i j Øket sebumsekresjon er kjent å ha tilknytning til tilstander' såsom fett hår og skalp og Acne Vulgaris, en kronisk forstyr-relse av pilosebakusenheter som primært innebefatter ansii k- :! tet, brystet og ryggen. Sebum produseres av sebakuskjertlene, som normalt bare finnes i forbindelse med hårfoliklene (der-j
i for uttrykket pilosebakus). Kjertelen danner kontinuerlig en kompleks fettblanding som er kjent som sebum, hvilken! i finner vei opp hårfoliklen.

<!>Sykdommer såsom akne kommer ofte når det produseres overskudd av sebum, når pilosebakusfolikularåpningene er for små til j at sebum kan passere, eller når begge tilstander foreligger.;
i ! Dertil viser aknelesjoner i allminnelighet Propionibacterium acnes ved dyrkning. P. acnes viser seg å være hovedkilden til lipolytiske enzymer som hydrolyserer triglyceridene i sebum og danner irriterende frie fettsyrer.
Den primære lesjon i akne er comedo. Det åpne comedo (svar-hode) består av en fast masse av ke.ratin og sebum som blok^ kerer og utvider folikulærporen. Den øvre del av svarthodet blir mørkere ved langsomme oksydasjonsforandringer (ikke av skitt), de nedre deler er hvite. Den lukkede comedo (hvit-
,hode) som er en oppsamling av keratin og sebum med den folikulære åpning blokkert.
Det er fremstilt mange midler for behandling av tilstander som har sammenheng med øket sebumsekresjon såsom fet hode-i
bunn, fett hår og akne. Rensemidler såsom slipemidler, astrihg-
enter, shampoos og spesielle såper har vært brukt, men disse fjerner imidlertid bare overflatelipider. Disse midler får huden, hodebunnen og håret temporært til å se mindre fet ut, men har bare en forbigående virkning. Topiske tørkemidlef såsom svovel, resorcinol, salicylsyre har vært brukt for|
r behandling av akne i forskjellige melker og kremer. Deres virkeevne ligger bare i evnen til å fremkalle erytema (r^d-farging) og dyskvamasjon (avskalling), og for å få eksister-ende hudormer, papeler og pusteler til å gå av.
Antibiotika har vært blant de mest effektive i behandling av akne. Langtidsbruk av antibiotika er imidlertid utilfrleds<->stillende av flere grunner, hvorav den mest fremtredende!
er fare for utvikling av resistente organismer som da ville være immune mot behandling av antibiotika i andre situasjoner. Antibiotisk symptomiatisk terapi av akne har også værtkarakterisertav en ny oppflamming av symptomer og gir tilbake-fall når behandlingen avbrytes. Det er også fare for sensitivt serende reaksjoner overfor antibiotika som kan opptre i
noen tilfeller.

i Det er også tidligere kjent at stilbenderivater og retinp
syrer har vært brukt for å behandle hudsykdommer, se f.eks. US patent 4.326.055. Retinoider virker ved å øke folikulær epitelproduksjon av ikke-heftende celler som derved løsner og kvitter seg med comedoinnholdene. Retinoider har også| blitt vist å redusere sebakuskjertelstørrelsen og inhibere sebumsekresjon. Disse retinoider kan forårsake uheldige i bivirkninger såsom hypervitaminose.

Som ovenfor nevnt er det kjent at inhibering av sebumproduk-' sjon og/eller sekresjon er virksom i behandling og/eller forebygning av slike tilstander som akne. Øket sebumsekresjort gir også slike hudtilstander som seborrhe innebefattet flass, fet hud, fett hår, talgknopper i huden og hudormer.
Ifølge foreliggende oppfinnelse har man funnet at véd topisk administrering av forbindelsen med formel I ovenfor reduéeres sebumsekresjonen. Foreliggende oppfinnelse er basert på eiet foregående funn og vedrører derfor topiske blandinger inne-
holdende forbindelsen med formel I, fremstillingen av slike
i blandinger og bruken av slike blandinger enten i profylakse mot tilstander som forårsakes av sebumsekresjonsoverskudd
i såsom akne eller fet skalp og hår eller i behandling derav.
De ifølge foreliggende oppfinnelse tilveiebragte blandinger
er topisk virksomme ved reduksjon av sebumsekresjon, spesiiel<t>med hensyn til å bekjempe akne og som overraskende ikke har noen av de skadelige bibirkninger som følger tidligere kjient<s>forbindelser. Forbindelsen i foreliggende oppfinnelse fører ikke til bivirkninger forårsaket av tidligere kjente medikamenter såsom toksisitet, arrdannelse eller hypervitaminose. Videre fører bruk av forbindelsen med formel I ovenfor ikke

