Abstract

Description
本發明係關於表現G蛋白偶合受體相關分選蛋白1 (GASP-1)之顆粒(亦稱為GASP-1顆粒)、靶向GASP-1顆粒之結合蛋白及嵌合抗原受體T (CAR-T)細胞,及結合蛋白與CAR-T細胞用於偵測及治療癌症之用途。
癌症在美國為第二常見之死亡原因且僅次於心臟病。若癌症未在其早期發展中被偵測到,則其發病率會顯著增加。在症狀出現之前的癌症之早期偵測為最有效的抵抗癌症的手段。舉例而言,歸因於能夠在結腸直腸息肉發展成癌症之前偵測且移除結腸直腸息肉之結腸直腸癌篩選測試,結腸直腸癌發病率在過去二十年的大部分時間裏持續降低(自1985年時的每1,000,000人中66.3個病例下降至2007年時的45.3個病例)。
自癌細胞釋放之細胞外囊泡(EV)與癌症發展及癌轉移有關。EV包括胞外體(exosome)(直徑為約30-100 nm)、微細小胞(microvesicle)(直徑為約100-1000 nm)及大型癌小體(直徑為約1-10 μm)。胞外體來源於晚期胞吞路徑之微細小胞集合體(multivesicular body,MVB)且藉由與細胞膜融合而釋放。另一方面,微細小胞及癌小體係藉由自細胞膜出芽而釋放。
癌症起始及癌症發展皆需要GASP-1之過表現。舉例而言,在使用三陰性乳癌(TNBC)細胞之阻斷基因表現實驗中,GASP-1表現量低之細胞明顯比野生型細胞長得慢。當完全下調GASP-1時,細胞死亡且無法繁殖。
為了追求安全且有效的癌症療法,研究者已試圖識別僅在癌細胞上而不在健康細胞上存在之抗原。但並未成功找到此類腫瘤特異性抗原(TSA)。當前CAR-T療法靶向僅係優先存在於癌細胞表面上之生物標記,意謂該等生物標記亦存在於正常細胞上。此產生非吾人所樂見且嚴重的副作用。另外,當前CAR-T治療僅對血液腫瘤有效,而對實體腫瘤無效。若干原因造成當前CAR-T療法對實體腫瘤無效。首先,實體腫瘤係異質的。與表面上幾乎遍佈CD19之癌性B細胞不同,未必所有實體腫瘤細胞皆攜帶CAR-T細胞經設計以攻擊之抗原。其次,即使單目標CAR-T療法成功消滅腫瘤,但癌症有時可在擺脫該抗原之後復發。第三,實體腫瘤含有厚度達到數百或甚至數千層之實體細胞塊,使得CAR-T細胞難以浸潤。
仍需要藉由靶向癌細胞(尤其實體腫瘤中之癌細胞)來進行、對正常細胞具有最小副作用之有效癌症治療。
本發明提供一種識別及治療癌症患者之新穎方法,其係藉由靶向表現G蛋白偶合受體相關分選蛋白1 (GASP-1)之顆粒(亦稱為GASP-1顆粒),例如在細胞溶質中或在細胞表面上。
提供一種用於偵測細胞之方法,該等細胞具有表現G蛋白偶合受體相關分選蛋白1 (GASP-1)或其片段之顆粒。該偵測方法包含使細胞與有效量之結合蛋白接觸,及識別具有結合於結合蛋白之顆粒的細胞。結合蛋白包含特異性結合GASP-1之抗原結合片段。所識別之細胞為具有GASP-1顆粒之細胞。結合蛋白可為人類化抗體或嵌合抗原受體(CAR)。

