Description
TEKNISK OMRÅDE FOR OPPFINNELSEN
Foreliggende oppfinnelse vedrører anvendelse av forbindelse med formel (I) for fremstilling av et medikament for å inhibere et virus som avhenger av en aspartylprotease for en obligatorisk hendelse i sin livssyklus i sentralnervesystemet hos et pattedyr og et medikament som inhiberer den enzymatiske aktivitet av en aspartylprotease i sentralnervesystemet hos et pattedyr.
Mer spesifikt vedrører foreliggende oppfinnelse anvendelse av forbindelse (I) for fremstilling av et medikament for å behandle AIDS-relatert demens forårsaket av HIV og et medikament for å inhibere produksjon av infeksiøse HIV-virioner i sentralnervesystemet hos et pattedyr.
OPPFINNELSENS BAKGRUNN
Humant immunsviktvirus ("HIV") er årsaken til ervervet im-munsviktsyndrom {"AIDS") - en sykdom karakterisert ved øde-leggelsen av immunsystemet, spesielt CD4+ T-celler, med ledsagende susceptibilitet overfor opportunistiske infeksjoner, og dets AIDS-relaterte forløperkompleks ("ARC") - et syndrom karakterisert ved symptomer så som vedvarende generalisert lymfadenopati, feber og vekttap.
Slik som er tilfelle for en rekke andre retrovira, koder HIV for produksjon av en protease som utfører post-translasjonell spalting av forløperpeptider i en prosess som er nødvendig for dannelsen av infeksiøse virioner (S. Crawford et al., "A Deletion Mutation in the 5' Part of the pol Gene of Moloney Murine Leukemia Virus Blocks Proteoly-tic Processing of the gag and pol Polyproteins", J. Virol., 53, s. 899 (1985)). Disse genprodukter innbefatter pol som koder for virion RNA-avhengig DNA-polymerase {revers transkriptase), en endonuklease, HIV-protease og gag, som koder for kjerneproteinene i virionet (H. En rekke syntetiske antivirale agenter har blitt utformet for å rette seg mot en rekke trinn i replikasjonssyklusen for HIV. Disse agentene innbefatter forbindelser som blokkerer viral binding til CD4+ T-lymfocytter (for eksempel, løselig CD4), og forbindelser som forstyrrer viral replikasjon ved å inhibere viral revers transkriptase (for eksempel didanosin og zidovudin (AZT)) og inhibere integrasjon av viralt DNA i cellulært DNA (M.S. Hirsch og R.T. D'Aqulia, "Therapy for Human Immunodeficiency Virus Infec-tion", N. Eng. J. Med., 328, s. 1686 (1993)). Slike agenter som er rettet primært mot tidlige trinn av viral replikasjon, forhindrer imidlertid ikke produksjonen av infeksiøse virioner i kronisk infiserte celler. Ytterligere har administrasjon av noen av disse midlene i effektive mengder medført celletoksisitet og uønskede bieffekter, så som anemi og benmargssuppresjon.
I den senere tid har utforming av antivirale medikamenter fokusert på å skape forbindelser som inhiberer dannelsen av infeksiøse virioner ved å forstyrre prosesseringen av virale polyproteinforløpere. Prosessering av disse forlø-perproteiner krever handling av viruskodende proteaser som er essensielle for replikasjon (Kohl, N.E. et al. "Active HIV Protease is Required for Viral Infectivity", Proe. Nati. Acad. Sei. USA, 85, s. 4686 (1988). Det antivirale poten-sial for HIV-proteaseinhibering har blitt demonstrert ved anvendelse av peptidale inhibitorer. Slike peptidale forbindelser er imidlertid typisk store og komplekse molekyler som har en tendens til å utvise lav biotilgjengelighet og er generelt ikke overensstemmende med oral administrasjon. Følgelig eksisterer fortsatt behovet for forbindelser som effektivt kan inhibere virkning av virale proteaser for anvendelse som midler ved forebygging og behandling av kro-niske og akutte virale infeksjoner.
AIDS og andre HIV-relaterte sykdommer har ofte CNS-komponenter. Én slik komponent er AIDS-relatert demens.
Selv om det finnes en rekke behandlinger for HIV og dets relaterte sykdommer, f.eks. AIDS og ARC, har slike medikamenter liten eller ingen effekt på CNS-effekter av HIV-infeksjon.
Grunnen til at disse behandlingene ikke er effektive mot CNS-effektene av HIV, er at de farmasøytiske sammensetninger som karakteriserer dem, ikke evner å krysse blod-hjernebarrieren i en mengde som er tilstrekkelig til å på-virke og senke HIV-infeksjon i CNS.
AZT, den best kjente HIV-behandling, har for eksempel en hjerne-/bloddistribusjon på kun om lag 0,3. Etter 60 min finnes intet AZT i hjernevevet. De andre HIV-nukleosidene, ddC, ddl og d4T, har sågar verre distribusjonsprofiler i
CNS.
HIV-proteaseinhibitorer penetrerer heller ikke CNS i anvendelige mengder. Abbotts ABT 538 viser for eksempel svært begrenset CNS-penetrasjon. Searle sine inhibitorer har en hjerne-/bloddistribusjon på 0,2 til 0,3. Mercks L-535524 har om lag den samme fordeling.
