Способ осуществляется следующим образом. После резекции поджелудочной железы к ее культе подводится отключенная заранее по Ру петля тонкой кишки и в противобрыжеечном крае кишки мысленно выбирается участок для анастомоза в виде окружности, соответствующий по диаметру культе железы, и место для создания точечного отверстия в стенке кишки, соответствующее расположению просвета главного панкреатического протока. Непосредственно пересечение поджелудочной железы для меньшей травматизации рекомендуется выполнять скальпелем, a не сшивающим аппаратом, а гемостаз легко достигается неглубоким прошиванием краевых сосудов, идущих по поверхности поджелудочной железы. В стенке тонкой кишки создается точечное отверстие, равное по диаметру просвету главного панкреатического протока. Лучше это выполнить с помощью диатермокоагуляции, чем достигается гемостатический эффект и отверстие получается более ровное. Последовательно выполняется сначала нижняя губа анастомоза, причем вколы проводятся последовательно в радиарном направлении, снаружи поджелудочной железы через всю ее толщу к центру, к просвету главного панкреатического протока, без захвата последнего и далее к просвету выполненного точечного отверстия в стенке тонкой кишки серозно-мышечно-подслизисто (без захвата слизистой оболочки кишки) с выколом через толщу стенки кишки шириной около 1-1,5 см также в радиарном направлении от центра кнаружи (фиг. 1). Таким образом, выполняeтся около 6-8 витков непрерывного шва в радиарном направлении, причем первый вкол начинается через поджелудочную железу, а последний выкол нижней губы выполняется из стенки тонкой кишки с противоположной стороны (на 180o) окружности анастомоза (фиг. 2). Аналогично выполняется наложение верхней губы анастомоза. Обе нити сразу не завязываются, а берутся на держалки. После выполнения верхней и нижней губы еще незатянутый анастомоз выглядит непрерывно обшитым атравматичными нитями с полным соответствием просвета главного панкреатического протока выполненному точечному отверстию в стенке тонкой кишки. Диаметр окружности стенки тонкой киши, захваченной в шов, соответствует диаметру культе поджелудочной железы, причем окружность на стенке тонкой кишке должна быть на 1-1,5 см больше окружности культи железы. Далее с помощью ассистента натягиваются все 4 конца нитей, и рядом находящиеся концы противоположных нитей связываются между собой с достаточным для создания герметизма швов усилием (фиг. 3). Таким образом, нижняя и верхняя губы анастомоза с дозированным натяжением связываются между собой, завершая выполнение анастомоза. Более плотная ткань поджелудочной железы не дает стенке тонкой кишки полностью гофрироваться. При этом стенка тонкой кишки как бы накрывает культю поджелудочной железы, плотно прилегая к последней, создавая полное соответствие просвета главного панкреатического протока выполненному точечному отверстию в стенке тонкой кишке для оттока панкреатического секрета и надежный герметизм анастомоза. Дренированиe главного панкреатического протока не производится. Для панкреатикоэнтероанастомоза используется атравматичная монофиламентная нерассасывающаяся нить типа "SURGILEN-3.0".The method is as follows. The diameter of the circumference of the wall of the small intestine trapped in the suture corresponds to the diameter of the pancreatic stump, and the circumference on the wall of the small intestine should be 1-1.5 cm larger than the circumference of the gland stump. Then, with the help of an assistant, all 4 ends of the threads are pulled, and the adjacent ends of the opposite threads are connected with each other with sufficient force to create a hermetic seal (Fig. 3). Thus, the lower and upper lips of the anastomosis with metered tension connect with each other, completing the anastomosis. The denser pancreatic tissue prevents the wall of the small intestine from fully corrugating. At the same time, the wall of the small intestine, as it were, covers the stump of the pancreas, adhering tightly to the latter, creating a complete correspondence of the lumen of the main pancreatic duct to the puncture hole in the wall of the small intestine for the outflow of pancreatic secretion and reliable sealing of the anastomosis. Drainage of the main pancreatic duct is not performed. For pancreaticenteroanastomosis, an atraumatic monofilament non-absorbable suture type "SURGILEN-3.0" is used.
