1 Claim.
The present invention relates to improvement in a form le and guide, being particularly adapted to legal forms for facilitating their use.
One of the objects of the present invention resides in a form file of loose leaf construction having a perforated detachment edge.
Another object of the invention is to provide an improved form file and guide of loose leaf construction having a perforated detachable edge with an instruction sheet extension with an offset hinge line.
Another object of the present invention is to provide a form file guide bound in sections, each section being separated by an extension instruction sheet having its hinge line in extended offset relation with reference to the hinge line of the bound section.
Other objects and advantages of the present invention reside in the specific construction and arrangement of the component parts of my im# proved form file and guide.

Printed forms are extensively used byi the legal profession, realtors, and public agencies in the regular transaction of their business. These forms are usually in a loose leaf state with meager instructions for their use.
According to the present invention, the forni sheets are printed up with the lines on both sides of the sheet, and numerically indicated. Preferably the forms are punched to take a conventional loose leaf binder and to further facilitate their use are perforated along their inner vertical edge. Similar forms are grouped in sections and are preferably arranged in the order in which they are used in cases where they relate to certain procedural matters. Separting these sections are extension instruction sheets so arranged that their hinge line is adjacent to the outer vertical edge of the form sheet. The instructions for filling out the front and back of the form sheet are provided on the front and back of the instruction sheet and cross referenced. The arrangement of the extension instruction sheet is such that it may be both in juxtaposition with the front and back of the form sheet to which it relates to facilitate the use of the form.
In the illustrated. form of the invention the loose leaf form book comprises a front cover IIl, a back I2, sections III and I6, and extension instruction sheets I8 and 20. All of these elements of the book have punched holes along the edge 20 to receive binding members 22, which are preferably of an adjustable type to permit the addition of loose leaves in a manner wellknown in connection with loose leaf binding.
The section I4 Vconsists of a large number of similar loose leaf forms, designed for one purpose, while the section I5 is made up of a large number of similar loose leaf forms designed for another purpose. Between the sections I4 and IS and associated with section I4 is an extension instruction sheet I8 having a fold line at 24. The loose leaf form sheets of section I4 have a front portion 26, a rear portion 28, and a perforated line 3l, the latter being for the convenience of tearing the loose leaf forms from the le without removing the adjustable fasteners 22 of the file. It will be well understood that the form may be torn along the perforated line 3| to remove the same from the book, giving a neat appearing edge.
It should be observed from a consideration of Figs. 1 and 2 that the extension instruction sheets i8 and Ztl may serve to separate one section of 4 forms from another. The convenient manner in which the forms may be filled out according to the instruction sheet and removed from the form le will be readily apparent from a consideration of Figs. 1 and 2.
Having thus described my invention, what I desire to secure by Letters Patent and claim is:
A combination form file and instruction book, comprising a loose leaf bound construction made up of a plurality of sections of single sheet forms, the form sheets of different sections being different, said sheets each having a perforated tear line adjacent the loose leaf'bound edge, each of said sheets having consecutively numbered, printed and blanked lines on the face and back thereof, an extension instruction sheet separating said sections having a hinge line adjacent the outer edge of said form sheets, said instruction sheets each having a face and back with printed lines of instruction consecutively numbered for cross reference to correspondingly numbered lines on the face and back of the form of its associated section, each instruction sheet being hinged outwardly to present its face in juxtaposition to the face of the top form of its associated section for giving instructions for the filling in of said blank lines on the face of said top form, with said top form turned to present its back, said instruction sheet being hinged inwardly to overlay the face of the next form and to present the back of said instruction sheet in juxtaposition with the back of said turned top form for giving instructions for filling in the back of said top form, said top form being thereafter removable by tearing along said tear line.