Således har de foreliggende HIV-nukleosid- og protease-baserte terapier mindre enn ønskelige effekter på CNS-kom-ponentene av HIV.
OPPSUMMERING AV OPPFINNELSEN
Foreliggende oppfinnelse vedrører anvendelse av forbindelse med formel (I) for fremstilling av et medikament for å inhibere et virus som avhenger av en aspartylprotease for en obligatorisk hendelse i sin livssyklus i sentralnervesystemet hos et pattedyr og et medikament som inhiberer den enzymatiske aktivitet av en aspartyl-protease i sentralnervesystemet hos et pattedyr.
Mer spesifikt vedrører foreliggende oppfinnelse anvendelse av forbindelse (I) for fremstilling av et medikament for å behandle AIDS-relatert demens forårsaket av HIV og et medikament for å inhibere produksjon av infeksiøse HIV-virioner i sentralnervesystemet hos et pattedyr.
Forbindelsen med formel I er en HIV-proteaseinhibitor og er som følger:
DETALJERT BESKRIVELSE AV OPPFINNELSEN
Forbindelsen med formel I er en HIV-proteaseinhibitor. I motsetning til andre proteaseinhibitorer har den imidlertid en hjerne-blod-distribusjon på mer enn 1,0. Dette betyr at den meget effektivt krysser hjerne-/blodbarrieren. Faktisk foreligger den i hjernen i samme mengde som i blodet. Ytterligere har forbindelsene med formel I en uventet lang halveringstid i hjernen. Begge disse egenskapene medfører at forbindelsen med formel I er overraskende anvendelig ved behandling av CNS-effekter av HIV, spesielt AIDS-relatert demens.
Forbindelsen med formel I kan fremstilles fra tilgjengelige utgangsmaterialer ved anvendelse av en rekke velkjente syn-teseruter. Eksempler på slike synteser innbefatter de beskrevet i International Patent Application WO 94/05639, som herved er inkorporert heri ved referanse.
Generelt erholdes sulfonamider med formel I fordelaktig fra a-aminosyrederivater med den generelle formel P-N(G)-CH(D)-COOH, hvori P er definert som THF-O-C(O)- eller en aminosyrebeskyttende gruppe, D er definert som benzyl og G er H eller benzyl. Egnede aminosyrebeskyttende grupper er beskrevet i en rekke referanser, innbefattende T.W. Greene og P.G.M. Wuts, Protective Groups in Organic Synthesis, 2<nd >Ed., John Wiley and Sons (1991). Eksempler på slike aminosyrebeskyttende grupper innbefatter, men er ikke begrenset til, karbamatinneholdende grupper så som Boe, Cbz eller Alloc, eller aminet kan alternativt beskyttes som et alkyl-derivat så som N,N-dibenzyl eller trityl. Slike a-aminosyrederivater er ofte kommersielt tilgjengelige eller kan hensiktsmessig fremstilles fra kommersielt tilgjengelige a-aminosyrederivater ved anvendelse av kjente teknikker. Selv om denne oppfinnelse ser for seg anvendelsen av race-miske blandinger av slike utgangsmaterialer, foretrekkes en enkelt enantiomer i S-konfigurasjon.
Ved anvendelse av kjente teknikker kan a-aminosyrederi-vatet med den generelle formel P-N(G)-CH(D)-COOH enkelt omdannes til et aminoketonderivat med den generelle formell P-N(G)-CH(D)-CO-CH2-X, hvori X er en utgående gruppe som på egnet vis aktiverer a-karbonet (dvs. øker susceptibilitet av metylengruppen for nukleofilt angrep). Egnede utgående grupper er velkjente innen faget og innbefatter halider og sulfonater, så som metansulfonater, trifluormetansulfonater eller 4-toluensulfonat. X kan også være et hydroksyl som omdannes in situ til en utgående gruppe (f.eks. ved behandling med trialkyl- eller triarylfosfin i nærvær av dial-kylazodikarboksylat). Fremgangsmåter ved dannelse av slike aminoketonderivater er også velkjent for fagmannen (se for eksempel S.J. Fittkau, J. Prakt. Chem., 315, s. 1037
(1973)). Alternativt er visse aminoketonderivater kommersielt tilgjengelige (f.eks. fra Bachem Biosciences, Inc., Philadelphia, Pennsylvania).
