Neutrofiler, en underklasse av leukocytter er involvert i immunforsvar og andre reaksjoner i blodkarsystemet og tilgrensende vev (Roitt, I.M., Leitfaden der Immunologie, Steinkopf-Verlag Darmstadt, 2. Utgave, 1984).
Neutrofiler kan aktiveres ved eksogene stoffer, så som for eksempel celle-veggkomponenter (et kommersielt oppnåelig produkt er "Zymosan"). I
sammenheng med dette blir såkalte oksygenarter dannet i de såkalte respiratoriske utbrudd (burst).
Figur 1 beskriver et forenkelt skjema av neutrofil stimulering: Etter binding av en ligand (for eksempel Zymosan A) til en spesifikk reseptor på neutrofil-membranen, blir guanosin trifosfat (GTP) aktivert, som i sin tur aktiverer fosfatidyl-inositol-4,5-bifosfat (PIP2) spesifikk fosfolipase C (PLC). PLC katalyserer hydrolyse av PIP2 til inositol-l,4,5-trifosfat (IP3) og diacylglyserol (DG). IP3 stimulerer
frigjøring av Ca<2+> ioner fra intracellulære lagre. Ca<2+> ioner aktiverer, sammen med DG enzyme, protein kinase C (PC) som fosforylerer et antall proteiner og på denne måte aktiverer blant andre NADPH oksidase-kompleks. NADPH oksidase katalyserer i sin tur reaksjonen:
hvori de reaktive oksygenarter Oy resulterer.
Skjønt aktiverte neutrofiler er viktig for funksjonen til immunforsvaret, kan sykdomssymptomer, så som akutte eller kroniske inflammasjoner eller andre allergireaksjoner resultere ved overreaksjoner forårsaket spesielt av reaktive oksygenarter. På den annen side kan neutrofilåktiyitet være altfor lav eller utilstrekkelig for et vellykket immunforsvar med en generell immundefekt, så som for eksempel med AIDS. Følgelig eksisterer det et behov, spesielt fra et medisinsk standpunkt, for å være i stand til påvirke en regulerende og/eller modulerende aktivitet til neutrofiler in vivo og in vitro. I denne forbindelse bør den neutrofile aktivitet enten økes eller reduseres avhengig av indika-sjonen, det vil si skulle være kontrollerbar etter behov.
Derfor er det foreliggende oppfinnelses hensikt å gjøre tilgjengelig et middel for regulering og/eller modulering av aktiviteten til neutrofiler. Denne hensikt er oppnådd ved foreliggende oppfinnelse, kjennetegnet ved det som fremgår av de vedlagte krav.
Det er nå overraskende blitt funnet at visse aminoadamantanforbindelser har en regulerende og/eller modulerende virkning på aktiviteten til neutrofiler. Den foreliggende oppfinnelse angår dette funn. Det er avgjørende for dette at neutrofilene allerede er blitt aktivert av andre stimulerende midler.
Aminoadamantanene i seg selv viser ingen aktiverende effekt på neutrofiler.
Den ovennevnte hensikt er følgelig løst ved anvendelse av memantin (l-amino-3,5-dimetyl adamantan) eller et farmasøytisk akseptabelt salt derav til fremstilling av et legemiddel for regulering av aktiviteten av allerede aktiverte neutrofiler.
Aminoadamantanforbindelser av denne type er kjent. Således er
1-aminoadamantaner beskrevet i DE 22 19 256, DE 28 56 393,
DE 22 32 735, US 3 450 761 eller 4 122 193. Produksjonen av memantin skjer generelt ved kjente fremgangsmåter, så som for eksempel alkylering av halogen adamantaner. Påfølgende videre halogener-ing og alkylering fremstiller individuelle di- og/eller tri- substituerte adamantaner. EP 392 059 blir referert til for fremstilling av aminoadamantaner.
Aminoadamantanforbindelser er også allerede blitt anvendt i farmasøytiske
hensikter. Således beskriver EP 392 059 anvendelse av 1 -aminoadamantan-
forbindelse forbehandling av Alzheimers sykdom eller hjernecelleskade som et resultat av cerebral ischemi. US 3 450 761 beskriver aminoadamantaner med antiviral aktivitet.
Om aktiviteten til neutrofiler blir forhøyet eller delvis eller fullstendig inhibitert avhenger av konsentrasjonen av den anvendte aminoadamantan-forbindelse.
Således har konsentrasjoner fra IO"<6> til 10"<5>M en aktivitets-økende virkning. En konsentrasjon på cirka 5.10"<6>M har vist seg spesielt effektivt til å øke neutrofilaktivitet, en konsentrasjon som vel kan oppnås in vivo i blodplasma. I
kontrast er resultatet en inhibert aktivitet med én økning av
aminoadamantankonsentrasjonen. En konsentrasjon som er egnet for denne hensikt er 10"<4> til 10'<3>M.
Oppfinnelsen omfatter adamantanforbindelser substituert med en eller flere aminogrupper.
Foretrukne adamantanerforbindelser er l-amino-3,5-dimetyl-adamantan.
