Description
薬液注入ポート 技術分野
本発明は、 進行癌患者の化学治療における抗癌剤の動脈内注入、 栄養補給のた めの静脈注入、 鎮痛のための髄硬膜等への鎮痛剤の投与、 糖尿疾患者へのインシ ュリン投与等の如く、 患者の体明内の特定箇所に長期間継続して、 定期的に薬液を 注入するための、 皮下埋め込み可能な薬液注入システムに関するものである。 背景技術
切除不能な悪性腫瘍に対しては、 抗癌剤投書与による化学療法が行われているが、 全身投与法では副作用を生じるため、 選択的投与法としてカテーテル先端を腫瘍 局所へ通じる動脈上流に留置し、 端末を抗癌剤注入口として体外に露出させたま まにする力、、 或は抗癌剤投与の都度カテーテルを再揷入していた (動中療法) 。 また、 高カロリー $命液の持続注入の場合にも、 カテーテル先端を中心静脈内に留 置し、 端末を輸液注入口として体外に露出させていた (I V H)
このためカテーテルを揷入した皮膚創部からの感染の防止は懸案課題である力、 一方皮膚創部を常に清潔に保つことは実生活上は極めて困難であり、 かつ患者の 自由な行動や入浴等にも制約があり、 社会復帰に大きな妨げとなっていた。
このような問題の解決手段として、 体内に長期間留置した状態で使用可能な皮 下埋込型の力テーテル組立体が開発されている。 この力テ一テル組立体とは薬液 を貯留するための内部空間と、 この内部空間に連通する弾性体を装着した薬液注 入口およびコネクターが付設された薬液流出路を有するポート本体と、 薬液注入 用のルーメンが形成されたカテーテルとを、 前記流出路にル一メンが連通するよ うに前記コネクタ一にカテーテルを持続し、 更にカテーテルの固定及びキンク防 止のための保護ロックをポート本体に嵌合させたものである。
本皮下埋込型カテーテル組立体は、 カテーテルを目的の血管内等まで揷入し、 薬液注入ポートを皮下組織に固定した状態で留置される。 そして、 薬液を注入す る際は、 皮膚の上から触診により薬液注入ポートの薬液注入口を確認し、 薬液注
入口の弾性体を注射針にて穿剌することにより薬液をポ一ト本体内に送り込み、 カテーテルを介して目的の血管内等へ薬液を注入する。
しかし、 これらの従来品はポート本体外周にコネクタ一が付設しており、 ネジ 嵌合等により持続固定される保護ロックは、 コネクター先端によるカテーテルの キンクを防ぐためコネクタ一先端よりも更に突出することになる。 従って本皮下 埋込型カテーテル組立体を皮下に埋め込む場合はポ一ト本体外周分の埋め込みス ペースでは足りず、 カテーテルの鉛直線上に保護口ックを含めたスペース分の深 い切開を施す必要があり、 患者の負担が大きいだけでなく、 術者は薬液注入口を 確認しにくく、 或はポート本体と保護ロックを含めた直線が長い (ハードな部分 が長い) ほど、 患者の動きに対して追随できずにカテーテルキンク等の種々の事 故力起きる可能性力《大きかった。
一方、 薬液注入ポートを埋め込む部位としては主に胸部、 腹部、 大腿部が挙げ られるが近年は動中療法、 I V H共に鎖骨下の動 ·静脈を利用することが多く、 従って胸部に埋め込まれることが多い。
ところで従来の薬液注入ポ一トを側面から見ると、 上面は凸形状で底面が平面 である。 図 7はこれらの従来の薬液注入ポー卜を皮下に埋め込んだ際の断面図で あるが皮膚がかなり隆起する形となる。 これは前記胸部等の皮下脂肪の薄 ヽ部位 に埋め込む場合に侵襲が大きく、 特に痩せ型の患者に埋め込まれた場合に皮膚の 頂部が衣服等と擦れ、 発赤や炎症を弓 Iき起こす原因となっていた。
本発明の開示
本発明は、 従来このような問題点を解消することを目的とするもので、 保護口 ックを含めた外周が、 ポ一ト本体の外周にほぼ一致するため薬液注入ポ一トの埋 め込みスペースとしてポート本体の外周分のみの切開を行うだけで済み、 術者が 容易に埋め込み可能で、 患者にとって低侵襲であり、 更にカテーテルキンク等の 種々の事故が起きにくい、 より安全な薬液注入ポートを提供するものであり、 又、 側面から見たとき形状力上下に膨らんだ凸レンズ形状をなしているため、 ポート の上下に位置する体組織は均一に膨らみ、 上側の皮膚だけに無理なテンションを かけることなく皮下に埋め込むことができ患者にとって低侵襲な薬液注入ポート を提供するものである。