Aminoketonderivatet kan deretter reduseres til den korres-ponderende aminalkohol, representert ved formelen P-N(G)-CH(D)-CH(OH)-CH2-X. Alternativt kan aminoketonderivatet reduseres senere i synteseruten. Mange teknikker for reduksjon av aminoketonderivater, så som P-N(G)-CH(D)-CO-CH2-X, er velkjent for fagmannen (Larock, R.C. "Comprehensive Organic Transformations", s. 527-547, VCH Publishers, Inc.© 1989 og referanser sitert deri). Natriumborhydrid er et foretrukket reduksjonsmiddel. Reduksjonsreaksjonen ut-føres ved en temperatur i området -40 °C til om lag 40 °C (fortrinnsvis ved om lag -10 °C til om lag 20 °C) i et egnet løsningsmiddelsystem så som for eksempel vandig eller ublandet tetrahydrofuran eller en lavere alkohol så som metanol eller etanol. Selv om denne oppfinnelse ser for seg både stereospesifikk og ikke-stereospesifikk reduksjon av aminoketonderivatet P-N(G)-CH(D)-CO-CH2-X, foretrekkes stereoselektiv reduksjon. Stereoselektiv reduksjon kan oppnås ved anvendelse av kirale reagenser kjent innen faget. Ifølge foreliggende oppfinnelse kan stereoselektiv reduksjon oppnås hensiktsmessig for eksempel under ikke-chelate-rende reduserende betingelser, hvor kiral induksjon av den nydannede hydroksylgruppe er gitt ved stereokjemi av D-gruppen (dvs. Felkin-Ahn-tilsats av hydrid). Vi foretrekker spesielt stereoselektive reduksjoner hvori det resulterende hydroksyl er syn til D. Vi har funnet at når hydroksylgruppen er syn til D, er det endelige sulfonamidprodukt en HIV-proteaseinhibitor med høyere styrke enn anti-diastereo-meren.
Hydroksylgruppen av aminalkoholen kan alternativt beskyttes ved enhver kjent oksygenbeskyttende gruppe (så som trial-kylsilyl, benzyl eller alkoksymetyl) for å gi en beskyttet aminalkohol med formelen P-N (G) -CH (D) -C (OR7) -CH2-X, hvori R<7 >er H eller annen egnet hydroksybeskyttende gruppe. Den beskyttende aminalkohol kan deretter reageres med en nukleofil aminforbindelse for å danne et intermediat med formelen III:

I en spesielt fordelaktig synteserute kan det oppnås akti-vering av metylen og beskyttelse av alkoholen ved å danne et N-beskyttet aminoepoksid fra oksygenet og dets tilstø-tende metylen for å gi et intermediat med formelen II:

Egnede løsningsmiddelsys-tem for fremstilling av N-beskyttet aminoepoksid innbefatter vannfrie eller vandige organiske løsningsmidler så som etanol, metanol, isopropanol, tetrahydrofuran, dioksan, dimetylformamid og lignende (innbefattende blandinger derav). Egnede baser for fremstilling av epoksid innbefatter alkalimetallhydroksider, kalium t-butoksid, DBU og lignende. Kaliumhydroksid er en foretrukket base.
Fortrinnsvis lages forbindelsen med formel I ved å frem-stille det N-beskyttede aminoepoksid ved å reagere et dianion av et eddiksyrederivat inneholdende en potensiell utgående gruppe på et a-karbon med et syklisk N-karboksyan-hydrid av en beskyttet a-aminosyre (så som BOC-Phe-NCA, tilgjengelig fra Propeptid) eller annet egnet beskyttet og aktivert aminosyrederivat. Denne fremgangsmåten innbefatter anvendelsen av haloeddiksyrer eller, generelt heteroatom-substituerte eddiksyrer hvori heteroatomer kan omdannes til en utgående gruppe. Et foretrukket eddiksyredianion er (metyltio)eddiksyredianion. Det resulterende aminoketon kan deretter reduseres (f.eks. med natriumborhydrid). I det tilfellet hvor nukleofilen er et dianion av metyltioeddik-syre, omdannes den resulterende aminalkohol lett til aminepoksid ved alkylering (f.eks. med metyljodid) etterfulgt av ringlukning (ved anvendelse av for eksempel natriumhyd-rid) .
Reaksjon mellom det N-beskyttede aminepoksid (eller annet egnet aktivert intermediat) med et amin utføres ublandet, dvs. i fravær av løsningsmiddel, eller i nærvær av et polart løsningsmiddel så som lavere alkanoler, vann, dimetylformamid eller dimetylsulfoksid. Reaksjonen kan hensiktsmessig utføres i området -30°C og 120°C, fortrinnsvis i området -5°C og 100°C. Alternativt kan reaksjonen utføres i nærvær av et aktiveringsmiddel, så som aktivert alumina i et inert løsningsmiddel, fortrinnsvis en eter, så som die-tyleter, tetrahydrofuran, dioksan, eller tert-butylmetyl-eter, hensiktsmessig fra om lag romtemperatur til om lag 110 °C, som beskrevet av Posner og Rogers, J. Am. Chem. Soc., 99, s. 8208 (1977). Andre aktiveringsmidler innbefatter lavere trialkylaluminiumarter, så som trietylaluminium, eller dialkylaluminiumhalidarter, så som dietylaluminium-klorid (Overman and Flippin, Tetrahydron Letters, s. 195
(1981)). Reaksjoner innbefattende disse artene utføres hensiktsmessig i inerte løsningsmidler så som diklormetan, 1,2-dikloretan, toluen eller acetonitril i området 0-110 °C. Ytterligere fremgangsmåter for å forskyve utgående grupper, eller åpne epoksider med aminer eller deres ekvi-valenter så som azider eller trimetylsilylcyanid (Gassman og Guggenheim, J. Am. Chem. Forbindelser med formler II og III og funksjonalitetsbe-skyttede derivater derav er anvendelige som intermediater
