Abstract
Description
The invention relates to medicine, namely to restorative medicine and neurology, and can be used to treat patients in a vegetative state (VS) and states of small consciousness.
A vegetative state - a prolonged coma - a stable pathological condition (UPS) characterized by the absence of cortical functions, prolonged impaired consciousness while maintaining the basic stem functions. It is formed under the influence of the primary damaging factor and is stabilized by the action of secondary factors that develop in the process of brain disease. The sun can last for years, but can be unbalanced, with the patient moving to a new level of consciousness [N. Ankylosing spondylitis. Healthy and diseased brain / 2nd ed. L .: Nauka, 1988.262 s.].
[Fielders A.I., Coble D.N., Pollak M.M. at al. Outcomes of children in a persistent vegetative state / Crit. Care Med - 1993 - Vol. 21, №12. Loftus - Lippincott - Raven, Philadelphia - New York - 1998/ - P. 117.]. - Т. 36, №1. - С. 138-141].Thanks to the success of modern resuscitation, the number of patients in the sun is growing worldwide, so in the USA at the end of the last century up to 25,000 adults and 10,000 children in this state were observed [Fielders A.I., Coble D.N., Pollak M.M. at al. Outcomes of children in a persistent vegetative state / Crit. Care Med - 1993 - Vol. 21, No. 12. - P. 1890-1894], and each year their number increased by 2000 people [Principles of Neurosurgery / Grossman SM. Loftus - Lippincott - Raven, Philadelphia - New York - 1998 / - P. 117.]. The effectiveness of treating patients in the sun remains extremely low [Vegetative state (prolonged coma) as a manifestation of a stable pathological condition / Vainschenker Yu.I., Ivchenko IM, Korotkoe A.D. et al. // Human Physiology, 2010. - T. 36, No. 1. - S. 138-141].
The accumulation in the population and the extreme severity of the patients' condition, on the one hand, and the low efficiency of their treatment, on the other hand, dictate the need to find treatment options.
А., Мамадалиев A.M. 30.06 - 01.07.2016. Currently, there is no single approach to the treatment of these patients. So, there is a method for carrying out endolumbal ozone-nootropic therapy [Aliev MA, Mamadaliev A.M. The study of the effectiveness of endolumbal ozone-nootropic therapy in post-traumatic chronic autonomic conditions / Abstracts V m-nar. conf. Fundamental and applied aspects of recovery of consciousness after a brain injury: an interdisciplinary approach. June 30 - July 1, 2016. Nizhny Novgorod].
The disadvantage of this method is that the introduction of nootropics and oxygen is not accompanied by modulation of afferent influx, stimulation of the reflex activity of the brain.
A method for the treatment of severe traumatic brain injury, proposed by Kasumov R.D. et al. [RF patent for the invention No. 2262356, IPC A61N 1/05, publ. 10/20/2005. - Bull. No. 29] consists of electrical stimulation of the brain stem using electrodes, one of which is inserted into the anterior horn of the uncompromised lateral ventricle, the second is epidural between the lower edge of the occipital bone and the posterior arch of the C 1 vertebra or into the large occipital cistern. The method provides the flow of electrons to the stem formations of the brain through the cerebrospinal fluid of the cisterns of the base, 4th ventricle, sylvian water supply and 3rd ventricle of the brain.
The disadvantage of this method is that the electron flow is undifferentiated. It cannot replace the natural multimodal afferent influx stimulating various reflex reactions that provide an ordered, phylogenetic and ontogenetically determined electrical activity of the brain.
- С. 89-90].Closest to the proposed method is kinesiotherapy in medical conductive rehabilitation [Isanova V.A., Sergeeva R.A., Mukhamedshina Ya.O. Author's methods of rehabilitation to restore consciousness in autonomic conditions / cerebral palsy and other disorders of movement in children: mat. 6 interdisciplinary n-pr conf. with m-nar participation, November 7-8, 2016, Moscow. - S. 89-90].
The method includes two steps. At the first stage, proprioceptive stimulation of the muscles of the face, stoma, tongue, neck, and oculomotors is performed to initiate chewing, swallowing, blinking, and communication. In this case, the patient is in a position on his side and sitting. Next, spiral-diagonal motor samples are connected using the Atlant rehabilitation pneumosuit.
