Abstract
Fremgangsmåte ved totalbestemmelse av hormoner og farmasøytika
Description
Foreliggende oppfinnelse vedrører en fremgangsmåte ved totalbestemmelse av hormoner og farmasøytika. Nærmere bestemt ved-rører oppfinnelsén totalbestemmelse av hormoner eller farma-søytika som er bundet partielt til spesifikke eller ikke spesifikke bindende proteiner ved enzymatisk hydrolyse av de bindende proteiner,, omsetning av hormonene eller farmasøytika med et antistoff og radiologisk immunologisk bestemmelse av hormoner eller farmasøytika.
Flere metoder for totalbestemmelse av hormoner ellér farma-søytika som foreligger i humant serum partielt bundet til spesifikke eller ikke spesifikke bindende proteiner er kjente. I senere år har relativt nye radiologisk immunologiske teknikker bidratt vesentlig til den nøyaktige bestemmelse av hormoner.

Innebyggningen av antistoffet i.gelen anser forfåtterene som fordelaktig fordi på denne måten utelukkes påvirkninger med molekyler med høyere mole-kylvekt. Imidlertid er disse kjente arbeidsmetodene ikke ment for totalhormonbestemmelse. Ikke desto mindre er fra et medisinsk synspunkt totalhormonbestemmelse meget viktig.
Metoder for totalbestemmelsen av hormoner, ved enzymatisk hydrolyse av de bindende proteinene og med radiologisk immunologiske teknikker er kjent fra J. Clin. Endocrinol. Metab. 42, 189 , 1976 og Clinical Chemistry, Vol 22, nr .11, 1976 , s. 1850. Disse artiklene viser at en en enzymatisk spaltning har betydelige fordeler sammenlignet med tidligere anvendte metoder som f. eks. løsningsmiddelekstraksjon og varme-denaturering. Spesielt nevnte fordeler er åt ekstraksjonen med organiske løsningsmidler og den kromatografiske rensning, av prøvene ellimineres, at tid og omkostninger ved bestem-elsen reduseres, at scintiliseringstelling i væsken ellimineres, at spesifitet og nøyaktighet forbedres, at bestemmelsesmetoden kan automatiseres og at metoden er universelt anvendelig.
Imidlertid angriper enzymet som brukes for å hydrolysere
det bindende proteinet også antistoffet som innføres i det neste arbeidstrinnet og ødelegger dette. Av denne grunn må enzymet deaktiveres for tilsettningen av dette antistoffet. Inaktivering av en enzymatisk aktivitet er mulig på flere måter, f.eks. kan enzymet denatureres ved en betydelig for-andring i pH-verdien eller ved oppvarming.
Imidlertid er dette denatureringstrinnet meget uheldig. Det krever enten tilsettning av ytterligere kjemikalier som inn-fører enda flere inteferensfaktorer i systemet eller det kreves lange tidsrom for varmedeanturering. Varmeeffekten må vare rundt 5 min., mens kjøleperioden vanligvis strekker seg til rundt 15 min. Dette deantureringstrinnet represen-terer derfor en vesentlig hindring for automatiseringen av bestemmelsen. Videre, avhengig av det anvendte systemet, spesielt det benyttede enzymet, må forskjellige typer dena-turering utføres som forhindrer den generelle anvendelig-heten av bestemmelsesmetoden og automatiseringen av denne.
For å unngå tidligere kjente vanskeligheter tilveiebringer foreliggende oppfinnelse en fremgangsmåte for totalbestemmelse av hormoner og farmasøytika hvor ikke noe denaturerings-trinn er nødvendig. Således er et formål for oppfinnelsen å gjøre bestemmelsesmetoden universelt anvendelig, redusere tid og kostnader for bestemmelsen og gjøre det mulig og automat-isere bestemmelsen.
