Abstract
Kontrastmidler bestående av mikroboble-dannende karbohydrat-mikropartikler med surfaktant innblandet i den mikropartikulære struktur (med forbehold at surfaktanten ikke er en mettet C10-20 fettsyre når det mikropartikulære karbohydratet er galaktose) viser nyttige nivåer av kontrastvirkning og/ eller stabilitet og kan brukes i diagnostisk avbildning innen ultralyd og MR.
Description
Denne oppfinnelsen angår nye kontrastmidler, mer spesielt nye mikxopartikulære kontrastmidler for bruk i diagnostisk avbilding.
Det er godt kjent at ultralydavbilding er et potensielt verdifullt diagnostisk verktøy, for eksempel i studier av blodåresystemet, spesielt innen kardiografi og av hårrørsnettet i vevene. Et variert utvalg av kontrastmidler er blitt foreslått for å forbedre de akustiske bildene oppnådd på denne måten, inkludert suspensjoner av faste partikler, emulgerte væskedråper, gassmikrobobler og innkapslede gasser eller væsker. Det er generelt akseptert at kontrastmidler som har lav tetthet, og er lett komprimerbare, er spesielt effektive i egenskap av det gode akustiske ekko de gir, og betydelig interesse er derfor blitt viet framstillingen av gass-inneholdende og gass-genererende systemer.
Innledende studier av frie gassmikrobobler generert in vivo ved injeksjon i hjertet av fysiologisk aksepterbare substanser har demonstrert den potensielle effektivitet av slike bobler som kontrastmidler i ekkokardiografi. Slike teknikker er derimot kraftig begrenset i praksis, på grunn av den korte levetiden til de frie boblene. Det er derfor av stor interesse å framstille og/eller stabilisere gassmikrobobler for ekkokardiografi og andre ultralydundersøkelser, for eksempel ved å bruke emulgatorer, oljer, fortykningsmidler eller sukkerarter.
Metoder som omfatter bruk av sukkerarter i ultralydkontrastmidler er beskrevet i for eksempel, US-A-4.681.119, US-A-4.442.843 og US-A-4.657.756, som omtaler nytten av partikulært tørrstoff med pluralitet av gassfylte tomrom og fortrinnsvis også en pluralitet av kjerner for mikroboble-dannelse. EP-A-0123235 og EP-A-0122624 antyder ultralyd-kontrastmidler bestående av surfaktant-belagte eller surfaktant- og gass-inneholdende mikropartikler som kan inkludere ulike sukkere. Hvor surfaktantinneholdende mikropartikler er beskrevet, er disse tillaget ved å blande surfaktanten med mikropartikkelmaterialene, for eksempel ved findeling.
DE-A-3834705 foreslår bruk av suspensjoner inneholdende mikropartikler av blandinger av idet minste én C10-20 fettsyre med i det minste én ikke-overflateaktiv substans, inkludert sukkere slik som cyclodekstriner, monosakkarider, disakkarider eller trisakkarider, så vel som andre polyoler og uorganiske og organiske salter. I praksis er det kun bruk av galaktose som det ikke-overflateaktive materialet og kun bruk av mettede fettsyrer som er eksemplifisert. Mikropartikulære materialer er typisk tillaget ved kopresipitering av fettsyren og den ikke-overflateaktive substans og pulverisering av det resulterende produkt, f.eks. ved bruk av luftstrålemølle.

Gass-holdige kontrastmidler er også kjent for å være effektive i magnetisk resonans (MR) avbilding, f.eks. som følsomhetskontrastmidler som vil redusere MR signalintensitet. Oksygen-inneholdende kontrastmidler representerer også potensielt nyttige paramagnetiske MR kontrastmidler.
Videre har det på røntgenavbildningsområdet blitt observert at gasser slik som karbon dioksid kan nyttes som negative orale kontrastmidler.
