건성안 질환에 대한 동물 모델에서, 마우스는 제어된 환경 챔버 (CEC)에 놓여 앞서 기술된 건성안이 유도되었다 (Barabino et al., 2005). 일반적으로 누액의 고삼투압 (HOP)이 안구 염증 및 상처에 대한 원인인 것으로 알려져 있다. HOP는 세포 대사의 부산물인 반응성 산소 종 (ROS)의 생성을 유도할 수 있다. 재료 및 방법
카르복시메틸셀룰로오스 나트륨 (CMC), 과요오드산 시프 (PAS) 시약 및 덱사메타손은 모두 Sigma-Aldrich(St. Louis, MO, USA)로부터 얻었다. 평형 염 용액 (BSS; Alcone) 중의 CMC (1% 중량/부피)를 PDSP에 대한 비히클로서 사용하였다. PDSP 및 음성 대조군 펩타이드 (ConP)를 합성하였고 안정성을 위해 NH2 말단에서 아세틸화 및 COOH 말단에서 아미드화로 변형시켰으며 GenScript (Piscataway, NJ, USA)의 지시대로 질량 분석에 의해 특성화하였다.
동물
생후 7 내지 8주령의 암컷 C57BL/6 마우스를 이 실험에 사용하였다. 실험 과정은 맥케이 메모리얼 병원 검토 위원회(Mackay Memorial Hospital Review Board)에 의해 승인받았고 안과 및 시력 연구에서 동물의 사용(Use of Animals in Ophthalmic and Vision Research)에 대한 ARVOStatement를 따라 수행되었다.
건성안 모델
건성안은 마우스를, 앞서 기술된 것과 같이 (Barabino et al., 2005) 제어된 환경 챔버 (CEC)에 넣음으로써 유도되었다. 간단히 설명하면, CEC에 넣은 마우스를 25%를 초과하는 상대 습도 (RH), 20 내지 22℃의 온도, 및 15 L/분의 기류에, 1일당 12시간 동안 노출시켰다. 스트레스-유도 건성안을 갖지 않은 비-스트레스 (NS) 마우스는 정상 환경 (RH >50%, 기류 없음, 21 내지 23℃의 온도)에서 동일한 기간 동안 유지시켰다.
각막 플루오레세인 염색 및
동물들을 졸레틸(zoletil) (6 mg/kg) 및 자일라진(xylazine) (3 mg/kg)의 혼합물의 복강내 주사에 의해 마취시켰다. 각막 상피 상처를 국소 플루오레세인 (Fluor-I-Strip, Ayerst Laboratories, Philadelphia, PA)으로의 염색에 의해 측정하였다. 각막 플루오레세인 염색을 코발트 블루 광 하에서 슬릿-램프 생체현미경으로 조사하였고 디지털 카메라로 촬영하였다. 각막의 염료 염색을 다음과 같은 방식으로 채점하였다: 점상 염색(punctuate Staining)이 없는 경우에 0점; 각막의 1/3 미만이 염색되었을 때 1점; 각막의 2/3 이하가 염색되었을 때 2점; 및 각막의 2/3 이상이 염색되었을 때 3점 (Horwath-Winter J 2013).
눈물 생성의 측정
눈물 생성을 페놀 레드-침지 면사 (Zone-Quick; Oasis, Glendora, CA)로 측정하였다. 마우스에서 이 테스트의 타당성은 이전에 기술된 것과 같이 수행하였다 (Dursun et al., 2002). 실들을 보석 겸자로 잡고 60초 동안 가쪽 눈구석에 놓았다. 눈물 생성을 눈물에 의해 젖어서 적색으로 변한 실의 밀리미터로 표시하였다.
고블렛 세포의 PAS 염색
동물들을 안락사시킨 후, 눈을 수술로 적출하여 10% 포르말린에 고정하고, 파라핀에 내장한 후, 5-μm 섹션으로 절단하였다. 결막의 위쪽 및 앞쪽의 고블렛 세포를 측정하기 위하여 섹션들을 과요오드산 시프 시약 (PAS; Sigma-Aldrich)으로 염색하고 조사하고 디지털 카메라가 장착된 현미경으로 촬영을 하였다. 결막의 PAS-양성 고블렛 세포를 각 눈으로부터의 5개의 섹션에서 조사하였다.
