Teknisk område
Foreliggende oppfinnelse tilveiebringer anvendelse av albumin til fremstilling av farmasøytiske preparater. Preparatene kan anvendes til behandling av henholdsvis skade på hornhinne og øyets bindehinne, og tørt øye. Preparatene har en evne til å øke øyeoverflateepitelmucinsekresjon og har også en evne til å diffundere olje.
Teknikkens stand
Skade på hornhinne og øyets bindehinne forårsakes av at det dannes defekter fra overflaten til epitelet. Årsaken kan omfatte slike patogene faktorer som keratokonjunktivitt sicca (tørt øye), forskjellig keratokonjunktivitt, allergi og infeksjon av mikroorganismer (f eks virus, bakterier, sopp etc), slike kjemiske faktorer som cytotoksisitet av kjemikalier og korrosjon på grunn av syre og base, slike fysiske faktorer som xeroftalmi, skade på grunn av fremmedstoff (f eks kontaktlinser etc), og varmt vann, og lignende. Det er nylig blitt rapportert at antiseptika inneholdt i et oftalmisk preparat (f eks benzalkoniumklorid, klorbutanol etc) og oftalmiske midler (f eks aminoglykosidantibiotika, ikke-steroidholdige antiinflammatoriske legemidler, IDU, pimaricin etc) , forårsaker en skade på hornhinneepitelet (ectocornea).
For å behandle skaden på hornhinne og øyets bindehinne administreres for tiden kondroitinsulfat, glutation, hyaluronsyre, fibronektin, EGF og lignende, eller også administreres en kunstig tåreoppløsning for det formål å supplere en tåreoppløsning, men effekten av disse behandlingene er ennå ikke tilstrekkelig.
Tørt øye er definert som en tilstand med reduksjon eller endring i tårekvalitet, uansett tilstedeværelsen eller fravær av skade på hornhinne og øyets bindehinne (Yamada et al. GANKI 43, 1289-1293 (1992)). Det er forskjellige faktorer som forårsaker tørt øye, men ingen egnet metode for å gjenopp-rette den reduserte tåremengde til det normale, er ennå blitt funnet.
For å behandle det tørre øye administreres for tiden en kunstig tåreoppløsning med det formål å supplere tårer, og kondroitinsulfat, glutation, hyaluronsyre, fibronektin, EGF og lignende for det formål å lindre subjektive symptomer, men effektene er ennå ikke tilstrekkelige.
Ut fra en senere undersøkelse menes det at normalt øyeoverflateepitel uttrykker mucinlignende glykoprotein, og at glykoproteinet tar aktiv del i opprettholdelse av tårefilm. Dessuten har mucin utskilt fra kimceller for øyets bindehinne vært kjent for å være ansvarlig for å stabilisere tårefilm. Så defekt i øyeoverflateepitel forårsaket av hvilken som helst faktor kan indusere unormal øyemucinsekresjon og derved ustabil tårefilm. Den ustabile tårefilm kan senke interak-sjonsnivået mellom epitelet og tårefilmen, og derved kan skaden på hornhinne- og bindehinneepitel bli verre (Norihiko Yokoi i GANKA CHIRYO NO KOTSU TO OTOSHIANA (Techniques for treatment in Ophtalmologic field), redigert av FUMIO KOGURE, utgitt av KABUSHIKI KAISHA NAKAYAMA SHOTEN, Tokyo, Japan, sidene 26-27 (1995)) .
Ut fra disse synspunkter er det foreslått at slike tilstander som skade på hornhinne og øyets bindehinne, eller tørt øye, kan behandles dersom tårefilmen stabiliseres ved å øke mucinsekresjon eller hvilket som helst annet middel.
Albumin er et protein som har stor utbredelse i et dyre- eller vegetabilsk vev eller en kroppsvæske, slik som humanserum, tåreoppløsning. F eks brukes humanserumalbuminet til behandling av hypoalbuminemi, hemoragisk sjokk o.l. På det oftalmiske området er det også kjent å anvende det som et sta-biliseringsmiddel for proteinpreparater, slik som fibronektin eller interferon. Det er blitt foreslått å anvende slike preparater som fibronektin og interferon til behandling av skade på hornhinnen (japansk patent-kokai nr Sho 61-103838 og nr Hei 6-271478), men det er ikke noen kunnskap om at albumin selv er effektivt til behandling av skade på hornhinne og øyets bindehinne, eller tørt øye.