ved fremstilling av forbindelsen med formel I. Der L representerer isobutyl, kan forbindelser med formel III omdannes til forbindelsen med formel I ved å reagere med sulfonylaktiverte arter for å danne sulfonamid. Fremgangsmåter ved
fremstilling av slike sulfonylaktiverte arter ligger innenfor fagmannens kunnskap. Typisk anvendes sulfonylhalider for å erholde sulfonamider. Mange sulfonylhalider er kommersielt tilgjengelige, andre kan enkelt erholdes ved anvendelse av konvensjonelle synteseteknikker (Gilbert, E.E., "Recent Developments in Preparative Sulfonation and Sulfation" Synthesis 1969: 3 (1969) og referanser sitert deri; Hoffman, R.V. I tilfelle av forbindelser med formel III hvori L er hydrogen, kan omdannelse av det resulterende primære amin til et sekundært amin utføres ved kjente teknikker. Slike teknikker innbefatter reaksjon med et alkylhalid eller alkylsul-fonat, eller ved reduktiv alkylering med et aldehyd ved anvendelse av for eksempel katalytisk hydrogenering eller natriumcyanoborhydrid (Borch et al., J. Am. Chem. Soc., 93, s. 2897 (1971)). Alternativt kan det primære amin acyleres etterfulgt av reduksjon med boran eller annet egnet reduksjonsmiddel, for eksempel som beskrevet av Cushman et al., J. Org. Chem., 56, s. 4161 (1991). Denne teknikk er spesielt anvendelig i forbindelser med formel III hvori P representerer en beskyttende gruppe så som tert-butoksykarbonyl
(Boe) eller benzyloksykarbonyl (Cbz) og G er hydrogen, eller hvor P eller G er begge benzyl.
Dersom variablene P og G av en spesiell forbindelse med formel IV representerer fjernbare beskyttende grupper, vil fjerning av én av eller begge grupper etterfulgt av reaksjon mellom det resulterende amin med et egnet aktivert reagens fordelaktig gi en annen forbindelse med formel IV. For eksempel kan karbamater erholdes ved reaksjon med klor-karbonater eller med karbonater esterifisert med utgående grupper så som 1-hydroksybenzotriazol (HOBT) eller HOSu, eller 4-nitrofenol (protonerte arter). Et eksempel på et slikt karbonat er N-succinimidyl-(3S)-tetrahydrofuran-3-yl-karbonat. Det skal forstås at for å lette spesifikke reaksjoner, kan det kreves beskyttelse av én eller flere poten-sielle reaktive grupper etterfulgt av påfølgende fjerning av gruppen. Slik modifikasjon av reaksjonsskjemaet skissert over ligger innenfor fagmannens kunnskaper.
Som kan forstås av en fagmann, skal ikke synteseskjemaene forstås som om de omfatter en fullstendig liste av alle midler hvorved forbindelsene beskrevet og krevd i denne søknaden kan syntetiseres. Ytterligere fremgangsmåter vil være åpenbare for fagmannen.
Forbindelsen ifølge oppfinnelsen kan modifiseres ved å til-føye egnede funksjonaliteter for å fremme selektive biolo-giske egenskaper. Slike modifikasjoner er velkjent innen faget og innbefatter de som øker biologisk penetrasjon til en gitt biologisk system (f.eks. blod, lymfatisk system, sentralnervesystemet), øke oral tilgjengelighet, øke løse-lighet for å tillate administrasjon ved injeksjon, endre metabolisme og endre ekskresjonshastighet.
Forbindelsen anvendt ifølge foreliggende oppfinnelse er en utmerket ligand for aspartylproteaser, spesielt HIV-1- og HIV-2-protease. Følgelig evner forbindelsen å rette seg mot og inhibere de senere trinnhendelser i HIV-replikasjon, dvs. prosessering av virale polyproteiner ved HIV-enkodede proteaser. Forbindelsen inhiberer den proteolytiske prosessering av virale polyproteinforløpere ved å inhibere aspartylprotease. Fordi aspartylprotease er essensiell ved fremstilling av modne virioner, blokkerer inhibering av denne prosessering effektivt spredning av virus ved å inhibere produksjon av infeksiøse virioner, spesielt fra kronisk infiserte celler. Forbindelsen ifølge oppfinnelsen inhiberer fordelaktig evnen HIV-l-viruset har til å infisere im-mortaliserte humane T-celler i løpet av noen dager, som be-stemt ved et assay av ekstracellulær p24-antigen - en spesifikk markør for viral replikasjon. Andre antivirale assay har bekrefter effekten av denne forbindelsen.
Forbindelsen anvendt ifølge oppfinnelsen kan anvendes på konvensjonelt vis ved behandling av vira, så som HIV og HTLV, som avhenger av aspartylproteaser for obligatoriske hendelser i deres livssyklus. Slike behandlingsmåter, deres doseringsnivå og krav kan velges av fagmannen ved tilgjengelige metoder og teknikker. For eksempel kan forbindelsen anvendt ifølge oppfinnelsen kombineres med en farmasøytisk akseptabel adjuvans for administrasjon til en viralinfisert pasient på en farmasøytisk akseptabel måte og i en mengde som effektivt lindrer alvorsgraden av den virale infeksjon eller lette patologiske effekter assosiert med HIV-infek-s jon.