RNA 추출 및 정량적 실시간 PCR
총 RNA를 TRIzol (Invitrogen)을 사용하여 세포로부터 추출하고 RNase 유리 DNase I (Qiagen, Santa Clarita, CA)로 처리하여 게놈 DNA를 제거한 후 RNA 정제 키트 (Dynabeads; Invitrogen)로 정제하였다. cDNA의 합성을 Superscript III (Invitrogen)에 의해 수행하였다. 정량적 실시간 PCR을 GeneAmp 7700 서열 검출 시스템 (Applied Biosystems, Foster City, CA)으로 수행하였다. 증폭을 3 pmol의 프라이머, 연속적으로 희석된 RT 생성물 및 SYBR Green PCR 코어 시약 (Applied Biosystems)을 함유한 40 μl의 총 부피로 수행하였다. 특이적 PCR 프라이머의 서열은 마우스 TNF-α(승인 번호: NM_013693) 센스, 5'- CTACCTTGTTGCCTCCTCTTT -3' (SEQ ID NO:79), 안티센스, 5'- GAGCAGAGGTTCAGTGATGTAG -3' (SEQ ID NO:80); 마우스 IL-1β (승인 번호: NM_008361) 센스, 5'- GGTGTGTGACGTTCCCATTA -3' (SEQ ID NO:81), 안티센스, 5'- ATTGAGGTGGAGAGCTTTCAG -3' (SEQ ID NO:82); 마우스 IL-6 (승인 번호: NM_031168) 센스, 5'- GTCTGTAGCTCATTCTGCTCTG -3' (SEQ ID NO:83), 안티-센스, 5'- GAAGGCAACTGGATGGAAGT -3' (SEQ ID NO:84); 마우스 MCP-1 (승인 번호: NM_011333) 센스, 5'- CTCGGACTGTGATGCCTTAAT -3' (SEQ ID NO:85), 안티-센스, 5'- TGGATCCACACCTTGCATTTA -3' (SEQ ID NO:86); 토끼 MMP-9 (승인 번호: NM_001082203) 센스, 5'- TGCGAGTTTCCGTTCATCTT-3' (SEQ ID NO:87), 안티-센스, 5'- GTAGAGCTTGTCCTTGTCGTAG-3' (SEQ ID NO:88)이었다. 스텝-사이클 프로그램(step-cycle program)을, 95℃에서 15초 동안 변성시키고, 어닐링시키고 62℃에서 1분 동안 연장시키는 것을 총 40 사이클 동안 설정하였다. 모든 측정을 3개 한벌로 측정하였다. 형광 방출이 실시간으로 기준선 방출보다 높은 한계에 도달했을 때의 PCR 사이클 수에 상응하는 사이클 한계 (Ct) 값을 GeneAmp 7700 SDS 소프트웨어를 사용하여 분석하였다. 그런 다음 관심의 PCR 생성물 및 대조군 mRNA (GAPDH)의 Ct 값을 사용하여 샘플 간 mRNA의 상대적인 양을 계산하였다.
면역조직화학
산화성 스트레스-유도 지질 과산화를 각막 및 결막에서 4-하이드록시-2-노네날(4HNE)의 면역조직화학 검출에 의해 평가하였다. 포르말린-고정된, 파라핀-내장된 눈 시편들을 자일렌 중에서 파라핀을 제거하고 단계별 에탄올 농도에 재수화시켰다. 슬라이드를 10% 염소 혈청으로 60분 동안 차단한 후 4-HNE (1:100 희석) (ab46545, Abcam)에 대한 일차 항체와 함께 4시간 동안 실온에서 인큐베이션하였다. 슬라이드를 계속해서 적절한 과산화효소-표지된 염소 면역글로불린 (1:500 dilution; Chemicon, Temecula, CA)과 함께 20분 동안 인큐베이션한 후 크로모겐 기질 (3,3'-다이아미노벤지딘)과 함께 2분 동안 인큐베이션한 후 헤마톡실린으로 대조염색하였다.