The second stage is the preparation of the patient for passive verticalization and the formation of sensations of foot support.
The method has the following disadvantages.
1. Does not include balancing the body in a gravitational field.
2. There is no stimulation of vestibular perception and overcoming of tonic reflexes.
3. There is no stimulation of tactile perception.
4. No stimulation of auditory and vibrational perception
5. There is no stimulation of taste perception and smell.
All this limits the possibility of using the method.

The advantages of the proposed method: physiological, reflex participation of the patient himself, the possibility of use in conditions of minimal technical support.
When developing this method, it was assumed that the restoration of the central nervous system activity during the unbalancing of the autonomic OPS repeats the path of brain development in ontogenesis [Vegetative state (prolonged coma) as a manifestation of a stable pathological condition / Vainschenker Yu.I., Ivchenko IM, Korotkov A .D., Melyucheva L.A., Kataeva G.V., Medvedev SV.// Human physiology. - 2010. - T. 36, No. 1. - S. 138-141]. During the implementation of the above method, the patient actualizes the old and forms new interneuronal connections, forming new UPS, which is manifested, inter alia, by a decrease in the depth of neurological symptoms. This mechanism is described by N.P. Ankylosing spondylitis [Ankylosing spondylitis NP Healthy and diseased brain / 2nd ed., Revised. and add. L .: Nauka, 1988.262 s.].
The sequence of stages is determined by the order of maturation of the central nervous system in ontogenesis. The main emphasis is placed on the period of fetal development. The vestibular system is the first sensory system that myelinates by five months of fetal development. She perceives the position and movement of the head, is responsible for static and dynamic balance. Irritation of the vestibular apparatus during head movements triggers tonic reflexes with a redistribution of skeletal muscle tone, in particular, determining the fetal position. In the process of further development of the central nervous system, these reflexes go under the influence of other coordination, which are formed later.
In a vegetative state, the patient remains ontogenetically the earliest appearing, including vestibular tonic influences that inhibit the formation of motor activity, delay the central nervous system in the early stages of development. The formation of more complex, ontogenetic later coordination leads to the elimination of these influences at the first stage of rehabilitation and opens up the possibility of expansion of motor activity.
Touching the body stimulates the release of brain tissue nerve growth factor (NGF), which activates the central nervous system, in particular, stimulates the development of the nervous network, maturation of sensory neurons during fetal development. In the postnatal period, NGF stimulates sympathetic neurons, the development of the nervous network, helps the functioning of the central nervous system, and increases the synthesis of acetylcholine. Therefore, the touch system makes an important contribution to the development of sensors and is performed in parallel.
Through vibrational perception, environmental information is transmitted through the skeletal system. Auditory perception develops on the basis of vibrational. Therefore, stimulation of vibrational, then auditory perception is carried out first.
Stimulation of taste perception is carried out in conjunction with oral stimulation. The last step is the stimulation of smell, since this sensory channel is associated with respiration and begins to work only in the postnatal period.
The method is as follows.
The method includes several successive steps. The patient is placed on a support - a couch of the Exarta kinesiotherapeutic unit, for which the patient’s body or body parts are fixed in suspensions, sensory stimulation of the patient is performed, giving the body a certain position on the couch or lifting individual parts of the body above the support and keeping it in this position, changing the body position in space that provides balancing of the patient’s body in the gravitational field, opening up the possibility of performing movements in three planes in response to sensory stimulation. The Exarta kinesiotherapeutic unit is produced by the MADIN company, 603062, Russia, Nizhny Novgorod, website: http://www.madin.ru/. The installation includes a special couch with a variable height and a system of suspensions, harnesses and loops. Exarta suspension systems provide neuromuscular activation of the deep muscles that stabilize the spine, co-activation of the deep and superficial muscles to form optimal postural and locomotor stereotypes, sensorimotor training in three planes in conditions of unstable equilibrium with balancing the body in the gravitational field.
