Description

吸入裝置及在吸入裝置中提供反饋控制的方 法
本發明大體上是關於吸入裝置,且特別是關於利用震動來協助粉末(如粉狀藥物)懸浮於吸入氣流(如吸入空氣)的吸入裝置。
某些呼吸道疾病可直接投予治療劑進行治療。這些藥劑最常以乾粉狀形式存在,最方便的使用方式則是經由鼻或口吸入這些粉狀材料。粉末形式可提高藥劑效用,因為藥物是確實用於預期產生作用的位置;故很微量的藥物效果常和利用其他手段給予大劑量的藥效一樣,因此可大幅減少不當的副作用和藥劑費用。或者,此形式的藥物可用來治療呼吸系統以外的疾病。當藥物用在相當大面積的肺臟時,其可快速吸收進入血液流動;故此給藥方式可代替注射、服用藥片或其他傳統手段。
製藥業認為,輸送到呼吸道的藥粒大小為1-5微米時有最佳的藥物生物利用度。要將藥粒製作成此尺寸,乾粉輸送系統需解決一些問題:
(1)在製造及儲存時,小粒子本身會產生靜電電荷。此將導致粒子結塊或聚集,使得粒子簇(cluster)的有效尺寸大於5微米。這些大粒子簇可能會進入肺臟深處而減少。因而造成可供病患吸收的包裝藥物百分比減低。
(2)需輸送給病患的速效藥用量可能為數十微克。以氣喘用藥albuterol為例,其通常使用25-50微克。目前製造設備在可接受的精確度內可有效輸送毫克範圍的藥量。故標準作法為混合速效藥與填料或增體劑,例如乳糖。添加劑亦能讓藥物”更易流動”。填料也稱為載體,因藥粒會透過靜電或化學鍵黏在這些填料粒子上。載體粒子遠比藥粒大。乾粉吸入器分離藥物與載體的能力是設計的要素。
(3)大於5微米的速效藥粒子會沉積在口中或喉嚨。此將引起另一不確定性,即該位置的藥物生物利用度和吸收效果不同於肺臟。乾粉吸入器需減少藥物沉積在這些位置,以降低藥物生物利用度的不確定性。
先前技術之乾粉吸入器(DPI)通常具有將藥物(速效藥和載體)引至高速氣流的裝置。高速氣流做為散開微米級粒子簇或分離藥粒與載體的主要機制。先前技術已知數種用來分配粉狀藥劑的吸入裝置。例如,美國專利證書號3,507,277、3,518,992、3,635,219、3,795,244和3,807,400揭露的吸入裝置具有刺穿含粉狀藥劑之膠囊的裝置,且吸入劑是從刺穿之膠囊釋出及進入使用者口中。其中數個專利揭露推進裝置,此有助於吸入時將粉末分配出膠囊,因而不需只仰賴吸入空氣來抽吸膠囊內的粉末。例如,美國專利證書號2,517,482提出之裝置具有含粉末之膠囊放在下腔室,吸入前,由使用者壓下刺針而刺穿。刺穿後,開始吸入,膠囊則被引到裝置的上腔室,在此朝各方向四處移動,以分配粉末通過穿孔及進入吸入氣流。美國專利證書號3,831,606披露之吸入裝置具有多個刺針、推進裝置和透過外在手動操作控制推進裝置的自給電源,故吸入時,推進裝置有助於將粉末分配至吸入氣流。
這些先前技術裝置呈現的數個問題和缺點可由本發明之吸入裝置解決。例如,先前技術裝置需要使用者賦予相當大的吸入效果才能將刺穿膠囊的粉末分配或吸引至吸入氣流。利用先前技術裝置,藉由吸入使粉末穿過膠囊穿孔的方式通常無法吸引膠囊內全部或大部分的粉末,以致浪費藥劑。又,先前技術裝置會產生不受控制的粉狀材料量或團塊吸入使用者口中,而非固定、分配良好的粉末吸入控制量。
