Description
Foreliggende oppfinnelse vedrører et implantat og en fremgangsmåte for fremstilling av det.
Implantater som består av eller har et overflatelag av vevforenlig materiale, slik som titan og visse typer keram-iske materialer, er allerede kjent, f.eks. fra SE-7902035-0.
Et felles trekk ved de fleste av disse implantatene er at de er utformet for å ligge tett an mot det biologiske vev, slik at dette hurtig kommer i kontakt med og vokser inn i implantatet. For dette formål utformes ofte lårproteser f.eks. med utspring hvorved protesen får et feste på den innvendige vegg i hulrommet i lårbenet.
Dette resulterer imidlertid i en uelastisk tilknyt-ning mellom overflaten av implantatet og det motstående kroppsvev, noe som er ufordelaktig spesielt i tilfellet med svampvev, slik som i kjever.
Til tross for at utspring er tilveiebrakt, vil til-helingsprosessen ta ganske lang tid, noe som ikke bare for-årsaker ulempe for pasienten, men som også kan gi opphav til dannelse av bindevev i sammenføyningen mellom implantatet og vevet.
Formålet ved foreliggende oppfinnelse er å overvinne, eller i det minste i det vesentlige redusere, de ovenfor nevnte ulemper ved kjente implantater.
Ifølge oppfinnelsen oppnås dette formål ved et implantat (en protese) som omfatter biologisk materiale i den overflate som er ment å vende mot kroppsvevet, og som er kjennetegnet ved at overflaten er dannet av et lag av materiale tilveiebrakt på en protesestamme eller et lag av materiale hvorfra ett eller flere lagforankrende elementer stikker frem, idet stammen eller forankringselementene består av eller har et overflatelag av vevforenlig materiale, idet laget av materiale består av en blanding av korn/pulver-materiale av vevforenlig type og desintegrert, vevforenlig, biologisk materiale, slik som benmel, som ved tilsetning av næring har fått vokse både ut til overflaten og inn til stammen eller forankringselementene for forankring til disse, hvor tilveksten også binder sammen bestanddelene i blandingen, og hvor det biologiske materiale er vev av den samme type som implantatet skal hhv. innføres eller plasseres i eller mot.
For å fremme slik vekst gjøres det bruk av en egnet, kommersielt tilgjengelig næringsoppløsning.
Stammen eller dens overflatelag er fortrinnsvis av titan, og dens ytre overflate er fortrinnsvis porøs for optimal vevforankring.
De vevforenlige korn kan være valgt fra forskjellige materialer, hovedsakelig titan, men andre materialer som er kjent for fagfolk innen teknikken, kan anvendes, slik som bio-keramer, bioglass og hydroxyapatitt.
Det vil forstås at implantatet ifølge oppfinnelsen som inneholder biologisk materiale i sitt overflatelag, vil redusere tiden som er påkrevet for tilheling etter implanta-sjonen, dvs. tiden som er påkrevet for biologisk forankring av implantatet i eller mot kroppsvev.
Ifølge oppfinnelsen er det videre tilveiebrakt en fremgangsmåte for fremstilling av implantatet, kjennetegnet ved at en implantatstamme eller implantatforankringselementer og en blanding av korn/pulver-materiale av vevforenlig type og desintegrert, biologisk materiale, samt en næringsoppløsning for det sistnevnte, plasseres i en støpeform hvor det biologiske materiale får vokse, både ut til avgrensningsveggen for formrommet og inn til implantatstammen.
Implantatet ifølge oppfinnelsen lages fortrinnsvis i en form hvor hulrommet inneholder stammen (eller delen derav som skal forsynes med laget) og en blanding av materialet i rommet mellom stammen og den avgrensende vegg i formhulrommet. Til denne blandingen inneholdt i formhulrommet, tilsettes en næringsoppløsning som fremmer (eventuelt etter påfylling) veksten av det biologiske materiale i blandingen. Vekstpara-metrene er tilgjengelige i litteraturen for fagfolk innen teknikken.
En fordel ved en slik teknikk er at implantatet lett kan skreddersys for pasienten. Formhulrommet formes prelimi-nært etter f.eks. en tannseng, og eventuelle modifikasjoner av implantatet som senere kan være påkrevet, bestemmes ved hjelp av radiografi etter implantering av implantatet formet i støpeformen. Modifikasjoner av støpeformhulrommet utføres så for å oppnå et mer korrekt implantat.
På den vedlagte tegning er det vist to utførelses-former av oppfinnelsen. Figur 1 viser en tannprotese 1 som skal skrus inn i en kjeve. Stammen 2 i protesen er av vewekstfremmende titan med en porøs ytre overflate (erholdt ved sintring, metall-avsetning ved fordampning etc). Til stammen er det festet et lag 3 bestående av en blanding av benmel 4 oppmalt fra kjeve-benvev tatt fra pasienten, og titanpulver 5. Blandingen er blitt tilført en næringsoppløsning 6 som får benmelet i blandingen til å vokse og danne vev 7 som forbinder bestanddelene 4, 5 i lag 3 til hverandre og til stammen 2. Protesen er blitt laget i en silikonform 8 med gjenger 9 på innsiden, slik at det dannes gjenger 10 på utsiden av tannprotesen. Figur 2 viser et bruskimplantat 11 som er forsynt med flere titanstifter 12 som i én ende er innelukket i et lag 13 av blandingen beskrevet ovenfor. Med de motsatte, frie endene av stiftene 12 kan implantatet 11 festes til kroppsvev.
Claims (5)
1. Implantat (protese) som omfatter biologisk materiale i den overflate som er ment å vende mot kroppsvevet, karakterisert ved at overflaten er dannet av et lag av materiale (3) tilveiebrakt på en protesestamme (2) eller et lag av materiale (13) hvorfra ett eller flere lagforankrende elementer (12) stikker frem, idet stammen eller forankringselementene består av eller har et overflatelag av vevforenlig materiale, idet laget av materiale (3, 13) består av en blanding av korn/pulver-materiale (5) av vevforenlig type og desintegrert, vevforenlig, biologisk materiale (4), slik som benmel, som ved tilsetning av næring har fått vokse både ut til overflaten og inn til stammen eller forankringselementene for forankring til disse, hvor tilveksten også binder sammen bestanddelene (4, 5) i blandingen, og hvor det biologiske materiale (4) er vev av den samme type som implantatet skal hhv. innføres eller plasseres i eller mot.
2. Implantat ifølge krav 1,
karakterisert ved at stammen (2) eller forankringselementene (12) omfatter titan med en porøs ytre overflate.
3. Implantat ifølge et av kravene 1-2, karakterisert ved at det granulære/pulver-formige materiale (5) omfatter titan.
4. Implantat ifølge et av kravene 1-3, karakterisert ved at det biologiske materiale (4) er endogent.
5. Fremgangsmåte for fremstilling av et implantat ifølge hvilket som helst av kravene 1-4,
karakterisert ved at en implantatstamme eller implantatforankringselementer og en blanding av korn/pulver-materiale av vevforenlig type og desintegrert, biologisk materiale, samt en næringsoppløsning for det sistnevnte, plasseres i en støpeform hvor det biologiske materiale får vokse, både ut til avgrensningsveggen for formrommet og inn til implantatstammen.
