Description
Den foreliggende oppfinnelse vedrører et nytt salt av (1S,4R)-ds-4-[2-amino-6-{cyklopropyiamino)-9H-purin-9-yl]-2-cyklopenten-1 -metanol, eller et solvat derav, farmasøytiske formuleringer som inneholder en slik forbindelse, eller en farmasøytisk formulering derav og fremstilling av saltet. Det kan anvendes i medisinsk terapi eller metoder for behandling eller profylakse av virussykdommer, spesielt mot menneskers immunsviktvirus (HIV) og heptatitt B virus (HBV) infeksjon.
Forbindelsen (1S,4R)-cjs-4-[2-amino-6-(cyklopropylamino)-9H-purin-9-yl]-2-cyklopenten-1 -metanol og dens antivirale anvendelse, spesielt mot HIV-infeksjoner, er beskrevet i europeisk patentsøknad nr. 0434450, som også viser til farmasøytisk akseptable derivater, spesielt salter, estere og salter av slike estere av forbindelsen, og beskriver spesielt hydrokloirdsalter av forbindelsen.
Anvendelsen av et satt av en forbindelse er velkjent på området, og syreaddisjonssalter av forbindelsen ovenfor er beskrevet i EPO434450. Det er vanskelig å forutsi de fysikalske egenskapene til et spesielt salt av en forbindelse, og små, men signifikante forskjeller i fysikalske egenskaper, kan være ensbetydende med store besparelser ved fremstilling og formulering av et farmasøytisk produkt som inneholder denne forbindelsen.
Forbindelsen (1S.4R)-cis-4-[2-amino-6-(cyklopropylamino)-9H-purin-9-yl]-2-cyklopenten-1-metanol er for tiden under klinisk undersøkelse som et anti-HIV farmasøytisk reagens. Det foreligger et behov for at forbindelsen skal fremstilles i en form som er egnet for lett isolering ved fabrikasjon i stor skala, og for å gjøre formuleringen av et akseptabelt produkt for administrering til mennesker lettere. Vi har funnet at fremstilling av den frie basen av forbindelsen gir en gummi som fanger løsningsmidler, og derfor er uegnet for rensing i stor skala, eller for formulering uten ytterligere renseprosesser.
Det er et mål for den foreliggende oppfinnelse å tilveiebringe et syreaddisjonssalt av (1 S,4R)-cjs-4-[2-amino-6-(cyklopropylamino)-9H-purin-9-yl]-2-cyklopenten-1-metanol eller et solvat derav som er lett og reproduserbart fremstillbart fra ekvimolare mengder av syren og stamforbindelsen i stor skala i høye utbytter, og hvor saltet som fremstilles er stabilt, lett å isolere, f.eks. ved rask og effektiv filtrering, og farmasøytisk akseptabel.
Det er nå funnet at fordelene ved succinatsaltet av den ovennevnte forbindelse fremfor hydrokloridsaltene som er beskrevet, er at' succinatsaltet er spesielt egnet og fordelaktig å fremstille i stor skala, og kan brukes ved fremstilling av farmasøytiske formuleringer for administrering til mennesker.
Ifølge det første aspekt av oppfinnelsen tilveiebringes succinatsaltet av (1S,4R)-cJs-4-[2-amino-6-(cyklopropylamino)-9H-purin-9-yl]-2-cyklopenten-1-metanol eller et solvat derav, innbefattet hydratet derav, heretter vist til som forbindelsen ifølge oppfinnelsen.
Videre aspekter av oppfinnelsen medfører:
a) Forbindelsen ifølge oppfinnelsen for anvendelse ved medisinsk terapi, spesielt ved behandling eller profylakse av virusinfeksjoner, spesielt
en HIV- eller HBV-infeksjon.
b) Anvendelse av en forbindelse ifølge oppfinnelsen ved fabrikasjon av et medikament for behandling eller profylakse av en virusinfeksjon,
spesielt en HIV- eller en HBV-infeksjon.
Forbindelsen ifølge oppfinnelsen er spesielt anvendelig for profylakse eller behandling av HIV-infeksjoner.
