Description
Område for oppfinnelsen
Foreliggende oppfinnelse vedrører et farmasøytisk preparat samt anvendelse av et farmasøytisk preparat til fremstilling av en oral væske for behandling og/eller forebyggelse av bentap hos et individ som har problemer med å svelge.
Bakgrunn for oppfinnelsen
Normale ben er levende vev som konstant gjennomgår resorpsjon og ny avsetting av kalsium, med en nettoeffekt av opprettholdelse av en konstant mineralbalanse. Denne tosidige prosess er vanligvis kalt "ben-turnover". I normalt voksende ben er mineralavsetningen i likevekt med mineralresorpsjonen, mens benresorpsjon overskrider benavsetting ved særlige patologiske tilstander, f.eks. som følge av sykelighet eller hyperparatyreoidisme, eller ved osteoporose. Ved andre patologiske tilstander kan kalsiumavsetningen finne sted i ubestemmelige mengder og områder og føre til f.eks. heterotrop forkalkning, osteoartritt, nyre- eller blærestener, aterosklerose og Pagets sykdom, som er en kombinasjon av en unormalt høy benresorpsjon etterfulgt av en unormal kalsiumavsetting.
Alendronat, 4-amino-1 -hydroksybutyliden-1,1 -bisfosfonsyremono-natriumtrihydrat, er et middel for bekjempelse av benresorpsjon ved bensykdommer, deriblant osteoporose, og er beskrevet som et preparat, en fremgangsmåte for anvendelse og en syntese, sammen med andre farmasøytisk akseptable salter i U.S. patentene 4922007 og 5019651 (begge overdratt til Merck).
Det er situasjoner der f.eks. en aldrende kvinnelig pasient som gjennomgår alendronatbehandling for osteoporose (f.eks. rarefikasjon av ben) eller er ansett å ha risiko for å utvikle osteoporose og samtidig har problemer med å svelge.
Imidlertid administreres alendronat for tiden oralt til alle pasienter i form av en tablett. Tilgjengeligheten av kun en fast oral doseform er en ulempe for enkelte pasienter som ikke har evne til lett å svelge tabletter. Disse pasienter kan dessuten omfatte en betydelig prosentandel av alendronat- pasientpopulasjonen, ettersom medikamentet også er ment for den aldrende kvinnelige pasientpopulasjon.
Det er i disse tilfeller ønskelig med en effektiv behandling for optimalt å behandle denne aldrende kvinnelige pasientpopulasjon med et forbedret oralt preparat for å overvinne problemet med vanskeligheter ved svelging.
O ppsummering av oppfinnelsen
Foreliggende oppfinnelse tilveiebringer et farmasøytisk preparat kjennetegnet ved en farmasøytisk effektiv mengde av alendronat i en farmasøytisk akseptabel bærer og en tilstrekkelig mengde av en buffer slik at pH i preparatet opprettholdes i området fra 2 til 8, og et kompleksdannende middel for å forebygge utfelling av alendronat i vandig løsning samt anvendelse at et farmasøytisk preparat til fremstilling av en oral væske for behandling og/eller forebyggelse av bentap hos et individ som har problemer med å svelge. Det flytende preparat kan være i form av en sirup, vandig løsning eller et rekondisjonert alendronatpulver i en vannløsning, inneholdende en buffer for å regulere pH i løsningen og et kompleksdannende middel for å forebygge dannelse av uløselige komplekser av alendronat.
Det orale, flytende farmasøytiske preparat ifølge foreliggende oppfinnelse, inneholder en farmasøytisk effektiv mengde av alendronat i en flytende farmasøytisk akseptabel bærer, f.eks. renset vann, og en buffer, f.eks. sitrat, for å opprettholde pH mellom 2 og 8, og fortrinnsvis mellom 4 og 6, og et kompleksdannende middel, f.eks. sitrat eller EDTA, for å hemme utfelling av alendronat i et vandig medium. Tilveiebragt er også den ovennevnte vandige løsning inneholdende en høy konsentrasjon av sukker for å tilveiebringe en sirup, noe som også kan tilsettes smak for å bedre salgbarheten.
Det er også tilveiebragt et farmasøytisk preparat omfattende et pulver for rekondisjonering, inneholdende en farmasøytisk effektiv mengde av alendronat i en farmasøytisk akseptabel tørr eksipiens, også i nærvær av en buffer, f.eks. sitrat, og et kompleksdannende middel for nevnte pulver med evne til å oppløses i vann.
