Description of the invention
The invention relates to the field of medicine, namely to surgery, and it can be applied in practical health care as an improved scheme for the prevention of the formation of adhesions after operations on the organs of the abdominal cavity.
Known means of prevention of adhesion formation after various operations on the organs of the abdominal cavity, traditionally consist of sanitation of the abdominal cavity during surgical intervention with the help of physiological solution, aqueous solution of chlorhexidine or furatsilin and its drainage |11. 70 But, thanks to these measures, only mechanical cleaning of the abdominal cavity from peritoneal exudate takes place, causes a rather difficult course of the postoperative period, long intestinal paresis, and the development of early adhesive intestinal obstruction.
A significant drawback of the given means is that they do not contain any drugs that can specifically inhibit the formation of adhesions in the abdominal cavity. In the future, this can lead to 12 different complications of the postoperative period (adhesion or strangulation intestinal obstruction, often gangrene of the intestine), which, as practice shows, is observed in 3 - 1095 patients.
The closest to the proposed method is the prevention of adhesions in the abdominal cavity, which, in addition to the above-mentioned medicines, is supplemented with a solution of rheopoliglukin. This drug reduces the adhesive ability of peritoneal exudate and, thus, to some extent inhibits the formation of adhesions. |2). At the same time, reopoliglyukin is injected in the amount of 200 ml into the abdominal cavity during the operation.
«However, the effectiveness of the use of rheopoliglukin has been proven only when performing operations on the organs of the small pelvis, which is used in the practice of gynecological hospitals. The possibility of using reopoliglukin is limited in hospitals of a general surgical profile. In addition, as practice shows, reopolyglukin has a number of negative properties (suppresses blood clotting, is slowly eliminated from the abdominal cavity), in addition, 29 it is not a natural substance for the human body. "
The basis of the invention is the task of improving the method of preventing the formation of adhesions after operations on the organs of the abdominal cavity due to the use of a synthetic analog of the endogenous hormone somatostatin - the drug sandostatin with the preparation of a natural tissue proteolytic enzyme - fibrinolysin, which will allow to specifically inhibit the formation of adhesions between the organs of the abdominal cavity, and therefore - reduce the number of postoperative complications, shorten the treatment period, improve long-term results.
The task is solved by the fact that, according to the invention, during the surgical intervention, a solution of 100 μg of sandostatin and 40,000 units of fibrinolysin in 200 ml of 0.990 "sodium chloride is additionally injected into the abdominal cavity.
Somatostatin or okreotide is a drug, a synthetic analogue of natural somatostatin, which was first obtained by Zapdo2 (Switzerland) in 1982. Its half-life is 90 - 120 minutes, and the pharmacological action lasts up to 8 - 12 hours, which allows it to be used in patients 2 - 3 times a day.
Sandostatin has all the qualities inherent in the natural hormone somatostatin, namely, it suppresses the activity of proliferative metabolic processes in the connective tissue, the activation of which is the basis of Z 70 adhesion formation. Fibrinolysin, as a tissue proteolytic enzyme, contributes to the breakdown of loose bonds between organs |Z). with" The method is carried out as follows.
There were 80 patients aged 27 to 85 who were operated on for various surgical pathologies of the abdominal cavity (acute appendicitis, acute and chronic cholecystitis, perforated ulcer of the stomach or duodenum, etc.) under supervision. All patients underwent the corresponding i-th surgical intervention, which, as usual, ended with sanitation and drainage of the abdominal cavity. According to the invention, in order to prevent the formation of adhesions after these operations, a solution of 100 μg of sandostatin and 40,000 units of fibrinolysin in 2000 ml of 0.995 sodium chloride was additionally injected into the abdominal cavity during surgery. Thanks to such tactics, the postoperative period of such patients went smoothly,
Gee! 20 complications of any kind (adhesion obstruction, severe postoperative intestinal paresis, etc.) were not noted, the average length of stay of the patients in the hospital was 11 x 1 day.