각막 상피 세포 배양 및 치료
생후 6개월의 뉴질랜드 화이트 토끼로부터 윤부 줄기 세포를 분리하고, 이전에 기술된 것과 같이 (Ho et al., 2013) 계속해서 14일 동안, DMEM/F-12 기준 배지 기반 세포-현탁액 배양에 의해 각막-유사 상피 세포 분화가 이루어질 때까지 배양하였다. 고삼투성 스트레스에 의해 유발된 ROS 또는 MMP-9 활성을 유도하기 위해, 세포를 밤새도록 90 mM NaCl의 첨가에 의해 이루어진 고장성 배지 (463 mOsm)에서 인큐베이션하였다. DMEM/F-12 기준 배지 (309 mOsm)에서 배양된 세포를 음성 대조군으로서 사용하였다. 고삼투성 스트레스-유도 ROS 또는 MMP-9 활성에 미치는 예방 효과를 검출하기 위하여, 세포를 10 μM PDSP로 20시간 동안 전처리한 후 NaCl로 치료하였다.
젤라틴 자이모그래피
MMP-9 활성을 검출하기 위하여, 10 μL의 배양 배지를 사용하여 앞서 기술된 방법과 같이 (Li et al., 2004) 젤라틴 자이모그래피를 수행하였다. 밴드 세기를 자이모그래피에서 모델 GS-700 영상화 밀도계 (Bio-Rad Laboratories, Hercules, CA)로 평가하고 Labworks 4.0 소프트웨어를 사용하여 분석하였다.
세포내 ROS 및 글루타티온 함량의 측정
세포내 ROS 생성을, ROS에 의해 산화되었을 때 녹색 형광 화합물 2',7'-다이클로로플루오레세인 (DCF)을 방출하는 2',7'-다이클로로다이하이드로플루오레세인 다이아세테이트 (H2DCFDA; 분자 프로브, Eugene, OR)를 사용하여 검정하였다. 분광형광계 검정에 의해 ROD를 검출하기 위해, 세포를 PBS로 세척한 후 NP-40 용해 완충액 (10 mM Tris-Cl, pH 7.4, 10 mM NaCl, 및 0.5% NP-40)에 의해 용해시키고 5 μM H2DCFDA를 함유하는 PBS와 함께 암실에서 15분 동안 37℃에서 인큐베이션하였다. 형광 (여기, 488 nm; 방출, 520 nm)을 스펙트라 MAXGEMINI 판독기 (Molecular Devices, Sunnyvale, CA, USA)로 측정하였다. GSH 수준을 글루타티온 검정 키트 (BioVision Research Products, Mountain View, CA)를 사용하여 제조사의 권고를 따라 정량화하였다. 간단히 설명하면, 과요오드산 처리된 세포 용해물을 OPA (o-프탈알데하이드) 프로브 및 GSH 완충액과 함께 40분 동안 실온에서 인큐베이션하였다. 형광을 SPECTRAmax GEMINI XS 형광 마이크로플레이트 형광분광계 (Molecular Devices, Sunnyvale, CA) 상에서 여기의 경우 340 nm에서 및 방출의 경우 420 nm에서 판독하였다.
국소 PDSP 치료는 건조 스트레스에 의해 유도된 안구 표면 손상을 회복시킨다
PDSP가 건조 스트레스 (DS)-유도 안구표면 결함에서 치료 효과를 갖는지를 측정하기 위하여, 마우스를 제어된 환경 챔버 (CEC)에 14일 동안 하우징하여 안구 표면 파괴를 유발시켰다. CEC에서 14일 후에, 본 발명자들은 처음 실험에 대해 2보다 높은 형광 점수를 가진 마우스들을 사용하였다 (도 1A). 계속해서, 건성안을 25 내지 200 μM의 PDSP로 국소적으로 (3회/일) 또는 PDSP 비히클 (BSS 중의 1% CMC)로 또 다시 5일 동안 치료하는 한편으로, 동일한 건조 스트레스 프로토콜을 유지하였다.
제 0일에, 평균 눈물 부피는 면사 테스트에 의해 측정되는 바 비-스트레스 (NS) 마우스와 비교하여 유의하게 감소되었다 (5.4±0.4 mm에 대비하여 3.8±0.5 mm; 도 1B). 눈물 생성은 마우스가 PDSP로 5일 동안 처리된 후에 눈에서 비히클 그룹과 비교하여 유의하게 증가하였다 (4.5 ± 0.2에 대비하여 5 ± 0.3).