til sensitivitetsreaksjoner eller utvikling av resistente organismer. Denne forbindelse i motsetning til noen andre medikamenter som er brukt for å behandle akne er ikke teratogen og virker ikke irriterende på huden.
i i Ifølge foreliggende oppfinnelse kan forbindelsen med formIelI anvendes som et middel for å redusere sebumsekresjon og er derfor nyttig som profylakse eller middel for behandling|av tilstander såsom akne når den gis i topiske blandinger og metoder. Blandingene i oppfinnelsen inneholder forbindelsen med formel I sammen med et fordragelig bærermateriale.Ethvert vanlig bærermateriale kan anvendes. Bærermaterialet kan være

'vis som salver, tinkturer, kremer, geler, løsninger, me lii, sprays, suspensjoner, shampoos, hårsåper, parfymer o.l. Faktisk kan enhver vanlig blanding som brukes for påføring på hodebunn eller hud anvendes ifølge oppfinnelsen. Blant!
de foretrukne påføringsmetoder av blandingen inneholdendé forbindelsen med formel I er anvendelse av geler, melk og kremer. Preparat for topisk administrering på hud kan frem-stilles ved å blande den foran nevnte aktive bestanddel med ikke toksiske, inerte, faste eller flytende bærere som vanlig brukes i slike preparater. Disse preparater bør minst inne-holde ca. 0, 0005 vekt% av den aktive bestanddel basert på. blandingens samlede vekt. Da den aktive bestanddel, forbindelsen med formel I,ikke er toksisk, ikke teratogen og ikke irriterende kan den brukes i topiske blandinger i mengdel betydelig utover 10%. Imidlertid gir bruk av forbindelsen med formel I i større mengder enn 10% ingen vesentlig gevinst utover det som oppnås opptil 10%. Det foretrekkes derfor at disse preparater inneholder ca. 0,01 til 10 vekt% og den aktive bestanddel basert på blandingens totalvekt.
Det foretrekkes også å påføre disse preparatene en gang eller to ganger daglig på huden. Disse preparater kan påføres av-hengig av den enkeltes behov. Ved utførelse av oppfinnelsen kan den ovennevnte bestanddel påføres i en vandig løsning eller en alkoholløsning såsom etylalkohol.
Under fremstilling av de ovenfor beskrevne topiske preparater kan additiver såsom preserveringsmidler, fortykningsmidlér, parfymer o.l. som er brukt på området for formulering av topiske preparater brukes. Dertil kan vanlige antioksydanter eller blandinger av vanlige antioksydanter inngå i de topiske preparater som inneholder den forannevnte aktive bestanddi el, il Blant de vanlige antioksydanter som kan brukes i disse preparater er N-metyl-X-tokoferolamin, tokoferoler, butylert liyd-roksyanisol, butylert hydroksytoluen, etoksykin o.l. Krem-baserte formuleringer inneholdende den aktive bestanddel som brukes ifølge foreliggende oppfinnelse består av vandige 'emulsjoner som fortrinnsvis inneholder en fettsyrealkohol, halvfast petroleumshydrokarbon, 1,2-etylenglykol og e.t emul geringsmiddel.
Salvef ormuleringer som inneholder den aktive bestanddel ijf øle e oppfinnelsen omfatter blandinger av et halvfast petroleumshycrc karbon med en løsningsmiddeldispersjon av det aktive materia].et Kremblandinger som inneholder den aktive bestanddel for bruk
i denne oppfinnelse omfatter fortrinnsvis emulsjoner dannet fra en vannfase av et fuktemiddel, en viskositetsstabilisator og vann, og oljefase av fettsyrealkohol, et halvfast petroleums hydrokarbon og et emulgeringsmiddel og en fase inneholdende den aktive bestanddel dispergert i en vandig stabilisator-pufferløsning. Stabilisatorer kan tilsettes det topiske preparat. Alle vanlige stabilisatorer kan anvendes ifølge oppfinnelsen. I oljefasen virker fettsyrealkoholbestanddelerj som en stabilisator. Disse fettsyrealkoholbéstanddeler avledes fra reduksjon av en langkjedet mettet fettsyre med minst 14 karbonatomer. Også vanlige parfymer og melk som vanligvis brukes i topiske preparater for håret kan anvendes ifølge oppfinnelsen. Videre kan om ønsket vanlige emulgeringsmidler benyttes i de topiske preparater i denne oppfinnelse.
Doseringen for behandling avhenger gjerne av individets tilstand .
Denne forbindelsen med formel I er effektiv for bruk mot akne
• i hos forskjellige pattedyr slik det fremgår fra de følgende forsøk: A) Virkningen av forbindelsen Ia på sebakuskjertler hos pattedyr ble målt ved å bruke hamstere ifølge den følgende fremgangsmåte. Hanhamstere ble kastrert 8 uker gamle og én uke atter operasjonen ble de behandlet med 4 p. g testosteronpropionat og forbindelsen Ia topisk i aceton på flankeorganene. Dyrene ble behandlet 14 dager og reaksjonen ble målt ved vektforand::-ing i flankeorganer fjernet ved autopsi. Separate grupper av hamstere tjente som bæremiddelkon troller og testosteron alenegrupper. Aktiviteten ble målt ved inhibering av vekt-økningen som opptrer som reaksjon på stimulering med testosteronpropionat. Resultatene er vist i tabell I.
p<0,001, NS.= ikke signifikant sammenlignet med gruppen som får T.P. (testosteronpropionat) alene.

