Abstract

A kit 1 arranged for use in short-stem hip replacements comprising a rod 2 arranged for use as an intramedullary guide and a short-stem femoral prosthesis 3. The prosthesis is formed with a hole 3d passing through it wherein the hole is sized and shaped to allow at least part of the rod 2 to completely pass through. The rod 2 is arranged to accurately position the prosthesis relative to the femoral canal. The rod may be removed through a hole in the stem of the prosthesis. The prosthesis may comprise a femoral head having a securable pin 4c which is arranged to fit inside the hole 3d. The kit may also comprise a broach 6 and/or a reamer 7 which are arranged to pass over the rod in use.

Description

INTRAMEDULLARY ROD
This application relates to a rod arranged for use in short-stem hip replacements as an intramedullary guide and a kit for use in short-stem hip replacements allowing precise placement of a short-stem hip replacement.
There exists in the field of short-stem hip replacements the problem of accurately locating the short-stem hip replacement on the femur. At present, short-stem hip replacements will toggle if they are inaccurately positioned on the femur and therefore precision is vital to ensure the comfort of the patient and durability of the hip replacement. Long-stem hip replacements are often easier to locate on the femur but can leave the patient with stiffness in the joint. Accordingly, there is the need for apparatus that can accurately establish the centreline of the femoral canal when carrying out short-stem hip replacements.
In a first aspect, the present invention provides a kit arranged for use in short-stem hip replacements comprising a rod arranged for use as an intramedullary guide and a short-stem femoral prosthesis, wherein the prosthesis is formed with a hole passing through the prosthesis, wherein the hole is sized and shaped to allow at least part of the rod to completely pass through the hole, and wherein the rod is arranged to accurately position the prosthesis relative to the femoral canal.
Preferably the kit further comprises a prosthetic femoral head arranged to attach to the short-stem femoral prosthesis.
Preferably the kit further comprises a prosthetic acetabulum arranged to engage with the prosthetic femoral head.
Preferably the rod is cylindrical and comprises a diameter of from 1 to 15 mm, more preferably from 5 to 12 mm, even more preferably from 6 to 11 mm, most preferably from 7 to 9 mm. The diameter of the rod is preferably slightly smaller than the diameter of the femoral canal in question. Accordingly, when the rod is inserted into the femoral canal it will eventually contact the naturally curved inner wall of the femur and jam in place.
The hole in the short-stem femoral prosthesis is preferably cylindrical in shape. Preferably the hole comprises a diameter that is slightly larger than the diameter of the rod.
Preferably the rod is removable through the hole in the short-stem femoral prosthesis.
Preferably the rod comprises a length of from 20 to 90 cm, more preferably from 25 to 70 cm, even more preferably from 30 to 50 cm, most preferably from 35 to 45 cm.
The rod may preferably be rigid. The rod may be manufactured from one or more metals, one or more plastics or a combination of any one of the above.
The short-stem femoral prosthesis may preferably be manufactured from one or more metals, one or more plastics or a combination of any of the above.
The short-stem femoral prosthesis is arranged to slot into a femoral canal.
Preferably the short-stem femoral prosthesis comprises a first end and a second end wherein the prosthesis tapers from the first end to the second end and the second end is arranged to slot into a femoral canal.
Preferably the short-stem femoral prosthesis has a length of from 10 to 70 cm, more preferably from 15 to 50 cm, even more preferably from 20 to cm, most preferably from 25 to 35 cm.
Preferably the hole of the short-stem femoral prosthesis comprises a diameter of from 3 to 17 mm, more preferably from 7 to 14 mm, even more preferably from 8 to 13 mm, most preferably from 9 to 11 mm.
Preferably the prosthetic femoral head comprises a securable pin arranged to fit inside the hole of the short-stem femoral prosthesis. The securable pin of the prosthetic femoral head may be secured inside the hole of the short-stem femoral prosthesis by any suitable means such as through tightness of fit, a locking system, using cement or a combination of the above. Alternatively, the securable pin and the hole of the short-stem femoral prosthesis may be correspondingly screw-threaded, enabling the securable pin to be screwed into the bole.
Preferably the securable pin of the prosthetic femoral head comprises a diameter of from 1 to 15 mm, more preferably from 5 to 12 mm, even more preferably from 6 to 11 mm, most preferably from 7 to 9 mm.
The prosthetic femoral head may be manufactured from one or more metals, one or more plastics, one or more ceramics, or a combination of any of the above.
The kit may further comprise one or more broach comprising a hole wherein the hole is arranged to allow at least part of the rod to completely pass through the hole. Preferably a plurality of broaches are present.
When the kit further comprises a plurality of broaches, preferably they are of different sizes. This enables successively larger broaches to be used until the desired size and shape of the hole for the implant is made in the femur.
The hole of the one or more broach is preferably cylindrical in shape.
Preferably the hole comprises a diameter that is slightly larger than the diameter of the rod.
Preferably the hole of the one or more broach comprises a diameter of from 3 to 17 mm, more preferably from 7 to 14 mm, even more preferably from 8 to 13 mm, most preferably from 9 to 11 mm.
The one or more broach may be manufactured from one or more metals.
The kit may further comprise one or more reamer comprising a hole wherein the hole is arranged to allow at least part of the rod to completely pass through the hole. The hole is preferably cylindrical in shape.
Preferably the hole comprises a diameter that is slightly larger than the diameter of the rod.
Preferably the hole of the one or more reamer comprises a diameter of from 3 to 17 mm, more preferably from 7 to 14 mm, even more preferably from 8 to 13 mm, most preferably from 9 to 11 mm.
The one or more reamer may be manufactured from one or more metals.