已知先前技術包括用來協助吸入粉狀藥劑的裝置,其包括設有主要與次要空氣入口通道和出口通道的主體部分。次要入口通道圍住含粉狀藥劑之膠囊,出口通道形成伸出主體的口。膠囊刺穿結構乃設置於旋轉時形成一或多個孔洞於膠囊中,故當電機震動器震動膠囊時,膠囊可釋出粉狀藥物。Wilke等人提出之刺穿裝置包括三個放射配置且裝在圓錐形腔室的彈簧偏壓刺針。用手轉動腔室後,刺針同時朝內徑向移動而刺穿膠囊。進一步轉動腔室,可利用彈簧底座讓刺針縮回到原位而將刺針抽出膠囊。
電機震動器的最內端包括震動活塞桿,其伸進入口通道與出口通道的交叉點。機械螺線管蜂音器連接活塞桿,用以供給桿子震動能量。蜂音器由高能量電池提供動力且由外部按鈕開關啟動。一旦經由出口通道吸入並同時壓下開關啟動電機震動裝置,空氣被抽吸通過入口通道,通過次要入口通道的氣流則抬起膠囊抵著震動活塞桿。膠囊因此快速震動而流體化粉末及從穿孔分配粉末。此技術常用於製造分配通過送料斗的粉末,其震動送料斗以流體化粉末及移送粉末通過送料斗出口。膠囊的穿孔代表送料斗出口。通過入口通道的氣流有助於抽出膠囊內的粉末及經由出口通道將粉末送到使用者口中。電機震動裝置可設置垂直入口腔室,震動幅度和頻率可經改變來調整吸入器的分配特性。
故如上述,震動器的吸入器為電機裝置,設有由螺線管蜂音器驅動的桿子。電機裝置可為驅動凸輪的馬達。使用此吸入器的缺點在於,桿子有效震動膠囊所需的機械動作很大。因膠囊殼的彈性和藥物與膠囊的慣性所致,桿子的移動範圍通常需為約數百微米。
再者,螺線管蜂音器的操作頻率一般小於5仟赫(kHz)。此操作頻率易產生噪音,從病患的觀點看來並不適合用於乾粉吸入器。電化學致動器的另一缺點為需使用高能源,因此需要大量的電池,或者常需更換攜帶型單元的電池組。從病患的安全性和”方便使用”的方面考量皆不期有上述特徵。
與專利提及的其他吸入器相比,此吸入器主要用來減少留在膠囊的粉末。然上述裝置無法將粉末分散成小於6微米的粒子或團簇,以符合有效輸送藥劑至肺臟的需求;反而像先前技術之吸入器一樣,仍繼續仰賴氣流速度將噴入氣流中的粉末分散成適合輸送至肺臟的粒子大小。
在先前技術之另一吸入裝置中,液態藥劑經超音波裝置霧化,例如壓力元件。氣流(通常呈高速)或推進物接著將霧狀粒子輸送給病患。噴霧器霧化液態藥劑所需的能量很高,故此輸送藥物至肺臟的方式只適合桌上型單元。驅動偏壓元件及形成必要的機械位移所需的高電壓亦對裝置重量和尺寸的影響甚大。噴霧器操作原理顯然也不能應用到輸送粉狀藥物給肺臟的乾粉吸入器。
先前技術裝置具有一些缺點,故不適合用來輸送乾粉至肺臟。部分缺點為:
先前技術之吸入器性能與使用者產生的流率有關。流率太低無法完全分散粉末,因此會影響輸送至病患的劑量。
因缺乏一致的分散過程,故各劑間的藥物生物利用度不一致。
驅動電機型吸入器需要高能量,如此會增加裝置體積,故不利於攜帶使用。
又一先前技術裝置包括一吸入器,其利用震動促使粉末懸浮於氣體中,藉以克服上述和其他的先前技術缺點。更特別地,吸入器包括壓電震動器,用以震動粉末。控制器設置來控制供應震動器震動粉末的致動電力(即振幅及/或頻率),使得至少一部分的粉末懸浮於氣體中。控制器包括使用者啟動控制,供使用者選擇最適合目前用於吸入器之粉末懸浮於氣體中的震動頻率及/或振幅。控制器可預先校正使用者啟動控制,讓控制器調整供給震動器的致動電力,使之符合震動使用者啟動控制選擇之粉末類型所需的頻率及/或振幅,使至少一部分的粉末懸浮於氣體中。就待懸浮於氣體中的平均粉粒大小及/或預定之震動頻率和振幅而論,使用者啟動控制可包括選擇變化層次。震動頻率一般宜調整成至少為約12kHz,以將常用的粉狀藥物懸浮於氣體中。震動頻率和振幅可經調整以有效地讓待使用之特定粉狀藥物懸浮。