Detaljert beskrivelse av oppfinnelsen og foretrukne utførelsesformer
Orale løsninger av alendronat, i form av sirup, vandig løsning eller en rekondisjonert, vandig løsning av et pulver gir fordelene med lettvint administrasjon og økt føyelighet hos pasienter som har problemer med å svelge faste orale doseformer. Et pulver for rekondisjonering gir også den ytterligere fordeler med redusert lagerplass i klinikker, apotek, sykehus og lagerhus. Disse preparater har fordelen av å kunne tillate dosetitrering dersom dette skulle være ønsket.
Preparatet kan anvendes for behandling av mennesker, særlig post-menopausale kvinner med en osteogenisk effektiv mengde av alendronat for å hindre benresorpsjon, dersom det er behov for slik behandling. Et slikt behov oppstår lokalt ved tilfeller av benbrudd, sviktende sammenvoksing av benbruddsender, bendefekt og lignende. Et slikt behov oppstår også i tilfeller av systemisk bensykdom, som ved osteoporose, osteoartritt, Pagets sykdom, osteomalasi, myelomatose og andre former for cancer, steroidbehandling og aldersrelatert tap av benmasse.
Uttrykket "hemning av benresorpsjon" som anvendt heri, refererer seg til behandling og forebyggelse av bentap, spesielt hindring av tap av eksisterende ben eller fra mineralfasen og/eller organisk matriksfase, ved direkte eller indirekte forandring av osteoklastdannelse eller -aktivitet. Uttrykket "hemmer av benresorpsjon" som anvendt heri refererer seg til midler som forebygger bentap ved direkte eller indirekte endring av osteoklastdannelse eller -aktivitet, og som kan øke benmassen hos populasjoner av behandlede pasienter.
Med begrepet "osteogenisk effektiv" som anvendt heri, menes den mengde som bevirker omsetningen av fullt utviklet ben. Som anvendt heri, er en osteogenisk effektiv dose også "farmasøytisk effektiv".
Uttrykket "behandling", som anvendt heri, skal bety (1) tilveiebringelse av en mengde av alendronat til et individ, tilstrekkelig til å virke profylaktisk for å forebygge utvikling av en svekket og/eller sykelig tilstand; og/eller (2) tilveiebringelse av en tilstrekkelig mengde av alendronat til et individ, for å lindre eller eliminere en sykdomstilstand og/eller symptomene av en sykdomstilstand, og en svekket og/eller sykelig tilstand.
Farmasøytiske preparater ifølge foreliggende oppfinnelse, som omfatter alendronat for administrering, vil generelt omfatte en osteogenisk effektiv mengde av alendronat til å fremme benvekst, i tillegg til en farmasøytisk akseptabel eksipiens. Preparatene kan med fordel fremstilles sammen med tørre, inerte bærere for pulverformer slik som sukker, deriblant sakkarose og laktose, stivelse og dets derivater, cellulose og dets derivater, gummier, fettsyrer og deres salter.
For flytende preparater er renset vann og saltløsning egnede flytende eksipienser/bærere.

I tillegg er tilstedeværelsen av en buffer nødvendig for å opprettholde den vandige pH i området fra 2-8, fortrinnsvis 4-6.
Buffersystemet er vanligvis en blanding av en svak syre og et løselig salt derav, f.eks. natriumsitrat/sitronsyre; eller et monokation- eller dikationsaltet av en divalent syre, f.eks. kaliumhydrogentartrat, natriumhydrogentartrat, natriurnhydrogenftalat, kaliumhydrogenftalat, kaliumdihydrogenfosfat og dinatriumhydrogenfosfat.
Mengden av buffersystem anvendt er avhengig av (1) den ønskede pH; og (2) mengden av alendronat. Generelt er en anvendt buffermengde fra 0,5 til 50:1 molforhold av buffenalendronat i preparatet for å opprettholde en pH i området fra 2 til
9 og generelt, 1 til 10 molforhold av buffer i forhold til alendronat til stede.
Én anvendbar buffer er natriumsitrat/sitronsyre i området fra 5 til 50 mg pr. ml natriumsitrat til 1 til 15 mg pr. ml sitronsyre.
