Description

脊椎骨骼穩定用之穩定裝置及桿連接器
本發明係關於骨骼穩定用之穩定裝置,包括第一桿、第二桿,以及連接第一桿和第二桿之桿連接器。桿連接器適於連接不同型的桿,尤其是彈性桿和剛性桿。
已知有各種裝置,供脊柱各部份之剛性固定用。一般而言,此等裝置包括剛性脊椎穩定桿。必須將不同類型的剛性桿加以連接。例如WO 2005/099603 A1揭示一種脊椎桿連接器,供連接剛性桿之用。
對某些用途,需要動態穩定,使相連接椎骨運動至一特定角度。又,脊椎的剛性和動能穩定組合使用,適合特殊臨床情況。US 2007/0005063 A3記載脊椎節段多位階、多功能穩定用之系統和方法,使用運動保存部(容許至少一部份脊椎位階運動)和運動防止部(實質上防止至少一部份相鄰脊椎位階運動)。
因此,為需要剛性和/或動態穩定之各種臨床情況,亟需組合使用剛性桿和撓性桿。
本發明之目的,在於提供一種穩定裝置,用途多樣,構造簡單。此外,應設桿連接器,適於連接不同類型之桿,尤其是撓性和剛性桿。
此目的是利用申請專利範圍第1項之穩定裝置,和申請專利範圍第16項之桿連接器達成。進一步發展見申請專利範圍附屬項。
本發明穩定裝置可例如用在剛性固定系統之末端部,使一部份脊柱固定不動。在此情況下,穩定裝置可作為保護元件,以防相鄰運動節段之超載。
另外,可用於分段穩定,以輪替順序進行動態和剛性固定。
桿連接器亦可僅用於連接剛性桿,故用途多樣。
在特別具體例中,桿連接器可連接於骨骼固定器。
桿連接器適用撓性桿(由彈料性材料製成為佳)和剛性桿。旋緊鎖定元件以固定撓性桿時,彈性材料變形會導致撓性桿、桿連接器和鎖定元件間之間接或動態型套連接,無損桿的整體結構。
此外,由少量組件即可達成桿之固定,故方便手術當中的處置。
本發明其他特點和優點由參見附圖所示具體例之詳細說明,即可瞭解明白。
茲見第1-4圖說明穩定裝置之第一具體例。穩定裝置包括第一桿1、第二桿2,和桿連接器3。圖示具體例中,第一桿1呈圓形斷面,直徑d1 。由彈性之生理相容性材料製成,以塑膠為佳。例如,桿可由彈料性材料製成,尤指聚碳酸酯、聚胺酯、聚碳酸酯-胺酯(PCU)或聚醚-聚胺酯(PEU)為基質。第一桿在施加外來負荷下,呈現彈性變形。第一桿表面最好是光滑表面。
第二桿2在圖示具體例中呈圓形斷面,直徑d2 ,比第一桿之直徑d1 為小。第二桿2可由剛性材料製成,諸如生理相容性金屬或合金,或生理相容性塑膠。惟本發明不限於第一桿1和第二桿2分別由彈性和非彈性材料製成和/或桿呈不同直徑之具體例。第一桿可由剛性材料製成。桿的斷面可呈任何形狀。桿之直徑可不同,亦可相同。再者,桿一辭係指長形構件,適於跨越至少二脊椎,或至少二破裂或受傷之骨骼部份的距離。
桿連接器3在圖示具體例中係單件製成。包括第一連接部4連接第一桿1,和第二連接部5連接第二桿2。第一連接部4包括相反側壁4a,4b,相反側壁4a,4b之連接側壁,和連接側壁之底壁。底壁內表面形成第一桿1的底座。壁形成之容納部呈大約U形斷面,形成容納第一桿1用之通道。在相反側壁4a,4b的自由端,設有陰螺紋6,供容納鎖定元件7。鎖定元件7可為內螺釘,與陰螺紋6嚙合。當第一桿1插入第一連接部內時,側壁4d形成第一桿1之銜接表面或止動部。
詳見第2至4圖,在底座4c表面設有複數肋條狀凸部8,肋條狀凸部8按通道軸線的垂直方向延伸,即在桿1插入時,是垂直於第一桿之縱軸線L1 。在圖示具體例中,凸部8呈實質上三角形斷面,有圓滑尖端。凸部8的長度是實質上跨越底座。各或若干肋狀凸部8可在底座表面提供凹陷(圖上未示)的凹溝狀凹部內一側或任一側伸出。底坐4c形成桿接觸表面。凸部形成桿接觸表面與桿1表面崎嶇不平。
內螺釘7在其桿接觸表面7a具有環狀凸部9。環狀凸部9形成有中央腔室之圓環形肋條,環形凸部9的斷面,類似底座的肋條狀凸部8斷面。此具體例中環形凸部之直徑,相當於底座上所形成二肋條狀凸部8之距離。因此,當桿1插入第一連接部內,把內螺釘旋入時,肋條狀凸部8和肋條狀凸部9與桿接觸之部份,位於桿之兩側。具體而言,肋條狀凸部8和肋條狀凸部9與桿接觸之部份,分別在徑向相反對位置,與桿表面結合。
