ND Изобретение относитс к медицинё а именно восстановительной травматологии . Известен способ оперативного лечени застарелого подвьгаиха стопы кнаружи путем остеотомии большеберцовой кости 1. Однако этот способ травматичен и не устран ет всей патологии ввиду своей, несосто тельности, а именно в тех случа х, где с момента травмы проходит длительный период (свыше 1 г.) и в местах возникшего диастаза образовались грубые рубцы. Цель изобретени - снижение трав матичности операции и восстановление анатомических ,суставных взаимоотношений . Цель достигаетс тем, что соглас но способу оперативного лечени застарелого подвывиха стопы кнаружи путем остеотомии большеберцовой кости производ т косую остеотомию. у основани внутренней лодьркки до, сус тавного хр ща в направлении к. углу голеностопного сустава, затем лодыж ку придавливают к таранной кости, ввод т в образовавшийс клиновидный дефект аутотрансплантат из оставшегос выступа большеберцовой кости, фиксируют костные фрагменты к метафизу . Способ осуществл ют следунмцим образом.. Под внутрикостной анестезией про дольным разрезом длиной 5-7 см, про , веденным через серюдину внутренней лодыжки, освобождают последнюю от м гких;тканей, отвод их в стороны. Лальпаторно определ ют внутренний угол голеностопного сустава дл ориентира производства остеотомии. Далее широким остеотомом ( ширина его должна соответствовать поперечному размеру лодыжки у ее основани производ т косую остеотомию в направлении сверху вниз и снутри кнаружи с ориентиром на угол голеностопного сустава. При этом остеотоми осуществл етс на всю глубину костной ткани до суставного хр ща . Очень важно сохранить и не травмировать суставную поверхность, дл этого надо стремитьс провести операцию внесуставно. После остеотомии лодыжку плотно придавливают к таранной кости и, в образовавшийс в результате смещени лодыжки дефект , внедр ют костный трансплантат, вз тый здесь же, выше линии остеотомии с метафизарной части большеберцовой кости. Костный трансплантат должен соответствовать величине диастаза избыть плотно в.недрен в дефект с таким расчетом , чтобы смещенна лодыжка удерживалась им, С целью более прочной фиксации лодьгаски и трансплантата производ т остеосинтез их с помощью шурупа, который вводитс через верхушку лодыжки снизу вверх и в направлении центра метафиза большеберцовой кости. . После синтеза костных элементов определ ют подвижность в голестопном суставе и боковые качательные движени стопы в нем. Критерием правильности проведенной операции вл етс ограничение боковых движений и неизменность функции сгибани и разгибани в нем в сравнении с данными перед вмешательством. Затем накладывайт послойные кетгутовые швы на ткани, включа и кожу . Стопу в правильном положении иммобилизируют гипсовой пов зкой до верхней трети голени на срок до 2,5-3 мес.