Beskrivelse av oppfinnelsen
Foreliggende oppfinnelse tilveiebringer anvendelse av albumin til fremstilling av et farmasøytisk preparat for behandling av skade på hornhinne og øyets bindehinne, hvor albumin er den eneste aktive bestanddel i preparatet. Videre tilveiebringer oppfinnelsen anvendelse av albumin til fremstilling av et farmasøytisk preparat for behandling av tørt øye, hvor albumin er den eneste aktive bestanddel i preparatet.
Opprinnelsen til albumin som anvendes til preparatet ifølge oppfinnelse, er ikke spesifikt begrenset. Når albumin har en antigenisitet, oppstår imidlertid et slikt problem som allergi, og derfor er det ikke foretrukket. Albumin av human opprinnelse, f eks humanserumalbumin, anvendes fortrinnsvis.
Humanserumalbumin som renses til den renhet som er egnet for normal anvendelse i medisinske applikasjoner, kan fortrinnsvis anvendes ved foreliggende oppfinnelse uten noe særlig problem. Det vil si at de som ikke inneholder mindre enn 80 % albumin (i tilfelle analysering med elektroforesen), er foretrukket. For å inaktivere virus, etc. er de som er erholdt ved hjelp av varmebehandling, foretrukket. Særlig humanserumalbumin som er kommersielt tilgjengelig som et legemid-del, anvendes fortrinnsvis.
Albumin fremstilt ved hjelp av mikroorganisme erholdt ved hjelp av genrekombinasjon anvendes også fortrinnsvis ved foreliggende oppfinnelse. Fremstillingsfremgangsmåten i henhold til genrekombinasjonsteknikken er godt kjent for fagfolk innen fagområdet. Kort forklart kan en vektor som inneholder et gen som koder for et ønsket albumin (f eks humanalbumin), innføres i en vertscelle for å transformere den. Den transfor-merte celle som produserer det ønskede protein, kan velges ut og dyrkes, så kan humanalbumin isoleres og renses fra den dyrkede supernatant eller celleekstrakt. Eksempler på vertscelle omfatter gjær, Escherichia coli og lignende, som vanligvis anvendes av fagfolk innen fagområdet for å produsere et protein. I den hensikt å unngå en risiko for innlemmelse av virus, etc. er albumin som et produkt av slik genrekombinasjon mer foretrukket.
Slik det her er brukt omfatter uttrykket "skade på hornhinne og øyets bindehinne" skade på hornhinne og øyets bindehinne forårsaket av slike patogene faktorer som kerato-conjunctivitt sicca (tørt øye), forskjellig keratocon-junctivitt, allergi og infeksjon med mikroorganismer (f eks virus, bakterier, sopp, etc), slike kjemiske faktorer som cytotoksisitet av kjemikalier, og korrosjon på grunn av syre og base, slike fysiske faktorer som xeroftalmi, skade på grunn av fremmedstoff (f eks kontaktlinse, etc), og varmt vann, og lignende, antiseptika inneholdt i et oftalmisk preparat (f eks benzalkoniumklorid, klorbutanol, etc) og oftalmiske midler (f eks aminoglykosidantibiotika, ikke-steroidholdige antiinflammatoriske legemidler, IDU, pimaricin, etc); defekter i ectocornea; hornhinneerosjon; hornhinnesår og lignende.
Slik det her er brukt omfatter uttrykket "tørt øye" ikke bare enkelt tørt øye (tåreunderskudd) definert som "en tilstand med reduksjon eller endring i tårekvalitet, uansett tilstedeværelsen eller fraværet av skade på hornhinne og øyets bindehinne", men også forskjellige tørrøye-tilstander, slik som alacrima, xeroftalmi, Sjogrens syndrom, tørr keratokonjunktivitt, Stevens Johnsons syndrom og okular pemfigoid, ble-faritt. Videre omfatter uttrykket "tørt øye" tørt øye etter kataraktoperasjon og det som ledsages av allergisk konjunk-tivitt, samt tørr-øyelignende tilstand, slik som tåresvinn hos VDT-arbeider (Visual Display Terminal) og et tåresvinn uten noe systemisk symptom forårsaket f eks av tørt rom på grunn av luftkondisj onering.
Slik det her er brukt henviser uttrykket "behandling" eller "behandle" til hvilken som helst måte for å kontrollere tilstandene, inkludert forhindring, helbredelse og lindring av tilstandene, og stans eller lindring av utvikling av tilstanden .
Oppfinneren fant videre at albumin øker øyeoverflate-epitelmucinsekresj onen.
Oppfinneren fant videre at albumin har en overflate-aktiv aktivitet for diffundering av oljer. Det er foreslått at slik overflateaktivitet hos albumin bidrar til øyeoverflate-filmstabilisering.