Alternativt kan forbindelsen anvendt ifølge oppfinnelsen anvendes ved forebyggelse og fremgangsmåter ved beskyttelse av individer mot viral infeksjon under en spesifikk hendelse, så som fødsel, eller over en forlenget tidsperiode. Forbindelsen kan ved slik forebyggelse anvendes enten alene eller sammen med andre antiretrovirale midler for å fremme effektivitet av hvert middel. Slik kan de nye proteaseinhibitorer anvendt ifølge oppfinnelsen administreres som midler for behandling eller forebyggelse av HIV-infeksjon i et pattedyr.
Forbindelsen med formel I kan enkelt absorberes inn i blod-strømmen hos pattedyr ved oral administrasjon. Forbindelsen med formel I med en molekylvekt på mindre enn om lag 600 g/ml og vandig løselighet større enn eller lik 0,1 mg/ml vil sannsynligvis utvise høy og konsistent tilgjengelighet. Den overraskende imponerende orale tilgjengelighet gjør forbindelsen til et utmerket middel for oral-administrert behandling og forebyggingsregimen mot HIV-infeksjon.
I tillegg til å være oralt tilgjengelig har forbindelsen anvendt ifølge oppfinnelsen også en imponerende høy terapeutisk indeks (som måler toksisitet mot antiviral effekt). Følgelig er forbindelsen ifølge oppfinnelsen effektiv ved lavere dosenivåer enn mange tidligere beskrevne konvensjonelle antiretrovirale agenter og unngår mange av de alvor-lige toksiske effekter assosiert med disse medikamentene. Det potensialet denne forbindelse har til å bli tilført i doser som sterkt overskrider dets effektive antivirale nivå er fordelaktig ved reduksjon av eller forebyggelse av mulighet for utvikling av resistente varianter.
Forbindelsen anvendt ifølge oppfinnelsen kan administreres til en frisk eller HIV-infisert pasient enten som et enkelt middel eller i kombinasjon med andre antivirale midler som forstyrrer replikasjonssyklusen av HIV. Ved å administrere forbindelsen ifølge oppfinnelsen med andre antivirale midler som retter seg mot forskjellige hendelseer i den virale livssyklus, styrkes den terapeutiske effekt av disse forbindelser. For eksempel kan det koadministrerte, antivirale middel være ett som retter seg mot de tidlige hendelser i virusets livssyklus, så som celleinntrenging, revers transkriptase og viral DNA-integrasjon inn i det cellulære DNA. Anti-HIV-midler som retter seg mot slike tidlige syklushendelser innbefatter didanosin (ddl), dideoksycytidin (ddC), d4T, zidovudin (AZT), 3TC, 935U83, 1592U89, 524W91, polysulfaterte polysakkarider, sT4 (løse-lig CD4), ganiclovir, trinatriumfosfonoformat, eflornitin, ribavirin, asyklovir, alfa-interferon og trimenotreksat. <y>tterligere kan ikke-nukleosidinhibitorer av revers transkriptase, så som TIBO, delavirdin (U90) eller nevira-pin, anvendes for å styrke effekten av forbindelsene ifølge oppfinnelsen, og dette gjelder også virale ikke-beleggende inhibitorer, inhibitorer av transaktiverende proteiner, så som tat eller rev, eller inhibitorer av den virale inte-grase.
Kombinasjonsterapier ifølge oppfinnelsen utviser en additiv eller synergistisk effekt ved inhibering av HIV-replikasjon fordi hvert komponentmiddel av kombinasjonen virker på et annet sete av HIV-replikasjon. Anvendelsen av slike kombi-nas jonsterapier reduserer også fordelaktig dosen av et gitt konvensjonelt, antiretroviralt middel som ville kreves for en ønsket terapeutisk eller profylaktisk effekt, sammenlig-net med når middelet administreres som en monoterapi. Slike kombinasjoner kan redusere eller eliminere bivirkningene av konvensjonelle behandlinger med et enkelt antiretroviralt middel uten å forstyrre den antiretrovirale aktivitet av disse midlene. Disse kombinasjonene reduserer motstands-potensial for behandlinger med ett enkelt middel samtidig som man minimerer enhver assosiert toksisitet. Disse kombinasjonene kan også øke effektivitet av det konvensjonelle middel uten å øke den assosierte toksisitet. Spesielt har vi oppdaget at i kombinasjon med andre anti-HIV-midler, virker forbindelsen anvendt ifølge oppfinnelsen på en additiv eller synergistisk måte ved forebygging av replikasjon av HIV i humane T-celler. Foretrukne kombinasjonsterapier innbefatter administrasjon av forbindelsen ifølge oppfinnelsen med AZT, ddl, ddC, d4T, 3TC, 935U83, 1592U89, 524W91 eller en kombinasjon derav.
