Abstract
Description
The invention relates to medicine, namely to dentistry, maxillofacial surgery, and can be used for surgical treatment of patients with fibrous epulis.
The treatment of tumor-like formations (epulis) of the oral mucosa, including fibrous epulis, is surgical, which results in the formation of a postoperative tissue defect that requires certain closure methods. However, their results are not always satisfactory from an aesthetic and functional point of view.
There is a method of closing such defects with a gauze cloth impregnated with iodoform, which is fixed with silk seams, wire or a protective plate made of quick-hardening plastic [Epulis [Electronic resource] Access mode: http://www.medical-enc.ru/17/stomatologia/hirurgicheskaya /epulis.shtml. Date of appeal: 04/07/2014]. The disadvantage of this method is the healing of open wound surfaces through secondary tension; contraction of soft tissues of the wound edges, which leads to an increase in the size of the defect with the formation of deforming scar tissue.
A known method of excising pathological formation and rapprochement of the edges of the wound, developed at the beginning of the twentieth century. The essence of the method is to close the resulting defect by mobilizing and drawing together the edges of the wound with suturing. The disadvantage of this method is that in conditions of tissue deficiency that occurs when removing pathological formations on the mucous membrane of the alveolar part of the jaw, the connection of the edges of the wound along the entire plane of the incision does not always provide uniform and tight contact. This is due to excessive stress on the edges of the wound, due to the intractability of the mucous membrane of the alveolar part of the jaw. In turn, the tension of the tissues leads to the eruption of sutures, disruption of microcirculation and trophism in the tissues.
One of the most common methods for closing a postoperative soft tissue defect is the use of a mucosal-periosteal gingival gingival flap. It can have any shape corresponding to the shape of the defect, its base is facing the transition folds of the upper arch of the vestibule of the oral cavity. To give the flap more mobility at its base parallel to the transitional fold, a periosteum is cut through a linear section along the entire length to the submucosal layer. Due to this, the flap lengthens and freely contacts the edges of the defect [Yu.I. Vernadsky, 1998, etc.]. The disadvantages of this method are a decrease in the depth of the vestibule of the oral cavity, the formation of a stretched fold from the mucous membrane, which subsequently leads to the development of periodontitis and the difficulty in manufacturing and wearing removable dentures, and all this requires additional surgical correction.
It is known that to close postoperative soft tissue defects in the alveolar part of the jaw materials of biological origin are used, for example, an autogenous vascularized mucous flap from the buccal region [M. Hatoko, M. Kuwahara, 2002]; skin flap [M.V. Mukhina, 1963; Yu.I. Vernadsky, 1973, etc.]. The disadvantages of these methods are additional trauma when taking automaterial; the formation of a tissue defect in the donor site, which requires closure using other plastic methods; reduction of flaps, which leads to deformation of the vestibule of the oral cavity, slow healing, lengthening the rehabilitation period.
It is known to use artificial materials for closing postoperative defects in soft tissues of the oral cavity, such as Teflon membranes (GoreTex® and others) and silicone rubber [D.S. Shinkevich, Features of the healing of postoperative wounds of the soft tissues of the face in the vestibule of the mouth, isolated by silicone membranes. - An abstract of a dissertation on a competition. Ph.D. - Moscow, 2008]. The disadvantage of this method is that the tissues under the membrane are often exposed because of their failure, they are infected with the contents of the oral cavity, all this provokes the development of inflammatory processes.
Прототипом изобретения является способ закрытия дефекта мягких тканей альвеолярной части челюсти после удаления гигантоклеточной гранулемы с использованием свободного соединительно-тканного трансплантата, взятого с неба [Lev R., Moses О., Holtzclaw D., Tal H. Esthetic treatment of peripheral giant cell granuloma using a subepithelial connective tissue graft and a split-thickness pouch technique. - Journal of Periodontology. 81(7): 1092-8, 2010 Jul.]. The prototype of the invention is a method for closing a soft tissue defect in the alveolar part of the jaw after removal of a giant cell granuloma using a free connective tissue graft taken from the palate [Lev R., Moses O., Holtzclaw D., Tal H. Esthetic treatment of peripheral giant cell granuloma using a subepithelial connective tissue graft and a split-thickness pouch technique. - Journal of Periodontology. 81 (7): 1092-8, 2010 Jul.]. The disadvantages of this method are:
1. Adverse anatomical features of the hard palate for graft collection, the proximity of the neurovascular bundle when there is a risk of damage to the palatine artery.
2. The submucosa rich in glandular or adipose tissue disrupts the processes of graft engraftment, being a barrier to plasma soaking and germination of periodontal ligament cells.
3. The level of cleavage of the mucous membrane of the alveolar part of the jaw was not noted for the preparation of the recipient bed, taking into account the histological structure of the tissue.