De farmasøytiske preparatene fremstilt ved anvendelsene ifølge foreliggende oppfinnelse kan være i slike doseringsformer som tabletter, piller, pulvere, suspensjoner, kapsler, suppositorier, injeksjonspreparater, salver, øyedråper o.l. Det er særlig foretrukket å administrere øyedråper lokalt.
I tilfelle preparatene er formulert som øyedråper, kan de inneholde albumin i en mengde på ca 1 til 1000 mg/ml, mer foretrukket ca 10 til 1000 mg/ml, videre fortrinnsvis ca 50 til 1000 mg/ml. Preparatene kan videre inneholde et farmasøytisk akseptabelt fortynningsmiddel.
Slik det her er brukt kan "farmasøytisk akseptabelt fortynningsmiddel" være hvilket som helst fortynningsmiddel som anvendes til oftalmiske preparater, og som er kjent for fagfolk innen teknikken, f eks vann, fysiologisk saltoppløs-ning, kunstig tåreoppløsning og lignende.
De farmasøytiske preparatene kan videre omfatte forskjellige komponenter som anvendes i et normalt oftalmisk preparat, slik som stabiliseringsmidler, steriliseringsmidler, buffermidler, isotone midler, chelateringsmidler, pH-reguleringsmidler, overflateaktive midler og lignende.
Eksempler på stabiliseringsmidlet omfatter normale L-type-aminosyrer, slik som glysin og alanin o.l., slike mono-sakkarider som glukose og mannose o.l., slike disakkarider som sukrose og maltose o.l., slike sukkeralkoholer som mannitol og xylitol o.l., og slike polysakkarider som dekstran o.l.
Eksempler på steriliseringsmidler omfatter benzalko-niumsalt, klorheksidinsalt og ester av paraoksybenzoat o.l.
Eksempler på buffermidlet omfatter borsyre, fosfor-syre, eddiksyre og sitronsyre, eller et salt derav.
Eksempler på det isotone middel omfatter natrium-klorid, kaliumklorid og sakkarider o.l.
Eksempler på chelateringsmidlet omfatter natriumede-tat og sitronsyre o.l.
Ettersom det er et oftalmisk preparat, reguleres pH fortrinnsvis fra 5 til 8.
Preparatet kan administreres ved en dosering på ca 1 til 100 ul/øye, fortrinnsvis ca 10 til 50 ul/øye, og mer foretrukket ca 30-50 ul/øye.
Slik det her er brukt omfatter uttrykket "et individ som trenger slik behandling av skade på hornhinne og øyets bindehinne" både en pasient som faktisk lider av skade på hornhinne og øyets bindehinne, og en pasient som mistenkes for å lide av slik skade. Det omfatter ikke bare pasienten hvis skade på hornhinne og øyets bindehinne faktisk er blitt erkjent, men også pasienten som mistenkes for skade på hornhinne og øyets bindehinne, og pasienten i den tilstand hvor det foreligger en stor mulighet for at tilstanden skal oppstå, slik som en pasient etter keratoplasti.
"Den effektive mengde" av albumin, som er en mengde for den ønskelige behandling, kan velges optimalt etter pasientens symptomer, alder, kjønn, kroppsvekt, kost, andre legemidler som anvendes i kombinasjon, og forskjellige faktorer som er kjent for fagfolk innen medisinen. Denne effektive mengde kan også variere avhengig av typen av eller aktiviteten til albumin, i tillegg til faktorene ovenfor. Bestemmelse av den effektive mengde er en operasjon som vanligvis utføres av fagfolk innen medisin.
De farmasøytiske preparatene administreres ved en dosering på ca 1 til 100 ul/øye, fortrinnsvis ca 10 til 50 ul/øye, og mer foretrukket ca 30 til 50 ul/øye, ca 1 til 20 ganger per dag og mest foretrukket ca 5 til 10 ganger per dag.
Formuleringseksempel
I eksemplene i denne søknaden ble det brukt albumin-preparater fra donert blod fremstilt av The Green Cross Cor-poration (Osaka, Japan). Disse albuminpreparatene ble erholdt ved å anvende en albuminfraksjon som ble isolert/renset fra plasma fra en blodgiver som et råmateriale i henhold til en Cohns fraksjoneringsmetode i kald etanol, fremstilling ifølge de følgende formuleringseksempler 1 og 2, og varmebehandling ved 60 °C i 10 timer.

Testeksempel 1
Tre tørrøyepasienter med skade på hornhinne og øyets bindehinne (kvinne 64 år, kvinne 61 år og kvinne 34 år) fikk administrert til øynene 25 % humanserumalbumin (albumin->innhold: 250 mg/ml) ifølge formuleringseksempel 1, 10 ganger per dag med en dosering på 30-50 ul/øye. Sammen med administrering av albumin ble en kunstig tåreoppløsning også administrert til øynene.