Alternativt kan forbindelsen anvendt ifølge oppfinnelsen også koadministreres med andre HIV-proteaseinhibitorer, så som saquinavir (Ro 31-8959, Roche), L-735,524 (Merck), ABT 538 (A-80538, Abbott), AG 1341 (Agouron), XM 412 (DuPont Merck), XM 450 (DuPont Merck), BMS 186318 (Bristol-Meyers Squibb) og CPG 53,347 (Ciba Geigy) eller prodroger av disse eller relaterte forbindelser for å øke effekten av behandling eller profylakse mot en rekke virale mutanter eller medlemmer av HIV kvasiarter.
Vi foretrekker å administrere forbindelsen ifølge oppfinnelsen som ett enkelt middel eller i kombinasjon med retro-virale revers transkriptaseinhibitorer, så som derivater av AZT, eller annen HIV-aspartylproteaseinhibitorer, innbefattende multiple kombinasjoner omfattende 3-5 midler. Vi antar at koadministrasjon av forbindelsen anvendt ifølge oppfinnelsen med retroviral revers transriptaseinhibitorer eller HIV-aspartylproteaseinhibitorer kan utøve en vesentlig additiv eller synergistisk effekt, hvorved man forebyg-ger, vesentlig reduserer, eller fullstendig eliminerer viral replikasjon eller infeksjon eller begge deler, samt symptomer assosiert derved.
Forbindelsen ifølge oppfinnelsen kan også administreres i kombinasjon med immunomodulatorer og immunostimulatorer (f.eks. bropirmin, antihumant alfa-interferon-antistoff, IL-2, GM-CSF, interferon-alfa, dietylditiokarbamat, tumor-nekrosefaktor, naltrekson, tuskarasol og rEPO); og anti-stoffer (f.eks. pentamidinisetiorat) for å forhindre eller forebygge infeksjon og sykdom assosiert med HIV-infeksjoner, så som AIDS og ARC.
Når forbindelsen anvendt ifølge oppfinnelsen administreres i kombinasjonsterapier med andre midler, kan de administreres til pasienten sekvensielt eller samtidig. Alternativt kan farmasøytiske sammensetninger fremstilt ved anvendelse ifølge foreliggende oppfinnelse omfatte andre terapeutiske eller profylaktiske midler.
Selv om oppfinnelsen fokuserer på anvendelsen av forbindelsen beskrevet heri ved forebyggelse og behandling av HIV-infeksjon, kan forbindelsen ifølge oppfinnelsen også anvendes som et inhibitorisk middel mot andre vira som avhenger av lignende aspartylproteaser for obligatoriske hendelser i deres livssyklus. Disse vira innbefatter andre AIDS-lignende sykdommer forårsaket av retrovira, så som simian immunsvikt-vira HTLV-I og HTLV-II. Ytterligere kan forbindelsen anvendt ifølge oppfinnelsen også anvendes for å inhibere andre aspartylproteaser og spesielt andre humane aspartylproteaser, innbefattende renin og aspartylproteaser som prosesserer endotelinforløpere.
Sammensetningene ifølge oppfinnelsen tas typisk oralt. De inneholder en mengde av forbindelsen med formel I, hvilken mengde er effektiv ved replikasjon av HIV ved å inhibere dets HIV-protease i CNS.
Forbindelsen med formel I anvendes ifølge foreliggende oppfinnelse i kombinasjon med en farmasøytisk akseptabel bærer. Typisk anvendes den også i kombinasjon med andre AIDS-terapier, spesielt AZT og 3TC.
Farmasøytiske sammensetninger fremstilt ved anvendelse
ifølge oppfinnelsen omfatter forbindelsen ifølge oppfinnelsen og farmasøytisk akseptable salter derav med enhver far-masøytisk akseptable bærer, adjuvans eller vehikkel. Farma-søytisk akseptable bærere, adjuvanser og vehikler som kan
anvendes i farmasøytiske sammensetninger fremstilt ved anvendelse ifølge oppfinnelsen, innbefatter, men er ikke begrenset til, ionebyttere, alumina, aluminiumstearat, lecitin, selvemulgerende medikamentforsyningssystemer (SEDDS) så som da-tokoferolpolyetylenglykol 1000 sukkinat, serumproteiner, så som humant serumalbumin, buffersubstan-ser så som fosfater, glysin, sorbinsyre, kaliumsorbat, del-vise glyseridblandinger av mettede vegetabilske fettsyrer, vann, salter eller elektrolytter, så som protaminsulfat, dinatriumhydrogenfosfat, kaliumhydrogenfosfat, natriumklo-
rid, sinksalter, kollodialt silika, magnesiumtrisilikat, polyvinylpyrrolidon, cellulosebaserte substanser, poly-etylenglykol, natriumkarboksymetylcellulose, polyakrylater, vokser, polyetylenpolyoksypropylen-blokkpolymere, poly-etylenglykol og ullfett. Syklodekstriner så som a-, p- og
y-syklodekstrin eller kjemisk modifiserte derivater, så som hydroksyalkylsyklodekstriner, innbefattende 2- og 3-hydrok-sypropyl-p-syklodekstriner, eller andre solubiliserte derivater kan også fordelaktig anvendes for å fremme leveranse av forbindelsen med formel I.
