Abstract

The method for the surgical treatment of the postoperative ventral hernias using the polypropylene mesh comprises the access to the hernial defect, the dissection of the postoperative scar, the excessive skin and underlying fat, the isolation and the incision of the hernial sac, the partial or complete removal of the hernial sac, the separation of the adhesions between the peritoneal organs and the hernial sac, the separation of the greater omentum from the adhesions, the fixation of the omentum and the polypropylene explant, the closure of thehernial defect with the muscular and aponeurotic tissues, the draining of the operation site, and layer-by-layer sewing of the underlying tissues and the skin. When the continuous greater omentum is absent, the separate flaps with the vascular peduncles are isolated and sewed together generating the large flap, which is further fixed by the uninterrupted suture around the perimeter of the hernial defect to the peritoneum along the internal surface with 5 cm indent from the edge of the hernial defect. The generated fragment of the greater omentum is attaches separately from the mesh explant.