En generell ulempe ved de fleste eksisterende gass-inneholdende/gass-genererende partikulære kontrastmidler, slik som de sukkerbaserte midlene diskutert ovenfor, er deres relative mangel på stabilitet in vivo. Dette er et spesielt problem for anvendelser som ved ekkokardiografi, der det er behov for forbedrede kontrastmidler som kombinerer tilstrekkelig stabilitet med liten mikroboblestørrelse (typisk mindre enn omtrent 10 Um, fortrinnsvis mindre enn 7 Um) for å tillate passasje gjennom kapillærnettet i lungene, og derved muliggjøre forbedret visualisering av den venstre siden av hjertet, fortrinnsvis ved mer enn en gangs omløp. Det er derfor behov for kontrastmidler som framkaller mikroboblesystemer med god stabilitet og som samtidig gir et effektivt kontrastnivå.
Denne oppfinnelsen er basert på vår oppdagelse av at kontrastmidler som omfatter mikropartikler av et karbohydrat med innblandet surfaktant (men med utelukkelse av tidligere framlagte blandinger av galaktose og mettede Ci0-20 fettsyrer) kan benyttes til å framkalle mikroboblesystemer med vesentlig økt stabilitet og/eller kontrasteffekt sammenlignet med tidligere foreslåtte karbohydrat-baserte kontrastmidler. På ultralyd-området kan dette bli demonstrert for eksempel ved in vitro måling av initiale dempningsnivåer og av halveringstid for attenueringen (dempningen); en nyttig indikasjon på kombinert effekt av disse egenskaper er integralet av arealet under kurven av dempingen mot tiden.
Med begrepet "surfaktant" forstås en hvilken som helst forbindelse med amfifile egenskaper som er i stand til å modifisere overflate-spenningen.
Ifølge et aspekt tilveiebringer den foreliggende oppfinnelsen kontrastmidler som består av mikrobobleframkallende karbohydrat-mikropartikler med en surfaktant innblandet i den mikropartikulære strukturen, med forbehold av at surfaktanten ikke er en mettet C10-20 fettsyre når det mikropartikulære karbohydrat er galaktose.
Det mikropartikulære karbohydratet er fortrinnsvis vannoppløselig, og kan med nevnte forbehold for eksempel bli utvalgt fra heksoser som glukose, fruktose eller galaktose; disakkarider som sukrose, laktose eller maltose; pentoser som arabinose, xylose, eller ribose; og polysakkarider som P- og Y-syklodekstriner, maltodekstrin og glykogen; begrepet "karbohydrat" som her benyttes er også ment å omfatte sukkeralkoholer, for eksempel alditoler som mannitol eller sorbitol. Mikropartikler av ovennevnte karbohydrater vil normalt inneholde gass innesluttet i hulrom i krystallstrukturen og/eller festet til overflaten. Gassen kan framkalle mikrobobler, for eksempel når mikropartiklene blir suspendert eller løst i en injiserbar bærervæske, for eksempel vann for injeksjon, en vandig løsning av ett eller flere uorganiske salter (for eksempel fysiologisk saltvann eller en fysiologisk bufferoppløsning), en vandig løsning av monosakkarid (f.eks. glukose eller galaktose) eller disakkarid (f.eks. laktose), eller en vandig løsning av en fysiologisk akseptabel enverdig eller flerverdig alkohol (f.eks. etanol, propanol, isopropanol, etylenglykol, propylenglykol, glyserol eller polyetylenglykol).
I tillegg til eller alternativt til luft, kan enhver biokompatibel gass benyttes i kontrastmidlene ifølge oppfinnelsen, for eksempel nitrogen, oksygen, hydrogen, dinitrogenoksid, karbondioksid, helium, argon, svovelheksafluorid og (eventuelt fluorerte) hydrokarboner med lav molekylvekt slik som metan, acetylen eller karbontetrafluorid. Begrepet "gass" slik det brukes her omfatter ethvert stoff i gassform ved 37 °C. Gassen kan foreligge i kontrastmidlet på en slik måte at produktet ikke gir kontrast før bruk men blir effektivt ved administrering, f.eks. som et resultat av at gassen danner mikrobobler når et løselig karbohydrat går i oppløsning.