Eksempler på kliniske tilstander forårsaket av HIV-infeksjoner som kan behandles ifølge oppfinnelsen omfatter tilegnet immunsviktsyndrom (AIDS) eller symptomer som ofte går forut for AIDS, eller beslektede kliniske tilstander såsom AIDS-relatert kompleks (ARC), progressiv generalisert lymfadenopati (PGL), Kaposis sarcoma, thrombocytopen purpura, AIDS-relaterte nevrologiske tilstander, såsom multippel sklerose eller tropisk paraparese og også anti-HIV anttstoff-positive og HIV-positive tilstander innbefattet AIDS asymoptomatiske pasienter.
Som et ytterligere trekk ved oppfinnelsen tilveiebringes en fremgangsmåte for fremstilling av forbindelsen ifølge oppfinnelsen som består i å oppløse (1S,4R)-cjs-4-[2-amino-6-(cyklopropylamino)-9H-purin-9-yl]-2-cyklopenten-1-metanol og ravsyre i vann, fortrinnsvis i ekvimolare mengder, i et egnet løsningsmiddel, f.eks. vandig etanol eller isopropanol. Slike løsninger fremstilles med fordel ved tilbakeløp. Etter avkjøling til romtemperatur utfelles krystaller av forbindelsen ifølge oppfinnelsen fra romtemperatur utfelles krystaller av forbindelsen ifølge oppfinnelsen fra nevnte løsning i høyt utbytte. Eventuell vasking og omkrystallisering kan anvendes for å øke produktets renhet om ønsket. At forbindelsen ifølge oppfinnelsen foreligger i krystallinsk form gir et middel til fabrikasjon i stor skala ved rask og effektiv filtrering.
Forbindelsen ifølge oppfinnelsen kan gis alene eller i kombinasjon med andre terapeutiske midler egnet ved behandling av HIV-infeksjoner,
såsom Nukleosid Revers Transkriptase-inhibitorer (NRTTer), f.eks. zidovudin, zalcitabin, lamivudin, didanosin, stavudin, S-klor^^-dideoksy-^-fluoridin og (2R,5S)-5-fluor-1-[2-{hydroksyetyl)-1,4-oksatiolan-5-yl]cytosin, ikke-NRTTer, f.eks. nevirapin og a-APA, HIV-proteaseinhibitorer, f.eks. saquinavir eller VX-478, andre anti-HIV-midler, f.eks. løselig CD4, immunmodulatorer, f.eks. interleukin II, erytyropoetin, tucaresol og interferoner, f.eks. a-interferon. I tillegg kan forbindelsen ifølge oppfinnelsen gis i kombinasjon med andre terapeutiske midler egnet ved behandling av HBV-infeksjoner, f.eks. lamivudine, (2R,5S)-5-fluor-1-[2-(hydroksymetyl)-1,3-oksatiolan-5-yl]cytosin, immunmodulatorer og interferoner som beskrevet ovenfor. Slike kombinasjoner kan gis sammen eller i rekkefølge, forutsatt at en avstand mellom administreringene av hvert terapeutisk middel ikke reduserer deres additive effekt.
Selv om det er mulig å gi forbindelsene ifølge oppfinnelsen alene, er det foretrukket og fordelaktig å presentere forbindelsen ifølge oppfinnelsen som en farmasøytisk formulering, og dette utgjør et videre trekk av oppfinnelsen. Den farmasøytiske formulering består i forbindelsen ifølge oppfinnelsen sammen med en eller flere akseptable bærer(e) for den og eventuelt andre terapeutiske midler. Bæreren(e) må være "akseptabel" i den betydning at de(n) er forenlig(e) med de andre bestanddeler i formuleringen og ikke skadelige for mottakerene derav.
Forbindelsene ifølge oppfinnelsen kan administreres ad enhver hensiktsmessig vei for tilstanden som skal behandles, og egnede veier innbefatter oral, rektal, nasal, topisk (innbefattet transdermal, bukal og sublingual), vaginal og parenteral (innbefattet subkutan, intramuskulær, intravenøs, intradermal, intratekal og epidural). Det vil være klart at den foretrukne rute kan variere med f.eks. mottakerens tilstand.
