Abstract
Det er beskrevet anvendelse av en kombinasjon av forbindelser fa gruppe A, spesielt opioider, og forbindelser fra gruppe B, spesielt antimuskarinika og andre overveiende perifert virkende stoffer, for fremstilling av et legemiddel for behandling av forsterket urineringstrang, henholdsvis urininkontinens, samt tilsvarende legemidler og fremgangsmåter for behandling av forsterket urineringstrang henholdsvis urininkontinens.
Description
Foreliggende oppfinnelse vedrører anvendelsen av en kombinasjon av forbindelser fra gruppe A, spesielt opioider, og forbindelser fra gruppe B, spesielt antimuskarinika og andre overveiende perifert virkende stoffer, for fremstilling av et legemiddel for behandling av forsterket urineringstrang, henholdsvis urininkontinens, samt tilsvarende legemidler.
Urininkontinens er uvilkårlig urinavgang. Dette opptrer ukontrollert når trykket i urin-blæren overskrider trykket som er nødvendig for lukking av urinlederen. Årsaker kan for det ene være et forhøyet indre blæretrykk (for eksempel ved detrusorinstabilitet) med følgende tranginkontinens, for det andre et redusert sfinktertrykk (for eksempel etter fødsel eller kirurgiske inngrep) med følgende stressinkontinens. Detrusor er den grovt buntede flersjikts blæreveggmuskulaturen, hvis kontraksjon fører til urintømming, sfinkter er lukkemuskelen av urinrøret. Det opptrer blandeformer av disse inkontinens-typene, samt den såkalte overstrømsinkontinens eller refleksinkontinens (for eksempel etter ryggmargsskader). Følgelig er eksempelvis urininkontinens, urineringstrang og forhøyet miksjonsfrekvens alle mulige symptomer på godartet prostatahyperplasi. Nærmere opplysninger vedrørende dette komplekset finnes hos Chutka, D. S. og Takahashi, P. Y., 1998, "Drugs", 560: 587-595.
Urintrang er tilstanden som retter seg mot urintømming (miksjon) med forsterket blære-muskelspenning ved tilnærming til blærekapasiteten (henholdsvis ved overskridelse av
denne). Denne spenningen virker som miksjonsstimulus. Under en forsterket urineringstrang forstår man derved spesielt opptreden av for tidlig eller ofte i mange tilfeller sågar smertefull urineringstrang inntil såkalt urineringstvang. Dette fører deretter til en lydelig hyppigere miksjon. Årsaken kan blant annet være urinblærebetennelser og neurogene
blæreforstyrrelser, samt også blæretuberkulose. Imidlertid er fremdeles ikke alle årsakene klarlagt.
Forsterket urineringstrang som også urininkontinens oppfattes som ekstremt ubehagelig og det består et tydelig behov hos personer som er berørt av disse indikasjonene for en mest mulig langvarig forbedring.
Vanligvis behandles forsterket urineringstrang og spesielt urininkontinens medikamen-telt med stoffer som er delaktige i refleksen av den nedre urinkanalen (Wein, A. J., 1998, "Urology", 51 (Suppl. 21): 43-47). For det meste er dette medikamenter som har en hemmende virkning på detrusormuskelen, som er ansvarlig for det indre blære-trykket. Disse medikamentene er for eksempel parasympatolytika som oksybutynin, propiverin og tolterodin, trisykliske antidepressiva, som imipramin eller muskel-relakserende midler som flavoksat. Andre medikamenter som spesielt forhøyer motstanden av urinrøret eller blærehalsen, viser affiniteter til a-adrenoreseptoren som efedrin, til P-adrenoreseptorer som clenbutarol, eller er hormoner som østradiol.
Et nøyaktig innblikk i de anvendte terapeutika og terapimetodene, spesielt med hensyn til antimuskarinika og andre perifert virkende stoffer, finner i oversiktsartikkelen til K. E. Anderson et al., "The pharmacological treatment of urinary incontinence", BJU International (1999), 84, 923-947.
