Description
En rekke fenantrenmetanol forbindelser er påvist å ha antimalariavirkning i mennesker mot både klorokin-sensi-tive og resistente stammer av Plasmodium falciparum. Be-dømmelsen av antimalariaaktiviteten til fenantrenmetanol, halofantrin eller 1-/1,3-diklor-6-trifluormetyl-9-fenant-ryl/-3-di-(n_-butyl) aminopropanol hydroklorid ble beskrevet i American Journal of Tropical Medicine and Hygiene, Vol. 31(6), sidene 1075-79 (1982).
Halofantrin var virksom når den ble gitt over .et kort tidsrom og med et minimum av to doser mot den multi-medika-mentresistente Vietnam Smith stammen og Cambodian Buchanan stammen av jg_. f alciparum og Chesson stammen av J?_. vivax. Imidlertid forble problemer med systemisk biotilgjengelig-het. Foreliggende oppfinnelse vedrører fremstilling av klassen antimalariaforbindelser inneholdende halofantrin (som fri base) og dens analoge, og nærmere bestemt beta-glycerofos-fatet, tartratet og bikinatsaltene. Disse salter viser markert øket aktivitet mot malariafremkallende parasitter sammenlignet med hydrokloridsaltene som er beskrevet i litteraturen. Farmasøytiske blandinger og fremgangsmåter for behandling av pasienter med malaria er også beskrevet.
Forbindelsene fremstilt ifølge foreliggende oppfinnelse

Disse forbindelser har markert øket aktivitet mot malariafremkallende parasitter.
En spesiell klasse forbindelser fremstilt ifølge foreliggende oppfinnelse er slike forbindelser med formel (I) hvor Ri og R2 er n-butylrester. Eksempler på denne type klasse forbindelser er 1-/1,3-diklor-6-trifluormetyl-9-fenantryl/-3-di-(n-butyl)aminopropanol-beta-glycerofosfat, 1-/1, 3-di klor-6-tr i f luormetyl-9-f enantryl/-3-di ( n_-butyl) aminopropanol 1/2-tartrat,
1-/1, 3-diklor-6-trif luor me tyl- 9-f enantryl /-3-di (n_-butyl) aminopropanoltartrat, og
1-/1, 3-di kl or-6-tr i f luor me tyl-9-f en an tr yl/-3-di (n_-butyl) aminopropanolbikinat.
Forbindelsene i foreliggende oppfinnelse fremstilles lett ved å omsette beta-glycerofosforsyre, tartarsyre eller kininsyre (1,3,4,5-tetrahydroksycykloheksan-karboksylsyre) med en forbindelse med formel (II):
hvori og R~ er som ovenfor beskrevet i et inert løsningsmiddel. Mengden beta-glycerofosforsyre som anvendes i denne reaksjonen kan være mellom 0,75 og 1,25 mol pr. mol av forbindelsen med formel (II), men ekvimolare mengder av begge reaktanter foretrekkes. For å fremstille tartratsaltene med formel (I), hvor n er 1/2, kan mengden av vinsyre som anvendes i reaksjonen være mellom 0,25 og 0. 55 mol pr. mol av forbindelsen med formel (II), men 0,50 mol vinsyre foretrekkes. For på lignende måte å fremstille tartratsaltene med formel (I) hvor n er en, kan mengden vinsyre som anvendes i reaksjonen være mellom 0,75 og 1,25 mol pr. mol av forbindelsen med formel (II), men ekvimolare mengder av begge reaktanter foretrekkes. Mengden av 1, 3, 4, 5 - tetrahydroksycykloheksan-karboksylsyre som anvendes i ovennevnte reaksjon kan være mellom 1,5 til 2,50 mol pr. mol av forbindelsen med formel (II), men 2,0 mol av forbindelsen foretrekkes.
kininsyren som anvendes i ovennevnte reaksjon kan være mellom 1,5 til 2,50 mol pr. mol av forbindelsen med formel (II), men 2,0 mol kininsyre foretrekkes.