Karbohydratet kan i tillegg, eller som alternativ, inneholde en eller flere gass-forløpere, inkludert karbonater og hydrogenkarbonater (f.eks. natrium eller ammonium hydrogenkarbonat) og aminomalonat-estere.
Med ovennevnte forbehold kan en bred variasjon av surfaktanter bli brukt i oppfinnelsens ultralydkontrastmidler, og det vil selvfølgelig bli verdsatt at surfaktanten kreves å være biokompatibel, med andre ord at den skal være fysiologisk akseptabel i de mengder som brukes ved administrering. Surfaktanten er fortrinnsvis biologisk nedbrytbar in vivo eller blir på annen måte raskt fjernet fra systemet.
Surfaktanten kan for eksempel være et amfifilt lipid, for eksempel utvalgt fra fettsyrer eller deres salter (for eksempel alkalimetall-salter), steroidsyrer, steroler, fosfolipider og glykolipider. Slike lipider inkluderer substanser med høy molekylvekt (for eksempel C 10-50) som rettkjedede mettede og umettede alifatiske syrer, som kapron-, palmitin-, heksadekandion-, stearin-, linolen-, behen-, docosandion- og melissin-syre; aralkansyrer, for eksempel fenyl-lavalkansyrer som 2-fenylsmørsyre; salter av de foregående syrene; mono- og diglyserider, for eksempel glyserylestere av alifatiske syrer med høy molekylvekt (f.eks. C10-50), som glyseryl monolaurat; kolansyrer som 5P-kolansyre; kolesterol; sorbitan-fettsyreestere som av "Span"-type rettkjedede alifatiske alkoholer som stearylalkohol og cetylalkohol; fosfolipider som fosfatidylcholin (lecitin) og dioleoylfosfatidyl etanolamin (DOPE); og blandinger av disse.
Andre surfaktanter som kan benyttes inkluderer anioniske surfaktanter, for eksempel alkalimetall-alkyl-sulfater som natrium-lauryl-sulfat og sulfonerte estere som natrium dioktyl sulfosukksinat ("docusate") og ikke-ioniske surfaktanter, for eksempel polyoksyetylen-polyoksypropylen kopolymerer (for eksempel poloxamerer som "Pluronic" F68) og polyoksyetylerte sorbitanestere (for eksempel polysorbater som "Tween-type" materialer).
Surfaktantgruppen kan om ønskelig være kovalent bundet til et substrat som karbohydrat før innblanding i det hoved-mikropartikulære karbohydratet. For eksempel, en fettsyre som palmitinsyre (fortrinnsvis i form av et radioaktiv derivat, som en tilsvarende acyl-halid) kan brukes til å forestre et (fortrinnsvis passende O-beskyttet) sukker slik som galaktose, og det resulterende lipofilt modifiserte karbohydratet kan benyttes som surfaktant i henhold til oppfinnelsen.
Surfaktanten kan, for eksempel, være tilstede i en mengde av 0,01-5 vekt %, fortrinnsvis 0,1-2 vekt %, relativt til det mikropartikulære karbohydratet.
Oppfinnelsens kontrastmidler kan benyttes i variasjoner av diagnostiske avbildningsteknikker, inkludert ultralyd, MR og røntgenkontrast. Bruken i diagnostisk ultralydavbildning og MR-avbildning, for eksempel som følsomhets-kontrastmidler, utgjør oppfinnelsens foretrukne egenskaper.
Oppfinnelsens kontrastmidler kan framstilles ved en hvilken som helst egnet metode som leder til fysisk innblanding av surfaktanten i den mikropartikulære karbohydratstrukturen og til framstilling av mikropartikler av ønsket størrelse.
