Операцию начинают с пересечени пет ли кишки 5 выбранной дл наложени анастомоза с пищеводом,Дл этого в брыжейке у вершины петли кишки р дом со стенкой последней с помощью зажима образуют небольшое отверстие стара сь не повредить брыжечные сосуды. Через это отверстие ввод т браншу аппарата НЖКА с лазерным приводом и пережимают кишку. Затем прошивают и пересекают ее, на прошитые лазерно-механическим швом концы кишки накладывают второй р д серозно-мышечных швов. Привод щий и отвод сций отрезки кишки сшивают по брь1жечному краю между собой на прот жении 7-8 сМо После этого заднюю стенку конца пищевода подшивают швами, накладываемъЕми в шахматном пор дке, к стенке отвод щего отрезка кишки, добива сь тесного контакта наружных поверхностей их на площади, ограниченной по ширине полуокружностью пищевода а по длине - рассто нием не меньше 2-3 см. Швы накладьшают на рассто нии 1-1,5 см один от другого. На передней стенке отвод щего отрезка кшпки на 0,5 см ниже дистальных швов, фиксь-фующих кишку к пищеводу, сфокусированным лазерным лучом образуют сквозное отверстие. В это отвер стие ввод т экранирующую браншу Тобразного зажима дтй лазерного рассе чени стенок полого органа, а сверху накладывают вторую браншу этого зажи ма (щелевидную). Затем путем перемещени руко тки светопровода лазерной хирургической установки вдоль направл ющих пазов верхней бранши производ т рассечение стенки кишки в по перечном направлении на прот жении, равном величине полуокружности пищевода . На этом же уровне аналогичный Г-образный зажим накладывают на пище вод и в той же последовательности пересекают его лазерным лучом. Дл адаптации слизистых оболочек пищевод и накладывают два сквозных шва в области углов будущего анастомоза Затем формируют переднюю губу анаст моза путем наложени только одного р да непроникающих П-образных швов. Привод щий отрезок кишки швами, накладьшаемыми также в шахматном пор дке, подшивают к передней стенке пищевода и к отвод щей кишке таким образом, чтобы они плотно соприкасались своими наружными поверхност ми на прот жении 2-3 см вверх и 2-3 см вниз от линии анастомоза. При фиксации поверхностей органов швами, накладываемыми в шахматном пор дке, создаютс наилучшие услови дл их срастани , и уже к 5-7-му дню (времени, когда наиболее часто развиваетс несосто тельность швов) на прот жении всего участка соприкосновени органов образуетс соединительна спайка, чего в эти сроки не бывает, если поверхности органов сближены швами, наложенными по периметру площади соприкосновени . Если прот женность участка соприкосновени поверхностей привод щей кишки и пищевода вьш1е линии анастомоза , а также привод щей и отвод щей кишки ниже линии анастомоза будет по длине меньше 2 см, то прочность возникающих сращений окажетс недостаточной дл того, чтобы обеспечить повьшение надежности анастомоза. Увеличение длины участка соприкосновени поверхностей органов больше, чем на 3 см вверх и вниз от соусть , дальнейшего повьш1ени его герметичности по сравнению с уже достигнутым уровнем не дает. Привод щий и отвод щий отрезки тощей кишки соедин ют между собой орауновским анастомозом, накладываемым с использованием лазерной техники . Пример. Во врем операции по поводу рака желудка после удалени последнего дл наложени пищеводнокишечного анастомоза вз та петл тощей кишки. У вершины петли кишка пересечена через отверстие в брыжейке аппаратом НЖА с лазерным приводом. Швами, накладьгоаемыми в шахматном пор дке , задн стенка пищевода на прот жении 3 см по длине подшита к стенке отвод щего отрезка кишки, С помощью лазера рассечена стенка кишки и пересечен пищевод. Наложен пищеводно-кишечный анастомоз конец в бок с использованием методики однор дного шва. Передн стенка анастомоза при3 крыта привод щим отрезком тощей кишки , подшита швами, накладьтаемыми в шахматном пор дке, к пищеводу и отвод щему отрезку кишки на прот жении 3 см вверх и 3 см вниз от линии соусть . Операци закончена наложени ем анастомоза бок в бок между прив д пщм и отвод щим отрезками тощей кишки. Повьш1ение надежности соусть , наложенного по предлагаемому способу, обусловлено следующими факторами. Пересечение петли тощей кишки через отверстие в брыжейке позвол ет избежать перев зки ее сосудов и тем самы сохранить нормальное кровообращение 184 в привод щем и отвод щем отрезках кишки. Широка площадь тесного соприкосновени поверхностей пищевода и кишки в области анастомоза создает ему надежную герметичность и хорошие услови дл заживлени . Использование лазерной техники при рассечении стенок пищевода и кишки благодар совершенному гемостазу исключает проведение дополнительных гемостатических меропри тий, позвол ет вьшолнить операцию с минимальной травмой и применить методику однор дного шва.
