Abstract
Description
The invention relates to medicine, namely to vascular surgery, and can be used for surgical correction of relative insufficiency of venous valves.
It is known that the main method for eliminating the relative insufficiency of venous valves is their extravasal correction. There are many ways of its practical implementation, the most effective and common of which are the method of extravasal correction of wireframe spirals according to A.N. Vedensky and the method of three ligatures.
However, the variant of the anatomical location of the venous valve, which consists in its localization directly above the mouth of a large vein, the ligation of which is undesirable, does not allow to apply any of the existing methods of extravasal correction. Such an anatomical situation really arises for a surgeon operating on a femoral vein valve located in the projection of the mouth of the deep vein of the thigh.
The purpose of the invention is to increase the possibilities of adequate correction relative to insufficient valves located in the mouths of large venous tributaries, not subject to ligation.
The method is as follows.
The area of confluence of large veins with the relatively insufficient valve to be corrected in one of them is isolated from the surrounding tissues according to the generally accepted technology for this anatomical region. Having discovered the anatomical variant of the location of this valve, which consists in its localization over the mouth of a large inflow, they block blood flow through both veins using three provisional turnstiles (one above the confluence, two below the valve and 1 cm below the mouth of the inflow) with preliminary displacement of blood from the blocked plot of veins. A fragment of the corrector A. N. Vedensky (1.5–2 turns based) was extravasally established on the rest of the inflow. The corrector to the venous inflow can not be hemmed. Then, the correction of the relatively relatively insufficient valve by three extravasal ligatures is performed. Two ligatures in the base and middle of the valve are installed as in the three-ligature method, and the third half-ligature above the tops of the valves and commissures is applied in the form of a half ring, which is fixed to the rigid corrector in the rest of the inflow with capture of the adventitia vein. Both lower ligatures and the upper half-ligatures are superimposed and tied on the calibration cylinder of the previously selected diameter as in the three-ligature method, after which they must be fixed to the vein by stitching to the adventitia along the front wall with separate interrupted sutures.
An example of the method
Patient G.T.G., 38 years old, and / b 1373/98 was admitted to the NIIPK 05.05.98, for varicose veins art. 2 with lesions in the pool of the left large saphenous vein, a combination of relative insufficiency with commissural defects of the supostalally located proximal valve of the left femoral vein, flounder and calf perforators on the left (by ultrasound of the veins of the right lower limb from 04/19/98).
12.05.98 г. 05/12/98, a large saphenous vein (BPV) was removed on the left, commissural suture, and extravasal correction of the supostalally located proximal femoral vein valve using the described method, subfascial ligation of insolvent shin perforants from separate accesses. The course of the postoperative period is not complicated; she was discharged under the supervision of a surgeon home on 05/25/98.
A follow-up examination dated 06.06.98 with an ultrasound scan with color mapping of the veins of the left lower extremity: no relapse, the proximal femoral vein valve located in the extravasal corrector is consistent.
The proposed method for extravasal correction allows for the effective correction of the relative insufficiency of venous valves located at the mouths of large venous tributaries, the ligation and intersection of which are undesirable.
