А 61 В 17/ОА 1975.The invention relates to medical technology, namely to surgical suture instruments, and is intended to tighten edges, wounds when a primary or secondary suture is applied. A device for shrinking the edges of a wound is known, which includes abutment plates with holes for the ligature Dl. However, the known device does not ensure uniform gradual tightening of the wound edges of an arbitrary configuration, which leads to delayed treatment time and an unsatisfactory cosmetic result. The aim of the invention is to provide a uniform gradual tightening of the wound edges of an arbitrary configuration. The goal is achieved by the fact that in a device containing stop plates with holes for a gatura. The stop plates are connected by flexible jumpers, forming two rows of plates, and one of them has racks installed on which they are fixed with an angle of rotation. using gears and locking springs, shafts with holes for a ligature. FIG. 1 shows a device for shrinking wound edges, top view; in fig. 2 is the same, side view, in FIG. 3 shows the thrust plates of the device; a general view; in FIG. 4 shows a tool for actuating the toothed hinges of the anvil, general view; in fig. 5-8 is a diagram of the process of tightening the edges of a wound of various configurations using a device. The device consists of shrinkable and tightening parts, each of which contains stop plates 1 with openings 2 for ligatures. The fins 1 are connected in two rows by flexible bridges 3. Allowing the plates to be placed at different angles to each other or to separate them if necessary from each other. Racks with holes 5 are mounted on the stacking part plates 1 into which shafts 6 are inserted, which have holes 7 for fixing the ends of the ligatures 8. To the side parts of the shafts 6 are fixedly attached gear wheels 9 and 10 that interact with stop springs 11, which one end rigidly attached to the racks. A pedal 13 is disposed on the crossbar 12 connecting the springs 11 to separate the springs 11 from the gears 9. The apparatus operates as follows. After sterilizing the apparatus and treating the skin around the wound, the retractable part is applied to the wound edge and a dye solution, for example, methylene blue solution, through the holes 2 for the ligatures of the plates 1 make marks on the skin. Extra plates depending on the length of the wound, cut off. Similar markings are made on the skin on the other side of the wound. According to the length of the stacked part, the extra plates 1 of the stiffening part are cut off. Then, ligatures are carried out through the holes 2 of the plates 1 at the end of the bendable, the ends of which are alternately put on the surgical needle, and the marks on the skin are passed through the wound edges. Each end of the ligature is passed through the corresponding apertures of 2 Plates 1 of the twisting part, holes 7 of the shaft 6 and fixed on it (Fig. 5 and 7) - After connecting the ligature of the extensible and tightening parts by alternating rotation of the gears 9 and 10 with a finger or a toothed plate 1, the ligature B is wound on the shafts 6, the tension of the ligatures leads to the convergence of the shaken and tightening parts of the apparatus and with them the edges of the wound (Fig. 6 and 8). The removal of the apparatus is carried out as follows. The pedal 13 is pressed down, which causes the springs 11 to separate from the gear teeth. Then the ligature 8 with the shaft 6 and the tensioning part of the apparatus are moved to the side, and the ligature 8: is crossed at the surface of the skin in the usual way. The remaining parts of the ligatures 8 are removed together with the apparatus part to be shunted. Wounds from ligatures are treated with iodine. Thus, the proposed device provides uniform gradual tightening of the wound edges of an arbitrary configuration, which contributes to a reduction in the duration of treatment and a satisfactory cosmetic result. Apparatus of the invention for tightening the edges of a wound, comprising abutment plates with holes for a ligature, characterized in that, in order to ensure uniform gradual contraction of the edges of a wound of arbitrary configuration, the abutment plates are connected by flexible jumpers to form two rows of plates, and of these, racks are installed, on which are reinforced with the possibility of turning at a given angle with the help of gears and locking springs, shafts, with holes for a ligature.
