Abstract
Description
The invention relates to medicine and can be used in dermatology and cosmetology.
Acne vulgaris is the most common complication of seborrhea and is a purulent inflammation of the sebaceous gland of the periglandular tissue. According to various sources, acne vulgaris of one or another severity are observed in 40-60% of young people. Different methods have been proposed for the treatment of acne vulgaris: antibiotic therapy, immunostimulating therapy, rational external treatment, vitamin therapy, UV radiation, etc. Antibiotic therapy is a well-known method of treating acne vulgaris. The task of treating acne vulgaris cannot be solved by changing antibiotics, as well as their combined use. All this necessitates the development of effective methods of treating this pathology.
As a prototype, a method for treating acne vulgaris using an antibiotic tetracycline was chosen. Tetracycline gives a noticeable effect on acne vulgaris, acting bacteriostatically and reducing the secretion of the sebaceous glands. The advantage of this antibiotic is also a wide range of its antibacterial action and the possibility of oral administration.
The essence of the method lies in the fact that patients are prescribed tetracycline treatment intramuscularly at 1 00 00 IU / day for 30 days. If necessary, antibiotic treatment is repeated after 1-2 months. At the same time, for the purpose of prevention, patients are prescribed nystatin 500,000
In parallel, in the treatment of acne vulgaris, external use of anti-inflammatory, disinfectants and UV radiation was used.
The disadvantages of the method include the relative duration of treatment, insufficiently expressed clinical effect, the accumulation of tetracycline-resistant strains in the microbial population.
The low efficiency of the applied methods for the treatment of acne vulgaris and its complicated forms necessitates the search for new tools and methods. The invention is aimed at increasing the effectiveness of treatment by stimulating non-specific immunity, reducing treatment time and reducing the frequency of relapses.
The essence of the invention lies in the fact that according to the method of treating acne vulgaris, simultaneously with the use of antibiotics, the patient is injected intravenously with oxidized autoblood daily, moreover, the administration begins with 12 ml of venous blood and 2 ml of a 0.04-0.06% sodium hypochlorite solution through 45-60 s, gradually increasing the ratio of sodium hypochlorite and blood to 6: 1, followed by a return to the original ratio, with a parallel 25-30-minute laser irradiation of blood at a power density of 6-8 mW / cm 2 and a wavelength of 0, 63 μm in echenie 2-3 days and simultaneous action on the affected area of the skin applications with 0.08-0.1% solution of sodium hypochlorite for 30-40 minutes, 4-6 times a day. In complicated forms of the disease, a second course of treatment is prescribed, when the patient is administered autohemotherapy with oxidized blood intramuscularly every other day. The method is implemented in the Krasnodar Regional Dermatovenerologic Dispensary.
Preliminarily, by the electrolysis method of a 0.89% isotonic solution of sodium chloride, sodium hypochlorite is obtained on the apparatus "electrical detoxifier of the body" (EDO-3). The concentration of sodium hypochlorite (NaOCl) is controlled by iodometric titration.
Laser irradiation of blood is carried out using a helium-neon laser device LAT-MP (Riga).
Н. А. According to Yu.N. Shevchenko, M.A. Porai-Kosits, laser irradiation of blood causes an increase in the saturation of blood and tissues with oxygen, in particular singlet.
To substantiate the duration and frequency of the introduction of sodium hypochlorite in order to stimulate the natural resistance of the animal organism in laboratory animals, humoral immunity in laboratory animals was studied.
In the experiment, male mice of F1 hybrids (CBAxC57BL) were used, in an amount of 30 individuals, weighing 21-23 g. Mice were injected once intraperitoneally with a 0.04-0.06% sodium hypochlorite solution in a volume of 0.5 ml / mouse. 24 hours after the last injection of sodium hypochlorite, mice were immunized by introducing sheep erythrocytes into the tail vein. On the 4th day after administration of the antigen, the level of JgM-AOK (antibody-forming cells) was examined.
1. The research results are presented in table 1.
167,8%. As can be seen from the table. 1, the maximum inducing effect on the immune system is exerted by a 0.04-0.06% solution of sodium hypochlorite, when the stimulation of humoral immunity reaches 167.8%.
In the complex, patients are prescribed oral antibacterial therapy (tetracycline, doxycycline), vitamin therapy (B 1 and B 5 ; and B 15 ), pathogenetic treatment (oral administration of drugs acting on gastric secretion: acidin-pepsin, gastric juice, bifungin, vitamin "U" etc.), physiotherapy: UV, UHF, etc.