En intravital fargeundersøkelse ble utført før og )etter administreringen for å beregne graden av skade på hornhinne og øyets bindehinne. Som den intravitale fargeunder-søkelse ble utført farging med rose bengal og fluoresceinfarging. Fargingen med rose bengal (RB) gir et tall for skade på hornhinne og øyets bindehinne, og poengbedømmelse ble utført i ved å beregne graden av farging på nese- og øresidene av bul-bærbindehinne og hornhinne med et poengtall på 0-3 (totale poeng på 9). Fluoresceinfargingen (F) gir et tall for skade på hornhinne og poengbedømmelsen ble utført ved å beregne graden av hornhinneskade med en poengbedømmelse på 0-3. I begge til-)fellene ble poengbedømmelse utført i henhold til en van Bijsterrelds evalueringsmetode. I begge tilfellene' gjelder det at dess høyere tallverdien er, dess alvorligere er skaden. Resultatene er vist i Tabell 1.
Testeksempel 2
Mann, 62 år, med defekter i ectocornea etter keratoplasti.
Etter penetrerende keratoplasti i det høyre øyet var det defekter i ectocornea fortsatt til stede og 0,1 % "Hyaleinmini" (inneholdende 0,1 % hyaluronsyre) ble administrert. Det ble imidlertid ikke observert noen effekt og administrering av 0,1 % "Hyaleinmini" ble derfor avsluttet. Deretter ble 25 % humanserumalbumin (albumininnhold: 250 mg/ml) ifølge formuleringseksempel 1 administrert til øynene 10 ganger per dag med en dosering på 30-50 ul/øye i en uke. En uke etter begynnelsen av administrering dannes ectocornea og defekter i ectocornea ble forbedret.
Testeksempel 3
Kvinne, 64 år, med defekter i ectocornea etter keratoplasti
Etter penetrerende keratoplasti i det venstre øyet, limbustransplantasjon og amniontransplantasjon ble defekter i ectocornea påvist ved hjelp av fluoresceinfargingen. Til øynene til denne pasienten ble det administrert 5 % humanserumalbumin (albumininnhold: 50 mg/ml) ifølge formuleringseksempel 2 10 ganger per dag med en dosering på 30-50 ul/øye i fire uker. To uker etter begynnelsen av administreringen opp-viste fluoresceinfarging forbedring av defekter i ectocornea. Fire uker etter begynnelsen av administreringen ble det ikke observert noen farging og en tilsynelatende bedring av defekter i ectocornea ble erkjent.
Testeksempel 4
Kvinne, 72 år, med Sjogrens syndrom
Til øynene til pasienten med defekter i ectocornea ledsaget av Sjogrens syndrom ble det administrert "Intal"
(inneholdende natriumkromglikat), 0,1 % "Flumetholon" (inneholdende fluormetolon) og en kunstig tåreoppløsning. Ingen forbedring av defektene i ectocornea ble imidlertid observert og administreringen ble avsluttet. Så ble det administrert 5 % humanserumalbumin (albumininnhold: 50 mg/ml) ifølge formuleringseksempel 2 til øynene til pasienten 6 ganger per dag med en dosering på 30-50 ul/øye i fire uker. Fire uker etter begynnelsen av administreringen ble det ikke observert defekter i ectocornea.
Testeksempel 5
Kvinne, 45 år, med Sjogrens syndrom
Til øynene til pasienten med skade på hornhinne og øyets bindehinne ledsaget av Sjogrens syndrom ble det administrert 5 % humanserumalbumin (albumininnhold: 50 mg/ml) ifølge formuleringseksempel 2 10 ganger per dag med en dosering på 30-50 ul/øye i fire uker.
Før og etter administreringen ble det utført en intravital fargeundersøkelse (farging med rose bengal og fluo-resceinf arging) for å beregne graden av skade på hornhinne og øyets bindehinne. Den intravitale fargeundersøkelse ble utført ved å påføre 2 ul av en blandingsoppløsning inneholdende 1 % rose bengal og 1 % fluorescein til pasientens nedre øyelokk under nøyaktig anvendelse av en mikropipette, noe som fikk pasienten til å blinke flere ganger, og så observere øynene. Farging med rose bengal (RB) ble målt med hvitt lys fra spaltelampe og fluoresceinfarging (F) ble målt med koboltblått lys. Omfanget av fargingen ble poengbedømt fra 0-9. Resultatene er vist i Tabell 2.