Spesielt foretrukket er l-amino-3,5-dimetyl-adamantan betegnet som mematin
(INN, Akationol Memantine<®>). Gortelmeyer, R., et al beskriver behandlingen av Demens syndrom med memantin i Spectrum of Neurorehabilitation, W.

Som forklart ovenfor har aminoadamantanforbindelser i henhold til oppfinn-elsen bare en regulerende virkning på neutrofiler som er preaktiverte eller som er blitt stimulert av spesielle aktivatorer.
Naturlige stoffer, så som Zymosan, N-formyl-Met-Leu-Phe (N-FLMP) eller A 23187 (en Ca antagonist) er kjent som aktivatorer. Disse stoffer kan anvendes hvis man har til hensikt spesifikt og mer intenst å aktiviere neutrofiler in vitro, mens også in vivo. Ved dette er det mulig å detektere neutrofiler i en prøve, for eksempel i en prøve av kroppsvæske spesielt en blodprøve, selv når bare få neutrofiler er til stede eller når deres aktivitet er meget svak. I denne forbindelse kan aktivatoren være til stede i kombinasjon med aminoadamantanforbindelsen: Graden av aktivering av neutrofilene stimulert av aktivatoren blir intensivert ved samtidig tilsetting av amino-adamantanforbindelse.
Et ytterligere anvendelsesfelt er forbedring av immunforsvaret hos personer med forskjellig former for immundefekter, spesielt AIDS pasienter. Her kan det administreres en farmasøytisk sammensetning som omfatter en amino-adamantanforbindelse i henhold til oppfinnelsen, eventuelt i kombinasjon med neutrofil aktivator. Videre er anvendelse i CGD (Agranulomatotose), Wegeners granulomatose og /eller glykogenlagringssykdommer mulig.
Som en selvfølge kan neutrofilene også aktiveres som en følge av en naturlig immunrespons hos organismen som en reaksjon på forskjellige immunogener og trigger allergiske reaksjoner, inflammasjoner og reumatiske symptomer på grunn av overreaksjon. Her kan den neutrofile aktivitet bli dempet ved egnet økning av aminoadamantankonsentrasjoner og pasienten kan bli gitt lettelse. Indre og ytre inflammasjonsbetingelser, for eksempel i kneet, hofte eller kjeve i tillegg til autoimmune sykdommer kan spesielt bekjempes effektivt med aminoadamantanholdig medisin. Et ytterligere anvendelsesfelt er i behandling av parasittiske sykdommer, så som Leishmannia.
I denne forbindelse er det opp til legens påpasselighet å sikre de egnede konsentrasjoner av aminoadamantanforbindelser for regulering og moduler-ing av neutrofil aktivitet ved seleksjon av den egnede dose og administra-sjonsform. Avhengig av anvendelsesområdet er parenterale former, for eksempel intravenøs eller oral administrering mulig her; former for å opprettholde virkning er også egnet. Oppfinnelsen omfatter også kombina-sjoner av aminoadamantanforbindelser i henhold til oppfinnelsen så vel som farmasøytisk akseptable salter, særlig syreaddisjonssalter, for eksempel hydroklorider, hydrobromider, sulfater, acetater, suksinater, tatrater eller tilsettingsforbindelse med fumarsyre, maleinsyre, sitronsyre eller fosforsyre kan navngis her. De farmasøytiske sammensetninger kan i tillegg inneholde en neutrofil aktivator avhengig av indikasjonsområdet. Vanlig farmasøytiske akseptable bærere og adjuvantia blir brukt for formuleringen.
Det følgende eksempel illustrerer den aktivitetsøkende eller inhiberende virkning av aminoadamantanderivater ved hjelp av den spesielle forbindelse med memantin.
Eksempel
Måleprinsipp:
Bestemmelse av aktivitet til stimulerte neutrofiler ved forskjellige memantin-konsentrasjoner ble utført ved kjemoluminiscens måling. Aktiverte neutro-filer danner reaktive oksygenarter i det respiratoriske utbrudd som er detekterbare som meget svakt kjemoluminiscens, såkalt lavnivå eller ultra-svak kjemoluminiscens. For å øke fotonutbyttet, blir luminol tilsatt som et sensitiverende middel (indikatoravhengig kjemoluminiscens). Luminol er et cyklisk hydrasid som kan oksideres til diazokinon gjennom reaktive oksygen-arter. Ved et nucleofilt angrep av et hydrogenperoksidanion, blir dette diazokinon videre konvertert til et hydroksyhydropéroksid som disintegrerer i aminoftalat under lysemisjon.
Reaksjonsprosion:
Analoge reaksjonsporsjoner kan fremstilles med ytterligere adamantanforbindelser.
Måling:
Måling ble utført med meget følsomme, lav-støy fotomultiplikatorer som konverterer fotoner på en fotokatode til primære elektroner og deretter fler-tal lsforsterker disse til et elektrisk impulssignal.
Reaksjonen startes med et stimuleringsmiddel og måles uten inkubasjonstid i 40 min (= 80 cykler).