PDSP는 결막의 고블렛 세포의 양을 부분적으로 회복시킨다
고블렛 세포는 결막 원개의 표면 상피에 주로 존재하고 점성 눈물 생성에 기여한다. NS 눈의 과요오드산 시프 (PAS) 염색은 고블렛 세포가 결막 상피에서 연속적인 균일한 패턴을 나타내는 것을 보여주었다 (도 2A). 그러나, 건조 스트레스 후 14일 후 (제 0일), 결막의 PAS 염색은 고블렛 세포의 수가 NS 그룹과 비교하여 현저하게 감소하였음을 보여주었다 (38±3.2에 대비하여 58±3.1; 도 2B). 5일 동안 점안액 중의 PDSP 또는 비히클로의 치료로 결막 고블렛 세포 수는 비히클-처리된 대조군에 비교하여 PDSP-처리된 눈에서 유의하게 더 높았다 (37±1.1에 대비하여 46±3.8). 종합적으로, PDSP 치료는 고블렛 세포의 수를 구제하지 못하였다.
국소 PDSP는 건조 스트레스에 의해 유도된 안구 표면 손상을 예방한다
PDSP가 DS-유도 각막 상피 파괴를 억제할 수 있는지를 조사하기 위하여, 본 발명자들은 14-일 건조 스트레스 프로토콜을 마우스에 적용하였고, 이 마우스들을 국소적으로 PDSP로 하루에 3회 처리하였다. 14일 후에, 각막 상피 결함을 플루오레세인 염료 염색에 의해 평가하였고 (도 3) 그 결과는 각막 플루오레세인 염색 점수가 PDSP-처리된 눈에 비해 비히클-처리된 눈에서 유의하게 더 높았음을 나타냈다 (0.7±0.2에 대비하여 1.7±0.3). 이런 결과는 PDSP가 또한 건조 스트레스에 대하여 안구 표면에 예방적 효과를 나타낼 수 있음을 가리킨다.
건조 스트레스 동물 모델에서 인간 및 마우스 PDSP의 비슷한 기능
인간 및 마우스에서 PEDF의 동일한 영역이 DED 대상체의 치료에 비슷하게 기능하는지를 조사하기 위한 것. 인간 PDSP 및 쥐과 PDSP의 활성을 마우스의 건조 스트레스-유도 건성안 모델에서 조사하였다. 인간 및 마우스 PDSP는 둘 다 건조 스트레스에 의해 유도된 안구 표면 손상 (도 11)뿐만 아니라 스트레스-유도 건성안을 가진 마우스에서 눈물 생성 (도 12)을 회복시킬 수 있는 것으로 나타난다.
PDSP는 건조 스트레스-유도 염증성 반응을 억제한다
염증이 실험 동물의 건조 스트레스-유도 건성안의 안구 표면 손상을 증가시킬 수 있을 것이라고 시사되었다 (Luo et al., 2004; De Paiva et al., 2006). 전염증성 매개물질 중에서, 건조 스트레스-유도 건성안은 TNF-α 또는 인터류킨-1 (IL-1) 차단제로 사전치료된 마우스에서 개선된 것으로 보고되었다 [Ji YW 2013; Okanobo A 2012]. 도 4에서 나타낸 것과 같이, 마우스가 CEC에서 14일 동안 하우징된 후에 (제 0일로서 설정됨; 미처리 마우스), IL-1β, TNF-α, IL-6 및 MCP-1을 포함한 전염증성 매개물질의 mRNA 수준은, 비-스트레스 (NS) 환경에서 하우징된 마우스와 비교하여, 각각 3.9-, 2.8-, 2.6-, 및 2.4-배씩 유의하게 상향조절되었다. 그러나, 마우스에서 5일 동안 국소 PDSP 치료는 안구 IL-1β, TNF-α, IL-6 및 MCP-1의 mRNA 발현을, 비히클-처리된 그룹에 비교하여, 각각 2.4-, 1.9-, 2.0-, 및 1.7-배씩 분명하게 억제하였다. 이것들을 함께 고려하면, 본 발명자들의 결과는 PDSP가 DS-유도 안구 염증성 반응을 약화시켰음을 나타낸다.
