Abstract
一種支撐骨骼強度的裝置與方法,包括一植入物,該植入物包含一外部葉片及一中央軸桿,該植入物可用一撐開器模組撐開,該撐開器模組包括:一套管與基座、一本體及一握把,該撐開器模組可接合該植入物,將植入物放置於欲醫治之骨骼內部調整擴張程度;將該撐開器模組握把置換成一注射器模組,該注射器模組包含一專屬骨水泥針筒及一注射握把,該注射器模組可將骨強化之混和物(例如骨水泥)經由出料孔、該中央軸桿之各第二軸向槽孔及該外部葉片之各第一軸向槽孔注入該骨骼內部,與已擴張之植入物結合,有效提升其支撐強度。
Description
本發明係屬一種骨骼植入物的技術領域,尤指其技術上提供一種支撐骨骼強度的裝置,透過撐開器模組,可於欲醫治之骨骼內部植入一可以調整不同擴張範圍、多層次弧形之可擴張植入物;藉由注射器模組可將骨水泥依序沿出料孔、通道、中央軸桿之各第二軸向槽孔及外部葉片之各第一軸向槽孔注入該骨骼內部,與已擴張之植入物結合。
骨壞死、骨腫瘤、或骨質疏鬆骨折,經常會合併骨缺損而導致骨塌陷缺損,因此需要接受外科手術以各種植入物或骨移植來治療,骨移植不論是自體骨、異體骨、或是人造骨,都需要經過移行性的骨再造(creeping substitution),才能重塑(remodeling)成正常的骨組織,而在此重塑過程,移植骨的力學結構會降低,當受到外力作用,很容易就會因支撐力不足而再次塌陷,因此必須藉由其他植入物以支撐骨骼結構強度,而這些植入物又必須同時不干擾移植骨的重塑過程。
以股骨頭缺血性壞死為例,目前各種的治療方式仍然不近理想,在早期發現、早期治療者,5年以上的成功率仍僅介於7至8成,失敗的最主要原因是股骨頭內的支撐力不足,而發生持續性的塌陷;以骨質疏鬆脊椎體壓迫性骨
折為例,目前常以經皮式骨水泥椎體成型術進行治療,然而因為骨水泥可能滲漏到脊髓神經周圍引起併發症,骨水泥本身硬度太高又無法被人體吸收,在復原的過程會阻礙骨骼癒合,因此又發展出佝僂成型術治療,利用氣球擴張將脊椎椎體撐開一個空洞,再用骨水泥灌入空洞內,佝僂成型術治療雖然改良了經皮式骨水泥椎體成型術,但是仍然使用高硬度的骨水泥,在骨質疏鬆的病人容易合併鄰近節脊椎椎體再次骨折,氣球擴張的醫療費用又相對高昂,因此有必要加以改良。
是以,針對上述習知結構所存在之問題點,如何開發一種更具理想實用性之創新結構,實消費者所殷切企盼,亦係相關業者須努力研發突破之目標及方向。
有鑑於此,發明人本於多年從事相關產品之製造開發與設計經驗,針對上述之目標,詳加設計與審慎評估後,終得一確具實用性之本發明。
習用之各種移植骨或使用大量異體骨加上去礦質化的骨基質,雖然短期的結果不錯,但是因為支撐力的不足,仍然有偏高比例的失敗率。
為改善上述之問題,本發明提供一種支撐骨骼強度的裝置,包括:一植入物,該植入物包含一外部葉片及一中央軸桿,
該外部葉片一端內部設有一第一迫合孔,其外周設有一接合部,該接合部呈多邊形且邊緣倒角之斷面設置,另一端內部設有一第二迫合孔與該第一迫合孔連通,該外部葉片緊臨該接合部設有一擴張部,該擴張部軸向設計為中央與二端厚薄不一之曲面設置,且環設複數個貫穿的第一軸向槽孔,各該第一軸向槽孔呈等角距設置,該中央軸桿內部呈中空狀,其一端外周設有一導引部,內周設有一第一內螺紋,另一端外周設有一擋止部,緊臨該擋止部設一第二迫合部,緊臨該第二迫合部設一中央軸管本體,該中央軸管本體外周環設複數個貫穿的第二軸向槽孔,各該第二軸向槽孔呈等角距設置,緊臨該中央軸管本體設一第一迫合部,該第一迫合部、第二迫合部分別與該第一迫合孔、第二迫合孔緊配。
前述,該植入物可用一撐開器模組撐開,該撐開器模組包括:一外套管,該外套管內部軸向貫設一通孔,其一端內周設有一接合孔,該接合孔對應該接合部呈多邊形且邊緣倒角之斷面設置,該接合孔與該接合部相套合,另一端固設一基座,並於該基座端面設一第一迫緊面;一迫緊塊,該迫緊塊一端之端面形成一第二迫緊面,另一端設一凹槽,該第二迫緊面抵靠該第一迫緊面,該凹槽二側形成一第三迫緊面,其軸向貫設一第一套合孔,其
