Abstract
Usage: in traumatology. The essence of the invention lies in the fact that the cavity of the synovial bag is opened, the cavity is washed with an antiseptic solution. Then, the entire inner surface of the synovial bag is photocoagulated with a COa laser beam without excision of the latter, then the wound is sutured and drained. The positive effect of the method is to shorten the treatment time and reduce relapse.
Description
Изобретение относитс к области медицины , в частности к травматологии, и может быть использовано дл хирургического лечени хронических локтевых и.коленных бурситов.. ; The invention relates to medicine, in particular to traumatology, and can be used for the surgical treatment of chronic ulnar and knee bursitis ..;
The purpose of the invention is to reduce treatment time by reducing the incidence of postoperative complications and relapses.
The proposed method is carried out as follows.
A skin incision is made over the tumor in the direction coinciding with the axis of the limb. The length of the incision depends on the size of the bursin - the upper and lower poles of the synovial bag are visualized. A wide opening of the bag cavity is made. The cavity is washed with an antiseptic solution (chlorhexidine solution, furatsillin, furagin, etc.), dried. Thoroughly photocoagulate the entire inner surface of the synovial bag with a CO2 laser (use, for example, the Scalpel-1 apparatus). After laser treatment, the wound is sutured, drained with a rubber graduate for 24 hours. A pressure bandage and a plaster cast are applied for 2-3 days. Sutures are removed on days 8–9. Disability periods are from 8 to 15 days, an average of 10.6 days from the time of surgery.
The proposed method facilitates, simplifies the surgical treatment of bursitis, eliminates significant trauma to the surgical area / ensures good homostasis, practically does not lead to the formation of infiltrates in the surgical area - wounds with this method heal with a gentle scar, the likelihood of relapse is reduced, and the period of incapacity for work is reduced.
Example 1. Patient R., 58 years old, was admitted to the hospital with a diagnosis of Chronic bursitis of the right knee joint. A typical skin incision was made above the tumor, the cavity of the synovial bag was opened, washed with a solution of chlorhexedine aqueous 0.05%. The inner surface of the bag is treated with a CO2 laser beam (Scalpel-1 installation). The wound was sutured, drained by a rubber graduate who was removed the next day. Aseptic posture. Plaster cast for 2 days. Sutures were removed 10 days after surgery. Wound healing by primary intention. Discharged from the clinic 15 days after surgery in

Example 2. Patient T., 73 years old, was admitted to the hospital with a diagnosis of Chronic bursitis of the left elbow joint. Surgery: Laser photocoagulation of the inner surface of the synovial sac of the left elbow joint. The operation was performed exactly as described in the previous clinical example. After the operation period was uneventful. Sutures were removed on the 9th day - healing by primary tension. Discharged to work. Examined 13 months after surgery: movement restrictions in the joint, no relapse.