PDSP는 지질 과산화 관련 안구 표면 손상을 부분적으로 예방한다
반응성 산소 종 (ROS) 과잉생성으로부터 유발된 안구 표면에서의 산화성 손상은 건성안의 병리에 포함되었다 (Wakamatsu et al., 2013). 본 발명자들은 PDSP가 지질 과산화 관련 막 손상을 억제할 수 있는지를 4-HNE 면역조직화학 염색을 통해 조사하였다. 도 5A에서 나타낸 것과 같이, 마우스가 CEC에 14일 동안 하우징 된 후에, 각막 상피의 4-HNE 염색은 상피의 윙 세포(wing cell) 상에서 핵 및/또는 핵주변에 국한된 것을 나타냈고, 이것은 이전의 보고와 일치하였다 (Nakamura et al., 2007). 4-HNE에 대해 양성으로 염색된 세포의 수는 NS, 비히클 및 PDSP 그룹에서 각각 1.9±0.6, 20.1±1.4, 및 5.3±1.0이었고 (도 5B), 이것은 PDSP가 지질 과산화를 방지할 수 있음을 나타낸다. 이에 더불어, 결막에서의 DS-유도 지질 과산화를 또한 4-HNE 염색에 의해 평가하였고, 면역조직화학 결과는 4-HNE 신호가 PDSP 치료를 받은 눈에서보다 비히클-처리된 눈에서 분명하게 더 강력하였음을 나타냈으며, 이것은 추가로 PDSP의 항산화 효과의 발견을 지지하였다 (도 5C). 종합적으로, PDSP 치료는 안구 표면에서 DS-유도 산화성 손상을 유의하게 억제하였다.
PDSP는 각막 상피 세포에서 고삼투성 스트레스-유도 ROS 축적 및 글루타티온 고갈을 예방한다
상승된 눈물 오스몰농도는 염증 및 안구 표면 손상을 유도하는 건성안의 핵심 메커니즘인 것으로 여겨진다 (Stahl et al., 2012). 건성안 진단에 대한 현재의 컷오프는 정상 눈에서의 300 내지 310 mOsm에 비교하여 316 mOsm이다 (Liu et al., 2009). 그러나, 각막 상피 세포에서 고삼투압의 시험관내 연구는, ROS 및 MMP-9 활성을 유도하기 위해서는 더 높은 수준의 눈물 오스몰농도 (350 내지 500 mOsm)에 대한 세포의 장기간 노출이 필요한 것을 보여준다 (Li DQ 2004; Li J 2016).
세포내 ROS 형성에 미치는 고삼투성 스트레스의 영향을 조사하기 위하여, 토끼 각막 상피 세포를 90 mM NaCl을 첨가함으로써 고삼투성 배지 (463 mOsM)에서 24시간 동안 배양하였다. 세포내 ROS 수준을 H2DCFDA 프로브에 의해 검출하였고 DCF-형광의 형성을 분광형광계에 의해 측정하였다.
도 6A에 나타낸 것과 같이, DCF-형광은 등장성 배지 (309 mOsm)에서 배양된 세포에 비교하여, 세포가 고삼투성 배지에서 24시간 동안 배양된 후에 1.6배 증가하였다. 그러나, PDSP로 사전처리된 토끼 각막 상피 세포는 NaCl 단독으로 처리된 세포에 비교하여, 1.3배의 인자만큼 억제된 DCF-형광을 나타냈다 (P<0.04). 대조군 펩타이드 (ContP) 사전치료는 그런 영향을 나타내지 못하였다. NaCl-유도 고삼투성 스트레스 또한 등장성 환경에서의 대조군 세포에 비교하여, 항산화제인 글루타티온 (GSH)의 수준을 현저하게 떨어뜨린 반면 (도 6B; 100±7.0%에 대비하여 46.7±15.4%), PDSP로 다시 처리된 세포는 GSH 수준을 최대 70.5±11.4%로 유의하게 증가시켰다. 이 결과들은 PDSP가 NaCl-유도 고삼투성 스트레스 (463 mOsM)로 처리된 각막 상피 세포에서, 적어도 부분적으로 GSH 수준을 향상시키는 것을 통해 항-산화 효과를 나타내는 것을 가리킨다.