根據偵測方法,細胞可在腫瘤中。腫瘤可為實體腫瘤或血液腫瘤。細胞可為癌細胞。
根據偵測方法,細胞可在患有癌症之個體中。癌症可選自由以下組成之群:膀胱癌、乳癌、結腸癌、子宮內膜癌、食道鱗狀細胞癌、神經膠質瘤、頭部癌症、肝細胞癌、浸潤性乳腺管乳癌、喉癌、肺癌、黑色素瘤、卵巢黏質囊腺癌、頸部癌症、卵巢癌、胰臟癌、前列腺癌、腎細胞癌、小腸惡性基質腫瘤及胃腺癌。癌症可選自由以下組成之群:膀胱癌、乳癌、結腸癌、神經膠母細胞瘤、肝癌、肺癌、黑色素瘤、卵巢癌、胰臟癌及前列腺癌。乳癌可為高度乳腺管原位癌(ductal carcinoma in situ,DCIS)乳癌或三陰性乳癌。肺癌可為非小細胞肺癌(NSCLC)。個體可已接受癌症治療。
偵測方法可進一步包含偵測細胞中之癌症生物標記。癌症生物標記可選自由以下組成之群:CA125、CA19-9、CA15-3、CA27.29、AFP、BRCA1/BRCA2、EGFR、HER-2、KIT及CEA。
提供一種結合蛋白。該結合蛋白包含抗GASP-1單鏈可變片段(抗GASP-1 scFv)。抗GASP-1 scFv可包含可變重(VH)鏈及可變輕(VL)鏈。
VH鏈可包含SEQ ID NO:1之胺基酸序列。VH鏈可包含由SEQ ID NO:2之胺基酸序列組成之第一互補決定區1 (VHCDR1)、由SEQ ID NO:3之胺基酸序列組成之第二互補決定區2 (VHCDR2)、由SEQ ID NO:4之胺基酸序列組成之第三互補決定區3 (VCCDR3)或其組合。
VL鏈可包含SEQ ID NO:9或17。VL鏈可包含由SEQ ID NO:10或18之胺基酸序列組成之第一互補決定區(VL1CDR1)、由SEQ ID NO:11之胺基酸序列組成之第二互補決定區(VL1CDR2)及由SEQ ID NO:12之胺基酸序列組成之第三互補決定區(VL1CDR3)。

本發明人亦已出乎意料地發現,隨著癌症發展,GASP-1顆粒之數目及尺寸均可上升。在癌症早期,GASP-1顆粒可主要為粉末狀至精細顆粒。在高侵襲性及侵略性癌症(諸如三陰性乳癌(TNBC)及神經膠母細胞瘤)之晚期,GASP-1顆粒可主要為粗糙GASP-1顆粒。
在正常細胞中,GASP-1可僅最低限度地表現於細胞溶質中或不表現,且若存在,則可呈粉末狀形式。GASP-1顆粒可最低限度地存在於正常細胞之細胞溶質中,但不存在於細胞膜上。GASP-1顆粒可存在於一些正常細胞之細胞核中。GASP-1顆粒僅存在於癌細胞表面上,而不存在於正常細胞表面上之驚人發現提供腫瘤特異性抗原(TSA)之首個實例。此發現使得GASP-1成為使用例如CAR-T細胞或含有GASP-1 scFv序列的人類化單株抗體之癌症免疫療法的理想目標,因為靶向GASP-1顆粒以進行癌症治療將對正常細胞具有最小副作用。