The first stage is aimed at stimulating proprioceptive, tactile, vestibular perception, overcoming tonic reflexes and includes the following exercises:
1. PI - lying on his back on a couch in an embryonic position, pendant with a slit under his head, head in a forward bend, legs bent at the knee and hip joints, pressed to the body, arms bent, pressed to the chest, patients take a pose on their own due to the severity spastic syndrome, kept in this position. The patient is held in this position of the patient from 30 seconds to 3 minutes, gradually increasing the time spent in this position. This exercise is intended to eliminate the labyrinthine tonic reflex. Staying in an embryonic position reduces irritation of the receptor cells of the otolith apparatus, which is accompanied by a decrease in the severity of tonic reflex movements of the neck, trunk and limbs.
2. IP - belly down on a wide suspension with support on the forearms and open palms, head straight. The patient is kept in this position from 30 seconds to 3 minutes, gradually increasing the time spent in this position. Staying in this position reduces the irritation of the receptor cells of the otolith apparatus, which is accompanied by a decrease in the severity of tonic reflex movements of the neck, trunk and limbs.
Then, stimulation of perception in the patient’s supine position on the couch, suspension with an incision under the head, wide pendants under the shoulder and pelvic girdles, and pendants under the ankle joints are performed.
Stimulation of auditory and vibrational perception.
Vibrational perception is stimulated using special vibrating devices, starting with the distal extremities. The device is applied precisely to the bone protrusions and is not biased. The following zones are stimulated: ankles, femoral condyles, styloid and ulnar processes, bone parts of costal arches. Tuning forks or vibrating massagers were used as vibrating devices.
To stimulate auditory perception, the patient is given calm stereo music through headphones. Use synchronous and asynchronous modes. Synchronous mode - simultaneous supply of stimuli to the right and left - stimulates the synchronization of the functional activity of the right and left hemisphere. Asynchronous mode - alternating stimulus delivery on the right and left - stimulates the alternate activity of the right and left hemisphere.
Stimulation of taste perception and oral stimulation.
Oral stimulation begins with touches in the cheeks, gradually approaching the lips and oral cavity. Then, with a chilled object, a rubber or plastic ring, or a ball, is carried out on the lips, first with a light, then with a more noticeable pressure. Items are pre-cooled in the refrigerator to 4-6 ° C.
Taste perception is stimulated using various types of sponges: finely porous or large-porous. Sponge dimensions: 1 × 1 cm. Use a soft toothbrush. Sponges and / or a soft toothbrush are dipped into a liquid (warm, cool; with various taste effects: sour, sweet, bitter, salty, womi) and gently run along the patient's lips. Then insert a sponge or toothbrush between the teeth and carefully enter into the oral cavity, holding it firmly with your fingers. The advantage of the sponge is that when it comes into contact with the teeth, it does not cause a reflex spasm of the jaws, which can happen when using harder objects. A toothbrush soaked in liquids with different tastes is used to treat teeth and tongue.
Stimulation of smell.
To stimulate the sense of smell, the patient is presented alternately with pieces of paper, pads, pomanders, on which aromatic oils are applied, for example, tangerine, cedar, fir or lavender.

Then the patient was transferred to a position on the stomach on a couch with support on the forearms, drugs were bent at a right angle, palms were open, legs were straight, wide suspension with elastic cables under the stomach. Holding the body in this position, carefully turned the head to the left, right, up, down. The exercise was performed once in the first lesson, gradually increasing the number of repetitions to four in subsequent ones. Then they touched the skin with balls in the direction from top to bottom and from inside to outside. Touching began on the body with the transition to the limbs. Stroking the back was carried out first across, then from top to bottom. This group of techniques was completed by touching the legs in the direction from top to bottom.
Vibrational perception was stimulated using tuning forks. Tuning forks were successively applied to the ankles, femoral condyles, styloid and ulnar processes, and bone parts of costal arches.

After the course of treatment: the patient is available for contact: answers questions with eye movement and blinking, at the request of performs movements with his hands, feet, slow, not in full. At the request he sticks out his tongue, makes an attempt to lick his lips. Spastic syndrome has decreased. Able to communicate through a personal computer: monosyllabic, slowly answers simple questions using the keyboard. The nystagmus became smaller, the asymmetry of the face remains. Pharyngeal reflexes appeared: fluid, gruel-like food began to be swallowed, salivation decreased. Learned to chew baby cookies. The installation of the head has become more correct. Holds a pose while sitting with support on his back and arms.
