На предоперационном этапе (фиг. 1) по Cobb определяют угол кифотической деформации. Далее определяют величину резекции суставных пар позвонков на уровне повреждения (фиг. 2). Для этого на уровне повреждения с обеих сторон расчерчивают угол резекции (CAB), который соответствует углу кифотической деформации следующим образом (фиг. 3, фиг. 4). С одной стороны суставных пар позвонков определяют вершину (А) каудо-дорзальную точку тела вышележащего позвонка от поврежденного. Точку (В) угла определяют на латеральной части нижнего суставного отростка вышележащего позвонка от поврежденного, точку (С) угла определяют по латеральной поверхности нижнего края верхнего суставного отростка поврежденного позвонка. Далее проецируют эти точки на противоположную сторону суставных пар позвонков. Скелетируют остистые, суставные отростки поврежденного и смежных позвонков, после чего в соответствии с анатомическими ориентирами под рентгеновским контролем выполняют транспедикулярную фиксацию с помощью металлоконструкции на смежных и более уровнях от поврежденного позвонка. Далее согласно предоперационному планированию проводят резекцию на полный поперечный размер суставных отростков с обеих сторон, а именно нижние суставные отростки вышележащего позвонка резецируют в плоскости (АА1ВВ1) по направлению кпереди и вверх. Верхние суставные отростки поврежденного позвонка резецируют в плоскости (АА1СС1) по направлению кпереди и вверх, тем самым формируя клиновидный дефект. Далее смыкают нижний суставной отросток вышележащего позвонка с верхним суставным отростком поврежденного позвонка в плоскости (ВВ1СС1) тем самым достигая коррекции кифотической деформации, при этом угол по Cobb становиться равен 0° (фиг. 5). The spinous and articular processes of the damaged and adjacent vertebrae are skeletonized, after which, in accordance with anatomical landmarks under X-ray control, transpedicular fixation is performed using a metal structure at adjacent or more levels from the damaged vertebra. Next, according to preoperative planning, resection is carried out to the full transverse size of the articular processes on both sides, namely, the lower articular processes of the overlying vertebra are resected in the plane (AA1BB1) in the anterior and upward direction. The upper articular processes of the damaged vertebra are resected in the plane (AA1CC1) anteriorly and upward, thereby forming a wedge-shaped defect. Пациентка Б. 1959 года рождения была госпитализирована в 2021 году в ФГБУ "ННИИТО им. Я.Л. Цивьяна" Минздрава России. Из анамнеза заболевания: травма бытовая - падение, при травмировании, ощутила боль в спине. При поступлении пациентка акцентировала внимание на выраженном болевом синдроме в грудо-поясничном отделе позвоночника. Было проведено комплексное клинико-рентгенологическое обследование. По результатам обследования установлен клинический диагноз: Закрытый неосложненный неполный взрывной перелом тела Th12 позвонка со смещением фрагмента в позвоночный канал. Посттравматическая кифотическая деформация на уровне Th11-Th12 позвонков. Было произведено лечение по предложенному нами способу. На предоперационном этапе выполняют многосрезовую компьютерную томографию грудо-поясничнго отдела позвоночника (МСКТ) (фиг. 6) на уровне Th10-L3 позвонков, на которой по Cobb определяют величину кифотической деформации на уровне Th11-Th12 позвонков, которая в данном примере равна 15 градусам. Далее определяют величину резекции суставных пар позвонков на уровне повреждения (фиг. 7). Для этого на томографическом срезе в сагиттальной плоскости с визуализацией суставных пар на уровне повреждения с обеих сторон расчерчивают угол коррекции (CAB), который соответствует углу кифотической деформации следующим образом. С одной стороны суставных пар позвонков вершиной (А) угла определяют каудо-дорзальную точку тела Th11 позвонка. Точкой (В) угла определяют на латеральной части нижнего суставного отростка Th11 позвонка. Точкой (С) угла определяют по латеральной поверхности нижнего края верхнего суставного отростка Th12 позвонка. Далее проецируют эти точки на противоположную сторону суставных пар позвонков. Проецируют вершину (А1) - каудо-дорзальную точку тела Th11 позвонка, точку (В1) угла определяют на латеральной части нижнего суставного отростка Th11 позвонка. Точку (С1) угла определяют по латеральной поверхности нижнего края верхнего суставного отростка Th12 позвонка. Точки В и В расположены так, что углы CAB и С1А1В1 равны углу кифотической деформации, то есть 15 градусам. После этого производят измерение высоты основания клиновидного дефекта (ВВ1СС1) которая составляет 8 мм. Вторым этапом выполняют оперативное вмешательство. Скелетируют остистые, суставные отростки поврежденного и смежных позвонков, после чего в соответствии с анатомическими ориентирами под рентгеновским контролем выполняют транспедикулярную фиксацию с помощью металлоконструкции, не менее чем на 2 позвонка выше и ниже вершины кифотической деформации. Далее согласно предоперационному планированию проводят резекцию на полный поперечный размер суставных отростков с обеих сторон, а именно нижние суставные отростки вышележащего позвонка Th11 резецируют в плоскости (АА1ВВ1) по направлению кпереди и вверх. Верхние суставные отростки поврежденного позвонка Th12 резецируют в плоскости (АА1СС1) по направлению кпереди и вверх, тем самым формируя клиновидный дефект. Далее смыкают нижний суставной отросток вышележащего позвонка Th11 с верхним суставным отростком поврежденного позвонка Th12 в плоскости (ВВ1СС1), тем самым достигая коррекции кифотической деформации. Затем выполняют ревизию положения спинномозговых корешков. Далее для стабилизации позвоночника выполняют фиксацию стержней металлоконструкции и окончательный монтаж металлоконструкции. A posterior median approach is performed at the level of vertebral injury. The spinous and articular processes of the damaged and adjacent vertebrae are skeletonized, after which, in accordance with anatomical landmarks, under X-ray control, transpedicular fixation is performed using a metal structure, at least 2 vertebrae above and below the apex of the kyphotic deformity. Next, according to preoperative planning, resection is carried out to the full transverse size of the articular processes on both sides, namely, the lower articular processes of the overlying T11 vertebra are resected in the plane (AA1BB1) in the anterior and upward direction. The upper articular processes of the damaged Th12 vertebra are resected in the plane (AA1CC1) anteriorly and upward, thereby forming a wedge-shaped defect. Next, the lower articular process of the overlying Th11 vertebra is closed with the upper articular process of the damaged Th12 vertebra in the plane (BB1CC1), thereby achieving correction of the kyphotic deformity. Then the position of the spinal roots is reviewed. Next, to stabilize the spine, fixation of the metal structure rods and final installation of the metal structure are performed. Then autologous bone grafting is performed in the area of the resected vertebrae Th11-Th12 under x-ray control. The wound is washed with saline solution, and the wound is drained. The wound is sutured in layers and an aseptic bandage is applied. Then postoperative MSCT control is performed (Fig. 8), which shows that the Cobb angle is 0°, which indicates that the kyphotic deformity has been eliminated. The patient makes no complaints. Notes the absence of pain. The patient is satisfied with the results obtained.
