Изобретение относится к медицине, а именно к травматологии и нейрохирургии и предназначено для лечения больных с травматическим повреждением позвоночника, а именно с компрессионным переломом тел грудных и поясничных позвонков. При переломах позвоночника достаточно большой процент занимает клиновидная деформация тела позвонка с наибольшей компрессией переднего его отдела, с возникновением кифотической деформации позвоночника и деформацией позвоночного канала на уровне перелома. Трудности лечения таких переломов заключаются в недостаточной разгрузке позвоночника на уровне поврежденного и смежных с ним позвонков, в достижении адекватной реклинации переднего отдела поврежденного позвонка, вследствие чего сохраняется кифотическая деформация позвоночника и деформация позвоночного канала в зоне поврежденного позвонка и последующее развитие дегенеративных процессов в спинном мозге. Поставленная задача достигается тем, что в способе хирургического лечения больных с травматическим повреждением позвонков, включающем установку крюков-упоров под дуги позвонков на 2-3 выше и 2-3 ниже поврежденного с двух сторон от остистых отростков, соединение крюков-упоров со стержнями дистракторов и дистракцию позвоночника, предварительно на стержни дистракторов устанавливают захваты и гайки с двух сторон от них, затем через захваты в смежные с поврежденным позвонки ввинчивают транспедункулярно шурупы и производят реклинацию поврежденного позвонка перемещением захватов вдоль стержня в направлении один к другому вращением гаек. Способ осуществляют следующим образом. The method is as follows.
Производят разрез мягких тканей от остистого отростка позвонка, расположенного на 2-3 выше поврежденного до остистого отростка позвонка, расположенного на 2-3 ниже поврежденного позвонка. На этом протяжении скелетируют остистые отростки и дуги позвонков. Под дуги крайних позвонков 1 и 2 с двух сторон от остистых отростков (фиг.1,2) устанавливают крюки-упоры 3 и 4 соединяют со стержнями дистракторов 5 и 6, на которых предварительно установлены захваты 7 с гайками 8 с двух сторон от них. Выполняют дистракцию позвоночника, путем перемещения крюков-упоров 3 и 4 по стержням дистракторов 5 и 6. Производят рентгенконтроль, по которому судят о достаточности выполненной дистракции. Затем через захваты 7 транспедункулярно в позвонки 9 и 10, смежные с поврежденным 11 ввинчивают шурупы 12 и перемещают захваты 7 по стержням дистракторов 5 и 6 навстречу друг другу вращением гаек 8 (фиг.2). При этом происходит реклинация переднего отдела тела поврежденного позвонка 11. Выполняют рентгенконтроль. При достижении адекватной реклинации тела поврежденного позвонка (восстановление его конфигурации на контрольных рентгенограммах), производят фиксацию всех захватов 7 и крюков-упоров 3 и 4 гайками. После этого рану послойно ушивают. На вторые сутки после операции больного переводят в вертикальное положение, что позволяет проводить раннюю его активизацию. Под дуги 11-го грудного и 3-го поясничного позвонков с двух сторон от остистых отростков подведены крюки-упоры, которые затем соединены стержнями дистракторов, на которых установлены заранее захваты и гайки с двух сторон от них. Выполнена дистракция позвоночника и рентгенконтроль, выявлено, что степень дистракции достаточная, однако сохраняется клиновидная деформация тела 1-го поясничного позвонка на одну треть. Через захваты в позвонки, смежные с поврежденным, ввинчены транспедункулярно шурупы. Путем перемещения захватов гайками на встречу друг другу выполнена реклинация тела 1-го поясничного позвонка. Выполнена миелография амнипаком, проходимость спинно-мозгового канала полностью восстановлена, деформация тела поврежденного позвонка устранена. Произведена фиксация крюков-упоров и захватов гайками. Контроль гемостаза. Послойные швы на рану. На вторые сутки после операции больная переведена в вертикальное положение. Больная выписана через три недели после операции с полным регрессом неврологической симптоматики. Транспедункулярно через захваты в позвонки, смежные с поврежденным ввинчены шурупы, путем перемещения захватов гайками на встречу друг другу выполнена реклинация 1-го поясничного позвонка. Рентгенконтроль - конфигурация тела 1-го поясничного позвонка практически полностью восстановлена. Выполнена миелография амнипаком. Проходимость спинно-мозгового канала полностью восстановлена. Произведена фиксация всех крюков-упоров и захватов гайками. Послойные швы на рану. На вторые сутки после операции больной переведен в вертикальное положение. Через один месяц больной выписан с полным регрессом неврологической симптоматики. Медико-клиническая эффективность способа заключается в:
- сокращении сроков лечения, обусловленном ранней активизацией больного (возможность перевода больного в вертикальное положение на вторые сутки после операции);
- возможности устранения деформации позвоночного канала и восстановления тем самым нормальных условий для спинного мозга, ведущих к ускорению репаративных процессов в нем;
- сокращении процента инвалидности.The medical and clinical effectiveness of the method consists in:
