Description
Det er vel kjent at ved vurdering av et legemiddels verdi er en hurtig aktivitetsstart en av de viktige faktorer av legemidlets totale terapeutiske profil. En tidlig start er viktig. Dette gir rask lindring og fremmer derfor pasientens føyelighet.
Sulindac er et ikke-steroidalt antiinflammatorisk og analgesisk legemiddel (NSAID) som er kjent fra begynnelsen av 1970-årene. Det er krevet i US patentskrift 3 654 349 og markedsført av Merck & Co. Inc. under varemerket CLINORIL.
Sulindac har imidlertid en langsom aktivitetsstart som skyldes dets dårlige løselighet i surt miljø slik som magen.
Sulindac er uløselig i vann under pH 4,5, men dets natriumsalt er meget løselig. Det var derfor antatt at natriumsulindac som natriumsaltene av andre NSAID-forbindelser, for eksempel indomethacin, ville ha en hurtigere biotilgjengelighet og meget kortere aktivitetsstart enn moderforbindelsen, sulindac i seg selv.
Ganske overraskende ble ingen merkbar forskjell i starten funnet mellom sulindactabletter og den vandige løsning av natriumsulindac til tross for utallige forsøk. Basert på historiske data utført på mennesker, utviste natriumsulindac gitt som en oral løsning ingen forbedret aktivitetsstart sam-menlignet med sulindac gitt som en tablett, 10 mg/ml eller 40 mg/ml oral suspensjon, en rectal stikkpille, en intramusku-lær injeksjonssuspensjon eller selv som dens aktive metabolitt (sulindac-sulfid).
Tromethamin eller natriumsalter av indomethacin, naproxen og aspirin er kjente (britisk patent 2 059 768 og tysk patent 134 672). Disse salter som beskrevet i disse patenter, utviser forbedret biotilgjengelighet av legemidlet. Det var således fullstendig uventet at ingen slik konvensjonell forbedring med hensyn til rask tilgjengelighet eller aktivitetsstart ble observert for natriumsulindac og sulindac-tromethamin-saltet når disse ble testet for seg selv.
Selv om natriumsulindac, natriumabicarbonat og tromethamin er kjent, er det ikke kjent noen kombinasjon av disse salter.
Når imidlertid en tilstrekkelig mengde av base slik som tromethamin og/eller natriumbicarbonat kombineres med natriumsulindac, oppstår en uventet forbedring når det gjelder biotilgjengelighet og en tidligere aktivitetsstart.
Det er derfor et mål med oppfinnelsen å tilveiebringe en ny kombinasjon av sulindac eller natriumsulindac og en base med en hurtigere absorpsjon og aktivitetsstart, kortere tid til toppaktivitet og forbedret biotilgjengelighet enn sulindac eller natriumsulindac alene.
Det er også et mål ved oppfinnelsen å tilveiebringe et farmasøytisk preparat av denne kombinasjon for behandling av inflammasjon og smerte.
Sluttelig er det et mål ved oppfinnelsen å utvikle en metode for behandling, omfattende administrering av den nye kombinasjon til pasienter som lider av inflammasjon og smerte.
Foreliggende oppfinnelse er rettet mot en kombinasjon av sulindac eller natriumsulindac og en base som kan være tromethamin eller natriumbicarbonat eller en kombinasjon derav. Basen er tilstede i en overskuddsmengde som vil opprettholde legemidlet oppløst i et surt miljø slik som magen, og således forbedre absorpsjonen og forkorte tiden til toppaktivitet ved oppløsning og avgivelse av sulindac fra magen inn i duodenum for effektiv og hurtig absorpsjon av kroppssystemet.
Det er et NSAID-legemiddel med lang virkningsvarighet og er et reduksjons-oxydasjonspro-legemiddel hvis in vivo aktivitet skyldes dets metabolitt, det aktive sulfid av formel (A<1>).
Tromethamin er representert ved formelen (B): NH2C(CH2OH)^; og natriumbicarbonat har formelen: NaHCO^.