For hver av de ovenfor angitte anvendelsesområder og indikasjoner vil mengden som kreves av den individuelle aktive bestanddel avhenge av en rekke faktorer innbefattet graden av tilstand som skal behandles og mottagerens identitet, og til slutt den behandlende leges skjønn. For hver av disse anvendelser og indikasjoner vil i alminnelighet, imidlertid, en egnet, effektiv dose være i området 3 til 120 mg pr. kg kroppsvekt mottager pr. dag, fortrinnsvis i området 6 til 90 mg pr. kg kroppsvekt pr. dag og helst i området 15 til 60 mg pr. kg kroppsvekt pr. dag. Dosen kan om ønsket presenteres som to, tre, fire eller flere sub-doser administrert med tilsvarende mellomrom over dagen.
Formuleringene innbefatter slike som er egnet for oral, rektal, nasal, topisk (innbefattet bukal og subltngual), vaginal eller parenteral (innbefattende subkutan, intramuskulær, intravenøs, intradermal, intratekal og epidural) administrering. Formuleringene kan hensiktsmessig presenteres i enhetsdoseform og kan fremstilles ved alle metoder som er velkjente på farmasiens område. Slike metoder innbefatter trinnet å bringe den aktive bestanddel i kontakt med bæreren som utgjør én eller flere tilleggsbestanddeler. Generelt fremstilles formuleringene ved å bringe den aktive bestanddel jevnt og tett i kontakt med flytende bærere eller finfordelte faste bærere eller begge, og deretter, om nødvendig, forming av produktet.
Formuleringene ifølge den foreliggende oppfinnelse er egnet for oral administrering kan presenteres som adskilte enheter såsom kapsler, cacheter eller tabletter som inneholder en forutbestemt mengde av den aktive bestanddel; som et pulver eller granulat; som en løsning eller suspensjon i en vandig væske eller en ikke-vandig væske; som en olje-i-vann flytende emulsjon eller vann-i-olje flytende emulsjon. Den aktive bestanddel kan også presenteres som en pille eller pasta eller kan foreligge inne i liposomer.
En tablett kan fremstilles ved kompresjon eller støping, eventuelt med én eller flere hjelpebestanddeler. Komprimerte tabletter kan fremstilles ved å komprimere den aktive bestanddel i en fritt-flytende form såsom et pulver eller granulat, eventuelt blandet med et bindemiddel (f.eks. povidon, gelatin, hydroksypropylmetylcelEulose), smøremiddel, inert fortynningsmiddel, sprengningsmiddel (f.eks. natriumstivelseglykollat, kryssbundet povidon, kryss-bundet natriumkarboksymetylcellulose), overflateaktivt eller dispergeringsmiddel i en egnet maskin. Støpte tabletter kan lages ved støping i en egnet maskin av en blanding av den pulveriserte forbindelse fuktet med et inert flytende fortynning smidde!. Tablettene kan eventuelt belegges eller innkapsles, og kan formuleres slik at de gir langsom eller kontrollert frigjøring av den aktive bestanddel deri ved å bruke f.eks. hydroksypropylmetyl-cellulose i varierende forhold for å gt den ønskede frigjøringsprofil.
En kapsel kan lages for å fylle et løst eller komprimert pulver i en tilsvarende fyllemaskin, eventuelt med ett eller flere additiver. Eksempler på egnede additiver omfatter bindemidler såsom povidon, gelatin, smøremidler, inerte fortynningsmidler, sprengningsmidler som for tabletter. Kapsler kan også formuleres slik at de inneholder pellets eller adskilte underenheter for å gi langsom eller kontrollert frigjøring av den virksomme bestanddel. Dette kan oppnås ved å ekstrudere eller kuleforme en våt blanding av legemidlet pluss en ekstruderingssyre (f.eks. mikrokrystallinsk celllulose) pluss et fortynningsmiddel såsom laktose. Sferoidene som derved dannes kan belegges med en halvgjennomtrengelig membran (f.eks. etylcellulose, Eudragit WE30D) for å gi vedvarende frigjøringsegenskaper.