I følge oppfinnelsen oppnås hensikten ved en fremgangsmåte for totalbestemmelse av hormoner og farmasøytika som er partielt bundet til spesifikke eller ikke spesifikke proteiner ved enzymatisk hydrolyse av de bindende proteinene, omsetning av disse hormonene eller farmasøytika med et antistoff og radiologisk immunologisk bestemmelse av hormonene og farmasøytika,karakterisert vedat det anvendes i mobiliserte antistoffer.
Antistoffet er fortrinnsvis innebygget i en polymergel.. Akrylamid-polymeret og akrylamid-kopolymeret er spesielt egnet som polymergel. Spesifikke fordeler ved kopolymeret er beskrevet mer detaljert i søkerens beslektede ansøkning.
Anvendelsen av immobiliserte antistoffer beskytter antistoffene mot enzymet. Antistoffene er innebygget i polymermatrisen som enzymet p.g.a. sin størrelse ikke kan trenge gjennom. Som resultat av dette enkle og elegante tiltaket i fremgangsmåten i følge oppfinnelsen kan denatureringstrinnet for enzymet ellimineres. Fremgangsmåten innbefatter således betydelig mindre arbeid og krever mindre tid. Fremgangsmåten er universelt anvendelig på flere systemer fordi den spesifikke enzym-denatureringen blir ellimineres. Videre er fremgangsmåten følge oppfinnelsen av særlig betydning da den egner seg spesielt godt for automatisering av bestemmelsen.
Fremgangsmåten i følge oppfinnelsen er egnet for bestemmelse av forskjellige hormoner og farmasøytika som er til stede i serum eller plasma i en tilstand bundet delvis til spelifikke eller ikke spesifikke bindende proteiner. Hormonene som skal bestemmes er tyroidhormoner, spesielt tyroksin og trijodtyrok-sin, steroidehormoner som kortisol, testosteron, progesteron, estron, estradiol og estriol og hjerteglykosidene som ditoksin og digoksin. Videre kan vitaminer, spesielt vita-min B12 og folinsyre samt farmasøytika med sterke protin-bindinger som f. eks. anti-koagulanter, dikumarin, analge-tika og salysilater bestemmes.

I utførelsen av bestemmelsen plasseres serumet eller plasmaet! som inneholder hormonet eller farmasøytikumet i kontakt med en løsning av et enzym hvor enzymet hydrolytisk avspalter det bindende proteinet. Valget av enzym er avhengig av det spesifikke bindingsproteinet. F. eks. kan følgende enzymer anvendes, avhengig av systemet: aminopeptidase, bromelin, karboksy-peptidase,cymotripsin, elastase, ficin, leucinaminope<p>tidase,. lipase, pankreatin, papin, pepsin, pronase, protease, pro-teunase, thermolysin og trypsin. For bestemmelsen av hormoner som thyroksin og kortisol er proteolytiske enzymer som protinase, pronase eller pepsin spesielt egnet.
Prøvens reaksjon med enzymatisk løsning kan utføres på en kontrollert temperaturanordning ved temperaturer rundt 35°C eller ved romtemperatur. Det nødvendige tidsrom for den enzym-atiske hydrolysen av proteinet avhenger av det anvendte enzym og ligger mellom 15 min. og 4 timer, i mange tilfeller mellom 15 og 30 min.
Etter hydrolysen av proteinet blandes prøven med en markert indikator-hapten og settes til det immobiliserte antistoffet.
Tilsettningen utføres ved hjelp av en stempelpum<p>e med flere kanaler. Når pumpen går bakover suges reaksjonsblandingen kvantitativt inn i små kolonner som inneholder det immobiliserte antistoffet. Et spesielt egnet apparat for dette er beskrevet i søkerens paraleltgående ansøkning. Etterpå er det et rointerval for reaksjonen til den markerte og ikke markerte substansen som skal bestemmes med det immobiliserte antistoffet.
En sluttekstraksjon med vann eller en pufferløsning gir separasjonen avhaptenen som er bundet til antistoffet og det frie haptenet. Den resterende radioaktiviteten i ekstraktet eller kolonnen er et mål for konsentrasjonen av substans som skal bestemmes.. Det radiologiske imunologiske prinsippet er kjent, f. eks. fra D. S. Skelley et al, Clinical Chemistry, Vol. 19, nr 2, 1973, s. 146ff.