Kombinasjonen anvendes i en mengde tilstrekkelig til
å behandle inflammasjon og smerte. Normalt vil kombinasjonen inneholde den aktive bestanddel, nemlig sulindac eller natrium-sulindac i. en mengde på 2 mg til 20 mg pr. kg kroppsvekt pr.
dag (ca. 100 mg til 1 g pr. pasient pr. dag), og eksempelvis oppnås den terapeutiske effekt ved oral administrering av en daglig dose på fra 200 til 600 mg/dag. Det skal imidlertid for-ståes at selv om de ovenfor angitte doseringsområder er angitt, vil doseringsnivået for en hvilken som helst bestemt pasient avhenge av alder, kroppsvekt, kjønn, administreringstid, administreringsmåte, grad av utskillelse, legemiddelkombinasjon, reaksjonssensibilitet og strenghet av pasientens sykdom.
For kombinasjoner inneholdende natriumsulindac eller sulindac og tromethamin er vektforholdet 25 mg til 1000 mg tromethamin til 25 mg til 750 mg sulindac eller natriumsulindac. Fortrinnsvis blandes 75 - 750 mg tromethamin med 50 - 450 mg sulindac eller natriumsulindac. Enda mer fordelaktig kombineres 150 til 700 mg tromethamin med 212 til 424 mg natrium-sulindac (svarende til 200 til 400 mg fri syre av sulindac). Det skal bemerkes at sammenpressede tabletter inneholdende natriumsulindac og tromethamin ikke oppbrytes eller oppløses hurtig ved simulerte mage-pH-verdier. Tilsetningen av 0,5 til 16 % av tablettvekten av enkelte vanlig anvendte oppbrytende midler eller super-oppbrytende midler, f.eks. Ac-Di-Sol<®>og Explotab<®>mislykkes i å bevirke hurtig oppbrytning og oppløs-ning i nærvær av det vanlig anvendte smøremidlet magnesium-stearat.
For å forbedre oppbrytningsgraden og oppløsningen omfatter kombinasjonen i tillegg til tromethamin og natrium-sulindac, 2-10 vekt% glycerylbehenat. For oppnåelse av enda mere forbedrede resultater tilsettes 2-10 vekt% maisstivelse og/eller 0,1 - 0,5 vekt% docusat-natrium. Følgende er en representativ kombinasjon av natriumsulindac og tromethamin.
For kombinasjoner inneholdende natriumsulindac og natriumbicarbonat er mengden av natriumbicarbonat ca. 0,8 g til 2,0 g som er den mengde som kreves for å nøytralisere magen og derved tillate tømming av natriumsulindac fra magen til tynntarmen som en løsning.
Den etterfølgende er en representativ kombinasjon
av natriumsulindac og natriumbicarbonat:
natriumsulindac 0,212 g (ekvivalent med
200 mg sulindac) natriumbicarbonat - 1,460 g
Selv om den foretrukne mengde av natriumsulindac er 0,212 til 0,424 g kan den variere fra 0,106 til 0,636 g.
Oppfinnelsen angår også en metode for behandling av inflammasjon og smerte hos pasienter under anvendelse av kombinasjonen. Eksempelvis kan kombinasjonen ifølge oppfinnelsen anvendes for å behandle sykdommer slik som rheumatoid arthritis, osteoarthritis, gikt, infektiøs arthritis og reumatisk feber. Den har en hurtigere og bedre biotilgjengelighet, kortere aktivitetsstart, kortere tid til toppaktivitet enn sulindac eller natriumsulindac alene, og utviser en mindre hyppighet av brekninger og andre ugunstige bivirkninger enn moderforbindelsen i seg selv.
Behandlingen av inflamamsjon og smerte kan utføres ved oral administrering til pasienter av en terapeutisk dose av kombinasjonen, i særdeleshet i en ikke-toksisk, farmasøytisk akseptabel bærer, fortrinnsvis i tablett- eller oppløsnings-f orm.
Den ikke-toksiske, farmasøytiske bærer kan for eksempel være fast eller væskeformig. Eksempler på faste bærere er lactose, maisstivelse, gelatin, talkum, sterotex, glyceryl-behenat, terra alba, docusot-natrium, sucrose, agar, pectin, cab-o-sil og acacia.
Eksempler på væskeformige bærere er peanøttolje, olivenolje, sesamolje og vann, fortrinnsvis vann.