Resultater:
Med neutrofiler som ble aktivert med naturlige stimuleringsmidler (Zymosan A, A23187; N-formyl.Met-Leu-Phe), kan RLU-baseraten (RLU = Relative Light Units) kan ytterligere økes med 10"<6>M memantin (Zymosan A ved cirka 90%; A 23287 ved cirka 60%; N-FLMP ved cirka 78% av basestimuler-ingen) [figur 2-4]. Dette antyder et øket respiratorisk utbrudd og en øket fagocytose aktivitet. Denne reaksjonen skjer ikke ved stimulering med toksisk Phorbol-Mvstrat-Asetåt (cirka 20% av basestimulering) [figur 5]. I kontrast inhiberer memantin nesten fullstendig basestimuleringen av alle stimuleringsmidler ved en konsentrasjon på IO<*3.>;I et kontrolleksperiment kan det vises at memantin i seg selv viser verken kjemoluminiscens eller kveler kjemoluminiscens (ikke vist). ;Spesifisiteten til aktiverin<g>sreguleringen: ;Den regulerende virkning av aminoadamantanforbindelser i henhold til oppfinnelsen er meget spesifikk for neutrofiler. Således viser aminoadamantan-forbindelsene ikke noen virkning på den deregulerende virkning av aktiverte leukocyter. Det viser seg fra figur 6 at ACC (l-aminocyklopropan-l-karboksylsyre) i seg selv ikke reagerer med memantin i relativt høye konsen-trasjoner. ACC er en spesifikk indikator for myeloperoksidase som blir frigjort ved degranulering av leukocytter og danner ødeleggende hypokloritt i ekstracellulære rom. Myeloperoksidase reagerer spesifikt med ACC under dannelse av etylen som kan detekteres gaskromatografisk. Derved betegner den dannede mengden av etylen en indirekte målmengde for leukocytt-aktiviteten. ;I kontrast er indikatoren for det respiratoriske utbrudd og oksygenradikalene som kommer derfra (superoksid, OH-radikaler og hydrogenperoksid) aktivert av memantin. Dette er allerede vist ved en konsentrasjon på bare 10"<7>M som lett kan oppnås in vivo. Aktiveringsvirkningen kan gjøres synlig ved tilsett-ing av Fe<3+> som forskyver reaksjonslikevekten gjennom Haber-Weiss reak-sjonen fra superoksid og hydrogenperoksid i retning av OH-radikaler. ;En følsom indikator for oksygenarter av denne type er KMB (a-keto-g-metyltioburat) som disintegrerer i etylen, blant annet under oksidative betingelser og som lett kan detekteres, [figur 5], ;Som en kontroll viser figur 8 at KMB-reaksjonen, men ikke ACC-reaksjonen, blir først stimulert ved tilsetting av Fe<3+> i konsentrasjon på > 5xlO<*4>M. Fe<3+> er alene uten innflytelse.
Claims (9)
1. Anvendelse av memantin (l-amino-3,5-dimetyl adamantan) eller et framasøytisk akseptabelt salt derav til fremstilling av et legemiddel som er nyttig for regulering av aktiviteten av allerede aktiverte neutrofiler, hvori legemidlet øker aktiviteten til neutrofilene i en pasient som allerede har aktiverte neutrofiler gjennom en konsentrasjon av memantin fra IO"<6> til IO"<5 >M i blodplasma.
2. Anvendelse som angitt i krav 1, hvori legemidlet er nyttig til behandling av CGD (agranulomatose), Wegeners granulomatose og/eller immunsviktsykdommer, særlig AIDS.
3. Anvendelse av memantin (l-amino-3,5-dimetyl adamantan) eller et farmasøytisk akseptabelt salt derav, sammen med en aktivator til fremstilling av et legemiddel nyttig til forsterket spesifikk aktivering av neutrofiler, hvori legemidlet øker aktiviteten til nevnte neutrofiler gjennom en konsentrasjon av memantin fra 10"<6> til 10"5 M i blodplasma.
4. Anvendelse som angitt i krav 3, hvori aktivatoren er valgt fra Zymosan, N-formyl-Met-Leu-Phe og A23187.
5. Anvendelse som angitt i krav 3 eller 5, hvori legemidlet er nyttig til behandling av CGD (agranulomatose), Wegeners granulomatose og/eller immunsviktsykdommer, særlig AIDS.
6. Anvendelse av memantin (l-amino-3,5-dimetyl adamantan) eller et farmasøytisk akseptabelt salt derav, for regulering av aktiviteten av allerede aktiverte neutrofiler in vitro, hvori aktiviteten til nevnte neutrofiler er øket gjennom en konsentrasjon av memantin fra IO<*6> til IO"5 M.
7. Anvendelse av (l-amino-3,5-dimetyl adamantan) eller et farmasøytisk akseptabelt salt derav, sammen med en aktivator for forsterket spesifikk aktivering av neutrofiler in vitro, hvori aktiviteten til nevnte neutrofiler er øket gjennom en konsentrasjon av memantin fra IO"<6> til IO"<5> M.
8. Anvendelse som angitt i krav 7, i en deteksjonsmetode for neutrofiler i en prøve.
9. Anvendelse som angitt i krav 7 eller 8, hvori aktivatoren er valgt fra Zymosan, N-formyl-Met-Leu-Phe og A23187.