圖2展示使用針對EEASPEAVAGVGFESK (SEQ ID NO:22)之多株抗體進行的高度DCIS乳癌之免疫組織化學(IHC)染色。在正常乳房細胞中,GASP-1僅表現於一些細胞核中且不表現於細胞溶質中(圖2左圖)。在癌細胞中,GASP-1顆粒與細胞膜結合,在細胞溶質中,聚集在核膜周圍及細胞核內(圖2右圖及圖3)。
GASP-1顆粒可來源於核內體。然而,不同於在成熟時具有0.5 µm直徑之核內體,此癌症中存在不同尺寸精細顆粒及粗糙顆粒之GASP-1顆粒(範圍涵蓋粉末狀顆粒、精細顆粒及粗糙顆粒)(圖3)。直徑為1.0至5.0 µm之粗糙GASP-1顆粒可大於核內體。此外,不同於描述於圖1中之細胞外GASP-1癌小體,粗糙GASP-1顆粒仍與細胞膜連接(參見圖3中之箭頭)且不自細胞膜釋放。GASP-1顆粒亦不形成可作為癌小體標誌之自細胞膜突出之膜泡。粗糙GASP-1顆粒可能係由較小顆粒之成熟或藉由較小顆粒之聚集而形成。細胞膜上許多粗糙GASP-1顆粒之結合有可能引起膜不穩定及細胞膜破裂(或片段化)。片段化細胞膜片仍含有許多與其連接之GASP-1顆粒(圖3)。細胞膜破裂的一個後果為許多最初存在於細胞內(在細胞溶質中,聚集於核膜上,或在細胞核中)之GASP-1顆粒釋放至腫瘤微環境中且隨後釋放至血液循環中。吾人亦已發現,隨著癌症變得更嚴重,GASP-1顆粒之尺寸變得更大且粗糙。
GASP-1顆粒存在於癌細胞表面上且不存在於正常細胞表面上可提供特異性靶向癌細胞且不影響正常細胞之機會。在癌細胞表面上存在大量粗糙GASP-1顆粒亦可使得GASP-1靶向癌症治療更有效。在如DCIS乳癌之實體腫瘤及其他癌症中,亦可在不同細胞層之表面上發現GASP-1顆粒。此亦將使得靶向GASP-1顆粒之免疫療法更有效,因為癌症治療劑將能夠找到該等GASP-1顆粒。
三陰性乳癌(其為侵略性極強之乳癌)展示與高度DCIS幾乎相同之GASP-1顆粒模式(圖4)。同樣地,此癌症中存在不同尺寸的GASP-1顆粒,其範圍涵蓋粉末狀顆粒、精細顆粒及粗糙顆粒。箭頭指出仍與細胞膜或膜片段連接之GASP-1顆粒。
對於男性及女性而言,肺癌均為最致命的癌症。其死亡率超過三種最常見癌症(結腸癌、乳房癌及胰臟癌)的死亡率之總和。非小細胞肺癌(NSCLC)占所有肺癌病例之約85%且大部分新近診斷之肺癌患者為晚期。早期偵測非常重要且在可獲得活檢樣品時,細胞溶質中GASP-1顆粒之存在及豐度可代表早期偵測。圖5展示NSCLC之代表性IHC染色。雖在正常肺細胞中,GASP-1顆粒僅存在於一些細胞核中且不存在於細胞溶質中(左圖),但在NSCLC中,各種尺寸之GASP-1顆粒存在於細胞溶質中或與細胞膜連接(右圖)。右圖中之箭頭指出GASP-1顆粒與細胞膜及膜片段之結合。因此,對來自懷疑罹患肺癌之個體的肺組織進行針對GASP-1顆粒之IHC染色將證實是否存在肺癌。
圖6展示神經膠母細胞瘤細胞中之GASP-1顆粒。GASP-1顆粒不存在於正常腦細胞中(左圖),但在神經膠母細胞瘤細胞中大量存在(右圖)。各種尺寸之GASP-1顆粒存在於神經膠母細胞瘤細胞之細胞溶質中及表面上。
發現在卵巢癌、結腸癌、黑色素瘤、胃癌及前列腺癌細胞之細胞溶質中或表面上存在類似的大量GASP-1顆粒。
癌細胞內存在如此多的不同尺寸之GASP-1顆粒為出人意料的。GASP-1顆粒存在於吾人所研究之所有癌細胞表面上;可假定其亦將存在於其他癌細胞表面上,諸如子宮內膜癌、食道鱗狀細胞癌、喉癌、卵巢黏質囊腺癌、腎細胞癌、小腸惡性基質腫瘤及胃腺癌。因此,GASP-1為普遍性癌症生物標記且產生GASP-1顆粒可代表癌症發展中之必要步驟。因此,可使用GASP-1顆粒之存在、其次細胞定位、其豐度等來評估癌症及癌症嚴重程度。
實例3. GASP-1 scFv序列
製備針對GASP-1肽片段EEASPEAVAGVGFESK (SEQ ID NO:22)之單株抗體。分離若干純系。純系14B8展示針對GASP-1肽之最高效價。將來自純系14B8之細胞溶解物儲存於TriZol溶液中。自細胞溶解物中之全部RNA反轉錄cDNA,隨後進行抗體之可變區(重鏈(VH)及輕鏈兩者)之PCR擴增。圖7展示使用VL特異性引子(泳道1)或VH特異性引子泳道2)之PCR產物的凝膠分析。隨後將所得PCR片段分別選殖至標準選殖載體中且測序。使用PCR,對純系14B8之重鏈及輕鏈進行測序,且發現兩個可變輕鏈序列。

實例4 抗GASP-1 scFv-Fc蛋白
基於純系14B8之序列產生抗GASP-1 scFv-Fc蛋白。使用不同可變輕(VL)鏈序列,製備兩種scFv-Fc表現載體scFv1-Fc及scFv2-Fc。表現載體構築體設計展示於圖8中。scFv1-Fc之VL為1468輕鏈#2 (SEQ ID NO:9),而scFv2-Fc之VL為1468輕鏈#1 (SEQ ID NO:17)。將表現載體短暫轉染至HEK293細胞中,且scFv-Fc蛋白藉由Gibco® FreeStyle™ 293表現培養基表現於HEK293細胞中。

