Fremgangsmåte for fremstilling av et preparat for å redusere fenylalanininnholdet i blodet. Foreliggende oppfinnelse vedrører en fremgangsmåte for fremstilling av et preparat som er i stand til å redusere fenylalanininnholdet i blodet, eller å redusere fenylalaninkonsen-trasjonen i fenylalaninholdige media. Det er kjent at fenylalanin er en såkalt essentiell aminosyre. I tillegg til at den spiller en rolle i syntesen av proteiner er imidlertid denne aminosyre sammen med tyrosin basis-substans for syntese av katekolaminer;adrenalin, tyroidalhormoner (trijod- og tetra-jodtyrosin) og det physiologiske pigment melanin.
Utgangstrinnet for 'dannelse av disse substanser er oksyda-sjonen av fenylalanin og tyrosin, som foregår i leveren ved innvirkning av fenylalanin-hydroksylase.
Blokkering av denne enzymatiske reaksjon fører til en akkumu-lering av fenylalanin i blodet.
Som et resultat oppstår en alternativ katabolisk rute som etter transaminering av fenylalanin til fenylpyrodruesyre fører til fenyleddiksyre som fjernes gjennom urinveien.
Fra et medisinsk synspunkt er en slik metabolisk blokkering kjent som fenylketonuria, eller fenylpyrodruesyre-sykdom eller også som såkalt Follings sykdom. Det er en medfødt sykdom og er arvelig, overført med en enkel resessiv karakter, som påvirker begge kjønn, spesielt i de nord-europeiske land.
Sykdommen bevirker mental tilbakeståenhet, kramper, eksem og dårlig hudpigmentdannelse.
Et overskudd av fenylalanin endrer på den annen side meta-bolismen av både tryptofan og tyrosin.
Oksydasjon av tryptofan inhiberes med medfølgende spaltning
av indolringen og dannelse av formyl-kynurein, hydroksylering til 5-hydroksytryptofan og dekarboksylering av det sistnevnte til 5-hydroksytryptamin.
Tryptofan må således ha en underordnet metabolisk rute til-gjengelig som medfører utskilling gjennom urinveien av indol-pyrodruesyre, indolmelkesyre og indoleddiksyre i mengder som er enndog 20 ganger større enn mengdene i normale personer.
Et overskudd av fenylalanin inhiberer tyrosinase og hindrer således dannelsen av melanin.
Fenylpyrodruesyre inhiberer i sin tur i det minste delvis oksydasjon av tyrosin til gentisinsyre og dannelse av katekolaminer som skyldes inhibering av dopa-dekarboksylase.
Den mentale patogenese skyldes antagelig, snarere enn en direkte toksisk virkning av fenylalaninet på den cerebrale metabolisme, den reduserte dannelse av de nevrohumorale midler som er essentielle for en normal utvikling av de cerebrale funksjoner, som f.eks. serotonin og katekolaminer.
De sykelige tilstander søkes i dag påvirket gjennom nærings-inntaket.
Det tilpasses en fenylalaninfattig diett hvori fenylalanin generelt må holdes under 10 - 30 milligram daglig pr. kilogram. Protein-hydrolysater med et lavt fenylalanin-prosentinnhold anvendes for dette formål.
Endelig har noen forfattere foregitt at eliminering av fenylalanin kan redusere utviklingen av leukemiceller. Det ville være teoretisk mulig å redusere fenylalanininnholdet i blodet. Det finnes faktisk et enzym, nemlig fenylalanin-ammoniakk-lyase, som omdanner fenylalanin til kanelsyre og ammoniakk.
Det er beklagelig vist umulig å innføre et slikt enzym direkte
i blodstrømmen,, på grunn av den markerte antigene karakter av enzymet som etter en eller to tilførsler forårsaker en hurtig eliminering av selve enzymet.
Det vises i denne forbindelse f.eks. til artikkelen av R.R. Fritz, D.S. Hodgins og C.W. Abell i J. of Biol. Chem. bind 251, side 4646 fra 1976. Disse forfattere har tilført enzymet til for-søksdyr og har sett at de halvveringstider som oppnås med den første tilførsel (22 timer) forkortes betraktelig i. de etter-følgende tilførsler. Nærværet av et antistoff som er spesifikt for enzymet iakttas samtidig.
Det er nå funnet at det er mulig å utnytte dette enzym i blod-strømmen av pasienter som lider av fenylketonuria uten å støte på det nevnte enzym-avstøtende fenomen på grunn av et lang-
varig opphold av enzymet i blodstrømmen.
Enzymet er da blitt fiksert ved hjelp av den metode som er beskrevet i det italienske patentskrift 836.462 i porøse fibre av cellulose-triacetat som er blitt gjort biologisk tålbare ved at det til den polymere fase tilsettes et blodplate-aggregasjonsforhindrende middel som f.eks. 4,5-difenyl-2-bis-(2-hydroksyetyl)aminooksazol. Ved å foreta dette tilveie-bringes et preparat som tillater å regulere konsentrasjonen av det fenylalanin som er tilstede i blodet uten at protein-katalysatoren som anvendes kommer i direkte kontakt med blodet.
I tillegg nedsetter det høye forhold mellom overflate og volum diffusjonseffektene til et minimum, dvs. at adgangen av substratet til de katalytiske punkter skjer hurtig.
Materialet som fremstilles på denne måte har biologisk tålbar-het, dvs. at det kan anvendes uten noen forstyrrende forhold som f.eks. blodplateadhesjon, dannelse av tromber og lignende.
