Description
本發明係關於骨骼固定裝置,包括固定元件,具有要固定於骨骼內之幹部,外表面呈球形截頭部之頭部;接納部,以接納該頭部;以及對該頭部之施壓元件,其中該幹部和該頭部係分開組件。
此種骨骼固定裝置見於美國專利第6,835,196 B2。固定元件的幹部具有圓筒形端部,接納於頭部之相對應腔孔,具有彈性屈服邊緣,對頭部施壓時,可夾持幹部之該端部。在另一具體例中,幹部的端部有陽螺紋,與接納該端部的頭部內之腔孔陰螺紋嚙合。以此骨骼固定裝置,可先旋入幹部內,必要時,調節幹部長度,再與含頭部之接納部連接。
US 6,074,391揭載一種骨骼固定裝置,包括骨釘,有螺紋幹部,球形截頭形成幹部自由端。骨骼固定裝置又包括接納件,內側有錐形環狀表面,向下輻輳。接納件內設有長孔籠,包容球形截頭,並有外部錐形表面,與接納件的錐形表面協合。對該籠施壓時,籠內長孔可夾持幹部之球形截頭。
WO 94/00066揭示一種接骨固定裝置,包含有錐形頭段的固設組件,設計來固定於骨骼內的毗鄰固定組件,以及球形截頭之有長孔夾持組件,有錐形腔孔可接納錐形頭段,在內側夾持有球形截頭腔孔之連接組件。夾持組件的外表面有球形截頭部。固設組件固定於連接組件內,是藉錐形頭段拉入夾持組件內達成,使夾持組件展開,壓緊於連接組件之球形截頭腔孔。
US 6,063,090揭載一種桿和腳釘之連接裝置,可將螺釘元件以球形頭部先旋入骨骼,然後把要與桿連接的扣頭套上球形頭部。要把螺釘元件相對於扣頭的角位固定,是利用錐形彈性夾頭夾持球形頭部。
本發明之一目的在於提供一種骨骼固定裝置,具有頭部和幹部呈分開組件,方便裝置之預組裝,當最後發生螺釘旋緊於桿時,可供增進固定。
此目的是以申請專利範圍第1項之骨骼固定裝置解決。進一步發展是列於申請專利範圍附屬項。
本發明骨骼固定元件之優點是,在手術之際和之前,可按照特殊情形之需要,將不同尺寸的幹部與分開頭部與接納部組合。
以本發明骨骼固定元件,可先把幹部固定於骨骼,然後把包含球形截頭的頭部之接納部安裝於幹部。由於幹部的端部錐形與頭部的協合凹部,可達成頭部相對於幹部預固定,即使頭部尚未鎖定於其最後多軸位置。
若幹部的端部和頭部的凹部之錐形角度,在約2°至10°的一定範圍,球形截頭之頭部和幹部間可達成自行鎖定連接,確保頭部不會脫落,或意外與幹部分開。
骨骼固定裝置又有優點是精巧,因不需外環或螺帽。
本發明其他特點和優點,由如下參照附圖所示具體例之說明,即可明白。
如第1圖至第3圖所示,骨骼固定裝置包括接納部1,呈實質上圓筒形,有第一端2,和在第一端相對面之第二端3。二端對縱軸線4垂直。設有與縱軸線4共軸的共軸腔孔5,從第一端2延伸至離第二端3預定距離處。在第二端3設有開口6,其直徑較腔孔5直徑為小,共軸腔孔5朝開口6斜縮。在圖示具體例中,斜縮成球形段7之形狀。
接納部1又有U形凹部8,從第一端2起,按第二端3方向,延伸至距該第二端3預定距離。利用U形凹部,形成二自由股部9,10,朝第一端2為止。相鄰於第一端2,接納部1包括在該股部9,10之陰螺紋11。
骨骼固定元件13包括有骨骼螺紋之幹部14,和頭部15,形成分開組件,在第1圖所示組合狀態,連接於幹部。頭部15有球形截頭外表面部16。頭部15的此部份半徑,使得頭部15接納於接納部1內時,頭部15之球形外表面16,受到第二端3與開口6相鄰球形段7的支持,或以開口邊緣支持,使頭部15的部份15a從開口6突出。對頭部15未旋壓時,頭部15樞動保持在球形段7內,類似球窩接頭。