Kombinasjonene kan fremstilles ved blanding av
sulindac eller natriumsulindac, en base og et egnet bindings-middel i en egnet blander og granulering ved tilsetning av en alkohol slik som vannfri SD3A alkohol. Den våte masse siktes, males, tørkes, siktes igjen og blandes med oppbrytende midler og smøremidler før sammenpresning.
De etterfølgende eksempler illustrerer oppløsnings-profilene av kombinasjonene:
Eksempel 1
Sammensetningen for 212 mg sulindac-natrium med tromethamin (TRIS) inneholdt 70 mg glycerylbehenat, 2 mg docusat-natrium og 60 mg maisstivelse. Formuleringen hadde følgende oppløsningsprofiler (USP ved 50 oiflidr. pr. min.) i 900 ml 0,6 milliekvivalenter og 2,45 milliekvivalenter HCl-media.
Eksempel 2
Effekten av glyceryl-behenat, docusatnatrium og/eller maisstivelse på kombinasjonen av sulindacnatrium (424 mg) og tromethamin (295 mg) er illustrert i den følgende tabell:
a. A = natrium-sulindac (424 mg)/tromethamin (295 mg)
b. B = natrium-sulindac (424 mg)/tromethamin (292 mg) +
4 vekt% glycerylbehenat
c. C = natrium-sulindac (424 mg)/tromethamin (295 mg) + 4 vekt% glyceryl-behenat + 0,2 vekt% docusat-natrium
d. D = sulindac (424 mg)/tromethamin (295 mg) + 4 vekt% glyceryl-behenat + 0,2 vekt% docusat + 5 % maisstivelse
Eksempel 3
Dette var en umerket, enkelt-dose, to dagers kryss-studie i 18 friske personer. 16 fullførte følgende behandling. Behandlingsdagene ble adskilt av minst 7 dager.
Behandling A: En CLINORIL (fremstilt av Merck & Co.
Inc.) tablett inneholdende 400 mg sulindac
Behandling B: En natriumsulindac-tromethamin-tablett inneholdende 424 mg natriumsulindac og 295 mg tromethamin
Behandling C: En natriumsulindac-tromethamintablett inneholdende 42 4 mg sulindac og 66 0 mg tromethamin
Behandling D: Løsning inneholdende 42 4 mg natrium-sulindac og 660 mg tromethamin Plasmaprøver ble oppsamlet ved 0, 0,25, 0,5, 0,75,
1, 1,5, 2, 3, 4 og 6 timer. Komplette urinprøver ble oppsamlet for følgende intervaller: -1 til 0 time (kontroll), 0-2, 2 - 4, 4 - 6, 6 - 24, 24 - 48, 48 - 72 og 72 - 96 timer. Alle prøver ble holdt fryst inntil analysetidspunktet.
Tabell 1 angir de fysiokjemiko-kjemikalske egenskaper av de to doseringsformer. Tabellene 2 angir de farmako-kinetiske parametre. Basert på de viste data er det innlysende at den total uringjenvinning av sulindac er sammenlignbar mellom de fire behandlinger. Tid-til-plasmatopp indikerer imidlertid en forbedring i absorpsjonsnivå og grad. Graden av absorpsjon, som bedømt ved de totale uringjenvinninger, ble ikke påvirket.
Behandling B - Tablett inneholdende 424 mg natriumsulindac
og 29 6 mg tromethamin.
Behandling C - Tablett inneholdende 424 mg natriumsulindac
og 6 60 mg tromethamin.
Behandling D- Tablett inneholdende 424 mg natriumsulindac og 66 0 mg tromethamin.
Alle verdier er middelverdi + S.D. for 18 personer

Behandling B - Tablett inneholdende 424 mg natriumsulindac og 296 mg
tromethamin.
Behandling C - Tablett inneholdende 424 mg natriumsulindac og 660 mg

Behandling D - Løsning inneholdende 424 mg natriumsulindac og 660 mg tromethamin
I nærvær av tromethamin ble både topp-plasmanivåer og topptid av sulindac etter oral administrering forbedret, mens uringjenvinning av sulindac forble uforandret. Disse resultater viser at tiden til topp-biotilgjengelighet av sulindac-natrium/tromethaminblandingen er fra 1,2 timer til 1,4 timer kortere enn den for sulindac alene, slik at en tidlig klinisk start med sulindacnatrium/tromethaminblandingen er forutsagt.
