Description
The invention relates to medicine, namely to neurology, orthopedics, reflexology, exercise therapy, rehabilitation, and can be used in the treatment of myofascial pain syndromes to improve gross and fine motor skills of the extremities in children with hemiparetic forms of cerebral palsy.
(https://fb.ru/article/400086/gemipareticheskaya-forma-dtsp-simptomyi-diagnostika-i-lechenie)Myofascial syndrome (MFS) is a neurological pathology characterized by involuntary muscle contraction and intense pain that impairs the patient's overall well-being. The area of hypertonicity in the muscles is a local and painful induration. These are trigger points located on the path of the motor nerve that provides contractile muscle activity. In response to the impact of negative endogenous and exogenous factors, a sudden, sharp pain in tense muscles and fascia occurs reflexively. IFS affects various muscle groups located on the neck, shoulders, chest, back, limbs, abdomen and can occur in an acute, subacute or chronic form. The main cause of MFS pathology is static muscle overstrain or its prolonged stay in a non-physiological position. MFS requires a whole range of therapeutic and prophylactic measures with an individual approach to each patient. General therapeutic measures include medication, physiotherapy and surgery. Physiotherapy is carried out under the supervision of a qualified specialist who will select a set of exercises specifically for each patient. The hemiparetic form of cerebral palsy is one of the most common. It develops when the brain is damaged during the intrauterine development of the fetus and is characterized by a violation of the structure of the structures of the central nervous system. And its main manifestation is central paresis and paralysis. (https://fb.ru/article/400086/gemipareticheskaya-forma-dtsp-simptomyi-diagnostika-i-lechenie)

(Патент РФ №2541757).A known method of treating patients with myofascial pain syndrome in pathology of the musculoskeletal system. The method includes displacement of the musculocutaneous fold (KMC) to act on the skin and muscle-fascial areas by kneading, traction, twisting, compression, rotation of the KMC with mobilization of the fascia above and below the joint. These effects are carried out on the myofascial areas of the limb in the projection of the fascial muscle sheaths, fascial intermuscular septa and fascial beds for the neurovascular bundles, as well as areas of tendon muscle attachment. They act in the transverse direction with respect to the longitudinally oriented muscle-fascial structures of the limb and in opposite directions with respect to the active rotational movements performed by the patient around the longitudinal axis of the limb with contraction of agonist muscles and synergists of movement in the joints, on which the affected areas are located. They act to the maximum possible physiological barrier with a delay on it for 3-5 seconds by both the doctor and the patient. Then, the muscle-fascial and tendon areas of the opposite limb participating in the gait pattern are similarly affected, where the arm on the opposite side of the body is the opposite for the lower limb. (RF Patent No. 2541757).
We have chosen this method as a prototype.
The objective of the proposed invention is to develop a method for treating myofascial syndrome in children with pathology of the musculoskeletal system with the study of deep myofascial structures to improve the large and fine motor skills of the limbs in children with hemiparetic forms of cerebral palsy.
The technical result of the implementation of the task is to restore mobility in large joints, maintain the length of the muscles, preventing the formation of contractures, maximally involving the myofascial chains in the work, which makes it possible to prevent the recurrence of the disease.
The task is solved by the fact that the methodologist-rehabilitation therapist conducts with the child a set of 4 exercises in physiological poses while simultaneously giving the body and limbs positions that allow the axial muscles and myofascial structures of the body, mediastinum and gastrointestinal tract to be involved in the work. The movement in its natural sequence starts from the stable central axis of the body. Each limb movement is possible under the condition of the optimal length of the myofiscial structures not only of the limbs themselves, but also of those included in the myofascial chain. So, the deep back line includes: rhomboid muscles, levator scapula muscle, rotator muscles of the shoulder joint, triceps muscle of the shoulder, periosteum of the ulna, ulnar collateral ligaments, muscles of the hypotenar. Work with the fascial axis is carried out before the beginning of peripheral gymnastics with the muscles of the paretic limb.
The method is carried out as follows.
A methodologist - a rehabilitation therapist conducts a complex of 4 exercises with a child with hemiparetic forms of cerebral palsy. Exercise 1: starting position (ip) - the child sits with his crossed legs tucked under him, the methodologist raises the child's hand up and straightens it in all joints, the child's shoulders must be divorced, the spine is in axial extension, then the methodologist helps the child tilt the body in the frontal plane, while the pelvis remains motionless. The pose is fixed for 30 seconds. Then the same is repeated with the other hand. 2nd exercise: I. p. - the child sits with crossed legs under him, the methodologist fixes the child's pelvis on the ischial tubercles and raises the child's hand up, straightens it, the spine is in axial extension, then helps the child to tilt the body in the frontal plane, as well as rotation of the body in the sagittal plane to the right , while the pelvis remains motionless. The pose is fixed for 30 seconds. Then the same is repeated with the other hand and turning to the other side. Exercise 3: ip - the child sits on the heels, on the ischial tubercles, with support on straight arms, knees are divorced, the methodologist, turning the child's torso with his own hands, puts the child on its side, twisting the child's spine, while the axis of the spine should not shift relative to the pelvis, the pose is fixed for 1 minute. Exercise 4: I. p. - lying on its side, the child's upper leg, bent at the knee joint, is set with the heel at the level of the hip joint, the foot is laid out, the thigh is perpendicular to the body. With the help of the methodologist, the child rises on the left elbow, rests on the forearm, straightens the right hand up in the frontal axis, the palm is turned inward. Hold the position for 2 minutes. Then repeat on the other side and with the other hand. The course of exercises is carried out daily for 8-10 days.
The technical result achieved in this case is to increase the efficiency of working out deep myofascial structures to improve the large and fine motor skills of the limbs in children with hemiparetic forms of cerebral palsy, as well as to normalize lymphatic drainage and vascular blood supply.

Patient A., 3 years 5 months, diagnosis: cerebral palsy, spastic hemiparesis. Received 3 courses of a complex of rehabilitation exercises using this technique. Exercise 1: the initial position (ip) the child sits, tucking his crossed legs under him, the methodologist raises the child's hand up and straightens it in all joints, the child's shoulders should be divorced, the spine is in axial extension, then the methodologist helps the child to complete tilt of the body in the frontal plane, while the pelvis remains motionless. The pose is fixed for 30 seconds. Then the same is repeated with the other hand. 2nd exercise: I. p. - the child sits with crossed legs under him, the methodologist fixes the child's pelvis on the ischial tubercles and raises the child's hand up, straightens it, the spine is in axial extension, then the methodologist helps the child tilt the body in the frontal plane, as well as rotation of the body in the sagittal plane to the right, while the pelvis remains motionless. The pose is fixed for 30 seconds. Then the same is repeated with the other hand and turning to the other side. Exercise 3: I. p. - with the help of the methodologist, the child sits on his heels, on the ischial tubercles, with support on straight arms, knees are divorced, the methodologist, turning the child's torso with his own hands, puts him on his side, twisting the child's spine, while the axis of the spine should not shift relative to the pelvis, posture fixed for 1 minute. Exercise 4: I. p. - lying on its side, the child's upper leg, bent at the knee, is set with the heel at the level of the hip joint, the foot is laid out, the thigh is perpendicular to the body. With the help of the methodologist, the child rises on the left elbow, rests on the forearm, straightens the right hand up in the frontal axis, the palm is turned inward. Hold the position for 2 minutes. Then repeat on the other side and with the other hand. As a result of the treatment, the amplitude of active movements in the lumbar spine and joints of the extremities increased.