To conduct the comparative effectiveness of various combinations of blood and sodium hypochlorite in the treatment of acne vulgaris and its complicated forms, a preliminary clinical study was conducted on 50 patients (see Table 2).
6:1. The results obtained allowed the use of a treatment regimen for acne vulgaris and its complicated forms, including the most effective ratio of blood components + sodium hypochlorite, equal to 6: 1.
Having accepted this ratio blood + sodium hypochlorite as the most effective, we conducted a comparative test on 20 patients with laser irradiation of blood for 25-30 minutes, with a power density of 6-8 kW / cm 2 and a wavelength of 0.63 microns for 2-3 days and at the same time applying to the affected areas of the skin applications with a 0.08-0.1% sodium hypochlorite solution for 30-40 minutes, 4-6 times a day.
3. The treatment results are presented in table.3.
Thus, the following parameters and modes of laser irradiation of blood were the most effective therapeutic action for acne vulgaris. The exposure time is 25-30 minutes, the power density is 6-8 mW / cm 2 , the wavelength is 0.63 μm, the course is 2-3 days with the simultaneous exposure of the affected areas of the skin to applications with 0.08-0.1% hypochlorite solution sodium for 30-40 minutes, 4-6 times a day.
The proposed method of treatment was used on 35 patients with various forms of seborrhea complicated by acne vulgaris, with a disease duration from 1 year to 12 years. The control group consisted of 15 people.
In patients with spherical and conglomerate acne, the average duration of the disease was 6.2 years (from 10 months to 12 years).
Conducted traditional treatment of 15 patients (control group) gives only a short-term effect. Of these, 7 patients were treated only with tetracycline, the rest with tetracycline in combination with other drugs (delagil, staphylococcal toxoid, trichopolum, levamisole, gamma globulin, phtivazide, PASK).
A month before the start of the application of the proposed method, patients are completely canceled previously prescribed treatment.
According to clinical signs, the patients were divided into 2 groups: the first group consisted of 17 people with superficial (papulopustular) acne, the second - 18 people with deep (spherical, conglabate, inductive, abscessed and phlegmonous) acne.
The results presented in table 4 show that the proposed method allows to achieve complete cure in 88.8% of patients with pustular and papular rashes, and to improve in 11.2% of patients. In the 2nd group, recovery occurred in 77.7% of patients, and improvement in 22.3% of patients. Deterioration of the process was not observed in any patient.
In the control group, the effect was distributed as follows: recovery occurred in 26.6% of patients, improvement - in 40%, and treatment without effect was observed in 33.4% of patients.
Complex pathogenetic treatment was prescribed for patients with acne vulgaris and its complicated forms, both experimental and control groups (tetracycline in the pustular stage of the process, sedatives, hyposensitizing and choleretic drugs, etc.).
The method was highly effective in all 35 patients. Observation of them continued throughout the year: 9 of them were in remission, 3 sometimes had small, small pustules that disappeared after a short local therapy with sodium hypochlorite.
Thus, the applied method in all cases had a positive therapeutic effect, which indicates its high efficiency. In patients, clinical improvement manifested itself already at the 1st week of treatment by the proposed method.
5. The dynamics of clinical manifestations in patients of the experimental and control groups are presented in Table 5.
Thus, the presented results indicate the high efficiency of the proposed method of treatment compared to traditional methods almost 3 times.

The patient is given daily intravenous autohemotherapy with oxidized blood, and the administration begins with 12 ml of venous blood and 2 ml of a 0.06% sodium hypochlorite solution after 60 s, gradually increasing the ratio of sodium hypochlorite to blood to 6: 1, followed by a return to the original ratio with parallel laser irradiation of blood at a power density of 8 mW / cm 2 and a wavelength of 0.63 μm for 30 minutes for 3 days and the simultaneous exposure to affected areas of the skin of applications with 0.1% sodium hypochlorite solution for 40
Comprehensive treatment also included antibiotic therapy with tetracycline (1.5 g per day for 10 days), vitamins, sedatives, choleretic drugs, etc.
Treatment tolerance is good. Such complex therapy made it possible to achieve a significant improvement in the condition of the skin integument in the lesion foci within 2 weeks.
On the 2nd day after the start of treatment, a complete cessation of the formation of skin elements was noted, mild hyperemia of the skin appeared. On the 4th day, papules and pustules completely resolved. On the 18th day, a full recovery occurred while maintaining the effect of preventive treatment for 12 months. The patient is under dispensary observation with a call frequency of 1 time in 3 months.