In use, a femoral head is separated from an acetabulum of a patient. The femoral head and the associated femoral neck are then removed from the femur (osteotomy), thereby exposing the femoral canal. Next the cartilage is removed from the acetabulum which is then shaped to accept the prosthetic acetabulum, before insertion of the prosthetic acetabulum into the modified acetabulum. The rod is then inserted into the femoral canal until it contacts the inner wall of the femur, which serves to lock the rod in place by virtue of the natural curve of the inner wall of the femur. The femur is proximally broached and/or reamed to accept the short-stem femoral prosthesis. This is carried out using broaches and/or reamers that comprise a hole arranged to allow at least part of the rod to completely pass through the hole. By passing the broaches and/or reamers over the rod the broaches and/or reamers are accurately positioned relative to the femoral canal during broaching and/or reaming respectively. Once the shaping of the bone is complete, any broaches and/or reamers are removed. The short-stem femoral prosthesis is then passed over the rod and accurately positioned on the femur relative to the femoral canal. Next the rod is removed and the prosthetic femoral head is attached to the short-stem femoral prosthesis in the correct orientation with respect to the centreline of the femoral canal. Finally the prosthetic femoral head is engaged with the prosthetic acetabulum.
In a second aspect, the present invention provides a method of carrying out short-stem hip replacements using a kit in accordance with the first aspect comprising (a) exposing a femoral canal, (b) inserting the rod into the femoral canal until it contacts the inner wall of the femoral canal, (c) broaching and/or reaming the femur to accept the short-stem femoral prosthesis using one or more broach and/or reamer respectively, wherein the one or more broach and/or reamer comprises a hole arranged to allow at least part of the rod to completely pass through the hole, (d) accurately positioning the short-stem femoral prosthesis on the femur relative to the femoral canal by passing over the rod, (e) removing the rod.
Preferably the method further comprises, following step (a), separating a femoral head from an acetabulum of a patient. The method may further comprise removing cartilage from and shaping the acetabulum to accept a prosthetic acetabulum. Additionally the method may comprise inserting the prosthetic acetabulum into the modified acetabulum.
Step (b) may comprise removing the femoral head and neck from a femur.
The method may further comprise attaching a prosthetic femoral head to the short-stem femoral prosthesis. This step may further comprise the use of cement to secure the prosthetic femoral head to the short-stem femoral prosthesis. The method may further comprise engaging the prosthetic femoral head with the prosthetic acetabulurn.
Step (e) may further comprise the use of cement and/or one or more pin to secure the short-stem femoral prosthesis on the femur.
A specific embodiment of the present invention will now be described, by means of example only, with reference to the drawings, in which: Figure 1 shows a perspective view of a kit according to the present invention; and Figure 2 shows a part cross-sectional view of a kit according to the present invention in use.
Figure 1 shows a kit 1 suitable for use in short-stem hip replacements.
The kit 1 comprises a rod 2. The rod 2 is 40 cm in length and of a diameter that is sized to enable the rod to fit inside the femoral canal in question. In general, femoral canals are between 8 and 12 mm in diameter.
The kit 1 also comprises a short-stem femoral prosthesis 3 with a first end 3a and a second end 3b wherein the prosthesis 3 tapers from the first end 3a to the second end 3b. The prosthesis 3 is 30 cm in length. The second end 3b of the prosthesis 3 has a diameter that is sized to slot into the femoral canal in question once the femur has been shaped appropriately. The first end 3a of the prosthesis 3 is shaped to match the opening in the appropriately shaped femur. In Figure 1 the first end 3a of the prosthesis 3 has a substantially oval shaped cross-section, whilst the second end 3b has a substantially circular cross-section.
The first end 3a of the prosthesis 3 has a substantially flat surface 3c which is angled at around 45° to a hole 3d which follows a straight path from the first end 3a to the second end 3b of the prosthesis 3, as shown in relief in Figure 1. The angle of the surface 3c with respect to the hole 3d makes it easier to locate the other prosthetic components. The hole 3d of the prosthesis 3 is of a diameter that is sized to enable the rod 2 to pass through the hole 3d.
The kit 1 further comprises a prosthetic femoral head 4 having a head section 4a, a neck 4b and a securable pin 4c. The head section 4a and neck 4b resemble a real femoral head and neck, sized accordingly in relation to the femur in question. The neck 4b has a first end 4d and a second end 4e, whilst the securable pin 4c has a first end 4f and a second end 4g. The first end 4d of the neck 4b is attached to the head section 4a, whilst the second end 4e of the neck 4b is attached to the first end 4f of the securable pin 4c. The second end 4g of the securable pin 4c is arranged to slot inside the hole 3d of the short-stem femoral prosthesis 3.
Once inside the hole 3d, the securable pin 4c can be secured using e.g. a locking system. The second end 4e of the neck 4b and the first end 4f of the securable pin 4c are attached at an angle of around 135° in order that the second end 4e of the neck 4b corresponds with the angle of the surface 3c at the first end 3a of the prosthesis 3.
The kit 1 further comprises a prosthetic acetabulum 5 which resembles a hollow hemisphere sized to engage with the head section 4a of the prosthetic femoral head 4.
The kit 1 further comprises a broach 6 with a first end 6a, a second end 6b, a hole 6c and an outer surface 6d. The hole 6c is arranged to allow the rod 2 to completely pass through the hole 6c. The outer surface 6d has a rough texture suitable for broaching the femur. The broach 6 tapers from the first end 6a to the second end 6b and is of similar dimensions to the short-stem femoral prosthesis 3. The second end 6b is arranged to pass over the rod 2 and enter a femoral canal in use.
The kit 1 further comprises a reamer 7 with a first end 7a, a second end 7b, a cylindrical portion 7c and a cylindrical cutting portion 7d. The cylindrical portion 7c has a hole 7e which is arranged to allow the rod 2 to completely pass through the hole 7e. The cutting portion 7d has a grater-like surface suitable for reaming a femur. The reamer 7 is arranged to pass over the rod 2 via the second end 7b in use.
Figure 2 shows a rod 2 and a short-stem fernoral prosthesis 3 in use inside a femur 8. The rod 2 was inserted into the femoral canal 8a and locked against the inner wall 8b. Next, the second end 3b of the prosthesis 3 was passed over the rod 2 and inserted into the femoral canal Ba until the correct position was established. Following this stage, the rod 2 can be removed and a prosthetic femoral head 4 can be attached to the short-stem femoral prosthesis 3.
The prosthesis 3 shown in figure 2 may be replaced with a broach 6 or a reamer 7 according to figure 1 to illustrate the use of a broach 6 or a reamer 7 in combination with the rod 2.