I en foretrukket metode i henhold til oppfinnelsen er karbohydratet og surfaktanten begge løst i passende innbyrdes blandbare løsningsmidler (f.eks. vann for karbohydratet og en lavere alkanol som etanol for lipidsurfaktanter som fettsyrer), de resulterende oppløsninger blandes, løsningsmidlene fjernes (f.eks. ved inndampning under redusert trykk), og den resulterende tørrstoffblanding blir mikronisert for å gi de ønskede mikropartikler. Det er verdsatt at alle slike operasjoner finner sted under sterile betingelser.
I en alternativ metode i henhold til oppfinnelsen blir en (foretrukket vandig) oppløsning av karbohydratet blandet med et liposomdannende materiale (f.eks. en tynn film av lipid som lecitin, dannet på den innvendige overflate av blandekaret ved å avdampe løsningsmidlet fra en oppløsning av lipidet i et passende organisk løsningsmiddel, for eksempel en klorert hydrokarbon som kloroform) slik at det dannes en liposom-inneholdende karbohydratoppløsning hvorfra løsningsmidlet kan fjernes ( f.eks. ved frysetørking) for å gi et produkt bestående av karbohydrat-inneholdende liposomer; dette produkt kan bli mikronisert til gitte mikropartikler av ønsket størrelse.
Generelt kan vanlige rnikroniseringsteknikker som kveming eller finmaling benyttes i prosesser i henhold til oppfinnelsen. Finmaling av tørrstoffblandingen i kulemølle er funnet å være særlig fordelaktig, og åpner muligheten for framstilling av mikropartiklene i form av aggregater (for eksempel med en aggregatstørrelse på 20-125 mikrometer, slik som 30-50 mikrometer) av partikler med en partikkelstørrelse på, for eksempel, 1-50 mikrometer, slik som 1-10 mikrometer. Slike aggregater vil være tilbøyelig til å inneholde et betydelig luftvolum absorbert på overflatene og inneholdt i hulrom f.eks. mellom partiklene eller på korn-grenser mellom krystallitiene. Partikkelstørrelsen kan, for eksempel, velges slik at man oppnår ønsket mikroboblestørrelse. For bruk i ultralyd slik som ekkokardiografi, for å tillate passasje gjennom lungesystemet og for å oppnå resonans med de foretrukne avbildingsfrekvenser på omtrent 0,1 - 15 MHz, kan det være fordelaktig å bruke mikrobobler og mikropartikler med en gjennomsnittlig størrelse på 0,1 - 10 Pm, for eksempel 1-7 Pm; bruk av mikropartikler med en gjennomsnittsstørrelse på 1 - 4 Pm som kan gi mikrobobler med en gjennomsnittsstørrelse på 4 - 7 <y>m er generelt fordelaktig. Vesentlig større bobler og partikler, for eksempel med gjennomsnittsstørrelser på inntil 500 Pm kan allikevel være nyttige til andre formål, for eksempel avbilding av mage- og tarmregionen.
Ultralydkontrastmidler i form av mikropartikler omfattende et mikroboble-framkallende karbohydrat innblandet med en amfifil organisk syre med mer enn 20 karbonatomer er søknadsgjenstand for vår internasjonale patentsøknad som er innlevert samtidig med krav om prioritet fra Britisk patentsøknad Nr. 9200387.0.
De følgende eksempler skal tjene til å illustrere oppfinnelsen:
Eksempel 1- 18
Generell framgangsmåte
D-(+) -galaktose (10,0 g) ble løst i destillert vann (14,2 g) ved 50 °C, sterilfiltrert og avkjølt på is til en temperatur på 4-8 °C. De angitte mengder av surfaktant oppført i tabell 1 ble hver løst i angitt mengde av 96% etanol, ved 50-78 °C, og den resulterende løsning ble sterilfiltrert og deretter aseptisk tilsatt til den avkjølte vandige galaktoseløsning under røring. Den resulterende blanding ble inndampet til tørrhet under redusert trykk (10 torr, 40 °C), og det resulterende faste produktet ble tørket i desikkator over natten og deretter malt i 10 minutter under aseptiske forhold i en rustfri stålkulemølle med en 50 ml beholder og 3 kuler (20 mm diameter) (Retsch centrifugal ball mill, Sl). Det malte produktet ble tørket i desikkator i 24 timer.