Et kompleksdannende middel er også til stede for å forebygge utfelling av alendronat ved metallkompleksdannelse med løste metallioner, f.eks. Ca, Mg, Fe, Al, Ba som kan lekke ut av glassbeholdere eller gummikorker, eller være til stede i vanlig ledningsvanri. Midlet virker som et kompetitivt kompleksdannende middel sammen med alendronat og danner et løselig metallkompleks hvor alendronat generelt danner et uløselig metallkompleks. Kompleksdannende midler omfatter sitratbufferen, som virker som et buffer/kompleksdannende middel eller EDTA. Når EDTA anvendes er det i en mengde på 0,005-0,1 vekt% av preparatet og 0,005-2 deler av EDTA til 1 vektdel alendronat og fortrinnsvis ca. 0,01 vekt% av preparatet. Det foretrekkes at sitratbuffer anvendes alene.
Eksempler på tre orale doseformer av alendronat er:
Vandig oral løsning
Alendronatmassemedikament ble oppløst i vann eller egnede koløsningsmidler for å nå ønsket konsentrasjon. Smaksstoffer, fargestoffer, viskositetsmidler, konserveringsmidler, stabilisatorer og buffermidler ble tilsatt som beskrevet. Løsningen ble fylt i multi- eller enhetsdosepakninger.
Den vandige løsning ble anvendt direkte fra flasken.
Generell sammensetning
Tilsetningsstoffer slik som løsningsmidler, smaksstoffer, fargemidler, konserveringsmidler, stabilisatorer og buffermidler kan også spesifikt inkorporeres i preparatet som følger:

c Sakkarose eller aspartam kan alternativt anvendes som
søtningsmidler.
Oral sirup
Alendronat massemedikament ble tilført en løsning av sakkarose (10-85 %) for å nå ønsket konsentrasjon. Tilsetningsstoffer slik som glyserol, sorbitol, smaksstoffer, fargestoffer, viskositetsmidler, konserveringsmidler, stabilisatorer og buffermidler ble tilsatt som beskrevet. Den endelige løsning ble fylt i multi- eller enhetlige dosepakninger. Sirupen kan anvendes direkte fra flaske eller tilsettes til små mengder av ledningsvann for å lette svelging.
Generell sammensetning
Tilsetningsstoffer slik som glyserol, sorbitol, smaksstoffer, fargestoffer, konserveringsmidler, stabilisatorer og buffermidler kan også spesifikt inkorporeres i preparatet som følger:
Spesifikt preparat
Pulver for rekondisjonering
Alendronatmassemedikament ble blandet til homogenitet med én eller flere av de følgende: smaksstoffer, fargestoffer, konserveringsmidler, stabilisatorer og spesifikt med et buffermiddel. Pulverblandingen ble så fylt i fler-dosebeholdere eller enhetsdosepakninger.
Pulveret kan oppløses i vanlig lednings vann for rekondisjonering av alendronat i en vandig løsning.
Pulverpreparat for rekondisjonering
Den nøyaktige nødvendige dose vil variere med alder, størrelse, kjønn og tilstand til pasienten, type og alvorlighetsgrad av forstyrrelse som behandles, og lignende; en nøyaktig effektiv mengde kan derfor ikke angis på forhånd og må bestemmes av behandleren. Passende mengder kan imidlertid bestemmes ved rutineforsøk med dyremodeller, som beskrevet nedenfor. Generelt er en effektiv dose av alendronat i enhver av de orale, flytende preparater omtrent 1,5 til 3000 jig/kg kroppsvekt og fortrinnsvis mellom omtrent 10 (ig/kg og 200 ug/kg kroppsvekt.
Fremgangsmåtene og preparatene ifølge foreliggende oppfinnelse er anvendbare for behandling av benbrudd, -defekter og -forstyrrelser som er resultat av patologiske tilstander av osteoporose, osteoartritt, Pagets sykdom, osteohalisterose, osteomalasi, bentap som følge av myelomatose og andre former for cancer, bentap som følge av bivirkninger av andre medikamentelle behandlinger (slik som steroider) og aldersrelatert tap av benmasse.
Claims (25)
1. Farmasøytisk preparat,
karakterisert ved en farmasøytisk effektiv mengde av alendronat i en farmasøytisk akseptabel bærer og en tilstrekkelig mengde av en buffer slik at pH i preparatet opprettholdes i området fra 2 til 8, og et kompleksdannende middel for å forebygge utfelling av alendronat i vandig løsning.

karakterisert ved at det er en væske.
3. Farmasøytisk preparat ifølge krav 2,
karakterisert ved at nevnte væske er en sirup.
