Abstract
Description
The invention relates to medicine, namely to traumatology and orthopedics, and can be used in the surgical treatment of children with a high position of the greater trochanter.
Such deformities are the result of dysfunction of the growth zone of the femoral neck and the epiphysis of the head and develop in children with post-reposition aseptic necrosis of the femoral head, the consequences of septic arthritis (hematogenous osteomyelitis), osteochondropathy of the femoral head with a total lesion of the epiphysis, etc. Violation of normal function (deceleration or blocking) of the growth zones of the femoral neck and the epiphysis of the head with the continued function of the growth zone of the greater trochanter leads to a shortening of the femoral neck and a high position of the greater trochanter with the growth of the child. The latter causes the development of lameness due to dysfunction of the gluteal muscles and limitation of abduction in the hip joint.
Ю.Ф. М. - С. 241-242.A known method of surgical treatment of children with a high position of the greater trochanter according to Wo-Lami, which consists in osteotomy and transplantation (transposition) of the greater trochanter, together with the abductor muscles of the thigh attached to it, in the distal direction to the lateral surface of the femur to restore the function of the gluteal muscles (Friedland M. O. Orthopedics. - M: Medgiz, 1954. - S. 361); Operative surgery with topographic anatomy of childhood, ed. Yu.F. Isakova, Yu.M. Lopukhin. - M.: Medicine, 1977. - S. 241-242.
The disadvantage of the known method of surgical treatment of children with a high position of the greater trochanter according to Wo-Lami and the like is that the fixation of the flat filing of the greater trochanter is carried out on the cylindrical base of the femur. This causes a number of negative features of this method, namely:
1. Increased risk of loss of correction due to insufficiently strong fixation of osteotomy fragments due to the small area of their contact.
2. Delayed consolidation of osteotomy fragments, up to the formation of a false joint.
3. Excessive lateralization of the greater trochanter.
The objective of the invention is to increase the efficiency of surgical treatment of children with a high position of the greater trochanter by providing reliable fixation of osteotomy fragments, as well as reducing the duration of treatment.
The method can be used in children from 3-5 to 17 years of age.
The technical result of the task is achieved by the fact that in the method of surgical treatment of children with a high position of the greater trochanter by its transposition, which consists in its osteotomy and transplantation together with the hip abductors attached to it in the distal direction and fixing it in the correction position with screws, it is proposed before fixation greater trochanter to perform for him the formation of a bed on the lateral surface of the femur.

In FIG. 5 shows a radiograph of the patient's hip joints with a high position of the greater trochanter on the left after surgical intervention according to the proposed method.
The method of surgical treatment of children with a high position of the greater trochanter is as follows:
3.The position of the patient on the operating table on the "healthy" side. Angular incision of the skin and subcutaneous tissue in the upper third of the thigh along the outer surface. After a cruciform dissection of the wide fascia of the thigh, the gap between m. gluteus Media, etc. tensor fascia lata. Mobilize the anterior and posterior edges of m. gluteus media. The upper third of the femur is exposed subperiosteally for 4 cm. The
On the lateral surface of the femur, in the place of the intended placement of the greater trochanter (the position in which its top will be at the level of the center of the head), a
The
The positive effect of the invention is as follows: the formation of a bed for the greater trochanter on the lateral surface of the femur allows reliable fixation of osteotomy fragments, prevents excessive lateralization of the greater trochanter and ensures rapid consolidation of bone fragments, thereby reducing the time of treatment of the patient.
