Технический результат достигается тем, что при выполнении антиглаукоматозной операции с имплантацией мини-шунта Ex-PRESS формируют склеральный лоскут трапециевидной формы основанием к лимбу размерами 6 мм в основании, высотой 7 мм и 3 мм в вершине лоскута, толщиной равной 1/2 общей толщины склеры, выполняют отсепаровку лоскута на глубине 1/2 толщины склеры по направлению к лимбу до проявления роговичной ткани. На расстоянии 1/3 от основания склерального лоскута в склеральном ложе лезвием с углом 45 градусов выполняют два надреза до сосудистой оболочки параллельно лимбу на всю ширину склерального ложа. Формируют тоннель под надрезами, выделяя таким образом полоску склерального ложа, отделяемую от сосудистой оболочки. Расстояние между надрезами составляет 1,2-1,5 мм. После имплантации дренирующего устройства бранши капсульного пинцета проводят под сформированной полоской склерального ложа, вводя с дистальной стороны и выводя наружу через медиальную. При проведении антиглаукоматозной операции ножницами производят разрез конъюнктивы длиной 6 мм в 2 мм от лимба в квадранте, выбранном для имплантации мини-шунта Ex-Press. Далее конъюнктиву отсепаровывают ножницами на глубину 10-12 мм, производят вертикальную насечку 3-4 мм справа от разреза и обнажают склеральную зону. Выполняют умеренную диатермокоагуляцию поверхностных сосудов склеры, не допуская выраженной ретракции склеральной ткани. Лезвием формируют склеральный лоскут трапециевидной формы основанием к лимбу размерами 6 мм в основании, высотой 7 мм и 3 мм в вершине лоскута, толщиной равной 1/2 общей толщины склеры. При наличии используется соответствующий разметчик. Расслаивателем выполняют отсепаровку лоскута на глубине 1/2 толщины склеры по направлению к лимбу до проявления роговичной ткани. На расстоянии 1/3 от основания склерального лоскута в склеральном ложе лезвием с углом 45 градусов выполняют два надреза до сосудистой оболочки параллельно лимбу на всю ширину склерального ложа. Формируют расслаивателем тоннель под надрезами, выделяя таким образом полоску склерального ложа, отделяемую от сосудистой оболочки. Расстояние между надрезами составляет 1,2-1,5 мм. Затем в сформированном склеральном ложе в области проекции трабекулы иглой 25-27 калибра выполняют входной канал, через который с помощью инжектора в переднюю камеру вводят и устанавливают мини-шунт, фиксируя его проксимальным концом на склере. Бранши капсульного пинцета проводят под сформированной полоской склерального ложа, вводя с дистальной стороны и выводя наружу через медиальную. Выполняют захват дистальной части склерального лоскута с дальнейшим проведением его в субсклеральном пространстве под сформированной полоской склерального ложа и выводом наружу, тем самым обеспечивая фиксацию лоскута к склеральному ложу. Осуществляют контроль фильтрации ВГЖ. В Тамбовском филиале «НМИЦ «МНТК «Микрохирургия глаза» им. акад. С.Н. Федорова была проведена имплантация мини-шунта Ex-Press в соответствии с предлагаемым способом семи пациентам на семи глазах. В данную группу вошли пациенты с диагнозами:This group included patients with diagnoses:

- вторичная открытоугольная глаукома (на фоне тампонады силиконовым маслом);- secondary open-angle glaucoma (against the background of silicone oil tamponade);
- вторичная неоваскулярная глаукома (на фоне посттромботической ретинопатии).- secondary neovascular glaucoma (against the background of post-thrombotic retinopathy).
Осложнений во время проведения операции и в послеоперационном периоде не наблюдалось. В послеоперационном периоде ВГД у всех пациентов пределах толерантного в соответствии со стадией глаукомы.No complications were observed during the operation and in the postoperative period.