在圖示具體例中,第二連接部5鄰接側壁4d形成圓筒形凸部。惟第二連接部之外形可具有任何外形。
第二連接部5包括腔孔10,其直徑足以容納第二桿2。腔孔10可有螺紋,而第二桿2可有螺紋端部2a,與腔孔10的陰螺紋嚙合,提供第二連接部5和第二桿2間之螺合連接。
使用時,第一桿1和第二桿2以任意順序以桿連接器3加以連接。例如,首先把端部2a旋入腔孔10內,將桿連接器和第二桿2連接。然後,把第一桿1插入第一連接部4之容納段內,直至定置於底座4c上,以其前面銜接後側壁4d。隨後,把內螺釘7插入並旋緊,直至環形凸部9與桿表面接觸。如第4圖所示,部份環狀凸部被向下壓到桿表面。同此,肋條狀凸部8從下面被壓到桿表面。凸部無損桿表面的整體性,在旋壓下,桿開始流動。此材料流動造成間接型套連接。直接摩擦力和間接型套力組合,把桿固定其位。若第一桿1和第二桿2二者均為剛性材料,則桿可利用摩擦力加以固定。
第一桿和第二桿的自由端可連接於又一連接器或骨骼固定器,把穩定裝置固定於骨骼內。
在修飾例中,環狀凸部9直徑可大於或小於最外肋條狀凸部8間之距離。如圖示具體例,設有二肋條狀凸部8。惟在內釘下側7a可設複數凸部。在鎖定元件下側和底座表面可設凸部和凹部組合,容許材料流入凹部內。
又一修飾例是,第二桿2和第二連接部間之連接,是利用壓套連接為之。
第5和6圖表示穩定裝置之第二具體例。與第一具體例相同之組件,標示同樣參考號碼,不再贅述。第二具體例與第一具體例不同,在於桿連接器31之設計。桿連接器31具有第一連接部4,與第一具體例一致。第二連接部51包括腔孔11,其直徑比第二桿2的直徑d2 稍大,故第二桿2可引進入腔孔11內。設有螺紋腔孔12,與腔孔11垂直。內螺釘13可旋入腔孔12內,利用夾持力,把桿2固定於腔孔11內。腔孔12與陰螺紋6平行延伸。惟腔孔12可以任何方式定向,例如繞二桿之縱向軸線,相對於陰螺紋6定向轉90°。
穩定裝置第三具體例參見第7至11圖說明之。與第一具體例一致之組件標示同樣參考號碼,不再贅述。第三具體例與第一具體例不同,在於桿連接器的設計。桿連接器301包括第一連接部,與第一具體例一致,但第二連接部不同。第二連接部501包括實質上U形凹部502,其寬度比第二桿2的直徑d2 稍大,故第二桿2可引進入凹部502內。U形凹部形成二股部,具有陰螺紋503可供容納鎖定元件504。容納桿2之U形凹部502底面,以及與桿2接觸之鎖定元件504下側,均為光滑面。因此,若桿2為硬質材料製成,施加外部負荷時,不會流動,桿2即利用摩擦力保持其位。側壁4d形成桿2和桿1之銜接表面。
第12至14圖表示穩定裝置之第四具體例。與前述諸具體例相同的組件,標示同樣參考號碼,不再贅述。
第四具體例與前述諸具體例不同,在於桿連接器之設計,可與骨骼固定器(例如骨釘20)多軸向連接。如第12至14圖所示桿連接器310包括與第一桿1連接之第一連接部40,和第二連接部5,後者在此例中與第5和6圖所示連接部5一致。第一連接部40包括在底座中心之腔孔41。腔孔41形狀以圓形為佳,腔孔41直徑較骨釘20的球體截面形頭部21的直徑為小。腔孔41邊緣可形成球形或錐形,或其他形狀,故骨釘20可樞動保持在第一連接部內。
設有施壓元件43,呈實質上圓筒形,可引進入第一連接部40之容納段內。施壓元件在其面向頭部21之側面,具有球形凹部44,包圍至少部份頭部21。施壓元件43又包括實質上U形凹部45,形成容納桿1用之通道。在U形凹部45的底面47設有至少二肋條狀凸部46,其設計像前述諸具體例之肋條狀凸部8。內螺釘7與前述諸具體例之內螺釘一致。因此,桿連接器310的第一連接部與前述諸具體例不同,在於施壓元件43內設有桿1用底座。施壓元件施壓於骨釘20之頭部21。