Også bestemte diarylmetylpiperaziner og -piperidiner er beskrevet for denne indikasjonen i WO 93/15062. Det ble likeledes påvist en positiv effekt på blærefunksjonen i en rottemodell med rytmiske blærekontraksjoner for tramadol (Nippon-Shinyaku, WO 98/46216). Videre finnes det i litteraturen undersøkelser for karakterisering av de opioide bivirkningene ved urinretensjon, hvorfra det fremgår noen antydninger ved-rørende påvirkningene av blærefunksj onene ved svake opioider som difenoksylat (Fowler et al, 1987, "J. Uroi", 138:735-738) og meperidin (Doyle og Briscoe, 1976, "Br J Uroi", 48:329-335) ved blandede opioidagonister/-antagonister som buprenorfin (Malinovsky et al., 1998, "Anesth Analg", 87:456-461; Drenger og Magora, 1989, "Anesth Analg", 69:348-353), pentazocin (Shimizu et al. (2000), "Br. J. Pharmacol.", 131 (3): 610-616) og nalbufin (Malinovsky et al, 1998, a.a.O), samt ved sterke opioider som morfin (Malinovsky et al., 1998, a.a.O; Kontani og Kawabata, (1988); Jpn J Pharmacol. Sep; 48(1):31) og fentanyl (Malinovsky et al., 1998 a.a.O). Riktignok fore-gikk disse undersøkelsene for det meste i analgetisk virksomme konsentrasjoner.
Ved de indikasjonene som her kommer på tale må det bemerkes at det generelt dreier seg om meget langvarige medikamentelle anvendelser og at de berørte i motsetning til mange situasjoner hvori analgetika anvendes, står overfor en meget ubehagelig, men ikke uutholdelig situasjon. Derfor må det her, enda mer enn ved analgetika, legges vekt på å unngå bivirkninger, fordi den berørte ikke skal måtte bytte ut ett ubehag med et annet. Videre er ved en langvarig urininkontinensbehandling analgetiske virkninger i stor grad uønskede.
Oppgaven ved foreliggende oppfinnelse var følgelig å tilveiebringe stoffer eller stoff-kombinasjoner som kan være av hjelp ved behandling av forsterket urineringstrang, henholdsvis urininkontinens og hvorved virksomme doser fortrinnsvis samtidig viser færre bivirkninger og/eller analgetiske virkninger enn kjent fra teknikkens stand, og spesielt viser en synergistisk effekt ved behandling av urininkontinens.
Overraskende er det nå funnet at en kombinasjon av forbindelser fra gruppe A, som omfatter opioider og andre sentralvirkende stoffer, som vekselvirker med opioid-reseptorer og deres effekter, kan antagoniseres ved nalokson eller spesielt stoffer som virker over en opiatreseptor, spesielt^-reseptoren, og forbindelser fra gruppe B, som omfatter muskarinantagonister, og andre overveiende perifert virkende, som kjente virksomme stoffer ved urininkontinens, har en fremragende virkning på blærefunksjonen. Videre viser denne kombinasjonen seg, tydelig utover det forventede, allerede ved meget lave doser som så virksom at de kombinerte virkestoffene kan anvendes lavdosert. Derved går de ved terapeutisk anvendelse ellers ved høyere nød-vendige doseringer opptredende bivirkningene tydelig tilbake, mens den terapeutiske virkningen blir beholdt fullt ut ved denne kombinasjonen av den perifert, overveiende direkte på blæren eller blæremuskulaturen virkende, antimuskarine effekten og den sentrale, opioide effekten henholdsvis u-reseptoreffekten.
Foreliggende oppfinnelse vedrører følgelig en virkestoffkombinasjon av minst én av forbindelsene A og minst én forbindelse B, med en forbindelse A valgt fra: Gruppe a) inneholdende: tramadol, O-demetyltramadol eller O-desmetyl-N-mono-desmetyl-tramadol eventuelt i form av deres racemater, deres rene stereoisomerer, spesielt enantiomerer eller diastereomerer eller i form av blandinger av stereoisomerene, spesielt enantiomerene eller diastereomerene, i et hvilket som helst blandingsforhold; i form av deres syrer eller baser eller i form av deres salter, spesielt de fysiologisk god-tagbare saltene, eller i form av deres solvater, spesielt hydratene;
Gruppe c) inneholdende:

Overraskende har det vist seg at kombinasjonen av de nevnte stoffene tydelig positivt påvirker bestemte fysiologiske parametrer som er av betydning ved forsterket urineringstrang, henholdsvis urininkontinens. Hver enkelt av disse forandringene kan bety en betydelig lettelse i det symptomatiske bildet for berørte pasienter.