Alternative bestemmelsesraetoder som fluorimmunologisk bestemmelse eller bestemmelse ved enzymatisk markering kan også anvendes.
Syntesen av antigenér, fremstillingen av antisera, f. eks. ved immunisering av kaniner og issolering og immobilisering av antistoffene er kjente (se f. eks. D. S. Skelley et al., Clinical Chemistry, Vol. 19, nr 2, 1973, s. 146ff.
Immobilise'rte antistoffer frembringes f. eks. ved å sette
en løsning av antistoffet til en monomerblanding. Blandingen utsettes for fri radikal polymerisasjon og den erholdte poly-mer knuses, vaskes og tørkes. Akrylamid er særlig egnet som monoraer. Ved å kopolymisere akrylamidet med akrylderivater med forskjellige funksjonelle grupper som f. eks. akrylsyre, metakrylsyre og metakrylamid kan fordelaktig variasjoner hos matrisen oppnås. Ved tilsetning av kopolymeret, kan særlig matrisens hydrofobeegenskaper og ladning påvirkes og således influeres reaksjonen til antistoffet med hormonet eller farmasøytikumet som skal bestemmes.
For å oppnå en passende porestørrelse hos polymermatrisen varieres monomerkonsentrasjonen. En monomerkonsentrasjon i området rundt 20 % fører til en porestørrelse på ca. 7 - 10 Å.
Bruken av en ionebyttet polymermatrise er også fordelaktig. Puffervirkningen som oppnås ved ionebytting kan brukes for for å endre pH-verdiene i prøven som skal bestemmes. Et fordelaktig system er f. eks. en kopolymer av akrylamid 20 - 60 % akrylsyre og/eller metakrylsyre fremstilt fra rundt 20 monomerløsning hvori minst en del av syregryppen overføres til de tilsvarende alkale eller jordalkaliesalter.
I det følgende skal oppfinnelsen forklares i nærmere detalj ved hjelp av eksempler.
Eksempel I ;
Bestemmelse av tyroksin ved enzymatisk hydrolyse med pronase
Antigensyntesen ble utført ved å begynne med tyroksinetylester og koblet på albumin med karbodimid. Antistoffet ble fremstilt ved immunisering av kaniner.
Antistoffet ble så innebygget i polymergel. En akrylamid-monomer med konsentrasjon på 2,9 mol pr. 1 ble anvendt. For utgangsblandingen ble 5 g akrylamid og 1,25 g N,N'- metylen-bis-akrylamid, oppløst i et begerglass i 24 ml fosfatpuffer med en pH på 7,2. Etter tilsettning av antiserum i 1 ml fosfatpuffer ble reaksjonen påbegynt med 0,15 g riboflavin og 0,10 ml N,N,W',N'-tetrametylendiamine og bestrålt med UV lys. Under bestrålingsperioden på 45 min. ble temperaturen holdt ved mindre enn 50°C. Gelblokken ble deretter knust, vasket med destillert vann og tørket.
Null-serumet ble fremstilt ved å omsette 1 ml serum og en pronase-løsning med en konsentrasjon på 2 mg pr. ml i 30 min. ved 35°C. Etterpå ble 2 g.av en ionebytter-harpiks (Amber-lite 400) tilsatt.
Etter dette ble 50 ul av null-serumet med et tyroksin-innhold på 10 g pr. 100 ml omsatt med 350 ul enzymløsning med en konsentrasjon på 2 mg-pr. ml i 30 min. ved en temperatur på 35°C.
100 ul av denne løsning ble tilsatt 700 ] il av tracer-løsningen med et innhold på 5,2 ng pr. ml av radioaktivt markert tyroksin og 5 ul pufferløsning med en pH-verdi på 7,2. 350 ul av denne løsningen ble satt til 60 mg av antistoffgelen. Inkuberings-perioden var 30 min., temperaturen 22°C. Etterpå ble løsningen ekstrahert i 5 min. med en pumpehastighet på 0,5 ml pr. min.