即ち、 本発明は、 薬液を貯留するための内部空間と、 この内部空間に連通する 薬液注入口と、 この薬液注入口を封止するセプタムと、 前記内部空間に連通する 薬液流出路とを有し、 該薬液流出路先にコネクタ一を備え、 更にカテーテル接続 の際の保護口ックを組み合わせた薬液注入ポートであって、 前記薬液流出路方向 のみポート本体が凹部をなし、 該凹部内にコネクターが設置されており、 前記保 護口ックをポート本体の凹部に嵌合させたときの外観がポート本体外周とほぼ一 体化する形状であることを特徴とし、 更には前記ポートと保護口ックの嵌合面が 互いに隙間なく嵌合する凹凸面であることを特徴とする薬液注入ポートである。 更に本発明は、 薬液を貯留するための内部空間と、 この内部空間に連通する薬 液注入口と、 この薬液注入口を封止するセプタムと、 前記内部空間に連通する薬 液流出路とを有し、 該薬液流出路先にコネクタ一を備え、 更にカテーテル接続の 際の保護ロックを組み合わせた薬液注入ポ一トであって、 側面から見たとき形状 力上下に膨らんでいることを特徴とし、 更には側面からみたとき厚みに対して中 心から上下 3 0 %の範囲に最大外径部が位置していることを特徴とする薬液注人 ポートである。

発明を実施するための最良の形態
以下、 図によって本発明をさらに詳細に説明する力 本発明はこれらに限定さ れるものではない。
図 1、 2の如く本発明の薬液注入ポートはポ一ト本体 1と保護ロック 1 1から なり、 ポート本体 1は薬液を貯留するための内部空間 4を有し、 この上部に薬液 注入口を封止するセプタム 3を装着した薬液注入口 2を有し、 更に一半径方向に 薬液流出路 5が設けてありその方向のポート本体 1は外周に対して凹部 7をなし、 凹部 7の中央にコネクタ一 6が付設されている。 すなわちポート本体 1の外観は 円盤形状の一部分が欠けた形状である。 なお薬液流出路 5は必要により薬液を流 出させる力 逆流を防ぐための一方弁を設けることも好ましい実施例の一つであ る。
ポート本体 1の材質としては、 生体適合性の優れていることが必要で、 例えば、 ポリウレタン樹脂、 ポリエーテルスルフォン樹脂、 シリコーンゴム等の樹脂や、 ステンレス、 チタニウム等の金属、 或はこれらの組合せでも何ら構わない。 また、 薬液注入口を封止するセプタム 3の材質としては、 弾性体が好ましく、 特に自己 封止性を有する弾性状物質が好ましく、 例えばシリコーン、 イソプレーン、 ラテ ックス等のゴム力く考えられる力く軟質の塩ィ匕ビニル樹脂ゃポリゥレタン樹脂でも針 剌し耐久性および生体適合性に優れれば使用可能である。 更にコネクター 6の材 質としては、 強度的にステンレスやチタニウム等の金属が望ましいが限定される ものではない。 ポート本体 1は以上の構成部材をィンサート成形や接着および溶 着等により組み立てられる。
次に保護口ック 1 1について説明すると外観は前記ポ一ト本体 1の円盤形状の 欠けた部分であり、 材質としては前記ポート本体 1と同様である力、 後端部のポ ―ト本体 1との嵌合部 1 3とカテーテルの圧着固定部 1 4は硬質材料が望ましく、 先端部のカテーテルと接触する部分はカテーテルの動きに追随しキンク等を防ぐ 目的でカテーテル接続部 (弹性部) 1 2が設けられていること力望ましい。
従ってポート本体 1と保護ロック 1 1を嵌合させたときの外観は保護ロック 1 1部分の突出のない円盤形状をなしている。 なお、 ポート本体 1と保護ロック 1 1の嵌合部はお互いにアンダーカツト等により嵌合できる形状がより安定し望ま しいが特に限定されるものではない。
次に本発明による薬液注入ポー卜の組立方法について図 1を用いて説明する。 予め保護ロック 1 1に通したカテーテル 2 1をポート本体 1のコネクター 6に揷 入接続し、 更に保護ロック 1 1をスライドさせポート本体 1に嵌合固定させる。 このようにして組み立てられた本実施例の薬液注入ポートは長さ Lと幅 Wがほぼ 等しく、 図 3のように従来の薬液注入ポートでは幅 Wに対して長さ Lが大きくな つし 'いる o
又、 図 4、 5の如く本発明の薬液注入ポートはポ一ト本体 1と保護ロック 1 1 力、らなり、 ポート本体 1は薬液を貯留するための内部空間 4を有し、 この上部に 薬液注入口を封止するセプタム 3を装着した薬液注入口 2を有し、 更に一半径方 向に薬液流出路 5が設けてありその先にコネクター 6が付設されていて、 カテ一 テル 2 1を接続後に固定するための保護ロック 1 1がネジゃアンダーカツ トを利 用して嵌合されている。 なお薬液流出路 5は必要により薬液を流出させるカ^ 逆 流を防ぐための一方弁を設けることも好ましい実施例の一つである。