Forbindelsen med formel (II) blandes med den riktige mengde syrereaktant (50% vandig løsning) og det inerte løsningsmiddel tilsettes for å bevirke oppløsning av reak-tantene ved en temperatur valgt i området romtemperatur til 100°C. Reaksjonsløsningen filtreres og filtratet oppvarmes under redusert trykk opptil 100°C for å fjerne løsningsmiddelet. Etter fjerning av løsningsmiddelet utfelles de respektive forbindelser med formel (I) og opp-samles og tørkes. Di-1, 3, 4, 5 - tetrahydroksycykloheksan-karboksylat-saltene med formel (I) er delvis vannløselige til ca. 20 prosent på vektbasis.
Eksempler på de inerte løsningsmidler som anvendes i fremgangsmåten er alkoholer såsom metanol, etanol, isopropanol og lignende amider, såsom dimetylformamid og dimetylaceta-mid.
Basene med formel (II) fremstilles ifølge de generelle fremgangsmåter som er beskrevet i Journal of Medicinal Chemistry, bind 15, nr. 7, side 771-5 (1972) hvor fremgangsmåten for å overføre substituerte fenantren-9-karbok-sylsyrer i de ønskede forbindelser som fri baseform er detaljert beskrevet.
Antimalariaaktivitetene til forbindelsene i foreliggende oppfinnelse påvises i et standard framakologisk in_ vivo forsøk mot P. berghei i sveitsiske mus.
Antimalariaaktiviteten til forbindelsene som beskrives nedenunder ble påvist ved å anvende den følgende metodikk. Forsøksdyr ble infisert ved intraperitoneal injeksjon av IO<6> parasittceller av _P. berghei i 0,5 ml 1:400 fortyn-ning av donatormusblod som var infisert med _P. berghei en uke tidligere. Tre dager etter infeksjon ble tre grupper på fire mus (2 hanmus og 2 hunmus) behandlet med forsøks-forbindelsen i et doseringsnivå på 1, 4 og 16 mg/kg/dag i 4 påfølgende dager. På lignende måte ble en positiv kon-troll på 3 grupper av 4 mus (2 hanmus og 2 hunmus) behand-les med halofantrin som hydrokloridsalt i en doserings-mengde på 3, 12 og 48 mg/kg/dag i 4 påfølgende dager. Administreringsveien for forsøksforbindelsen og halofantrin var oralintubering av en suspensjon av hver forbindelse i 0, 2% metylcellulose ved et konstant volum på 20 ml. En negativ kontrollgruppe på 4 mus (2 hanmus og 2 hunmus) forble ubehandlet. Resultatene av overnevnte forsøksprose-dyre (a) målt som overlevelsestid i dager og prosent røde blodceller parasitisert ved 28 dager i de behandlede over-levende mus og ved 3 dager i de ubehandlede kontrollmus eller (b) uttrykt som en ED^q (den effektive dose som reduserer celleparasittemien med 50% den fjerde dag av behandlingen) er vist i de følgende tabeller for de an-gitte forbindelser.
Forbindelse A: 1-/1,3-diklor-6-trifluormety1-9-fenantryl/-3-di(rv-butyl)-aminopropanol-beta-glycerofosfat;
Forbindelse B: 1-/1,3-diklor-6-trifluormetyl-9-fenantryl/-di(n-butyl)-aminopropanol 1/2-tartrat;
Forbindelse C: 1-/1, 3-di klor-6-tr if luor me tyl-9-f enantryl /-3-di (n-butyl) - aminopropanoltartrat; og
Forbindelse D:
1-/1, 3-di kl or-6-tr i f luor me tyl- 9-f enantryl /-3-di (ri_-butyl) - aminopropanolbikinat.
Forbindelsene A, B og C er minst 12 ganger mere effektive på vektbasis i behandlingen av P. berghei malaria hos mus basert på overlevelses tid og utslettelse av parasitter sammenlignet med halofantrinhydroklorid.