使用時,把骨釘引進入桿連接器310之第一連接段40內,再引進施壓元件43。骨釘20、施壓元件43和桿連接器310可以預組合。當骨釘20旋入脊椎或骨骼內時,由於骨釘20和桿連接器310間之多軸向連接,桿連接器310可對齊,以容納桿。
在旋緊內螺釘7時,可先壓住第一桿1。惟在第一桿1因彈性變形時,內螺釘7即接觸施壓元件43之突緣。因此,桿即鎖定其位。進一步旋緊螺釘7時,即壓住施壓元件43之突緣。施壓元件43即進而對頭部施壓,從而壓緊腔孔41邊緣。因此,骨釘即鎖定其角位。
又一修飾例是,形成第二連接部,與第一連接部一致,故可連接同型之二桿,例如二彈性桿彼此連接。
穩定裝置之第五具體例,如第15至17圖所示。與前述諸具體例一致之組件,標示前述諸具體例同樣之參考號碼。穩定裝置包括桿連接器311,與前述諸具體例不同,在於鎖定元件71和第一桿1之間設有充填件80。第五具體例之連接器311包括第一連接部410,有相反側壁410a,410b和底壁4c,提供桿1之底座,其高度是在桿1插入後,可引進充填件80。在陰螺紋6下方之側壁411a,410b內壁,可設導溝411a,411b,與導部81a,81b結合,後者可設在充填件80兩側的充填件上。導溝和導部亦可省略。充填件80包括在面向第一桿1側面的凹部82,其形狀適於桿之外形。在圖示具體例中,呈圓筒節段形凹部82。與桿接觸的凹部82表面,可設肋條狀凸部83,在桿1總軸線之基本上垂直方向延伸。充填件80在其兩側可包括開口84,鎖定元件71下側之圓形凸部可轉動自如結合其內,開口亦可省略。鎖定元件71與前述諸具體例之鎖定元件7相似。可有凸部(圖上未示)結合開口84,以提供轉動連接。
使用上,在插入第一桿1後,插入充填件,然後旋入鎖定元件。鎖定元件可相對於充填件轉動。施壓於充填元件,從而壓在桿1的表面。
第18圖簡略表示穩定裝置就脊柱100穩定之不同應用。第18a圖表示單型剛性桿2,例如鈦桿,可連接骨骼固定器101,定置於脊椎骨內,供剛性固定。第18b圖表示在具有剛性桿2的穩定構造末端,本發明連接器31把剛性桿2末段與彈性桿1連接。在此情況下,力量流動會延伸入脊椎的相鄰運動節段,加以保護,以免因穩定運動節段的剛性固定造成之力而超載。
第18c圖表示穩定系統應用在彈性桿1和剛性桿2輪替配置之構造。
可有其他修飾例。例如,第一連接段和第二連接段彼此相對位置,使桿之各底座在相同或不同高度,以副不同的解剖情況。再者,連接可彼此呈一角度定向,0角度除外。桿連接器可設計成,使第一和第二連接部設置成並列配置,即與第一連接部的側壁對立。第一和第二連接段可形成分開組件,可彼此永久性或可折式連接。分開第一和第二連接部的側壁4d,410d可省略。
上述各種具體例之元件可以組合,例如,各第二連接部可與各第一連接部組合。在底座的桿接觸表面上所設桿表面輪廓和/或鎖定元件和/或充填件,除肋條狀外,可有其他形狀,例如亦可設有點狀或尖狀凸部/凹部。

Claims (14)

  1. 一種脊椎骨骼穩定用之穩定裝置,包括:一第一桿(1),由彈性材料製成;一第二桿(2);一桿連接器(3;31;301;310;311),連接該第一桿(1)和該第二桿(2),該桿連接器包括容納該第一桿(1)用之一容納部(4;40;410),和把該第一桿固定於該容納部內用之一固定元件(7);其中在該容納部(4;40;410)內設有一桿接觸表面(4c;47;7a;44);且其中該桿接觸表面之輪廓包括與該第一桿(1)表面輪廓崎嶇不平(8;9;46;83),以致固定該第一桿(1)時,使該第一桿之該彈性材料變形;該第一桿(1)係配置成利用經由該固定元件(7)施壓產生之摩擦力,和利用該輪廓崎嶇不平(8;9;46;83)發生之間接型套助力,固定在該容納部(4;40;410)內定位者。
  2. 如申請專利範圍第1項之穩定裝置,其中該桿接觸表面上之該崎嶇不平係在該桿插入時,設在該桿之一側者。