外周設一外擋緣及一套合部;一本體,該本體呈中空狀,其一端內周依序設一第二套合孔及一第三套合孔,另一端內周設有一第二內螺紋,該第三套合孔固設一內套管,該內套管內部軸向貫設一通道,其一端設一卡合部,另一端外周設有一第一外螺紋,該第一外螺紋與該植入物之該第一內螺紋相螺合,該內套管依序穿套該第一套合孔及該通孔,將該套合部套入該第三套合孔,該外擋緣套入該第二套合孔,並使該卡合部嵌入該凹槽,限制該迫緊塊僅能於該本體上軸向位移並擋止於該外擋緣;及一握把,該握把外周設一第二外螺紋,其一端樞接一轉動件,該轉動件係為一培林,另一端設有一扭轉部,該扭轉部呈翼形設置,俾利於使用者扭轉施力,該第二外螺紋螺合於該第二內螺紋,該轉動件端面形成一第四迫緊面。
前述,該撐開器模組可於欲醫治之骨骼內部植入一可以調整不同擴張範圍、多層次弧形之可擴張植入物。

流體係為骨強化之混和物(例如骨水泥);該注射握把外周設一第二外螺紋,其一端內部凹設一容置部,另一端設有一扭轉部,該扭轉部呈翼形設置,俾利於使用者扭轉施力,該第二外螺紋螺合於該第二內螺紋,該管體套設於該容置部,使該推進桿抵靠該容置部底部,該出料孔和該通道連通。
前述,該注射器模組可將該流體依序沿該出料孔、該通道、該中央軸桿之各該第二軸向槽孔及該外部葉片之各該第一軸向槽孔流出至該骨骼內部。
一、本發明之支撐骨骼強度的裝置,該撐開器模組透過該握把施加不同力矩,藉由螺紋之扭轉力矩迫使該擴張部均勻地擴張變形,形成一可以調整不同擴張範圍、多層次弧形之可擴張植入物,藉此彈性的擴張構造使植入物和骨骼內部接觸面積增加,因此能夠有效的減少應力集中的現象,並且提供更廣泛的支撐效果。
二、本發明之支撐骨骼強度的裝置,該注射器模組可將骨強化之混和物(例如骨水泥)依序沿出料孔、通道、中央軸桿之各第二軸向槽孔及外部葉片之各第一軸向槽孔流出至骨骼內部,與已擴張之植入物結合,有效提升其支撐強度。
三、本發明之支撐骨骼強度的裝置,藉由植入物結合
骨強化之混和物(例如骨水泥)的支撐效果,配合各種骨移植策略,可提供骨修補癒合期間之生物力學支撐結構。
有關本發明所採用之技術、手段及其功效,茲舉一較佳實施例並配合圖式詳細說明於後,相信本發明上述之目的、構造及特徵,當可由之得一深入而具體的瞭解。

前述,該撐開器模組(1)之該握把(600)以一專屬骨水泥針筒(800)及一注射握把(900)置換,可組成一注射器模組(3),該專屬骨水泥針筒(800)包含一推進桿(810)及一外殼(820),該推進桿
(810)可軸向滑移的裝設於該外殼(820)內部,該外殼(820)一端設一出料孔(821),其內部設有一容置空間(822)和該出料孔(821)相連通,該容置空間(822)係用於盛裝流體(830),該流體(830)係為骨強化之混合物(例如骨水泥);該注射握把(900)外周設一第二外螺紋(901),其一端內部凹設一容置部(902),另一端設有一扭轉部(903),該扭轉部(903)呈翼形設置,俾利於使用者扭轉施力,該第二外螺紋(901)螺合於該第二內螺紋(503),該專屬骨水泥針筒(800)套設於該容置部(902),使該推進桿(810)抵靠該容置部(902)底部,該出料孔(821)和該通道(511)連通。
前述,該注射器模組(3)之使用方法,其步驟為:(a)一方面緊握該本體(500),另方面扭轉該注射握把(900)之扭轉部(903),使該容置部(902)底部推動該推進桿(810),讓該容置空間(822)中之該流體(830)依序沿該出料孔(821)、該通道(511)、該中央軸桿(200)之各該第二軸向槽孔(202)及該外部葉片(100)之各該第一軸向槽孔(101)流出至該骨骼(例如脊椎椎體)(700)內部(如第七圖所示);(b)放鬆該注射握把(900),一方面緊握該外套
管(300)之該基座(305),另方面反方向扭轉該本體(500),退出該注射器模組(3),參閱第八圖所示完成該植入物(10)之植入及該流體(830)之注入作業。
前述,本發明之支撐骨骼強度的裝置,其具支撐骨骼結構強度特性,非常適合應用在骨質疏鬆所造成的椎體壓迫性骨折,及其他骨缺損狀況,除了提供骨骼(例如椎體)的機械強度支撐外,亦可結合使用生物可吸收性的人造骨,如此不需要擔心因為骨水泥溢出而造成的併發症,同時也不會阻礙骨折的癒合,或也不至於造成後續鄰近脊椎因為受力過大而再發生骨折。
