk-Endoprothesensystem nach Anspruch 9 bis 10, 30 dadurch gekennzeichnet, daß der Schaft (5) mit einer oder mehreren Rippen und/oder Rillen (53) versehen ist, welche parallel zur Schaftachse in der Transversalebene verlaufen sollen, bzw. etwa parallel zum 35 Verjüngungswinkel (p) in der Sagittalebene. Dabei können die Rippen/Rillen vorzugsweise für das zementierte Operationsverfahren weich und rund ausgebildet werden, für das nicht- zementierte Operationsverfahren eher schärfer ausgeprägt sein.
40 12. Hüftgelenk-Endoprothesensystem nach einem der Ansprüche 9 bis 11, dadurch gekennzeichnet, daß der Schaft (5) einen Ein-/Ausschlagadapter (55) aufweist. Zudem kann der Schaft (5) 45 mit einer Schulterabflachung (59) versehen sein.
13. Hüftgelenk-Endoprothesensystem nach einem der Ansprüche 9 bis 12, 50 dadurch gekennzeichnet, daß der Schaft (5) eine zentrale Bohrung (56) aufweist. Über diese Bohrung kann intraoperativ der Schaft (5) mit Hilfe eines Führungsstabes (8) ausgerichtet werden. Weiterhin kann der Führungsstab (8) einen Flansch (81) und/oder eine Sperrenverbindung (82) aufweisen. Außerdem kann der Führungsstab (8) ebenfalls mit einer zentralen Bohrung (83) sowie zusätzlichen Entlüftungslöchern (84) versehen sein. Analog dazu kann die Markraumsperre (9) mit entsprechender Führungsstabverbindung (91) ausgebildet sein.
14. Hüftgelenk-Endoprothesensystem nach einem der Ansprüche 1 bis 13, dadurch gekennzeichnet, daß die Parameter Pfannen-, Inlay-, Kopf-, Hals- und Schaftdurchmesser, Halslänge (Ih) und Schaftlänge (I3), Verjüngungswinkel (π) und (p) sowie CCD- Winkel (ζ) und CCD- Korrekturwinkel (ε) in verschiedenen Größen zur besseren Anpassung an die anatomischen Gegebenheiten ausgebildet werden können.