Tre tørrøyepasienter (kvinne, 64 år, kvinne, 61 år og kvinne, 34 år) fikk administrert 25 % humanserumalbumin (albumininnhold: 250 mg/ml) ifølge formuleringseksempel 1 til sine øyne 10 ganger per dag med en dosering på 30-50 il/øye. Sammen med administrering av albumin ble det også administrert en kunstig tåreoppløsning til øyet.
Før og etter administreringen ble subjektive symptomer hos pasientene beregnet og poengbedømt. Det subjektive symptom omfatter øyesmerte, tørrhetsfølelse i øyet og fremmed-legemefølelse i øyet. Beregning av symptomene ble gjort av pasientene selv. Det verste subjektive symptom ble bedømt som 100 poeng og det beste eller normale som 0. Resultatene er vist i Tabell 3.
Testeksempel 7
Til øynene til en tørrøyepasient ledsaget av Sjogrens syndrom (kvinne, 74 år), ble det administrert 5 % humanserumalbumin (albumininnhold: 50 mg/ml) ifølge formuleringseksempel 2 6 ganger per dag med en dosering på 30-50 ul/øye i åtte uker.
Etter åtte uker var subjektive symptomer, inkludert øyesmerte, tørrhetsfølelse i øyet og fremmedlegemefølelse i øyet nesten perfekt fjernet. Administrering av albumin ble stanset.
Testeksempel 8
Albumins evne til å øke øyeepitels mucinsekresjon ble undersøkt. CCL-celler (bindehinneepitelcellestamme) ble dyrket i TCM199-medium (GIBCO) inneholdende 10 % (vekt/volum) humanserumalbumin (SIGMA) i 24 timer ifølge vanlig celledyrknings-betingelse. Som en kulturkontroll ble cellene dyrket med TCM199-mediet uten humanserumalbumin.
De dyrkede celler ble innhøstet fra kulturbeholderen under anvendelse av trypsin-EDTA, fiksert med paraformaldehyd i 30 minutter og vasket med fosfatbufret saltoppløsning (PBS) tre ganger. De erholdte celler ble blokkert (4 °C, 30 min.) med normalt geiteserum og de blokkerte celler ble omsatt
(4 °C, 30 min.) med muse-anti-mucinantistoff (Mue 1) og så vasket med PBS tre ganger. Cellene ble omsatt videre (4 °C, 30 min.) med FITC-merket anti-muse-IgG antistoff og så vasket tre ganger med PBS.

Testeksempel 9
Evne til å diffundere olje ved hjelp av albumin
10 volum% ricinusolje ble tilsatt til en kunstig tåreoppløsning. Oljen og oppløsningen skiltes ut og oljedråper ble dannet. I dette systemet ble det tilsatt 10 % (v/v av kunstig tåreoppløsning) av 5 % albuminoppløsning, oljen diffunderte gjennom overflaten til vannfasen og oljedråpene forsvant. I henhold til dette eksemplet er det ting som tyder på at albumin kan virke som et overflateaktivt middel til å beskytte fordamping av tåreoppløsning fra øyeoverflate.
Industriell anvendbarhet
De farmasøytiske preparater som fremstilles ved anvendelsene ifølge foreliggende oppfinnelse, kan anvendes til behandling av skade på hornhinne og øyets bindehinne, og tørt øye.
Claims (10)
1. Anvendelse av albumin til fremstilling av et farma-søytisk preparat for behandling av skade på hornhinne og øyets bindehinne, hvor albumin er den eneste aktive bestanddel i preparatet.
2. Anvendelse ifølge krav 1, hvor albuminet er albumin av humanopprinnelse.
3. Anvendelse ifølge krav 1, hvor det farmasøytiske preparat er egnet for applikasjon som øyedråper.
4. Anvendelse ifølge krav 4, hvor det farmasøytiske preparat inneholder 1-1000 mg/ml albumin.
5. Anvendelse ifølge krav 1, hvor det farmasøytiske preparat inneholder 10-1000 mg/ml albumin.
6. Anvendelse av albumin til fremstilling av et farma-søytisk preparat for behandling av tørt øye, hvor albumin er den eneste aktive bestanddel i preparatet.
7. Anvendelse ifølge krav 6, hvor albuminet er albumin av humanopprinnelse.
8. Anvendelse ifølge krav 6, hvor det farmasøytiske preparat er egnet for applikasjon som øyedråper.
9. Anvendelse ifølge krav 6, hvor det farmasøytiske preparat inneholder 1-1000 mg/ml albumin.
10. Anvendelse ifølge krav 9, hvor det farmasøytiske preparat inneholder 10-1000 mg/ml albumin.