Man ser fra disse resultatene at når forbindelsen Ia ble gitt i en dose på Q.,5 ug/dyr/dag topisk sammen med 5 jig T.P. topisk fikk man ca. 3 8% inhibering av flankeorganvektøkning sammLn-lignet med flankeorganvekten til hamstere som ble behandlet med testosteron alene.
B) Virkningen av forbindelsen Ia på sebakuskjertlene hos pattedyr ble videre bestemt ved å anvende ørkenrotter i hénhcld til følgende fremgangsmåte. Hanørkenrotter ble kastrert 8j uker gamle og en uke etter operasjonen ble de behandlet m!ed 5 ug fritt testosteron og forbindelsen Ia topisk i aceton) på
i
de abdominale sebakuskjertelputeorganer. Dyrene ble behandlet i 14 dager og reaksjonen ble målt ved vektforandring i disse organer fjernet ved autopsy. Separate grupper av ørkenrotter tjente som bæremiddelskontroller og testosteron alenegrupper. Aktiviteten ble målt ved inhibering av vektøkningen som opp-trådde som reaksjon på stimulering med téstosteron. Resultatene er vist i tabell II. i

Man ser fra disse resultater at når forbindelsen Ia blir gitt:
i en dose på 1,0 ug/dyr/dag topisk med 5 ,ug testosteron topisk fikk man ca. 54% inhibering av sebakuskjertelputevekt over nivået for sebumsekresjon når ørkenrotter ble behandlet tLpisk med 5 ;ag testosteron alene.
1,2,3, 4-tetrahydro-l ,1,4 , 4-tetrametyl-6- ( (E).-«<-metylstyryl) naftalen Ia viser ingen uønskede bivirkninger såsom hyperj-vitaminose i passende doseringer. Dette er også tidligere bemerket, se Loeliger et al., "Arotinoids, a new class of highly active retinoids, "Eur. J. Med. Chem., 15, 9 (1980). somV|iser at denne forbindelsen ikke har noen inhiberénde virkning I!på papillomavekst. Denne artikkel viser faktisk at etter xntra-r peritoneal administrering av denne forbindelse, fikk man en økning i papillomstørrelsen i forsøksdyret. Mangel på uønskede bivirkninger illustreres videre ved den følgende fremgangsmåte.
En hypervitaminose A prøve ble utført på mus som veide 25- •
27 g, hvilke fikk i.p. injeksjoner av forbindelsen Ia i arachisolje over en 14 dagers periode. Hypervitaminose A defineres som den tilstand for dyrene hvor symptomene (vekt-tap, huddeskvamsjon, tap av hår, bensprekkes i ekstremitetene). ,som gjør seg opptil minst grad 3, sammenlign W. Bollag, Cancer, il974, 1_0, 731. I dette forsøket gir forbindelse Ia ikke re su).-

tater i hypervitaminose A selv en dose opptil 4 00 mg/kg.