本發明提出改良先前技術之吸入器,包括如上述之壓電震動器(piezoelectric vibrators)。更特別地,申請人已發現過度位移恰與壓電換能器(piezoelectric transducers)的過早失效有關。故乾粉吸入裝置運作期間獲得換能器位移相關的反饋是有益的。壓電陶瓷具有機械力與電壓的雙向關係,而可同時做為驅動與反饋裝置。然就小巧、低成本的裝置而言,輔助反饋系統的體積太大又昂貴。本發明則可提供此反饋。
本發明之實施例提供乾粉吸入器的系統和方法。簡言之,一系統實施例的構造如下。系統包括震動機制。乾粉供應器操作耦接震動機制。電源連接震動機制。感測器連接震動機制。反饋控制器連接感測器和電源。反饋控制器依感測器提供的震動機制性能相關資訊,控制傳送給震動機制的功率。
本發明尚提出提供乾粉吸入器反饋控制的方法。在此,一方法實施例可總結成以下步驟:利用第一功率輸入,驅動震動機制達到趨近穩態;移除第一功率輸入,其中至少一部分的震動機制繼續震動;移除電壓輸入後,感應震動機制的震動;利用多個不同功率輸入來重複進行驅動、移除及感應之步驟;決定何種電壓輸入可產生最大感應震動;以及放置震動機制,藉以分散乾粉。
熟諳此技藝者在檢閱以下圖式和詳細說明後,將更清楚本發明之其他系統、方法、特徵和優點。說明書中提及的所有附加系統、方法、特徵和優點皆落在本發明之範圍內,本發明之保護範圍當視後附之申請專利範圍所界定者為準。
第1圖為根據本發明第一實施例之吸入器2的側視圖。如第1圖所示,空氣10或其他流體流入氣流通道12。透過病患吸入裝置2的呼吸動作可引發空氣10流動。空氣10從吸入器2的遠端14經由通道12流向吸入器2的近端46。吹口設於吸入器2的近端46供病患吸入之用。
震動機制28設置在鄰近吸入器2的第三開口16處。震動機制28可包括壓電元件、超音波換能器、或任何其他電子/機械震動機制,但不以此為限。容器20設置鄰近震動機制28。容器20和震動機制28至少夠接近讓震動機制28震動容器20。容器20可為泡殼膠囊,例如授予MicroDose Technologies公司之美國專利證書號7,318,434所述的泡殼膠囊,其一併附上供作參考。容器20容納待震動機制28分散的粉末50。吸入器2的結構可設計成在每次用完吸入器2後,丟棄及更換容器20。
吸入器2內含控制電路48。控制電路可設計成特定功能積體電路晶片及/或其他積體電路晶片。控制電路48可採用微處理器或單獨的電氣與電子組件形式,且可包括一或多個間接連接吸入器2的元件。控制電路48決定電源26供給震動機制28的功率。控制電路可控制電源26供給震動機制28的致動功率振幅及/或頻率,此將影響震動機制28震動的程度。震動機制28與電源26間的電性連接22提供致動功率,控制電路48則至少部分控制電性連接22。電性連接22包括電路裝置,用以將電源26供應的直流(DC)電源轉換成交流(AC)電源給震動機制28使用,電路裝置為電路設計領域的一般技藝人士所熟知。