前述,本發明之支撐骨骼強度的裝置可應用於脊椎骨、股骨頭及骨質疏鬆之骨骼的重塑工程。

Claims (14)
- 一種支撐骨骼強度的裝置,係包含有一植入物,該植入物包括:一外部葉片,該外部葉片一端內部設有一第一迫合孔,其外周設有一接合部,另一端內部設有一第二迫合孔與該第一迫合孔連通,該外部葉片緊臨該接合部設有一擴張部,且環設複數個貫穿的第一軸向槽孔;及一中央軸桿,該中央軸桿內部呈中空狀,其一端外周設有一導引部,內周設有一第一內螺紋,另一端外周設有一擋止部,緊臨該擋止部設一第二迫合部,緊臨該第二迫合部設一中央軸管本體,該中央軸管本體外周環設複數個貫穿的第二軸向槽孔,緊臨該中央軸管本體設一第一迫合部,該第一迫合部、第二迫合部分別與該第一迫合孔、第二迫合孔緊配。
- 如申請專利範圍第1項所述之支撐骨骼強度的裝置,其中該接合部呈多邊形且邊緣倒角之斷面設置。
- 如申請專利範圍第1項所述之支撐骨骼強度的裝置,其中該擴張部軸向呈中央與二端厚薄不一之曲面設置。
- 如申請專利範圍第1項所述之支撐骨骼強度的裝置,其中各該第一軸向槽孔及各該第二軸向槽孔皆呈等角距設置。
- 如申請專利範圍第1項所述之支撐骨骼強度的裝置,其可應用於脊椎骨、股骨頭及骨質疏鬆之骨骼的重塑工程。
- 一種組設如申請專利範圍第1項所述之支撐骨骼強度的裝置之工具,係包含有一撐開器模組,該撐開器模組包括:一外套管,該外套管內部軸向貫設一通孔,其一端內周設有一接合孔,該接合孔與該接合部相套合,另一端固設一基座,並於該基座端面設一第一迫緊面;一迫緊塊,該迫緊塊一端之端面形成一第二迫緊面,另一端設一凹槽,該第二迫緊面抵靠該第一迫緊面,該凹槽二側形成一第三迫緊面,其軸向貫設一第一套合孔,其外周設一外擋緣及一套合部;一本體,該本體呈中空狀,其一端內周依序設一第二套合孔及一第三套合孔,另一端內周設有一第二內螺紋,該第三套合孔固設一內套管,該內套管內部軸向貫設一通道,其一端設一卡合部,另一端外周設有一第一外螺紋,該第一外螺紋與該植入物之該第一內螺紋相螺合,該內套管依序穿套該第一套合孔及該通孔,將該套合部套入該第三套合孔,該外擋緣套入該第二套合孔,並使該卡合部嵌入該凹槽,限制該迫緊塊僅能於該本體上軸向位移並擋止於該外擋緣;及 一握把,該握把外周設一第二外螺紋,其一端樞接一轉動件,另一端設有一扭轉部,該第二外螺紋螺合於該第二內螺紋,該轉動件端面形成一第四迫緊面;藉此,持續對該握把施加扭力,使該外套管擠壓該植入物,透過螺紋之扭轉力矩迫使該植入物之該擴張部擠壓變形,藉以形成一可以調整不同擴張範圍、多層次弧形之可擴張植入物。
- 如申請專利範圍第6項所述之支撐骨骼強度的裝置之工具,其中該擴張部於擴張後軸向可呈不同梯度之曲面設置。
- 如申請專利範圍第6項所述之工具,其中該接合孔對應該接合部呈多邊形且邊緣倒角之斷面設置。
- 如申請專利範圍第6項所述之工具,其中該轉動件係為一培林。
- 如申請專利範圍第6項所述之工具,其中該扭轉部呈翼形設置,俾利於使用者扭轉施力。
- 一種組設如申請專利範圍第6項所述之支撐骨骼強度的裝置之工具,係包含有一注射器模組,該注射器模組係將該撐開器模組之該握把以一針筒及一注射握把置換而成,該針筒包含一推進桿及一外殼,該推進桿可軸向滑移的裝設於該外殼內部,該外殼一端設一出料孔,其內部設有一容置空間和該出料孔相連通,該容置空間係用於 盛裝流體;該注射握把外周設一第二外螺紋,其一端內部凹設一容置部,另一端設有一扭轉部,該第二外螺紋螺合於該第二內螺紋,該針筒套設於該容置部,使該推進桿抵靠該容置部底部,該出料孔和該通道連通;藉此,扭轉該注射握把,可使該容置部底部推動該推進桿,讓該容置空間中之該流體依序沿該出料孔、該通道、該中央軸桿之各該第二軸向槽孔及該外部葉片之各該第一軸向槽孔流出。
- 如申請專利範圍第11項所述之工具,其中該扭轉部呈翼形設置,俾利於使用者扭轉施力。
- 如申請專利範圍第11項所述之工具,其中該流體係為骨強化之混合物。
- 如申請專利範圍第11項所述之工具,其中該流體係為骨水泥,該針筒係為專屬骨水泥針筒。