En doseringseffekt-kurve for tyroksin ble oppstilt med tyroksin-løsninger med konsentrasjoner på 5 ng, 10 ng, 20 ng og 50 ng pr. 100 ml. Disse løsningene ble omsatt med 350 ul enzymløsning (konsentrasjon 2 mg pr. ml) i 30 min. ved en temperatur på 35°C.
100 ul av denne løsning ble tilsatt 700 ul tracer-løsning ved (5,2 ng pr. ml) og i stedet for puffer 50 ul null-serum.
Målingen resulterte i en gjenvinning på 99 %.
Eksempel 2
Totalbestemmelse av tyroksin ved enzymatisk hydrolyse med protinase
Arbeidsmåten fra eksempel 1 ble fulgt, men i stedet for pronase-løsning ble en protinaseløsning med en konsentrasjon på 2 mg pr. ml anvendt.
Det anvendte serum bestod av et null-serum med et innhold på .
6 mg pr. 100 ml.
Gjenvinningen var 9 8,5
Eksempel 3
Totalbestemmelse av kortisol ved enzymatisk hydrolyse med pronase
Metoden fra eksempel 1 ble fulgt, men et serum inneholdende 18Ug kortisol pr. 100 ml ble brukt. Som tracér-løsning ble en løsning av radioaktivt markert kortisol i en konsentrasjon på 0,15 ng pr. ml brukt. Det anvendte antistoffet ble fremstilt ved syntese av kortisol-C3-oksim og kopling ved albumin med blandet anydrid og antistoff-fremstilling ved immunisering av kaniner.
Gjennvinningen var 9 7 %.
Eksempel 4 ,
Totalbestemmelse av kortisol ved enzymatisk hydrolyse med protinase
Arbeidsmetoden fra eksempel 3 ble fulgt, men serumet som inne-holdt 6 ug kortisol pr. 100 ml og en protinaseløsning med en konsentrasjon på 2 mg pr. ml ble anvendt.
Det anvendte antistoffet i eksempel 4 var det samme som antistoffet i eksempel 3.

Eksempel 5
Totalbestemmelse av tyroksin ved enzymatisk hydrolyse med pepsin
Den generelle arbeidsmetoden fra eksempel 1 ble fulgt.
10 ul serum med et innhold på 18 ug tyroksin pr. 100 ml ble blandet med 160 ul av en enzymløsning som bestod av 2 mg pr. ml pepsin oppløst i 0,1 n saltsyre. Omsetningen med enzymet ble utført ved romtemperatur i 30 min.
Til disse 170 ul ble så 150 ul tracer-løsning med et innhold på 5,2 ng pr. ml radioaktivt markert tyroksin tilsatt.
Hele løsningen ble så satt i 60 mg av en antistoffgel. Polymermatrisen til antistoffgelen bestod av en kopolymer av akrylamid og og 40 mol % av natriumsaltet av metakrylsyre fremstilt fra en 20 % monomerløsning.
Inkuberingstiden var 30 min, temperaturen 22°C.
En ekstraksjon fulgte i 5 min. ved en pumpe.hastighet på 0,5 ml pr. min.
Målingen viste en gjenvinning på 98 %.

1. Fremgangsmåte ved totalbestemmelse av hormoner og farma-søytika bundet partielt til spesifikke eller ikke spesifikke bindende proteiner ved enzymatisk hydrolyse av de bindende proteiner, omsetning av hormonene eller farmasøytika med et antistoff og radiologisk immunologisk bestemmelse av hormonene eller farmasøytika, karakterisert ved at immobiliserte antistoffer anvendes.
2. Fremgangsmåte i følge krav 1, karakterisert ved at antistoffene er innebygget i en polymergel.
3. Fremgangsmåte i følge krav 1 eller 2, karakterisert ved at antistoffene er innebygget i en akrylamid-polymer eller akrylamidkopolymer.