I forbindelse med foreliggende oppfinnelse forstår man under alkyl-, henholdsvis syklo-alkylrester mettede og umettede (men ikke aromatiske), forgrenede, uforgrenede og sykliske hydrokarboner, som er usubstituerte. Derved står Ci-2-alkyl for Cl - eller C2-alkyl, Ci-3-alkyl for Cl-, C2- eller C3-alkyl, CM-alkyl for Cl-, C2-, C3- eller C4-alkyl. Videre står C3_4-sykloalkyl for C3- eller C4-sykloalkyl, C3_5-sykloalkyl for C3-, C4-eller C5-sykloalkyl, C3-6-sykloalkyl for C3-, C4-, C5- eller C6-sykloalkyl, C3-7-sykloalkyl for C3-, C4-, C5-, C6- eller C7-sykloalkyl, C3.8-sykloalkyl for C3-, C4-, C5-, C6-, C7- eller C8-sykloalkyl, C4.5-sykloalkyl for C4- eller C5-sykloalkyl, C4.6-sykloalkyl for C4-, C5- eller C6-sykloalkyl, C4.7-sykloalkyl for C4-, C5-, C6- eller C7-sykloalkyl. Under begrepet sykloalkyl faller imidlertid spesielt også én- eller flerumettede, fortrinnsvis enkeltumettede sykloalkyler uten heteroatom i ringen, så lenge sykloalkyl ikke utgjør noe aromatisk system. Fortrinnsvis er alkyl- henholdsvis sykloalkylrestene metyl, etyl, vinyl (etenyl), propyl, allyl (2-propenyl), 1-propinyl, metyletyl, butyl, 1-metylpropyl, 2-metylpropyl, 2-metylsyklopropyl, syklopropylmetyl, syklobutyl, syklopentyl, syklopentylmetyl, sykloheksyl, sykloheptyl, syklooktyl.
Under begrepet salt forstås i forbindelse med oppfinnelsen enhver form av virkestoffet ifølge oppfinnelsen hvori dette antar en ionisk form, henholdsvis er ladet og koblet med et motion (et kation eller anion) henholdsvis befinner seg i oppløsning. Derunder forstås også komplekser av det virksomme stoffet med andre molekyler og ioner, spesielt komplekser som er kompleksdannet via ioniske vekselvirkningene.
Under begrepet fysiologisk godtakbart salt (spesielt med kationer eller baser) forstår man i forbindelse med foreliggende oppfinnelse salter av minst én av forbindelsene ifølge oppfinnelsen - for det meste en (deprotonert) syre - som anion med minst ett, fortrinnsvis uorganisk, kation som er fysiologisk godtagbart - spesielt ved anvendelse i mennesker og/eller pattedyr. Spesielt foretrukket er saltene av alkali- og jordalkali-metaller, men også med NH4<+>, spesielt imidlertid (mono-) eller (di-)natrium-, (mono-) eller (di-)kalium-, magnesium- eller kalsiumsalter.
Under begrepet fysiologisk godtagbart salt (spesielt med anioner eller syrer) forstår man i forbindelse med foreliggende oppfinnelse videre salter av minst én av forbindelsene ifølge oppfinnelsen - for det meste eksempelvis protonert på nitrogen - som kation med minst ett anion som er fysiologisk godtakbart, spesielt ved anvendelse på mennesker og/eller pattedyr. Spesielt forstår man derunder i forbindelse med foreliggende oppfinnelse det med en fysiologisk godtakbar syre dannede saltet, nemlig saltet av det aktuelle vikestoffet med uorganiske, henholdsvis organiske, syrer som er fysiologisk godtakbare - spesielt ved anvendelse i mennesker og/eller pattedyr. Eksempler på fysiologisk god-tagkbare salter av bestemte syrer er salter av: saltsyre, bromhydrogensyre, svovelsyre, metansulfonsyre, maursyre, eddiksyre, oksalsyre, ravsyre, eplesyre, vinsyre, mandel-syre, fumarsyre, melkesyre, sitronsyre, glutaminsyre, l,l-diokso-l,2-dihydro-lb6-benzo[d]isotiazol-3-on (sakkarinsyre), monometylsebacinsyre, 5-oksoprolin, heksan-1-sulfonsyre, nikotinsyre, 2-, 3- eller 4-aminobenzosyre, 2,4,6-trimetyl-benzosyre, a-liponsyre, acetylglysin, acetylsalisylsyre, hippursyre og/eller asparaginsyre. Spesielt foretrukket er saltsyresaltet.