Forbindelsen D er minst 6 ganger mer effektiv på vektbasis ved behandling av P_. berghei malaria hos mus når man som virkningskriterium anvender EDj-q beregning av deparasi-tiserte celler den fjerde dag sammenlignet med halofantrinhydroklorid. /Se Annals of Tropical Medicine and Para-sitology, 73_, s. 505-525, (1979)./
Farmasøytiske blandinger som inneholder en forbindelse med formel (I) som har antimalaria-aktivitet fremstilles i konvensjonelle doserings-enhets-former ved å Innføre den kjemiske forbindelse med en ikke toksisk farmasøytisk bærer ifølge vanlige aksepterte metoder. En ikke-toksisk mengde av den aktive bestanddel velges som er tilstrekkelig til å gi den ønskede kjemo-terapeutiske aktivitet hos en pasient, dyr eller menneske, uten uakseptabel toksisitet. Blandingene vil inneholde den aktive bestanddel i en slik virksom men ikke toksisk mengde valgt fra ca. 125 til ca. 100 mg aktiv bestanddel pr. doseringsenhet, men denne mengde avhenger av den ønskede spesifikke biologiske aktivitet, og aktiviteten til forbindelsen og pasientens tilstander.
En rekke farmasøytiske former kan anvendes. Hvis således en fast bærer brukes for oral administrering kan preparatet tabletteres, plasseres i en hardgelatinkapsel i pulver eller pelletform, eller i form av en suppositorium, drage eller tablett. Mengden av fast bærer vil variere sterkt, men vil fortrinnsvis være fra ca. 125 mg til 500 mg. Hvis en flytende bærer anvendes, vil preparatet foreligge i form av en syrup, emulsjon, mykgelatinkapsel, steril inji-serbar væske såsom en ampulle eller en vandig eller ikke vandig væskesuspensjon.
De farmasøytiske preparater fremstilles ved å følge vanlig teknikk for den farmasøytiske kjemiker som innebefatter blanding, granulering og komprimering om nødvendig, eller å varierende blande og oppløse bestanddelene på passe måte til å gi det ønskede sluttprodukt.
Fremgangsmåten for å få antimalariaaktivitet, kurativt eller profylaktisk, består i å gi en pasient som har behov for slik aktivitet innvendig en forbindelse med formel (I), normalt kombinert med en farmasøytisk bærer, en ikke toksisk mengde som er tilstrekkelig til å gi den ovenfor beskrevne aktivitet. Administreringsvei kan være enhver vei som effektivt transporterer den aktive forbindelse til virkningsstedet, som skal påvirkes inne i legemet, såsom oralt eller parenteralt. Med fordel vil en enkel oral dose eller like orale doser gis flere ganger såsom fra en til tre ganger om dagen med et daglig doseringsområde valgt fra ca. 125 til 1000 mg.
De følgende eksempler illustrerer fremstilling av preparatene av forbindelsene med formel (I) og deres innføring i farmasøytiske blandinger.
Eksempel 1
Fremstilling av 1-/1,3-diklor-6-tyrifluormetyl-9-fenant-ryl/ -3-di ( n_-butyl) aminopropanol-beta-glycerof osf at ( Forbindelse A)
Til en 50% vandig blanding av beta-glycerofosforsyre (4,8 g) satte man ved romtemperatur under røring l-/-l,3-di-klor-6-trif luormetyl-9-f enantryl/-3-di (n-buty.l) aminopropanol (14,0 g). Til blandingen satte man etanol (500 ml) og blandingen ble oppvarmet til ca. 100°C for å bevirke oppløsning. Løsningen filtreres så og etanolen fjernes under vakuum inntil det ønskede produkt utfelles som et hvitt krystallinsk materiale. Etter oppsamling og tørking har det ønskede produkt et smp. på 60-65°C. Elementæranalyse er som følger: Beregnet C, 51,78; H, 5,80; N, 2,08 og Cl, 10,56 Funnet C, 51,83; H, 5,65; N, 2,10 og Cl, 10,20.