再者,設有施壓元件17,具備圓筒形構造,其外徑僅稍小於腔孔5的內徑,容許施壓元件可引進接納部1之腔孔5內,並在軸向運動。在朝向第二端3的下側,施壓元件17包括球形凹部18,其半徑相當於頭部15的球形部16半徑。在相反側,施壓元件17有U形凹部19,橫向延伸至縱軸線4。選用此U形凹部的側向直徑,使要接納於接納部內之桿20,可插入凹部19內,並在其內側向導引。選擇球形凹部18之深度,使得在組裝狀態,即桿插入並壓緊於U形凹部之底部時,施壓元件17對頭部15施加壓力。
骨骼固定元件又包括內螺釘21,可旋入股部9,10之間,把桿20固定,經由施壓玵件17對頭部15施壓。陰螺紋11和內螺釘21之協合螺紋,可有任何已知的螺紋形狀。然而,平板螺紋或負角度螺紋的優點是,股部9,10不會發生張開,故不必使用外螺帽或外環。
茲詳述幹部14和頭部15。幹部14包括端部22,可接納於頭部15內。端部22形如截頭錐體,朝直徑ds的自由端23加寬。宜選擇幹部的錐形端部之錐角α,在幹部連接於頭部時,可達成自行鎖定連接。特別由第2和3圖可見,頭部15輪廓為切平的球體,在其面朝第一端2的末端包括腔孔24,與接納部的縱軸線4共軸,直徑比錐形端部22的自由端23最大直徑ds稍大。腔孔24容許導引貫穿錐形端部22之外端23,使錐形端部22完全插入時,從頭部15突出,如第1圖所示。頭部15在指向接納部第二端3之側,有凹部25形成空心截頭錐體,其維度是在幹部插入頭部15內時,可與幹部的錐形端部22維度相配。錐形凹部25的角度α'宜相當於端部22之角度α,或比角度α大。凹部25形成的下開口26之直徑db,比幹部的錐形端部22之自由端23最大直徑ds為小。
如第2和3圖所示,頭部15有複數縱向細縫27,對面向幹部的端部22之末端開口,在沿周方向等距配置。細縫27尺寸足以容許凹部25展開,至下開口26加寬到容許錐形端部22插入的程度,並在插入後,彈性夾持端部。
如第4圖所示,錐形端部的自由端23有切角28,以方便插入頭部15內。切角28可彎曲,與施壓元件17的凹部18曲率相配,錐形凹部25亦可延伸貫穿頭部15。
頭部15的凹部25錐角α宜相當於端部22之錐角,其選擇是當錐形端部22插入頭部的凹部25內時,可達成自行鎖定連接。一旦建立自行鎖定連接,即需額外力量從凹部25取出錐形端部,比錐形端部插入凹部25必要之力為大。達成自行鎖定效果之錐角以大約2°至10°之間為佳,以約4°更好。
在第一種操作模態中,選用適當幹部14,以其端部22連接至頭部15。利用切角28可方便錐形端部22通過下開口26引進入頭部15。藉插入端部22,把下開口26撐寬,錐形凹部25即因細縫27而張開,以便容許全錐形端部22引進。若選用角度α,在幹部引進入頭部內時,達成自行鎖定連接,則一旦插入頭部內,頭部即不會脫落或意外掉出。