Claims (11)

  1. CLAIMS1. A kit arranged for use in short-stem hip replacements comprising a rod arranged for use as an intramedullary guide and a short-stem femoral prosthesis, wherein the prosthesis is formed with a hole passing through the prosthesis, wherein the hole is sized and shaped to allow at least part of the rod to completely pass through the hole, and wherein the rod is arranged to accurately position the prosthesis relative to the fernoral canal.
  2. 2. The kit of claim 1 further comprising a prosthetic femoral head arranged to attach to the short-stem fernoral prosthesis.
  3. 3. The kit of claim 1 or claim 2 further comprising a prosthetic acetabulum arranged to engage with the prosthetic femoral head.
  4. 4. The kit of any preceding claim wherein the rod is cylindrical and comprises a diameter of from 1 to 15 mm.
  5. 5. The kit of any preceding claim wherein the rod is removable through the hole in the short-stern femoral prosthesis.
  6. 6. The kit of any preceding claim wherein the short-stem femoral * * prosthesis comprises a first end and a second end, wherein the prosthesis tapers from the first end to the second end and the second end is arranged S.S to slot into a femoral canal. ** * .
  7. 7. The kit of any preceding claim wherein the short-stem femoral prosthesis has a length of from 10 to 70 cm. * .** * S S... *5 * *S
  8. 8. The kit of claim 2 or any of claims 3 to 7, as they depend from claim 2, wherein the prosthetic femoral head comprises a securable pin arranged to fit inside the hole of the short-stem fernoral prosthesis.
  9. 9. The kit of any preceding claim wherein the kit further comprises one or more broaches each comprising a hole wherein the hole is arranged to allow at least part of the rod to completely pass through the hole.
  10. 10. The kit of any preceding claim wherein the kit further comprises one or more reamers each comprising a hole wherein the hole is arranged to allow at least part of the rod to completely pass through the hole.
