Description
Foreliggende opprinnelse angår anvendelse av en forbindelse med formel (I) for fremstilling av et medikament for behandling av neuropatisk smerte.
er strukturelt nye antiepileptiske forbindelser som er svært effektive antikonvulsanter i dyrefester (Maryanoff, B.E., Nortey, S.O., Gardocki, J.F., Shank, R.P. og Dodgson, S.P. J. Med. Chem. 30, 880-887,1987; Maryanoff, B.E., Costanzo, M.J., Shank, R.P., Schupsky, J.J., Ortegon, M.E. og Vaught J.L. Bioorganic & Medicinal Chemistry Letters 3,2653-2656,1993). Disse forbindelsene er dekket av US-patent nr. 4.513.006. En av disse forbindelsene 2,3:4,5-bis-I-{l-metyletyliden)-B-D-fruktopyranosesulfamat kjent som topiramat har vist i kliniske forsøk på human epilepsi å være effektive som tilleggsbehandling eller som genbehandling ved behandling av enkle og komplekse delvise anfall og sekundært generaliserte anfall (E. Faught, B.J. Wilder, R.E. Ramsey, R.A. Reife, L.D. Kramer, G.W. Pleldger, R.M. Karim et al., Epilepsia 36 (S4) 33, 1995; S.K. Sachdeo, R.C. Sachdeo, R.A. Reife, P. LIM and G. Pledger, Epilepsia 36 (S4) 33, 1995), og er vanligvis markedsført for behandlingen av enkle og komplekse epilepsi-anfall med eller uten sekundære generaliserte anfall i Storbritannia, Finland, Sverige og Sveits. Forbindelser med formel I ble opprinnelig funnet å ha antikonvulsant aktivitet i den tradisjonelle maksimal-elektrosjokk-(MES)-testen hos mus (Shank, R.P., Gardocki, J.F., Vaught, J.L. Davis, C.B. Schupsky, J.J., Raffa, R.B., Dodgson, S.J., Nortey, S.O. og Maryanoff, B.E. Epilepsia 35,450-460,1994). Etterfølgende studier avdekket av forbindelser med formel I også var svært effektive i MES-testen hos rotter. Mer nylig ble topiramat funnet å effektivt blokkere anfall i flere gnagermodeller av epilepsi (J. Nakamura, S. Tamura, T. Kanda, A. Ishii, K. Ishihara, T. Serikawa, J. Yamada og M. Sasa, Eur. J. Pharmacol. 254,83-89,1994), og i en dyremodell av kindelepilepsi (A. Wauquier og S. Zhou, Epilepsy Res. 24,73-77,1996).
Nylige prekliniske studier på topiramat har avdekket tidligere ikke kjente farmako-logiske egenskaper som antar at topiramat er effektiv ved behandling av noen andre sykdommer. En av disse er neuropatisk smerte.
Således angår foreliggende oppfinnelse anvendelse av en forbindelse med formel (I):

Re og R7 er like eller forskjellige og er hydrogen, C1-C3 alkyl, hvor alkyl inkluderer rettkjedet og forgrenet alkyl, eller R$ og R7 sammen med karbonatomet til hvilket de er bundet, danner en syklopentyl eller sykloheksylring, for fremstilling av et medikament for anvendelse ved behandling av neuropatisk smerte.
Forbindelser med formel I kan syntetiseres ved følgende fremgangsmåter:
(a) Omsetning av en alkohol med formelen RCH2OH med et klorsulfamat med formelen CISO2NH2 eller CISO2NHR1 under nærvær av en base slik som kalium a-butoksid eller natriumhydrid ved en temperatur på ca. -20° til 25 °C og i et løsemiddel slik som toluen, THF eller dimetylformamid hvori R er halvdel med formelen III: (b) Omsetning av en alkohol med formelen RCH2OH med sulfurylklorid med formelen SO2CI2 under nærvær av en base slik som trietylamin eller pyridin ved en temperatur på ca. -40° til 25 °C i et løsemiddel slik som dietyleter eller metylenklorid som gir et klorsulfat med formelen RCH2OSO2CI.