Egnede salter i forbindelse med foreliggende oppfinnelse og i enhver beskrevet anvendelse og ethvert beskrevet legemiddel er salter av det aktuelle virkestoffet med uorganiske, henholdsvis organiske, syrer og/eller et sukkererstatningsstoff som sakkarin, sykla-mat eller acesulfam. Spesielt foretrukket er imidlertid hydrokloridet.
Forbindelser fra gruppe c) og deres fremstilling er kjent fra DE 44 26 245 Al, henholdsvis US 6 248 737.
I en foretrukket utførelsesform gjelder for anvendelsen ifølge oppfinnelsen at forbindelsen A i gruppe a) er valgt fra: tramadol, (+)-tramadol, (+)-0-demetyltramadol eller (+)-0-desmetyl-N-mono-desmetyltramadol, fortrinnsvis tramadol eller (+)-tramadol, spesielt (+)-tramadol.
I en foretrukket utførelsesform gjelder for anvendelse ifølge oppfinnelse at forbindelsen A i gruppe c) er valgt fra forbindelser med formel I, for hvilke gjelder: X er valgt fra OH, F, Cl, OC(0)CH3eller H, fortrinnsvis OH, F, OC(0)CH3

at forbindelsene med formel I anvendes i form av (+)-enantiomeren, spesielt i blandinger med høyere andel av (+)-enantiomeren sammenlignet med (-)-enantiomeren av en racemisk forbindelse eller anvendes som ren (+)-enantiomer.
Derved er det spesielt foretrukket når forbindelse A er valgt fra følgende gruppe:

Også når anvendelsen ifølge oppfinnelsen bare viser få bivirkninger kan det eksempelvis, for å unngå bestemte former av avhengighet, også være fordelaktig ved siden av kombinasjonen av forbindelser A og B også å anvende morfinantagonister, spesielt nalokson, naltrekson og/eller levallorfan.
Foreliggende oppfinnelse tilveiebringer også et legemiddel for behandling av forsterket urineringstrang, henholdsvis urininkontinens, som inneholder en virkestoffkombinasjon som omtalt ovenfor, samt eventuelt egnede tilsats- og/eller hjelpestoffer.
Dette legemidlet innholder følgelig den følgende en virkestoffkombinasjon av minst én av forbindelsene A og minst én av forbindelsene B, med forbindelse A valgt fra:

Derved er det for forbindelser fra gruppe c) spesielt fordelaktig når forbindelsene med formel I med R<3>= H foreligger i form av diastereomerer med den relative konfigurasjonen Ia spesielt i blandinger med høyere andel av denne diastereomeren sammenlignet med andre diastereomerer eller som ren diastereomer
og/eller
at forbindelsene med formel I foreligger i form av (+)-enantiomeren, spesielt i blandinger med høyere andel av (+)-enantiomeren sammenlignet med (-)-enantiomeren av en racemisk forbindelse eller foreligger som ren (+)-enantiomer.
Som nevnt tidligere omfatter foreliggende oppfinnelse også et legemiddel inneholdende den ovenfor omtalte kombinasjonen.