PDSP는 각막 상피 세포에서 고삼투성 스트레스-유도 MMP-9 발현을 억제한다
각막 상피 세포에서 고삼투성 스트레스-유도 MMP-9 발현에 미치는 PDSP의 효과를 조사하기 위하여, 세포를 PDSP 또는 대조군 펩타이드 (ConP)로 20시간 동안 사전처리한 후 세포를 고삼투성 배지 (463 mOsM)에 추가로 24시간 동안 노출시켰다. 도 7A에서 나타낸 것과 같이, ContP/ 고삼투성 배지로 처리된 세포에서 MMP-9의 mRNA 수준은 등장성 배지에서 배양된 세포 (미처리 대조군)에 비교하여, 3배만큼 유의하게 상향조절되었다. 그러나, PDSP 전처리는 MMP-9 mRNA 발현을 2배나 억제하였다.
젤라틴 자이모그래피는 또한 고삼투성 스트레스가 미처리 대조군에 비교하여, MMP-9 활성을 2.7배만큼 유도하였음을 증명하였다 (90 kDa에 가까운 크기; 도 7B). 자이모그래피는 추가로 PDSP 전처리가 MMP-9 활성을 3배나 억제하였음을 보여주었다. 이 결과들은 PDSP가 고삼투성 스트레스에 의해 유도된 MMP-9 발현 및 활성을 억제하는 것을 나타낸다.

Claims (16)

- 제 1 항에 있어서, 상기 펩타이드는 SLGAEQRTESIIHR (SEQ ID NO:2) 또는 SLGAEHRTESVIHR (SEQ ID NO:3)의 서열을 포함하지 않는 것을 특징으로 하는 제약학적 조성물.
- 제 1 항에 있어서, 펩타이드는 SEQ ID NO: 6 내지 75 중 임의의 하나의 서열을 포함하는 것을 특징으로 하는 제약학적 조성물.
- 제 1 항에 있어서, 펩타이드는 20, 22, 24, 또는 29개 아미노산 잔기로 구성되는 길이인 것을 특징으로 하는 제약학적 조성물.
- 제 1 항 내지 제 4 항 중 어느 한 항에 있어서,펩타이드는 N-말단에서 아세틸화로 변형되거나 또는 C-말단에서 아미드화로 변형되는 것을 특징으로 하는 제약학적 조성물.
- 제 1 항 내지 제 4 항 중 어느 한 항에 있어서, 안과 질환은 각막 손상 관련 질환인 것을 특징으로 하는 제약학적 조성물.
- 제 6 항에 있어서, 각막 손상 관련 질환은 건성안 증후군인 것을 특징으로 하는 제약학적 조성물.
- 제 5 항에 있어서, 상기 펩타이드의 농도는 10 내지 200 μM인 것을 특징으로 하는 제약학적 조성물.
- 제 5 항에 있어서, 상기 조성물은 용액, 연고, 또는 겔의 형태인 것을 특징으로 하는 제약학적 조성물.
- 제 5 항에 있어서, 상기 대상체는 인간 대상체인 것을 특징으로 하는 제약학적 조성물.
- 안과 질환을 예방 및/또는 치료하기 위한 방법으로서, 필요로 하는 대상체에게 제 1 항 내지 제 5 항 중 어느 한 항에 따르는 조성물을 투여하는 단계를 포함하는 방법.
- 제 11 항에 있어서, 안과 질환은 각막 손상 관련 질환인 것을 특징으로 하는 방법.
- 제 11 항에 있어서, 상기 각막 손상 관련 질환은 건성안 증후군인 것을 특징으로 하는 방법.
- 제 11 항에 있어서, 상기 펩타이드의 농도는 10 내지 200 μM인 것을 특징으로 하는 방법.
- 제 11 항에 있어서, 상기 조성물은 용액, 연고, 또는 겔의 형태인 것을 특징으로 하는 방법.
- 제 11 항에 있어서, 상기 대상체는 인간 대상체인 것을 특징으로 하는 방법.