De farmasøytiske sammensetninger fremstilt ved anvendelse ifølge oppfinnelsen kan administreres oral, parenteralt, ved inhalasjonsspray, topikalt, rektalt, nasalt, bukkalt, vaginalt eller via et implantert reservoar. Vi foretrekker oral administrasjon eller administrasjon ved injeksjon. De farmasøytiske sammensetninger fremstilt ved anvendelse ifølge oppfinnelsen kan inneholde enhver konvensjonell ikke-toksisk farmasøytisk akseptabel bærer, adjuvanser eller vehikler. I noen tilfeller kan pH av formuleringen justeres med farmasøytisk akseptable syrer, baser eller buffere for å fremme stabilitet av den formulerte forbindelse eller dets tilførselsform. Benevnelsen parenteral som anvendt heri, innbefatter subkutane, intrakutane, intrave-nøse, intramuskulære, intraartikulære, intrasynoviale, intrasternale, intratekale, intralesjonale og intrakraniale injeksjons- og infusjonsteknikker.
De farmasøytiske sammensetninger kan være i form av en steril injiserbar fremstilling, for eksempel i form av en steril injiserbar vandig eller oleaginøs suspensjon. Denne suspensjonen kan formuleres ifølge teknikker kjent innen faget ved anvendelse av egnede dispergerings- eller fuktgi-vende midler (så som for eksempel Tween 80) og suspensjons-midler. Det sterile injiserbare preparat kan også være en steril injiserbar løsning eller suspensjon i et ikke-toksisk parenteralt akseptabelt fortynningsmiddel eller løs-ningsmiddel, for eksempel som en løsning i 1,3-butandiol. Blant de akseptable vehikler og løsningsmidler som kan anvendes, er mannitol, vann, Ringers løsning og isoton nat-riumkloridløsning. I tillegg anvendes konvensjonelt sterile, fikserte oljer som løsningsmiddel eller suspensjons-medium. For dette formål kan enhver glatt fiksert olje anvendes, innbefattende syntetiske mono- eller diglyserider. Fettsyrer, så som oleinsyre og dets glyseridderivater, er anvendelige ved fremstilling av injiserbare midler, så som olivenolje eller lakserolje, spesielt i deres polyoksyety-lerte versjoner. Disse oljeløsningene eller suspensjonene kan også inneholde en langkjedet alkoholdiluent eller dis-persant, så som Ph. Heiv., eller en lignende alkohol.
De farmasøytiske sammensetninger ifølge oppfinnelsen kan administreres oralt i enhver oralt akseptabel doseringsform innbefattende, men ikke begrenset til, kapsler, tabletter, og vandige suspensjoner og løsninger. I tilfelle av tabletter for oral anvendelse, innbefatter bærere som normalt anvendes, laktose og maisstivelse. Lubrikerende midler, så som magnesiumstearat, settes typisk også til. For oral administrasjon i kapselform, innbefatter anvendelige diluenter laktose og tørket maisstivelse. Når vandige suspensjoner administreres oralt, kombineres den aktive ingrediens med emulgerende og suspenderende midler. Dersom øns-kelig, kan visse søtningsstoffer og smakstilsetningsstoffer og/eller fargestoffer settes til.
De farmasøytiske sammensetninger fremstilt ved anvendelse ifølge oppfinnelsen kan også administreres i form av suppo-sitorier for rektal administrasjon. Disse sammensetninger kan fremstilles ved å blande en forbindelse ifølge oppfinnelsen med en egnet ikke-irriterende eksipiens som er i fast form ved romtemperatur, men flytende ved rektal temperatur og følgelig vil smelte i rektum slik at de aktive komponenter frigis. Slike materialer innbefatter, men er ikke begrenset til, kakaosmør, bivoks og polyetylengly-koler.
Topikal administrasjon av de farmasøytiske sammensetninger ifølge oppfinnelsen er spesielt anvendelig når den ønskede behandling innbefatter områder eller organer som er lett tilgjengelige ved topikal applikasjon. For topikal applikasjon av huden bør den farmasøytiske sammensetning formuleres med en egnet salve inneholdende de aktive komponenter suspendert eller oppløst i en bærer. Bærere for topikal administrasjon av forbindelsene anvendt ifølge oppfinnelsen innbefatter, men er ikke begrenset til, mineralolje, flytende petroleum, hvit petroleum, propylenglykol, polyoksy-etylen-polyoksypropylenforbindelse, emulgerende voks og vann. Alternativt kan den farmasøytiske sammensetning formuleres med en egnet lotion eller krem inneholdende den aktive forbindelse suspendert eller oppløst i bæreren. Egnede bærere innbefatter, men er ikke begrenset til, mineralolje, sorbitanmonostearat, polysorbat 60, cetylester-voks, cetearylalkohol, 2-oktyldodekanol, benzylalkohol og vann. De farmasøytiske sammensetninger fremstilt ved anvendelse ifølge oppfinnelsen kan også påføres topikalt på den lavere intestinale kanal ved rektal suppositorieformulering eller i en egnet enemaformulering. Topikal-transdermale lapper er også innbefattet av oppfinnelsen.