ポ一ト本体 1の形状は側面から見ると上下に膨らんだ形状をなしており、 厚み に対して中心から上下 3 0 %の範囲に最大外径部が位置しているの力《好ましい。 最大外径部が中心に一致することが患者に対して最も侵襲が小さくなるが皮膚の 隆起が低すぎ、 薬液注入ポートの埋設位置力分かりにくい場合があるため、 望ま しくは中心から下向きに 5〜 1 5 %の範囲に最大外径部が位置するのが良い。 ポート本体 1、 薬液注入口を封止するセプタム 3及びコネクタ一 6の材質とし ては、 前述の通りである。 ポート本体 1は以上の構成部材をィンサート成形や接 着および溶着等により組み立てられる。
次に保護ロック 1 1について説明すると形状は特に限定しないが、 前記ポート 本体 1と嵌合させ一体ィ匕させたとき嵩張らないこと力く望ましく、 材質としては前 記ポ一ト本体 1と同様である力 後端部のポ一ト本体 1との嵌合させカテーテル 1を圧着固定させる後端部 1 5は硬質材料力望ましく、 先端部.1 6の力テーテ ルと接触する部分はカテーテル 2 1の動きに追随しキンク等を防ぐ目的で弾性体 が設けられていること力く望ましい。
次に本発明による薬液注入ポ一卜の組立方法について図 4を用いて説明する。 予め保護ロック 1 1に通したカテーテル 2 1をポート本体 1のコネクター 6に揷
入接続し、 更に保護ロック 1 1をスライドさせポート本体 1に嵌合固定させる。 組み立てられた薬液注入ポ一トは図 6のように皮下脂肪層 3 1と筋膜層 3 2の間 に埋め込まれ、 皮膚の上から触診により薬液注入ポ一卜の薬液注入口 2を確認し、 薬液注入口 2の薬液注入口を封止するセプタム 3を注射針にて穿刺することによ り薬液をポート本体内に送り込み、 カテーテル 2 1を介して目的の血管内等へ薬 液を注入する。
又、 図 8に示す本発明の薬液注入ポートは、 図 1の如く薬液注入ポートに対し、 更に形状が側面から見ると上下に膨らんだ形状をなしている特徵をも有するもの でめる。
産業上の利用可能性
以上に述べた如く、 本発明による薬液注入ポー卜は埋め込みのための切開スぺ ースが小さく (浅く) 済むため、 術者が容易に埋め込むことができ、 患者にとつ て低侵襲であり、 更にカテ一テルキンク等の種々の事故力起きにくく、 より安全 に選択的薬液注入が行える用具として極めて有用である。
又、 本発明による薬液注入ポートは、 患者の皮膚に無理なテンションをかける ことなく皮下に埋め込むことができ、 より低侵襲で安全に選択的薬液注入が行え る用具として極めて有用である。
Claims
1. 薬液を貯留するための内部空間と、 この内部空間に連通する薬液注入口と、 この薬液注入口を封止するセプタムと、 前記内部空間に連通する薬液流出路とを 有し、 該薬液流出路先にコネクターを備え、 更にカテーテル接続の際の保護ロッ クを組み合わせた薬液注入ポー卜であって、 前記薬液流出路方向のみポ一ト本体 が凹部をなし、 該凹部内に前記コネクタ一が設置されており、 前記保護ロックを ポート本体の凹部に嵌合させたときの外観がポ一ト本体外周とほぼ一体ィ匕する形 状であることを特徴とする薬液注入ポート。
2. ポ一トと保護口ックの嵌合面力互いに隙間なく嵌合する凹凸面である請求 項 1記載の薬液注入ポ一ト。
3. 保護ロックの先端側にカテーテル接触部を備え、 該カテーテル接触部が弾 性体である請求項 1記載の薬液注入ポ一ト。
4. 側面から見たとき形状が上下に膨らんでいることを特徴とする請求項 1記 載の薬液注入ポート。
5. 薬液を貯留するための内部空間と、 この内部空間に連通する薬液注入口と、 この薬液注入口を封止するセプタムと、 前記内部空間に連通する薬液流出路とを 有し、 該薬液流出路先にコネクタ一を備え、 更にカテーテル接続の際の保護ロッ クを組み合わせた薬液注入ポートであって、 側面から見たとき形状力上下に膨ら んでいることを特徴とする薬液注入ポート。
6. 形状が凸レンズ型である請求項 5記載の薬液注入ポート。
7. 側面からみたとき厚みに対して中心から上下 3 0 %の範囲に最大外径部が 位置している請求項 5又は 6記載の薬液注入ポート。