然後,由幹部14和所安裝頭部15組成之骨骼固定元件,從第一端2引進到接納部1內,有螺紋之幹部14導經接納部之開口6,直至頭部15緊靠開口6的邊緣。然後,引進施壓元件17,把有螺紋幹部旋入骨骼內。再調節接納部1相對於固定元件13之角位,把桿20插入。再旋入內螺釘21,把桿固定。內螺釘21對桿20施壓,把施壓元件17壓下。施壓元件17形成幹部的端部22自由端23之支座。如此一來,頭部15壓緊球形區7或開口6邊緣,使開口6邊緣因細縫27壓縮頭部,夾持幹部之端部22。此外,施壓元件17壓住端部22之自由端23,因而把錐形端部22壓入頭部之錐形凹部25內,於是,頭部和幹部間達成牢固連接,同時頭部鎖定於其轉動位置。
上述骨骼固定裝置之組件,最好是由人體相容性材料製成,例如鈦。
在第二種操作模態中,先把有螺紋幹部14旋入骨骼或脊椎內。為此目的,幹部14在其自由端有已知螺絲起子結合之可能性,諸如六角形凹部30,像第1圖所示。接納部1再和鬆弛預組裝的頭部15及施壓元件17,一同施壓於突出骨骼外的幹部14之錐形端部22。為完成此事,必須要有一定的壓力把頭部15推上錐形端部,即頭部由於細縫而張開,並套合於錐形端部22。把頭部放到幹部上所需壓力,視錐角和/或細縫提供的彈力而定。在此狀態下,頭部仍然可在接納部內樞動,而幹部則被頭部夾持。其次,把桿20插入,調節接納部1相對於頭部15之角位。然後,內螺釘21旋入股部9,10之間,直至壓到桿20上,因而對頭部15施壓。
由於施壓元件對頭部15所施壓力,此有長孔頭部15即牢靠連接或夾持於幹部14,防止運動,同時,頭部15即鎖定於其轉動位置。
必要時,接納部1連同頭部15即可用工具在原位與幹部脫離,如第8a至8d圖及第9圖所示。首先,將內螺釘和桿拆除,如第8a圖所示。在此狀態,頭部15在接納部1內解鎖,但仍夾持幹部之端部22。其次,如第8b圖所示,使用工具100把頭部15推入到接納部1內,故不再從接納部的下開口6突出。
工具100包括縱棒101,在垂直方向伸出二支臂102,103。支臂之一固定在離棒101的自由端104一定距離,使棒之一段突出於支臂103上方,方便夾持。在圖示具體例中,工具使用中是指上支臂103。另一支臂102可滑動安裝於縱棒101,且可變化固定於他端106的末端位置和朝向上支臂103所需位置之間,故可變化支臂間之距離。至少下支臂為叉形,得以包圍幹部14。下支臂102位於棒的末端106時,支臂間的距離比接納部1的長度為大。
使用時,工具應用於接納部,下支臂102在其末端位置,如第8b圖所示。即移動下支臂102,壓緊接納部1之第二端3,接納部1因而相對頭部15移動,直至頭部15完全在接納部內,如第8c圖所示。在此狀態,頭部有擴張空間,故當工具連同接納部和頭部拉離幹部時,幹部即會被拉開,如第8d圖所示。
可以修改,例如下支臂可換成上支臂103在滑動。滑動支臂不需固定於棒,但可用手動壓緊接納部。
選擇端部22、頭部15和接納部1之維度,使克服端部22和頭部15間自行鎖定連接之摩擦力所需力量,在手術當中可以容忍。
第5a至5c圖表示幹部14之修改。在第15a圖內,幹部14的直徑d較錐形部22的最小直徑更小。在第5b圖內,幹部14的直徑d比錐形端部22的最小直徑更大。在第5c圖內,幹部包括鄰接尖端141的螺紋段140,和無骨骼螺紋之段142。在所有修改中,錐形端部22的形狀可接納在頭部15之凹部25內。