Endelig kan materialet anvendes ved behandling av protein-hydrolysater for anvendelse som næringsmidler for selektivt å redusere fenylalanininnholdene innenfor et område som kan tåles av fenylketonuria-pasienter.
Hittil har fremgangsmåten for fremstilling av næringsmidler tilpasset for forbruk av fenylketonuriapasienter innbefattet omstendelige kromatografiske separasjonstrinn av proteinholdige hydrolysater og etterfølgende rekonstituering av
en passende mengdemessig blanding hvorfra fenylalanin ute-lukkes. Til forskjell herfra gjør den foreliggende oppfinnelse det mulig å fjerne fenylalanin fra proteinholdige hydrolysater ved enkelt å behandle disse med de ovennevnte fibere.
For detaljer vedrørende fremstillingen og bruk av disse mater-ialer vises det til de etterfølgende utførelseseksempler på foretrukne utførelsesformer av oppfinnelsen.
EKSEMPEL 1
Til en løsning sammensatt av 20 g cellulose-triacetat og 7 g 4,5-difenyl-2-bis(2-hydroksyetyl)aminooksazol i 930 ml metylen-klorid tilsettes det ved 2°C 140 ml av en løsning av fenylalanin-ammoniakk-lyase i en buffer av Tris (0,05 M HC1, pH
7,5) som også inneholder 30 % glycerol. Enzymløsningen ble oppnådd ved å ekstrahere enzymet fra Rhodotorula glutinis og rense det ved utfelling med protaminsulfat, ammoniumsulfat og natriumsitrat til en spesifikk aktivitet på 2,1 mikromol/
min. milligram protein.
Aktiviteten av løsningen var 113 U pr. milliliter.
Ved omrøring ble det oppnådd en emulsjon som så ble ekstrudert under nitrogentrykk gjennom en spinnedyse neddykket i et koaguleringsbad av toluen. I toluenbadet ble det ved koaguler-ing av polymeren dannet en fiber som ble opptatt på tromler og behandlet med en nitrogenstrøm for å fjerne spinne-løsnings-midlene.
To gram fiber ble anordnet i form av en ring på veggene av
et nylonrør med en indre diameter på 7 mm og en lengde på 500 mm.
Veggene i nylonrøret var tidligere behandlet i henhold til den metode som er beskrevet i belgisk patentskrift 869.323 inngitt 29. januar 1979 for å gjøre rørveggene ikke-trombo-gene.
En annen enzymatisk reaktor ble fremstilt ved hjelp av samme fremgangsmåte som ovenfor.
De to reaktorer ble forbundet i serie og innsatt i et system bestående av en beholder, termostatisk styrt ved 37°C, og en peristaltisk pumpe og selvfølgelig de to reaktorer.
Beholderen var fylt med 400 ml blod, tilsatt natriumsitrat og fenylalanin (0,7 mikromol pr. milliliter).
Blod ble brakt til å strømme gjennom reaktoren med en strøm-ningstakt på 4 ml pr. sekund. Fenylalanin-blodnivåprøver ble foretatt periodisk. Verdiene som ble oppnådd er gjengitt i denetterfølgende tabell:
Denne tabell viser at under de herskende betingelser fjernes fenylalanin fra blodet i løpet av omtrent 90 minutter. For-søket ble gjentatt 6 ganger på nytt med de samme reaktorer og den totale fjernelse av fenylalanin ble oppnådd i løpet av 90 minutter i hvert tilfelle.
EKSEMPEL 2
To enzymreaktorer anordnet som beskrevet i eksempel 1 og an-brakt i serie ble anvendt for ekstra-korporeal sirkulering ved å ta blod fra femoral-arterien å sende det tilbake til femoralvenen etter at det var blitt bragt til å strømme
gjennom enzymreaktorene.
Før tilkobling av reaktorene var det funnet 0,1 mikromol fenylalanin pr. milliliter i blodet. Etter 120 minutter var fenylalanin-blodinnholdet praktisk null.
EKSEMPEL 3
En fiber ble fremstilt ved hjelp av fremgangsmåten omhandlet i eksempel 1 men med utelatelse av de blodplate-aggregasjons-hindrende midler.
5 g fiber ble innsatt i en termostatstyrt rørreaktor med diameter 40 mm og en høyde på 220 mm. Reaktoren ble tilført kasein-hydrolysat i 0,4 % konsentrasjon i fosfatbuffer
(0,01 M, pH 7,5) med en strømningstakt på 600 milliliter
pr. time. Konsentrasjonen av fenylalanin var 0,62 mikromol pr. milliliter og konsentrasjonen av tyrosin var 0,45 mikromol pr. milliliter.
I utstrømningen fra kolonnen var konsentrasjonen av fenylalanin 0,04 mikromol pr. milliliter, mens konsentrasjonen av tyrosin var 0,05 mikromol pr. milliliter.
Claims (2)
1. Fremgangsmåte for fremstilling av et preparat for å redusere fenylalanininnholdet i blodet eller andre fenylalaninholdige medier,
karakterisert ved at man okkluderer enzymet fenylalanin-ammoniakk-lyase i porøse fibre av cellulosetri-acetat som er blitt gjort biologisk tålbare.
2. Fremgangsmåte som angitt i krav 1, karakterisert ved at fibrene gjøres biologisk tålbare ved tilsetning av et blodplate-aggregasjons-hindrende middel.
