В качестве аналога предлагается способ формирования концеконцевого тонкотолстокишечного анастомоза [2]. Недостатками данного способа являются несоответствие диаметров сшиваемых тонкой и толстой кишок; полное отсутствие клапанных свойств анастомоза, его воспаление и заброс каловых масс в тонкую кишку. Указанные недостатки в раннем послеоперационном периоде способствуют развитию несостоятельности анастомоза, в позднем - его рубцеванию, провоцируют развитие рефлюкс-энтерита и дисбактериоза.As an analogue, a method is proposed for the formation of a terminal-end small bowel anastomosis [2]. Недостатками данного способа являются нарушения кровоснабжения и иннервации в зоне анастомоза, его аперистальтичность, отсутствие клапанных свойств и, как следствие, зияние с неизбежным воспалением и забросом каловых масс в тонкую кишку. Указанные недостатки в раннем послеоперационном периоде способствуют развитию несостоятельности анастомоза, в позднем - провоцируют развитие рефлюкс-энтерита и дисбактериоза.As an analogue, a method is proposed for forming a longitudinal lateral colonic anastomosis [3]. Решение поставленной задачи позволяет уменьшить частоту осложнений со стороны анастомоза и улучшить его клапанные свойства, что достигается выполнением инвагинации тонкокишечного «хоботка» в просвет толстой кишки одновременно с его контролируемым выворачиванием и полной адаптацией слизисто-подслизистых слоев тонкой и толстой кишок за счет применения однорядных узловых швов, каждый из которых формируют следующим образом: первым этапом производят продольное серозно-мышечное прошивание стенки тонкой кишки шириной 0,5-0,6 см по проксимальной границе «хоботка», вторым этапом на расстоянии 0,5-0,6 см от свободного края «хоботка» осуществляют серозно-подслизистое прошивание стенки кишки с выколом в свободный край «хоботка», третьим этапом захватывают в шов стенку толстой кишки краевым подслизисто-серозным прошиванием шириной 0,5-0,6 см. Способ осуществляют следующим образом. На анастомозируемом дистальном конце тонкой кишки формируют «хоботок» длиной 3,5-4,0 см с участком брыжейки и питающим концевым сосудом (1). Стенку толстой кишки (2) по линии будущего анастомоза рассекают строго в поперечном направлении на длину, превышающую ширину тонкокишечного «хоботка» на 0,2-0,3 см (при выполнении резекционных вмешательств на толстой кишке расстояние от заглушенного проксимального конца толстой кишки до линии анастомоза должно составлять 5-6 см). Приступают к формированию анастомоза однорядными узловыми швами, каждый из которых накладывают следующим образом: первым этапом производят продольное серозно-мышечное прошивание стенки тонкой кишки шириной 0,5-0,6 см по проксимальной границе «хоботка» (3), вторым этапом на расстоянии 0,5-0,6 см от свободного края «хоботка» осуществляют серозно-подслизистое прошивание стенки кишки с выколом в свободный край «хоботка» (4), третьим этапом захватывают в шов стенку толстой кишки краевым подслизисто-серозным прошиванием шириной 0,5-0,6 см (5). В целях обеспечения контроля за процессом выворачивания тонкокишечного «хоботка», гемостазом и полнотой адаптации слизисто-подслизистых слоев тонкой и толстой кишок в качестве первых швов накладывают два шва с обеих сторон от брыжейки «хоботка» (6) и один шов - по его противобрыжеечному краю (7). Следующим этапом указанными швами формируют сначала переднюю, затем заднюю губы анастомоза. В итоге формируется однорядный концебоковой поперечный инвагинационный тонкотолстокишечный анастомоз, обладающий надежностью гемостаза, адекватностью процесса выворачивания погруженного в просвет толстой кишки тонкокишечного «хоботка», полнотой адаптации слизисто-подслизистых слоев обеих кишок и достаточными клапанными свойствами.The method is as follows. In order to provide control over the process of inverting the small intestine "proboscis", hemostasis and the full adaptation of the mucous-submucous layers of the small and large intestines, two sutures are placed on both sides of the proboscis mesentery (6) and one suture along its mesenteric edge (7). The next step, these seams form first the front, then the back lips of the anastomosis. As a result, a single-row transverse lateral invagination thin-colonic anastomosis is formed, which has the reliability of hemostasis, the adequacy of the process of eversion of the small intestine “proboscis” immersed in the lumen of the colon, the complete adaptation of the mucous-submucous layers of both intestines and sufficient valve properties.
Способ применен в клинике у 8 больных. Осложнений со стороны анастомоза не отмечено ни в одном случае.The method was used in a clinic in 8 patients. No complications from the anastomosis were noted in any case.