For infeksjoner i øyet eller annet utvendig vev, f.eks. munn og hud, påføres formuleringene fortrinnsvis som en topisk salve eller krem inneholdende den aktive bestanddel i en mengde på f .eks. 0,075 til 20 vekt-%, fortrinnsvis 0,2 til 15 vekt-% og helst 0,5 til 10 vekt-%. Formulert i en salve kan de aktive bestanddeler anvendes med enten en parafinisk eller en vannblandbar salvebase. Alternativt kan de aktive bestanddeler formuleres i en krem med en olje-i-vann-krembase eller som en vann-i-oljebase.
Om ønsket, kan den vandige fase av kremen inneholde f.eks. minst 40-45 vekt-% av en flerverdig alkohol, dvs. en alkohol med to eller flere hydroksylgrupper såsom propylenglykol, butan-1,3-diol, mannitol, sorbitol, glyserol og polyetylenglykol og blandinger derav. De topiske formuleringer kan om ønsket inneholde en forbindelse som forsterker absorpsjon eller gjennomtrengning av den aktive bestanddel gjennom huden eller andre påvirkede områder. Eksempler på slike dermale gjennomtrengningsøkere er dimetylsulfoksyd og beslektede analoger.
Den oljeaktige fase av emulsjonene ifølge denne oppfinnelse kan bestå av kjente bestanddeler på en kjent måte. Selv om denne fasen kan inneholde bare et emulgeringsmiddel (ellers kjent som en emulgator), omfatter den fortrinnsvis en blanding av minst ett emulgeringsmiddel med et fett eller en olje eller med både et fett og en olje. Fortrinnsvis inngår en hydrofil emulgator sammen med en lipofil emulgator som virker som en stabilisator. Det foretrekkes å innta både en olje og et fett. Sammen utgjør emulgatoren(e) med eller uten stabilisator(er) den såkalte emulgeringsvoks, og voksen sammen med oljen og/eller fett utgjør den såkalte emulsjons-salvebase som danner den oljedispergerte fase av kremformuleringene.
Emulgatorer og emulsjonsstabilisatorer egnet for bruk i formuleringen ifølge den foreliggende oppfinnelse omfatter Tween 60, Span 80, cetostearylalkohol, myristylalkohol, glyseryl-mono-stearat og natriumlaurylsulfat.
Forbindelsen (1 S,4R)-cjs-4-[2-amino-6-(cyklopropylamino)-9H-purin-9-yl]-2-cyklopenten-1-metanol kan syntetiseres ifølge europeisk patent nr. 0434450 eller alternativt PCT-søknad nr. GB/9500225 som herved inntas som henvisning.
Ravsyre kan kjøpes (Aldrich Chemical Company, Dorset, England).
Eksempel A
Fremstilling av (1 S.4R)-cis-4-[2-amino-6-(cyklopropylamino)-9H-purin-9-yn-2-cyklopenten-1-metanol-ravsyresalt.
En løsning av (1S,4R)-cjs-4-[2-amino-6-(cyklopropylamino)-9H-purin-9-yl]-2-cyklopenten-1 -metanol (30,93 g, 0,102 mol) i absolutt etanol (96 ml) ble satt til en løsning av ravsyre (Aldrich, 99%, 12,15 g, 0,102 mol) i vann (130 ml). Blandingen ble bragttil tilbakeløp, og den resulterende løsning ble behandlet med benkull (0,4 g) og filtrert gjennom et celittsjikt. Lysegule krystaller (38,0 g) dannet seg mens løsningen avkjøltes langsomt til romtemperatur. Dette faststoff ble omkrystallisert fra vann (500 ml) og ga (IS^RJ-cJs^-p-amino-e-tcyklopropylamino^H-purin-g<->yll^-cyklopenten-l-metanol-succinat som et hvitt pulver (34,9 g, 80%), smp. 168-169°C;

De følgende formuleringer A, B og C fremstilles ved våtgranulering av bestanddelene med en løsning av povidon, etterfulgt av tilsetning av magnesiumstearat og kompresjon.

