Description
Titel: Stabiele waterige folinaatcplossing.
De uitvinding heeft betrekking qp een stabiele waterige folinaatcplossing.
Folinezuur is een metaboliet van folzuur en is de actieve vorm waarin folzuur in het lichaam wordt omgezet. Deze omzetting wordt door sommige cytostatische middelen, zoals methotrexaat, geremd. Teneinde deze moeilijkheid cp te heffen, is het nodig folinezuur toe te voegen. Verder wordt folinezuur toegepast bij foliumzuurdeficiëntie en werkt het synergistisch in kaïbinatie met 5-fluoruracil. De gebruikelijke vorm waarin het folinezuur wordt toegevoegd, is calciumfolinaat, dat door infusie of injectie wordt toegediend. Calciumfolinaat is goed oplosbaar naar niet houdbaar bij kamerterrperatuur, en kan derhalve niet langdurig bij kamertemperatuur worden bewaard. Bij koelkasttemperatuur, waarbij het folinaat wel houdbaar is, is een relatief geconcentreerde oplossing niet stabiel; bij de normale koelkasttemperatuur van gemiddeld circa 4°C vindt reeds uitkristalliseren plaats bij een concentratie van ca. 15 mg/ml.
EP 0 401 895 verschaft folinaatcplossingen die bij een relatief hoge concentratie stabiel bij koelkasttemperatuur zijn, welke oplossingen f olinaat ionen, calciumionen en een complexvormend middel voor calcium bevatten, waarbij het complexvormende middel en calcium een complex vormen. Deze oplossingen kunnen in een concentratie van zelfs 50 mg/ml folinaat bij koelkasttemperatuur stabiel zijn. Als complexvormend middel kan bijv. een natriumzout van etbyleendiaminetetraazijnzuur worden toegepast.
Gebleken is dat men een stabiele waterige oplossing van folinezuur kan verkrijgen door eenvoudig een waterige oplossing van natriumfolinaat te bereiden, welke natriumfolinaatcplossing ook in een hoge concentratie tot maximaal 50 mg folinezuur per ml stabiel in de koelkast is, d.w.z. niet uitkristalliseert. De oplossing is stabiel in een pH trajekt van 4.0-8.8.
Bij voorkeur bevat de oplossing een stabilisator zoals natriumnitraat of natriumacetaat, welke zouten de stabiliteit van de oplossing verder verhogen.
Voorbeeld;
Men bereidde de onderstaande injectievloeistof:
Folinezuur (als natriumfolinaat) 25 mg
Natriumchloride 4,3 1¾
Natriumcitraat 0,5 irg
Water voor injectie tot 1 ml
De pH was met natriumhydroxide cp een waarde van 7,5 ingesteld.
Het natriumchloride diende voor het isotonisch maken van de oplossing.
De bovenstaande oplossing bleek na 3 maanden bij koelkasttempera-tuur tussen 2 en 8°C (gemiddeld circa 4°C) nog steeds stabiel te zijn, d.w.z. niet uitgekristalliseerd.
Claims (3)
1. Waterige folinaatcplossing voor medische toepassingen, met het kenmerk dat deze oplossing het folinaat in de vorm van natriumfolinaat bevat en een pH van circa 4,0-8,8 bezit.
2. Waterige folinaatcplossing volgens conclusie 1, met het kenmerk dat de oplossing tevens natriumnitraat bevat.
3. Waterige folinaatcplossing volgens conclusie 1 of 2, met het kenmerk dat de oplossing tevens natriumchloride als isotonisch-makend middel bevat.