藉由蛋白A親和性層析法純化scFv-Fc蛋白且分析其對於目標PC-3癌細胞之親和性。在流式細胞分析(FACS)中,5×105
個PC-3細胞用scFv1-Fc或scFv2-Fc蛋白,隨後用作為二級抗體之PE抗人類IgG Fc抗體進行染色。圖9展示scFv1-Fc蛋白識別49.4%之PC-3細胞,而scFv2-Fc蛋白僅識別8.39%之PC-3細胞,表明scFv1對PC-3細胞具有顯著更高的親和性。
實例5 癌細胞表面上之GASP-1表現

表2展示用於產生CAR卡匣之核苷酸及胺基酸序列。使用以下之核苷酸序列製備CAR構築體:Lenti-EF-1α啟動子(SEQ ID NO:27)、信號肽(SEQ ID NO:28)、scFv1 (SEQ ID NO:29)、CD28鉸鏈(SEQ ID NO:30)、CD28膜內序列(SEQ ID NO:31)、CD3z (SEQ ID NO:32)、T2A (明脈扁刺蛾(thosea asigna)病毒2A自裂解肽) (SEQ ID NO:33)及EGFRt (截短型人類表皮生長因子受體) (SEQ ID NO:34)。GASP-1 CAR構築體包含以下之胺基酸序列:信號序列(SEQ ID NO:35)、scFv1 (SEQ ID NO:36)或scFv2 (SEQ ID NO:43)、CD28鉸鏈(SEQ ID NO:37)、CD28跨膜序列(SEQ ID NO:38)、CD3z (SEQ ID NO:39)、T2A (SEQ ID NO:40)及EGFRt (SEQ ID NO:41)。scFv1可經scFv2取代(SEQ ID NO:42)。

使用GASP-1顆粒作為CAR-T目標,靶向癌細胞表面GASP-1顆粒之CAR-T療法代表一種新方法且將有效對抗血液腫瘤及實體腫瘤。
實例7 CAR-T細胞
為評估CAR-T百分比,將CAR-T細胞用抗EGFR及PE結合之抗人類IgG染色。流式細胞儀分析表明34.9%的細胞為CAR陽性(圖12)。

為測試CAR-T之有效性,將CAR-T細胞與目標細胞株PC-3以不同E/T比率共培養24小時,收集上清液以確定細胞介素之釋放。資料表明在與目標腫瘤細胞接合之後,由CAR-T細胞分泌之IL-2及IFN-γ增加(圖13)。
實例8.構築含有抗GASP-1 scFv序列之人類化抗體。
製備含有scFv1序列之人類化抗體。第一抗體可包含重鏈(SEQ ID NO:44)之胺基酸序列(包括可變區VH (SEQ ID NO:1)及人類恆定區(SEQ ID NO:45)),及輕鏈(SEQ ID:46)(包括由人類恆定區(SEQ ID NO:47)結合之可變區VL1 (SEQ ID NO:9))。第二抗體可包含重鏈(SEQ ID NO:44)之胺基酸序列(包括可變區VH (SEQ ID NO:1)及人類恆定區(SEQ ID NO:45)),及輕鏈(SEQ ID:48)(包括由人類恆定區(SEQ ID NO:47)結合之可變區VL2 (SEQ ID NO:17))。表3展示用於製備抗GASP-1人類化抗體之各種序列。合成所設計之嵌合抗體鏈且將其次選殖至哺乳動物表現載體中且短暫轉染至HEK293細胞中。藉由蛋白A親和性層析法及SEC-HPLC純化mAb。在超濾之後,對最終產物進行0.2微米無菌過濾。蛋白質純度>99%且內毒素低於1 EU/mg。

Claims (59)
- 一種用於偵測細胞之方法,該等細胞具有表現G蛋白偶合受體相關分選蛋白1 (GASP-1)或其片段之顆粒,該方法包含: (a)使該等細胞與有效量之結合蛋白接觸,其中該結合蛋白包含與GASP-1特異性結合之抗原結合片段;及 (b)識別具有結合於該結合蛋白之顆粒的細胞,其中該等所識別之細胞為具有GASP-1顆粒之細胞。
- 如請求項1之方法,其中該等細胞中之GASP-1顆粒之直徑在0.1至5.0 µm範圍內。