Klorsulfatet med formelen RCH2OSO2CI kan deretter omsettes med et amin med formelen R1NH2 ved en temperatur på ca. 40° til 25 °C i et løsemiddel slik som metylenklorid eller acetonitril som gir en forbindelse med formel I. Reaksjonsbetingelsene for (b) er også beskrevet av T. Tsuchiya et al. i Tet. Letters, nr. 36, s. 3365 til 3368 (1978). (c) Omsetning av klorsulfatet RCH2OSO2CI med et metallazid slik som natriumazid i et løsemiddel slik som metylenklorid eller acetonitril gir et azidosulfat med formelen RCH2OSO2N3 som beskrevet av M. HedayatuUah i Tet. Lett. s. 2455-2458 (1975). Azidosulfatet blir deretter redusert til en forbindelse med formel I, hvori Ri er hydrogen ved katalytisk hydrogenering, f. eks. med et edelmetall og H2 eller ved oppvarming med kobbermetall i et løsemiddel slik som metanol.
Utgangsstoffene med formel RCH2OH kan oppnås kommersielt eller slik det er kjent i litteraturen. F.eks. kan utgangsstoffene med formelen RCH2OH hvori både R2 og R3 og R4 og R5 er identiske og har formelen II oppnås ved fremgangsmåten til R.F. Brady i Carbohydrate Research, vol. 14, s. 35 til 40 (1970) eller ved omsetning av trimetyl-silylenoleteren med et ReCOR7-keton eller aldehyd med fruktose ved en temperatur på ca. 25 °C, i et løsemiddel slik som et halogenkarbon, f.eks. metylenklorid under nærvær av en protisk syre slik som saltsyre eller en Lewis-syre slik som sinkklorid. Trimetyl-silylenoleterreaksjonen er beskrevet av G.L. Larson et al. i J. Org. Chem. Volaa 38, nr. 22, s. 2935 (1973).
Videre kan karboksylsyrer og aldehyder med formlene RCOOH og RCHO reduseres til forbindelser med formelen RCH2OH ved vanlige reduksjonsteknikker, f.eks. reaksjon med litiumaluminiurnhydrid, natriumborhydrid eller boran-THF kompleks i et inert løsemiddel slik som diglym, THF eller toluen ved en temperatur på ca. 0° til 100 °C, f.eks. som beskrevet av H.O. House i "Modern Syntethic Reactins, 2. utg. s. 45 til 144
(1972).
Forbindelsene med formel I kan også fremstilles ved fremgangsmåten fremlagt i 5.387.700 som er omfattet heri med referanse.
Forbindelsene med formel I inkluderer de forskjellige individuelle isomerene så vel som racematene derav, f.eks. de forskjellige alfa- ogbeta-forbindelsene, dvs. under og over tegneplanet, av R2, R3, R4 og R5 på den 6-leddede ringen. Foretrukket er oksygenene på merylendioksygruppen II bundet på den samme siden av den 6-leddede ringen.
Aktiviteten til forbindelsene med formel I til å behandle neuropatisk smerte ble først klar i prekliniske studier utført for å evaluere effektiviteten av topiramat i en dyremodell av neuropatisk smerte. Denne modellen ble utviklet og først beskrevet av S.H. KIM og J.M. Chung, Pain 50,355-363,1992 og blir betegnet "Kim og Chung-modellen".
Hann Sprague-Dawley-rotter, som veier mellom 150-250 g, har L5 og L6 (lumbar-region) spinalnervene tett bundet (bundet med kirurgisk tråd) mellom spinalbåndet og innløpet til isjiasnerven (i det bakerste benet) bare på den ene siden av kroppen. Denne fremgangsmåten resulterer i allodynia (en smertefull respons på normalt uskadelig stimuli) og hyperalgesi (en overdreven respons på normalt smertefull stimuli) i den bakre foten på den samme siden av kroppen som ligasjonen (berørt pote), men gjør ikke foten ubrukelig. Subjektene er fremdeles i stand til å gå og anvende den berørte foten. Innen noen få dager blir subjektene plassert i opphøyde observasjonskamre (ca. 4" x 6" x 10") som har gulva av et metalltrådnett. Gradert trykk anbringes for å lokalisere området nederst på foten via anvendelsen av von Frey-hår (monofilamenter som er kalibrert til å bøye seg under en viss trykkmengde, som varierer fra 0,41 til 15,1 g). Taktil allodynia måles ved å registrere de forskjellige trykkene hvorved den berørte foten trekkes tilbake fra den graderte stimulien ifølge fremgangsmåten til S.R. Chaplan et al. (J. Neurosci. Meth. 53 55-63,1994). Dyr responderte ved 12-15 g trykk på deres ikke-berørte føtter, mens Kim og Chung-modelldyrene responderte ved 1-3 g trykk på deres berørte fot. Avbrytelsesverdien for en rotte som skal inkluderes i denne studien var en respons ved 4 g eller mindre av trykk på den berørte foten innen 7 dager etter kirurgi.