震動機制28可包括壓電元件28,其由具高頻之材料組成,較佳為具超音波共振頻率(如約15-100MHz),且按特定頻率和振幅震動,此視施加的激電頻率及/或振幅而定。用來製造壓電元件的材料例子包括石英和多晶陶瓷材料(如鈦酸鋇和鋯鈦酸鉛)。有利地,以超音波頻率震動壓電元件可減少以較低頻率(即小於超音波)震動壓電元件伴隨的噪音。
第2圖為根據本發明第一實施例,用於第1圖吸入器之震動控制系統的方塊示意圖。如同熟諳此技藝者所理解,第1圖繪示之功能組件是針對本發明的可行實施例之一。在不脫離本發明之範圍內當可修改、更動及/或重新配置第1圖組件,且其他吸入器構造亦受益於在此所述之震動控制系統。
控制電路48包括啟動控制器70和控制子系統72。啟動控制器70包括開關機制,其依氣流感測器40提供的信號來啟動電源26供電給控制子系統72。氣流感測器40會限制震動機制28,使其只在有人呼吸吸入器2的近端46時才啟動。控制電路48上也可設置有肘節開關32,以確保電源26因周遭氣流而耗盡。換言之,當肘節開關32設定為「開」位置、及氣流感測器40提供啟動控制器70信號並指示氣流通道12發生吸入時,控制器70允許電源26供應控制子系統72致動功率。然當肘節開關32設定為「關」位置、或氣流感測器40提供給控制器70的信號指示氣流通道12未發生吸入時,啟動控制器70不許電源26供應系統72致動功率。
若啟動控制器70先讓電源26供應控制子系統72致動功率,則可使控制子系統72進入初始化狀態,促使提供預定致動功率頻率和振幅的可控制電路74產生控制信號。控制信號使泵電路80依據儲存於初始化記憶體82的儲存值,傳送最初預定的致動功率頻率和振幅。可控制電路74包括頻率掃描產生器76和頻率產生器78。可控制電路74產生的信號可用來指示泵電路80,讓泵電路80供應控制信號明定的致動功率給震動機制28。
較佳地,供給震動機制28的最初致動電力頻率和振幅乃預先校正來驅使震動機制28達穩態條件。如同熟諳此技藝者所理解,以趨近穩態驅動壓電元件90震動時,實質上可從震動機制28傳送最大的震動功率至容器20內的粉末50。此結果已發現可有效分散容器20內的粉末50及懸浮於使用者吸入的空氣中。然若容器20或粉末50放置在震動機制28上,則容器20的重量與體積和待分散之粉末50的重量、體積與特殊尺寸會改變震動機制28的震動特性,導致震動機制28以共振頻率以外的頻率震動。如此將造成震動機制28傳送至粉末50的震動能量減低,因而降低震動機制28將粉末50分散及懸浮於使用者吸入空氣的效率。
在控制電路48中,一旦達到穩態,即停止泵電路80的供應信號。震動機制28因其動量而繼續震動。若震動機制28包括壓電元件,則在停止泵電路80之供應信號後的最早幾個循環,因壓電作用,持續震動將感生電壓,此可由感測器88(如伏特計)測量。測得電壓恰與壓電元件90的位移呈正比關係。

頻率掃描產生器76和頻率產生器78掃描完供給震動機制28的功率頻率和振幅後,取樣保持反饋控制器84促使可控制電路74循環完產生局部最大值的功率頻率和振幅,及決定何種頻率和振幅可產生整個震動機制28的偵測最佳功率傳送特性。
操作時,以前述方法刺穿容器20及嚙合震動機制28的表面。肘節開關32設定為”開”位置時,使用者由近端46吸入空氣。氣流感測器40感應到空氣10的吸入,因此發信號通知啟動控制器70,進而供電給控制子系統72。控制子系統72接著調整供給震動機制28的致動功率振幅和頻率,直到能適當地將容器20內的粉末50分散及懸浮於氣流中。