Eksempel 2
Fremstilling av
1-/1, 3-di klor-6- tr i f luor me tyl-9-f enantryl 3- (di (_n_-butyl) - aminopropanol 1/2-tartrat (Forbindelse B)
Til en 50% vandig løsning av vinsyre (1,35 g) ved romtemperatur under røring satte man 1-/-1,3-diklor-6-trifluor-metyl-9-f enantryl/-3-di (_n_-butyl) a minopropanol (9,0 g) . Til blandingen satte man 500 ml etanol og blandingen ble oppvarmet til ca. 100°C for å bevirke oppløsning. Løs-ningen filtreres deretter og etanolen fjernes under vakuum inntil det ønskede produkt felles ut som et hvitt krystallinsk materiale. Etter oppsamling og tørking har det ønskede produkt et smeltepunkt på 158°C. Elementæranalysen er som følger: Beregnet C, 58,43; H, 5,77; N, 2,43 og Cl, 12,34 og F, 9,91. Funnet C, 58,51; H, 5,40; N, 2,31; Cl, 11,99 og F, 9,70.
Eksempel 3
Fremstilling av 1-/1,3-diklor-6-trifluormetyl-9-fenant-ryl/-3-di(n-butyl)aminopropanoltartrat (Forbindelse C)
Ved å benytte den generelle fremgangsmåte fra eksempel 2 med 2,7 g vinsyre og 9,0 g 1-/1,3-diklor-6-trifluorme-tyl-9-fenantryl/-3-di(n-butyl)aminopropanol får man det ønskede produkt som et hvitt krystallinsk materiale med et smp. på 75-80°C. Elementær analysen er som følger: Beregnet C, 55,38; H, 5,53; N, 2,15; Cl, 10,92 og F, 8,77. Funnet C, 54,92; H, 5,50; N, 2,00; Cl, 11,30 og F, 8,95.
Eksempel 4
Fremstilling av 1-/1,3-diklor-6-trifluormetyl-9-fenant-ryl/- 3-di(n-butyl)aminopropanol bikinat (Forbindelse D)
Til en 50%ig vandig løsning av 1, 3, 4, 5 - tetrahydoksy-cykloheksan-karboksylsyre (4.62 g) ved romtemperatur under røring satte man 1-/-1,3-diklor-6-trifluormetyl-9-fenan-tryl/-3-di-(n-butyl)aminopropanol (6 g). Til blandingen satte man (500 ml) etanol, og blandingen ble oppvarmet til ca. 100°C for å bevirke oppløsning. Lønningen ble så filtrert, og etanolen og vannet fjernet under vakuum til tørrhet. Det ønskede produkt størkner som et hvitt krystallinsk materiale med et smeltepunkt på ca. 90°C og er oppløselig i vann opptil 20 prosent på vektbasis. Elementæranalysen er som følger : Beregnet C, 54,29; H, 6,32; N, 1,58 og Cl, 8,03; F, 6,44. Funnet C, 54,32; H, 6,24; N, 1,66 og Cl, 7,95; F, 6,49.
Eksempel 5
Som spesifikke utførelsesformer av blandinger i foreliggende oppfinnelse oppløses som aktiv bestanddel en slik del av forbindelse A, B, C eller D i 20 deler 0,2 prosent vandig metylcellulose, og kan gis oralt i en dose på 4 mg/kg til en pasient som trenger behandling for malaria.

- Analogifremgangsmåte ved fremstilling av en terapeutisk aktiv forbindelse med den følgende strukturformel (I) :hvor og R2 er n-butyl, og X er H203POCH (CH2OH) 2- n[H02CCH(OH)CH(OH)C02H] hvori n er 1/2 eller 1, og 2H02CC6H?(0H)^ karakterisert ved at man omsetter betaglycerofosforsyre, vinsyre eller 1, 3, 4, 5 - tetrahydroksycykloheksan-karboksylsyre med en forbindelse med formel (II) : hvori R og R2 er som ovenfor angitt, i et inert løsningsmiddel.
