Abstract
Изобретение относитс к медицине, в частности к хирургии и предназначено дл профилактики и терапии послеоперационной динамической кишечной непроходимости. Цель - повышение эффективности блокады путем прочной фиксации катетера, уменьшени веро тности спайкообразовани в брюшной полости и обеспечени надежного дозируемого блока забрюшинных нервных образований, иннервируюш.их органы брюшной полости. Дл этого катетер устанавливают через прокол боковой стенки живота транскутанно-забрюшинно под контролем пальцев к зоне намеченного забрюшинного сплетени .The invention relates to medicine, in particular to surgery, and is intended for the prevention and treatment of postoperative dynamic intestinal obstruction. Способ осуществл ют следующим образом .The method is carried out as follows.
На операции после обычной обработки кожи производ т прокол кожи и переднего листка апоневроза в точке, расположенной на боковой стенке живота слева, по подмышечной линии, посредине рассто ни между II ребром и гребнем подвздошной кости. Нисход щую ободочную кишку пальцами левой руки хирург отт гивает кпереди и с помощью длинного изогнутого зажима проводит тонкий полихлорвиниловый катетер под контролем пальцев в забрюшинном пространстве, под задним листком париетальной брюшины, и устанавливает его в корне брыжейки тонкой кишки, между верх- .не-брыжеечной артерией и двенадцатиперст- но-тощекишечным переходом дл блокады верхне-брыжеечного сплетени (ВВС) или на 2-3 см ниже места дуоденоеюнального перехода - дл блокады нижне-брыжеечного сплетени (НБС), или проводит катетер под тело поджелудочной железы, к зоне перешейка ее дл блокады чревного сплетени и самой железы. Катетер фиксируют швом к коже. Явлени послеоперационного пареза кишечника не наблюдались , на 3 день отмечено восстановление перистальтики. Катетер удален. Вечером был самосто тельный стул.