Eksempel 19- 22
Den generelle prosedyre for eksemplene 1-18 ble gjentatt bortsett fra at D-(+)-galaktose ble byttet med karbohydratene oppført i tabell U, i mengder som angitt, og ved bruk av vannmengdene som vist, og at surfaktanten var palmitinsyre (0,2% w/w relativt til karbohydratet) løst i 96% etanol (l,2g).
Eksempel 23
6- O- Palmitovl- D- galaktopvranose/ galaktose blandinger
(A) 6- 0- Palmitoyl- l, 2, 3. 4- diisopropvliden- D- galaktopvranose 1,2,3,4-Diisopropyliden-D-galaktopyranose (Sigma, 13,4 g, 51,3 mmol)
og trietylamin (7,15 ml, 51,3 mmol) ble løst i metylenklorid (150 ml) og avkjølt til 0°C. Palmitoylklorid (Aldrich, 14,1 g, 51,3 mmol) løst i metylenklorid (100 ml) ble tilsatt dråpe vis under røring i 1 time. Kjølebadet ble fjernet og reaksjonsblandingen ble satt under røring over natten. Utfelt trietylamin hydroklorid ble fjernet ved filtrering, filtratet ble overført til en skille trakt og ekstrahert med vann (3 x 50 ml) og tørket over MgSCU, hvoretter løsningsmidlet ble fjernet in vacuo. Residuet var en lett brunaktig olje som stivnet til voksaktige krystaller. Råutbytte: 23 g. Råproduktet ble benyttet uten videre rensing. En liten alikvot ble omkrystallisert for karakterisering. FT-
IR: CO-1734 cm"<1>.<13>C-NMR: CO ester 172,79. Sm. pkt. 124-127°C.
(B) 6- O- Palmitovl- D- galaktopvranose
6-0-Palmitoyl-l,2,3,4-diisopropyliden-D-galaktopyranose (6 g) ble løst
i eddiksyre (25 ml) og varmet til 100°C under nitrogen i 6 timer. Under
påfølgende avkjøling til romtemperatur presipiterte produktet fra løsningsmidlet, og ble satt i romtemperatur over natten. Krystallene ble samlet ved filtrering og tørket under vakuum. Utbytte: 3,3 g. Produktet ble karakterisert ved FT-IR: CO-1734 cm"<1>; OH-3464 cm"<1>. (C) 6- O- Palmitovl- D- galaktopvranose/ galaktose blandinger (i) D-(+)-galaktose (2 g) ble løst i renset vann (2,87 g) og sterilfiltrert. 6-0-Palmitoyl-D-galaktopyranose (0,25) framstilt som i (B) ovenfor ble løst i etanol (3 g) og sterilfiltrert. Palmitoyl-galaktopyranose-løsningen ble tilsatt til galaktoseløsningen under røring hvoretter hele blandingen ble tørket under vakuum (10 torr, 50°C). Produktet ble tørket i desikkator over natten. (ii) Framgangsmåten (i) ble gjentatt ved bruk av 6-O-Palmitoyl-D-galaktopyranose (0,50 g) løst i etanol (6 g).
Eksempel 24
Frysetørkede liposomer inneholdende D-(+)- galaktose- partikler
1 ml fosfatidylcholin (100 mg/ml) ble løst i 10 ml kloroform. Blandingen ble helt over i en rundkolbe, og den organiske fasen ble fordampet ved 40°C slik at en tynn film av fosfatidylcholinet ble dannet på innsiden av kolben. 10 ml steril pyrogenfri 40% vandig D-(+)-galaktose-løsning ble deretter tilført ved 40°C hvoretter kolben ble satt til 1 times rotering. Den vandige blandingen inneholdende liposomer og løst galaktose ble deretter frysetørket i 24 timer, og det resulterende produkt bestående av frysetørket galaktose og frysetørkede galaktosefylte liposomer ble deretter malt i en kulemølle for å gi et produkt med en partikkelstørrelsesfordeling på 1-20 Pm.