Kapsler fremstilles ved å dispergere den aktive bestanddel i lecitinet og arakisolje og fylling av dispersjonen i myke, elastiske gelatinkapsler.
Formulering C ( Kontrollert frigjøringskapsel)
Den følgende kontrollerte frigjøringskapselformulering fremstilles ved å ekstrudere bestanddelene a, b og c ved bruk av en ekstruder, etterfulgt av kuledannelse av ekstrudat og tørking. De tørkede pellets belegges så med frigjøringskontrollerende membran (d) og fylles i en to-biters hardgelatinkapsel.
Eksempel D: Iniiserbar formulering
Den aktive bestanddel oppløses i mesteparten av fosfatbufferen (35-40°C), deretter fylles det opp til volum og filtreres gjennom et sterilt mikroporefilter i en steril 10 ml ravfarget glassflaske (type 1) og tettes med sterile lukninger og overtetninger.
Eksempel E: Intramuskulær injeksjon
Den aktive bestanddel oppløses i glykofurolen. Benzylalkoholen tilsettes deretter og oppløses, vann tilsettes til 3 ml. Blandingen filtreres deretter gjennom et sterilt mikroporefilter og forsegles i sterile 3 ml glassflasker (type 1).
Natriumbenzoatet oppløses i en del av det rensede vann og

1/5 av Witepsol H15 smeltes i en dampkappepanne ved 45°C maksimum. Den aktive bestanddel siktes gjennom en 200 \ im sikt og settes til den smeltede base under blanding ved bruk av en silverson utstyrt med et skjærehode, inntil en jevn dispersjon oppnås. Mens blandingen holdes på 45°C tilsettes det gjenværende Witepsol H15 til suspensjonen og røres for å sikre en homogen blanding. Hele suspensjonen føres gjennom en 250 Im rustfri stålsikt og får under kontinuerlig røring avkjøles til 40°C. Ved en temperatur på 38°C til 40°C fylles 2,02 g av blandingen i egnede plastformer. Suppositoriene får kjøle til romtemperatur.
Eksempel H: Pessarer
Bestanddelene ovenfor blandes direkte og pessarer fremstilles ved direkte kompresjon av den resulterende blanding.
Eksempel 1: Tooisk formulering
Oppløs den aktive bestanddel i en blanding av renset vann og glyserol og oppvarm til 70°C. Oppvarm de gjenværende bestanddeler sammen på 70°C. Tilsett de to deler til hverandre og emulger. Avkjøl og fyll i beholdere.

1. Ravsyresaltet av (1 S,4R)-cis-4-[2-amino-6-(cyklopropylamino)-9H-purin-9-yl]-2-cyklopenten-1-metanol.
2. Et solvat av forbindelsen ifølge krav 1.
3. Et hydrat av forbindelsen ifølge krav 1.
4. Forbindelse ifølge ethvert av kravene 1 til 3 for anvendelse i medisinsk terapi.
5. Anvendelse av en forbindelse ifølge ethvert av kravene 1 til 3 ved fremstilling av et medikament for behandling eller profylakse av en virusinfeksjon.
6. Fremgangsmåte for fremstilling av forbindelsen ifølge krav 1, karakterisert ved blanding av(1 S,4R)-cis-4-[2-amino-6-(cyklopropylamino)-9H-purin-9-yl]-2-cyklopenten-1-metanol i et egnet løsningsmiddel med ravsyre i vann og ved slike betingelser at det oppnås dannelse av forbindelsen ifølge krav 1.
7. Farmasøytisk formulering,
karakterisert ved at den omfatter en forbindelse ifølge ethvert av kravene 1 til 3 og minst én farmasøytisk akseptabel bærer for denne.
8. Farmasøytisk formulering ifølge krav 7,
karakterisert ved at den er formet til en tablett eller en kapsel.
9. Farmasøytisk formulering ifølge krav 7,
karakterisert ved at den er tilpasset parenteral administrering.