Egnede tilsats- og/eller hjelpestoffer i forbindelse med med dette legemidlet er alle for fagmannen innen teknikkens stand kjente stoffer for å oppnå galeniske formuleringer. Valget av disse hjelpestoffene, samt mengdene som skal anvendes, avhenger av om legemidlet skal tilføres oralt, intravenøst, peritonealt, intradermalt, intramuskulært, intranasalt, bukkalt eller lokalt. For den orale tilførselen egner seg preparater i form av tabletter, tyggetabletter, dragéer, kapsler, granulater, dråper, safter eller siruper, for den parenterale, topiske og inhalative tilførselen egner seg oppløsninger, suspensjoner, lett konstituerbare tørrpreparater, samt sprayer. En ytterligere mulighet er suppositorier for anvendelse i rektum. Anvendelsen i et depot i oppløst form, en bærerfolie eller et plaster, eventuelt under tilsats av midler som fremmer hudpenetreringen, er eksempler på egnede perkutane tilførselsformer. Eksempler på hjelpe- og tilsatsmidler for den orale tilførselsformen er sprengmidler, glidemidler, bindemidler, fyllmidler, formslipp-midler, eventuelt oppløsningsmidler, smaksstoffer, sukker, spesielt bærermidler, for-tynningsmidler, fargestoffer, antioksidanter og så videre. For suppositorier kan det anvendes blant annet vokser, henholdsvis fettsyreestere, og for parenterale påførings-midler, bærerstoffer, konserveringsmidler, suspensjonshjelpemidler og så videre. Virkestoffmengden som administreres til pasientene, varierer avhengig av pasientenes vekt, tilførselsmåten og graden av sykdommen. Fra oralt, rektalt eller perkutant anvend-bare preparatformer kan forbindelsen ifølge oppfinnelsen frigis med forsinkelse. Ved indikasjonen ifølge oppfinnelsen er tilsvarende retardformuleringer, spesielt i form av et "once-daily"-preparat som bare må inntas én gang om dagen, spesielt foretrukket.
Ytterligere foretrukket er legemidler som inneholder minst 0,05 til 90,0 % av virkestoffet, spesielt lave virksomme doseringer, for å unngå bivirkninger eller analgetiske virkninger. Vanligvis tilføres 0,1 til 5 000 mg/kg, spesielt 1 til 500 mg/kg, fortrinnsvis 2 til 250 mg/kg kroppsvekt av minst en forbindelse med formel I. Likeledes foretrukket og vanlig er imidlertid også tilførselen av 0,01-5 mg/kg, fortrinnsvis 0,03 til 2 mg/kg, spesielt 0,05 til 1 mg/kg kroppsvekt.
Hjelpestoffer kan eksempelvis være: vann, etanol, 2-propanol, glyserol, etylenglykol, propylenglykol, polyetylenglykol, polypropylenglykol, glukose, fruktose, laktose, sakkarose, dekstrose, melasse, stivelse, modifisert stivelse, gelatin, sorbitol, inositol, mannitol, mikrokrystallinsk cellulose, metylcellulose, karboksymetylcellulose, celluloseacetat, skjellakk, cetylalkohol, polyvinylpyrrolidon, parafiner, vokser, naturlige og syntetiske gummier, akasigummi, alginater, dekstran, mettede og umettede fettsyrer, stearinsyre, magnesiumstearat, sinkstearat, glyserylstearat, natriumlaurylsulfat, spiselige oljer, sesamolje, kokosnøttolje, jordnøttolje, soyabønneolje, lecitin, natriumlaktat, poly-oksyetylen- og -propylenfettsyreestere, sorbitanfettsyreestere, sorbinsyre, benzosyre, sitronsyre, askorbinsyre, tanninsyre, natriumklorid, kaliumklorid, magnesiumklorid, kalsiumklorid, magnesiumoksid, sinkoksid, silisiumdioksid, titanoksid, titandioksid, magnesiumsulfat, sinksulfat, kalsiumsulfat, potteaske, kalsiumfosfat, dikalsiumfosfat, kaliumbromid, kaliumjodid, talkum, kaolin, pektin, krospovidon, agar og bentonitt.
Fremstillingen av legemidlet ifølge oppfinnelsen og farmasøytiske preparater foregår ved hjelp av standardteknikk for farmasøytisk formulering ved hjelp av vel kjente midler, innretninger, metoder og fremgangsmåter, som eksempelvis beskrevet i "Remington's Pharmaceutical Sciences", utgiver A. R. Gennaro, 17. utgave, Mack Publishing Company, Easton, Pa. (1985), spesielt i del 8, kapittel 76 til 93.