Eksempel 25
Ekkogenisitet in vitro
10 ml propylenglykol blandet med 90 ml 5% dekstrose i vann ble brukt som bærervæske for å bestemme produkt-ekkogenisitet i henhold til eksemplene. 1,0 g av hvert testet produkt ble dispergert i 3,0 ml av bærervæsken og ristet i 15 sekunder. Den resulterende blandingen ble tilsatt til 52 ml 5 % humant serum albumin infusjonsoppløsning i målecellen og produktenes akustiske effekt ble undersøkt ved å måle den akustiske transmisjon gjennom prøvene ved bruk av 5 MHz bredbånds-transducer med en pulsrefleksjonsteknikk. Temperaturen i målecellen ble stabilisert til 37°C og væskesirkulasjonen ble opprettholdt ved konstant røring. Ultralyd-transmisjon gjennnom prøvene ble målt som en funksjon av tiden i 390 sekunder. Resultater ble normalisert til målinger av en referanseløsning bestående av 55 ml av 5% humant serum albumin infusjonsoppløsning.
Resultater av representative eksemplifiserte produkter og sammenlignende resultater av umodifisert malt D-(+)-galaktose er vist i ledsagende tegning som figur 1. Det vil være åpenbart at disse produkter har en kraftig effekt på ultralyddempning in vitro, en effekt som varte i flere minutter.
Claims (8)
1. Kontrastmiddel omfattende mikrobbble-dannende mikropartikler omfattende et karbohydrat iblandet en surfaktant i den mikropartikulære struktur karakterisert ved at mikroboblene som dannes av nevnte mikropartikler omfatter gass valgt fra svovelheksafluorid og fluorerte hydrokarboner som er i gassform ved 37 °C med det forbehold at surfaktanten ikke er en mettet C fettsvre n^r karbohydratet er galaktose.
2. Kontrastmiddel ifølge krav 1 karakterisert ved at karbohydratet er en vannløselig pentose, heksose, disakkarid, polysakkarid eller sukkeralkohol.
3. Kontrastmiddel ifølge krav 2 karakterisert ved at karbohydratet er galaktose.
4. Kontrastmiddel ifølge et av de foregående krav karakterisert ved at surfaktanten er valgt fra rett-kjedede alifatiske karboksylsyrer eller deres salter, sorbitanestere eller mono- og diglyserider av slike; aralkansyrer eller deres salter; steroidsyrer; steroler; rettkjedede alifatiske alkoholer; fosfolipider; alkalimetall-alkyl-sulfater eller -sulfonerte estere; polyoksyetylen-polyoksypropylen kopolymerer; polyoksyetylerte sorbitanestere og blandinger av hvilke som helst av foregående.
5. Kontrastmiddel ifølge et av de foregående krav karakterisert ved at surfaktanten er et lipofilt modifisert karbohydrat.
6. Kontrastmiddel ifølge et av de foregående krav karakterisert ved at surfaktanten foreligger i en mengde av 0,1 - 2,0 % w/w i forhold til det mikropartikulære karbohydratet, og at mikropartiklene har en størrelse på 1-10 mikrometer og foreligger i aggregater med en størrelse på 30-50 mikrometer.
7. Kontrastmiddel ifølge et av de foregående krav karakterisert ved at det dannes mikrobobler omfattende luft i blanding med nevnte svovelheksafluorid eller fluorerte hydrokarboner.
8. Framgangsmåte for framstilling av et kontrastmiddel ifølge krav 1 karakterisert ved at (i) en løsning av karbohydratet blandes med en løsning av surfaktant hvoretter løsningsmidlene fjernes og (ii) den resulterende blandingen mikroniseres for å oppnå de ønskede mikropartikler, eventuelt ved maling i kulemølle.