For videre å bestemme sikkerheten av forbindelsen Ia ble teratogenforsøk på fetale abnormaliteter utført i rotter
ifølge den etterfølgende fremgangsmåte. Forbindelse Ia ble
gitt til Charles River CD rotter på drektighetsdagene 8 Cjg 9 jved bruk av en daglig dose på 10, 50 eller 150 mg/kg av denne;
substans oppløst eller oppslemmet i en polyetylenglykolformu-lering inneholdende spor av antioksydanter (askorbylpalmi-(tat og butylert hydroksyanisol) . Forbindelsen ble gitt oralt med et konstant volum på 5 ml/kg. Rottene ble så avlivet på
drektighetsdag 20 og undersøkt. I alle disse forsøk fant man ingen skadelige virkninger på morslegemevektene, absorpsjons-hastigheten eller fetal vekt, heller ingen ytre misdannelser ble funnet i noen av de 187 fetuser, hvilket videre viser sikkerheten av denne forbindelse.
At forbindelsen Ia ikke irriterer huden ble påvist ved den følgende fremgangsmåte. Forbindelse Ia ble påført topisk på ørene til 21 dager gamle Charles River CD-1 hanrotter i / påfølgende dager i 25^1/dag aceton. Under behandlingen obser-veres musene med hensyn til utvikling av erytemer på ørene, og jevne hudstansinger tas ved autopsy den 8. dag. Våt- cg tørrvektene av hudprøvene ble målt og sammenlignet med vekten til bærerbehandlede kontrolldyr. Økninger i væskeinnholdet av skinnstansingene tjente som et mål på ødem når irritasjon og inflammasjon hadde opptrådd. Forbindelse Ia ble vist å ikke ha uheldige virkninger ved topisk påføring i doser oppti]j 100 ;ug/dag, hvilket derved demonstrerte at forbindelsen ia ikke irriterte huden.
Det er helt klart fra det ovenstående at 1,2,3,4-tetrahydro-J 1,1,4,4-tetrametyl-5-((E)-X-metylstyryl)-naftalen Ia tolererés godt uten toksiske symptomer i passende doser og understøtter videre virkningen til det forannevnte naftalenderivat i behandlingen av akne.
'Selv om forbindelse Ia er en kjent forbindelse, beskrives •fremstillingen av den nedenunder for fullstendighets sk.yjjd.
■ 10 g benzaldehyd (nydestillert) ble satt til 60 g trifenyl- •
.(1-(5,6,7,8-tetrahydro-5,5,8,8-tetrametyl-2-naftyl)etyl)fos-
fbniumbromid. Blandingen ble tilbakeløpskokt under argon) i

130 timer. Den derved erholdte klare, gule løsning ble avkjølt og helt i 600 ml metanol/vann (6:4 volumdeler). Blandingen
i ; ble ekstrahert 4 ganger med heksan, den organiske fasen j/as-jket en gang med 'metanol/vann (6:4 volumdeler) og tre ganger med vann, tørket med natriumsulfat og inndampet. Den olje-aktige rest ble filtrert gjennom en kort kiselgelkolonne med heksan inneholdende 5% etylacetat og ble så rekrystallisert fira heksan som ga 1, 2 , 3 , 4-tetrahydro-l, 1, 4 , 4-tetrametyl-5-(|(E)-*<-metylstyryl)naftalen som fargeløse krystaller, smp. !83-85°C.
Claims (1)
1. Topisk blanding for reduksjon av sebumsekresjon hos individer karakterisert ved at den inneholder en virksom mengde 1,2,3,4-tetrahydro-l,1,4,4-tetrametyl-6-K-metylstyryl)naftalen som aktiv bestanddel . og en flordragelig bærer.'
2. Blanding ifølge krav lf karakterisert ved at den inneholder ca. 0,0005 til 10 vekt%, fortrinnsvis fra ca. 0,01 til 10 vekt% 1,2,3,4-tetrahydro-l,1, 4 , 4-tetrametyl-6- (od-metylstyryl) naftalen som aktiv bestanddel og en fordragelig bærer.
3. Blanding ifølge krav 1 eller 2, karakterisert ved at den aktive bestanddel er 1,2,3,4-tetrahydro-1,1,4 , 4-tetrametyl-6- ( (E).-<*i-metylstyryl) naftalen.
4. Fremgangsmåte ved fremstilling av en topisk blanding for reduksjonen av sebumsekresjon karakterisert ved at man som aktiv bestanddel bringoer en virksom mengde av 1,2,3,4-tetrahydro-l,1,4,4-tetra-metyl-6- (^-metylstyryl)-naftalen i en egnet form for topisk administrering.
5. Fremgangsmåte ifølge krav 4, karakt e >.r i - sert ved at man som aktiv bestanddel bringer ca. 0,0005 til 10 vekt%, fortrinnsvis ca. 0,01 til 10 vekt% 1,2,3, 4-tetrahydro-l ,1,4, 4-tetrametyl-6- (o^-metyl-styryl)naftalen i egnet form for topisk administrering.
6. Fremgangsmåte ifølge krav 4 eller 5, karakterisert ved at man som aktiv bestanddel anvender 1,2,3, 4-tetrah<y> dro-l ,1,4, 4-tetramet <y> l-6- ( (E) imetylstyryl ).naf talen.