Følgelig kan for eksempel for en fast formulering, som en tablett, virkestoffet av legemidlet granuleres med en farmasøytisk bærer, for eksempel vanlige tablettinnholds-stoffer som maisstivelse, laktose, sakkarose, sorbitol, talkum, magnesiumstearat, dikalsiumfosfat eller farmasøytisk akseptable gummier, og farmasøytiske fortynnings-midler, som for eksempel vann, for å danne en fast sammensetning som inneholder virkestoffet i homogen fordeling. Under en homogen fordeling forstås her at virkestoffet er fordelt jevnt over den samlede sammensetningen slik at dette uten videre kan opp deles i like virksomme enhetsdoseformer som tabletter, piller eller kapsler. Det faste preparatet oppdeles deretter i enhetsdoseformer. Tablettene eller pillene av legemidlet ifølge oppfinnelsen kan også overtrekkes eller komponderes på annen måte for å tilveiebringe en doseform med forsinket frigivelse. Egnede beleggingsmidler er blant annet polymere syrer og blandinger av polymere syrer med materialer som for eksempel skjellakk, cetylalkohol og/eller celluloseacetat.
Når legemidlet ifølge oppfinnelsen bare viser få bivirkninger kan det eksempelvis for å unngå bestemte former av avhengighet, være fordelaktig ved siden av kombinasjonen av forbindelsene A og B, også å anvende morfinantagonister, spesielt nalokson, naltrekson og/eller levallorfan.
Oppfinnelsen kan anvendes i en fremgangsmåte for behandling av forsterket urineringstrang, henholdsvis urininkontinens, hvorved virkestoffkombinasjonen av forbindelse A og forbindelse B anvendes, spesielt i en terapeutisk (i hvert tilfelle) virksom dosering.
De følgende eksemplene skal belyse oppfinnelsen nærmere.
EKSEMPLER
Eksempel 1
Testsystemcystometri på bedøvde, naive rotter
Den cystometriske undersøkelsen på naive hunnrotter ble gjennomført ved fremgangs-måten ifølge Kimura et al. (Kimura et al, 1996, "Int. J. Uroi", 3:218-227). På bedøvde, ventilerte rotter åpnes abdomen og urinlederen avbindes. Urinen ledes bort fra nyrene. Et kateter innføres og fikseres i blæren. Via dette infunderes saltvannsoppløsning ved hjelp av infusjonspumpe i blæren inntil denne viser rytmisk spontan aktivitet i form av kontraksjoner, som kan opptas ved hjelp av en tilknyttet trykkopptaker. Forsøksstoffet tilføres etter oppnåelse av stabile utgangsverdier i.v. En påvirkning av blærefunksjonen ytrer seg ved undertrykkelse av spontankontraksjonene. I foreliggende eksempel ble forbindelse A ((+)-(2R,3R)-1 -dimetylamino-3-(3-metoksyfenyl)-2-metylpentan-3-ol; hydroklorid) i dosering 0,1 mg/kg i.v. kombinert med forbindelse B (oksybutynin) i doseringen 0,03 mg/kg i.v. og virkningen av denne kombinasjonen ble sammenlignet med enkeltstoffene. Enkeltstoffene og kombinasjonen viste en hemning av kontrak-sjonsraten (miksjonstilfeller/min). Dataene er sammenfattet i den følgende tabellen. Ved alle de her opplistede stoffene og kombinasjonene var en undertrykkelse av spontankonsentrasjonene i rottene målbar.
Den undersøkte kombinasjonen av stoffene viser en positiv virkning på blære-reguleringen og er følgelig egnet for behandling av urininkontinens.
Eksempel 2
Parenteral administrasjonsform
20 g tramadol og 1 g tolterodin oppløses i 1 liter vann for injeksjonsformål ved rom-temperatur og innstilles deretter ved tilsats av NaCl på isotoniske betingelser.

1.
Anvendelse av en virkestoffkombinasjon av minst én av forbindelsene A og minst én av forbindelsene B, med forbindelse A valgt fra: Gruppe a) inneholdende:
tramadol, O-demetyltramadol eller O-desmetyl-N-mono-desmetyl-tramadol eventuelt i form av deres racemater, deres rene stereoisomerer, spesielt enantiomerer eller diastereomerer eller i form av blandinger av stereoisomerene, spesielt enantiomerene eller diastereomerene, i et hvilket som helst blandingsforhold; i form av deres syrer eller deres baser eller i form av deres salter, spesielt de fysiologisk godtakbare saltene, eller i form av deres solvater, spesielt hydratene; eller
Gruppe c) inneholdende: l-fenyl-3-dimetylaminopropanforbindelser med generell formel I

for fremstilling av et legemiddel for behandling av forsterket urineringstrang henholdsvis urininkontinens.
