С целью предупреждени деформации при врожденных сколиозах за счет нарушени формировани позвонков у детей до по влени прогрессировани деформации удал ют полупозвонок из передне-бокового доступа, резецируют межпозвонковые диски, эпифизарную часть смежных позвонков, дужку полупозвонка , сближают позвонки на выпуклой стороне тракцией за дужки, производ т корпороспондилодез.The invention relates to medicine, namely to orthopedics in the treatment of congenital scoliosis. Поставленна цель достигаетс тем, что операцию провод т по возможности в более ранние сроки, не ожида известных периодов прогрессировани деформации. Такими периодами вл ютс момент, когда ребенок начинает ходить (обычно конец 1-го года) и период первого физиологического выт гивани (обычно 4-6 лет). Учитыва то, что чаще всего диагноз может быть поставлен в конце первого полугоди жизни, когда ребенок начинает садитьс , уже с этого времени ставитс вопрос об оперативном вмешательстве . На первом этапе операции из передне-бокового доступа резецируют тело полупозвонка и маркируют его дужку введением в ее корень спицы с выколом на спину. На втором этапе операции из заднего доступа удал ют маркированную дужку. Такой пор док действий не требует обширного обнажени заднего отдела позвоночника, облегчает поиск дужки полупозвонка сзади и сокращает врем второго этапа операции.The goal is achieved by the fact that the operation is carried out as early as possible, without waiting for certain periods of deformation progression. Ребенок уложен на левый бок. Передне- боковой разрез по II межреберью. Тупо кпереди отслоена периетальна брюшина, обнажен передний отдел позвоночника. После перев зки сегментарных сосудов над телами Т , HI и Ли позвонков Н-образно рассечена передн продольна св зка. Проведено удаление тела полупозвонка, прилежащих межпозвонковых дисков и эпифизарных пластин. В обнажившуюс дужку полупозвонка (ее основание) введена спица Кирш- нера с выколом на кожу спины. Больной переведен в положение на животе. Продольный разрез над остистыми отростками. Удалена маркированна дужка полупозвонка. Под дуги позвонков и Л„ (бывший Л„) установлены крючки контрактора. Поставлен стержень контрактора, после чего проведена компресси до соприкосновени дуг позвонков Тн,,, и Л„. Послойный шов задней раны. Ребенок вновь уложен на левый бок. В значительно сократившийс дефект между телами позвонков уложены ауто- и алло- трансплантаты, часть из них - по передней поверхности позвоночника с перекрытием зоны бывшего полупозвонка.