Tre doser topiramat (3,10 og 30 mg/kg) ble testet for oral aktivitet mot neuropatisk smerte i Kim og Chung-modellen; en oral dose av ULTRAM™ (tramadolhydroklorid, 60 mg/kg) ble testet som en positiv kontroll (D. Bian et al., Analgesia 2 57-62,1996). Som forventet, nedsatte tramadolhydroklorid (60 mg/kg p.o.) sensitiviteten av de berørte føttene av Chung-modellrotter fra 3,0 g til en topp på 13,9 g ved 2 timer etter dosering; sensitiviteten gikk tilbake til 4,6 g i løpet av 8 timer (n = 4). Topiramat (30 mg/kg, p.o.) nedsatte sensitiviteten til de berørte føttene til Chung-modellrottene fra 3,0 g til en topp på 8,9 g ved 1 time; sensitiviteten gikk sakte tilbake til 6,3 g ved 8 timer og forble ved 5,6 g 24 timer etter dosering (n = 4). Mindre doser av topiramat hadde mindre effekt, som forandret sensitiviteten til en maksimum på 2,9 g ved 4 timer (3 mg/kg, p.o.) og 5,2 g ved 8 timer (10 mg/kg, p.o.), men disse effektene ble ikke ansett signifikante i denne studien (n = 4 hver).
Den langtids anti-allodyne effekten av topiramat i denne dyremodellen av neuropatisk smerte indikerer at den kan være nyttig ved behandling av neuropatisk smerte hos menneske.
For behandling av neuropatisk smerte kan en forbindelse med formel I anvendes med en daglig dosering i området fra ca. 50 til 400 mg administrert oralt, vanligvis i to oppdelte doser, for et gjennomsnittlig voksent menneske. En enhetsdose vil inneholde ca. 25 til 200 mg av den aktive ingrediensen.
For å fremstille farmasøytiske sammensetninger ifølge oppfinnelsen, blir en eller flere sulfamatforbindelser med formel I umiddelbart blandet sammen med en farmasøytisk bærer i henhold til vanlige farmasøytiske formuleringsteknikker, hvor bæreren kan være i mange forskjellige former avhengig av den ønskede fremstillingsformen for admini-strasjonen, f.eks. oralt, med stikkpiller, eller parenteralt. Ved fremstilling av sammensetningene i oral doseringsform kan en hvilken som helst av de vanlige farmasøytiske mediene anvendes. For orale væskepreparater slik som f.eks. suspensjoner, eliksirer og løsninger, inkluderer således egnede bærere og additiver vann, glykoler, oljer, alkoholer, luktstoffer, konserveringsmidler, fargestoffer og lignende; for faste orale preparater slik som f.eks. pulvere, kapsler og tabletter, inkluderer egnede bærere og additiver stivelser, sukker, fortynningsmidler, granuleringsmidler, smøremidler, bindemidler, disintegrasjonsmidler og lignende. På grunn av enkel administrasjon er tabletter og kapsler den mest fordelaktige orale doseringsenhetsformen, i hvilket tilfelle faste farmasøytiske bærere klart blir anvendt. Hvis ønskelig, kan tabletter være sukkerbelagt eller enterisk belagt med vanlige teknikker. Stikkpiller kan fremstilles, hvor kakaosmør kan anvendes som bæreren. For parenteraler vil bæreren vanligvis innbefatte sterilt vann, selv om andre ingredienser, f.eks. for formålene slike som bedring av løseligheten eller konservering, kan omfattes. Injiserbare suspensjoner kan også fremstilles hvor hensiktsmessige væskebærere, suspensjonsmidler og lignende kan anvendes. Topiramat er vanligvis tilgjengelig for oral administrasjon i runde tabletter som inneholder 25 mg, 100 mg eller 200 mg aktiv ingrediens. Tablettene inneholder følgende aktive ingredienser: laktose vannholdig, pregelatinert stivelse, mikrokrystallinsk cellulose, natriumstivelseglykolat, magnesiumstearat, renset vann, karnaubavoks, hydroksypropyl-metylcellulose, titaniumdoksyd, polyetylenglykol, syntetisk jernoksyd og polysorbat 80.
De farmasøytiske sammensetningene heri vil inneholde, pr. doseringsenhet, f.eks.
tablett, kapsel, pulverinjeksjon, teskjefull, stikkpiller og lignende, fra ca. 25 til ca. 200 mg aktiv ingrediens.