第6圖表示頭部修改之示意圖。頭部15'有截頭錐形之凹部25,一如第1至4圖所示具體例。然而,錐形腔孔24'的的直徑較幹部錐形端部的末端23直徑為小。因此,凹部25的上壁形成端部22之環狀支座28。腔孔24'直徑大到剛好可供旋入工具存取端部22。為使頭部15連同接納部1可在原位與幹部14分離,頭部具有結合機構,可與持有頭部之工具結合,故接納部1拉離幹部時,不會拉到碰到開口6。第7圖表示工具30之例,包含手柄,和從手柄延伸之棒32,具有螺紋端部33。在此情況,腔孔24'包括陰螺紋,與螺紋端部33結合。使用上,在除去內螺釘21和桿後,即釋除對頭部15之壓力,利用螺紋尖端33旋入螺紋腔孔24',以工具30持有頭部,同時以手動把接納部拉離幹部。如此一來,幹部的端部22被拉脫離頭部15。
本發明不限於上述具體例,可構思進一步修改。例如,本發明不限於具有螺紋幹部之固定元件,亦可應用於由鈎組成的固定元件。在此情況下,鈎的自由端具有錐形端部22。
此外,幹部14的形狀和直徑可以變化,骨骼螺紋之長度亦可變化。例如,第5c圖修改為幹部具備無螺紋段,亦可應用於第5a圖或第5b圖所示幹部。
可以選擇錐形端部22的錐角,和空心截頭錐體形之凹部25,使不具有自行鎖定效果。在此情況,必須選擇頭部15之細孔26數目和形狀,使頭部15可套合於錐形端部22上。
頭部15有若干細孔,亦可改為唯一細孔,按縱向延伸貫穿頭部之全壁。除第3圖所示細孔外,亦可有一連續細孔。可設額外細孔,朝下開口26相反端開啟。又,形成空心截頭錐體之凹部25亦可延伸貫穿全頭部15,因而省略腔孔24或24'。
在又一修改中,頭部係由具有彈性的材料形成,例如適用之塑膠材料。在此情況下,細孔可省略。
接納部亦可修改。球形段7半徑可較頭部半徑大,只要開口6小到夠使頭部15不會脫出即可。球形段7亦可改為有一段是其他形狀,例如錐形。
再者,施壓元件17可有不同形狀。施壓元件在縱向的長度可大到在桿插入時,由U形凹部19所發生的股部突出到桿上方。在此情況下,可經由施壓元件,以要旋入股部9,10中間的內螺帽,分開把頭部15固定,並以要旋入螺帽的內螺釘,分開將桿固定。
接納部可為骨骼板之一部份。在此情況,接納部不會有U形凹部,即可例如單獨由內螺釘發生對頭部15之壓力。

Claims (9)
- 一種骨骼固定裝置,包括:固定元件,包括要固定於骨骼或脊椎之幹部(14),和頭部(15),頭部在外表面有球形截頭部(16),其中該幹部(14)和該頭部(15)係分開組件;接納部(1),包括第一端(2),在第一端對立之第二端(3),貫穿二端之縱軸線(4),與縱軸線共軸之腔孔(5),鄰接第二端(3)之第一區(7),供接納該頭部之球形截頭部(16);以及其中接納部有二自由股部(9,10),尾部朝向第一端;內螺釘(21),可旋入自由股部(9,10)之間,把桿(20)固定,經由施壓元件施壓於頭部;其中設有施壓元件(17),適於施壓該頭部(15),把該頭部鎖定於接納部內;施壓元件係圓筒形構造,其外徑比腔孔(5)內徑稍小,容許施壓元件引進入接納部之腔孔內,並在軸向運動,且其中施壓元件之圓筒軸線係與接納部之縱軸線(4)共軸;其中該頭部有空心內部(25),接納該幹部之自由端部(22),且其中該頭部和該端部形成方式,使該頭部未鎖定時,端部即被該頭部彈性夾持,又其中該端部(22)呈錐形,直徑朝自由端(23)遞增,而該空心內部(25)具有相配形狀者。