4. Fixing the graft with interrupted sutures does not ensure its full adaptation and worsens the processes of graft revascularization, which can lead to graft necrosis.
5. The connective tissue graft used in this technique is deprived of the epithelial cover throughout, and therefore the graft site is in contact with the oral fluid, undergoes maceration and becomes infected.
Thus, an analysis of the literature indicates unresolved issues related to the closure of wound defects in the region of the vestibule of the oral cavity after removal of tumor-like formations, in particular epulis. The development of a methodology with more universal capabilities and a predicted result is an actual solution to the problem at the moment.
The objective of the invention is to expand the arsenal of methods for replacing a mucosal defect in the alveolar part of the jaw after removal of fibrous epulis.
The technical result of this invention is to increase the aesthetic and functional results by simultaneously eliminating the mucosal defect and creating the necessary volume of the attached gums.
The proposed method for closing a defect in the mucous membrane of the alveolar part of the jaw after removal of fibrous epulis is as follows.
1. Under local anesthesia, stepping back 2-3 mm from the epulis, it is removed by a p-shaped incision of the mucous membrane with the underlying part of the periosteum.
2. The mucous membrane of the gum surrounding the defect after removal of the epulis is split at the border of its own lamina (lamina propria) and periosteum to 0.3-0.5 cm. This forms a “niche” along the edges of the defect.
3. In the area of the tubercle of the upper jaw with an L-shaped incision of the mucous membrane, a free transplant of a rectangular shape is taken, including connective tissue and epithelial layers of the mucous membrane, the size of the defect is 0.3-0.5 cm larger at the edges.
4. The resulting wound in the donor site is closed by the approach of the edges of the wound and the application of nodal sutures.
Throughout the edge of the resulting connective tissue transplant, a de-epithelization of 0.3-0.5 cm wide is carried out, after which it is laid on the surface of the defect so that the de-epithelized sections of the graft are in a pre-prepared "niche" (described in paragraph 2 )
6. In the area of projection of the "niche" the graft is fixed with p-shaped sutures.
7. On top of the graft an additional crosswise compression suture is applied with a polyamide thread.
This graft is a medium graft, ideal in terms of aesthetic results. It is the first to undergo primary reduction due to the small number of elastic fibers in its composition.
In addition, the graft obtained from the hillock of the upper jaw contains the least amount of adipose and glandular tissue, which significantly improves its engraftment.
The preparation of the recipient bed for a transplant consists in the formation of a “niche” obtained by splitting the gingival mucosa surrounding the defect at the border of the lamina propria and periosteum by 0.3-0.5 cm.
In addition, the revascularization of the graft placed in this way (in the “niche”) occurs in the direction both from the periphery of the defect to the center and from the underlying part of the periosteum. This fact provides more predictable stabilization and a high percentage of transplant viability.
For reliable fixation of the transplant, 2 types of sutures are used. U-shaped sutures that not only fix the flap around the perimeter of the defect, but also prevent compression of the blood vessels inside the graft, providing better diffusion of fluids.
The cruciform suture contributes to the proper adaptation of the graft to the underlying tissues, and also prevents hemorrhage under the graft, which can lead to its detachment and subsequently to necrosis.
The invention is illustrated by the following example.
Patient A., born in 1975, was admitted to the department of maxillofacial surgery with a diagnosis of epulis of the anterior maxilla (sizes 0.9-1.2 cm). Clinical and laboratory examination.
Under Infiltration Anesthesia Sol. Ultracaini 1: 100000 - 1.7 ml, a p-shaped incision was made that borders the tumor, departing from it by 2-3 mm 2.0 cm long in the area of 2.1 tooth, including the lower-free gingival margin, with the base apical. The tumor is removed with the gingival margin of the tooth region 2.1, part of the periosteum within the healthy tissue. The neck surface of the 2.1 tooth is smoothed.
6-0.Under Infiltration Anesthesia Sol. Ultracaini 1: 100000 - 1.0 ml, an L-shaped incision of the mucous membrane was made in the region of the upper jaw hillock to the left to the periosteum 1.5x1.8 cm in size, the graft was taken by splitting it at the border of its own plate of the mucous membrane and submucosal layer. A nodal suture with a 6-0 polyamide thread was applied to the wound in the region of the tubercle of the upper jaw.
6-0.In the area of the postoperative wound, in the projection of the tumor, a “niche” was formed from the medial, distal and apical edges of the wound to a depth of 0.3-0.5 cm within the soft tissues using a scalpel blade of 15 s. The edges of the free graft are brought into the area of the postoperative defect, and the graft is fixed with p-shaped sutures in the projection of the “niche” with a 6-0 polyamide thread. A crosswise compression suture of 6-0 polyamide thread was applied to the surface of the graft.
The sutures were removed on the 7th day, the transplant is viable, there are no discrepancies. After 1 month, complete adaptation with the underlying tissue and epithelization of the donor bed.