Claims (10)

  1. 一種乾粉吸入器,其至少包含:一震動機制;一乾粉之一供應器,係可操作地耦接至該震動機制;一電源,該電源與該震動機制溝通;一感測器,該感測器與該震動機制溝通,其中該感測器係被定位以量測該震動機制的一震動;一反饋控制器,該反饋控制器與該感測器和該電源溝通,藉此該反饋控制器控制傳送至該震動機制的一功率;一記憶體,該記憶體與該反饋控制器溝通,藉此該記憶體儲存從該感測器傳送至該反饋控制器的至少一訊息,該訊息係與從該反饋控制器傳送至該電源的至少一訊息相對應;及一頻率掃描產生器,該頻率掃描產生器係連接於該電源與該震動機制之間,以用於控制傳送至該震動機制的一功率特性。
  2. 如申請專利範圍第1項所述之乾粉吸入器,其中該震動機制更包含一壓電換能器。
  3. 如申請專利範圍第1項所述之乾粉吸入器,其 中該感測器更包含一伏特計。
  4. 如申請專利範圍第1項所述之乾粉吸入器,其中該反饋控制器更包含一頻率掃描產生器,連接在該電源與該震動機制之間,用以控制傳送到該震動機制的一功率特性。
  5. 如申請專利範圍第1項所述之乾粉吸入器,更包含一尖峰功率偵測器,該尖峰功率偵測器與該感測器溝通。
  6. 如申請專利範圍第1項所述之乾粉吸入器,其特徵在於以下一或多個特徵結構:(a)其中該震動機制更包含一壓電換能器,且該感測器可偵測該壓電換能器的一電壓輸出;以及(b)其中該反饋控制器聯繫傳送到該震動機制的複數個功率形式與該感測器的複數個輸出,其中該反饋控制器可非必要地識別一傳送到該震動機制之該些功率形式的功率特性,且該功率特性與該感測器之該些輸出的一最大輸出有關。
  7. 如申請專利範圍第1項所述之乾粉吸入器,其中該乾粉吸入器係提供於一容器中,在該乾粉吸入 器使用後,該容器被更換。
  8. 一種在具有一震動機制之乾粉吸入器中提供反饋控制的方法,該方法包含以下步驟:感應一使用者何時吸入並啟動該震動機制,其中該震動機制係與一電源及一震動機制感測器溝通;提供一反饋控制器,該反饋控制器與該震動機制感測器及該電源溝通,藉此該反饋控制器控制了傳遞至該震動機制的功率;利用一第一功率輸入,驅動該震動機制達到一趨近穩態;移除該第一功率輸入,其中至少一部分的該震動機制繼續震動;移除一電壓輸入後,利用該震動機制感測器感應該震動機制的震動;利用複數個不同的功率輸入,重複進行驅動、移除及感應之多個步驟,其中該複數個不同的功率輸入係依據頻率而不同;在與該反饋控制器溝通的一記憶體中,儲存從該震動機制感測器傳送至該反饋控制器的至少一訊息,該訊息係與從該反饋控制器傳送至該電源的至少一訊息相對應;決定何種電壓輸入可產生一最大感應震動;以及 放置該震動機制,藉以分散一乾粉,其中感應該震動之該步驟進一步包括從一壓電換能器偵測一輸出電壓。
  9. 如申請專利範圍第8項所述之方法,其中該放置步驟係在該移除步驟之前進行。
  10. 如申請專利範圍第8項所述之方法,其特徵在於以下一或多個特徵結構:(a)其中該驅動該震動機制之步驟更包含驅使一壓電換能器震動;(b)其中該感應該震動機制之步驟更包含偵測該壓電換能器的一輸出電壓;(c)其中該些不同的功率輸入依一頻率變化;以及(d)更包含聯繫該些不同的功率輸入與複數個不同的感應震動。