De farmasøytiske sammensetninger fremstilt ved anvendelse ifølge oppfinnelsen kan administreres ved nasal aerosol eller inhalasjon. Slike sammensetninger fremstilles i hen-hold til teknikker velkjent innen faget av farmasøytisk formulering og kan fremstilles som løsninger i salin ved anvendelse av benzylalkohol eller egnede konserveringsmid-ler, absorpsjonspromotere for å fremme biotilgjengelighet, fluorkarboner og/eller andre solubiliserende midler eller dispergeringsmidler kjent innen faget.
Doseringsnivåer i området 0,01-100 mg/kg kroppsvekt per dag, fortrinnsvis i området 0,5-75 mg/kg kroppsvekt per dag av den aktive ingredienskomponent er anvendelig i forebyg-gelsen og behandlingen av viral infeksjon, innbefattende HIV-infeksjon. Typisk administreres de farmasøytiske sammensetninger ifølge oppfinnelsen i området 1-5 ganger per dag eller alternativt som en kontinuerlig infusjon. Slik administrering kan anvendes som en kronisk eller akutt terapi. Mengden av aktiv ingrediens som kan kombineres med bærermaterialene for å produsere en enkelt doseringsform, vil variere avhengig av verten som behandles og den spesi-elle administrasjonsmodus. Et typisk preparat vil inneholde i området 5-95 % aktiv forbindelse (vekt/vekt). Fortrinnsvis inneholder slike preparat i området 20-80 % aktiv forbindelse .
Ved behandling av en pasients tilstand kan det administreres en dose av forbindelsen, sammensetninger eller kombinasjon av denne oppfinnelsen dersom nødvendig. Deretter kan administrasjonsdosen eller -frekvensen eller begge reduseres, som en funksjon av symptomene, til et nivå hvorved den forbedrede tilstand opprettholdes når symptomene har blitt lindret til det ønskede nivå, behandling bør opphøre [sic.]. Pasienter kan imidlertid behøve intermitterende behandling på langtidsbasis ved tilbakefall av sykdomssymp-tomer.
Slik som fagmannen vil forstå, kan lavere eller høyere doser enn de gitt over være påkrevd. Spesifikk dosering og behandlingsregimet for hver enkelt pasient vil avhenge av en rekke faktorer, innbefattende aktivitet av den spesifikke anvendte forbindelse, alderen, kroppsvekt, generell helsestatus, kjønn, diett, administrasjonstidspunkt, ekskresjonshastighet, medikamentkombinasjon, grad og alvorlig-het av infeksjonen, pasientens disposisjon til infeksjon og evaluering av behandlende lege.

Eksempel 1
Forløper A. En løsning av 102 mg N-((2syn, 3S)-2-hydroksy-4-fenyl-3-((S)-tetrahydrofuran-3-yloksykarbonylaminobutyl-amin i 4:1 CH2Cl2/mettet vandig NaHC03 ble sekvensielt behandlet ved romtemperatur under en nitrogenatmosfære med 65 mg p-nitrobenzensulfonylklorid og 51 mg natriumbikarbo-nat. Blandingen ble rørt ved 14 t, fortynnet med CH2C12, vasket med mettet NaCl, deretter tørket over MgS04, filtrert og konsentrert in vacuo. Residuet ble renset ved lav-trykks-silikagelkromatografi ved anvendelse av 20 % die-tyleter/CH2Cl2 som eluent for å tilveiebringe 124 mg av tittelproduktet som et hvitt faststoff. TLC: Rf = 0,36, 20 % dietyleter/CH2Cl2 HPLC: Rt = 15,15 min (1H) -NMR (CDC13) i overensstemmelse med strukturen.
Eksempel 2
Forbindelse I. En løsning av 124 mg av den resulterende forbindelse fra Eksempel I i etylacetat ble behandlet, ved romtemperatur, med 13 mg 10 % palladium på karbon. Blandingen ble rørt i 14 t under en hydrogenatmosfære, filtrert gjennom en pute av Celittfiltermiddel og konsentrert in vacuo. Residuet ble underkastet preparativ HPLC for å gi 82 mg av tittelproduktet i form av et hvitt faststoff. TLC: Rf = 0,10; 20 % eter/CH2Cl2. HPLC: Rt = 13,16 min. (1H) -NMR (CDC13) i overensstemmelse med strukturen.

for fremstilling av et medikament for å inhibere et virus som avhenger av en aspartylprotease for en obligatorisk hendelse i sin livssyklus i sentralnervesystemet hos et pattedyr.
2. Anvendelse av en forbindelse med formel (I):
for fremstilling av et medikament for å behandle AIDS-relatert demens forårsaket av HIV.
3. Anvendelse av en forbindelse med formel (I) for fremstilling av et medikament som inhiberer den enzymatiske aktivitet av en aspartylprotease i sentralnervesystemet hos et pattedyr.
4. Anvendelse av en forbindelse med formel (I):
for fremstilling av et medikament for å inhibere produksjon av infeksiøse HIV-virioner i sentralnervesystemet hos et pattedyr.
5. Anvendelse ifølge ethvert av kravene 1-4 hvori medika-mentet ytterligere omfatter AZT, 3TC eller begge.
