Abstract
Description
カルボキシメチルセルロースナトリウム(CMC)、ピリオド酸(period acid)シッフ(PAS)試薬およびデキサメタゾンは全てシグマアルドリッチ(米国ミズーリ州セントルイス)から入手した。バランス塩溶液(BSS;Alcone)中のCMC(1% w/v)をPDSPのビヒクルとして使用した。PDSPおよび陰性対照ペプチド(ConP)を合成し、NH2末端をアセチル化し、安定性のためにCOOH末端をアミド化して修飾し、質量分析(純度>95%)によって特徴づけてジェンスクリプト(米国ニュージャージー州ピスカタウェイ)で注文した。
7~8週齢の雌C57BL/6マウスをこれらの実験に使用した。
実験手順はMackay Memorial Hospital Review Boardによって承認され、ARVO Statement for the Use of Animals in Ophthalmic and Vision Researchに従って実施された。
ドライアイを、前述のようにマウスを制御された環境チャンバ(CEC)に入れることによって誘発した(Barabinoら、2005)。手短に言えば、CECに入れたマウスを、25%未満の相対湿度(RH)、20~22℃の温度、および15L/分の空気流に1日12時間曝した。ストレス誘発性ドライアイを有していない非ストレス(NS)マウスを、同じ持続時間の間、通常の環境(RH>50%、空気流なし、温度21~23℃)中で保持した。
動物を、ゾレチル(zoletil)(6mg/kg)およびキシラジン(3mg/kg)の混合物の腹腔内注射によって麻酔した。角膜上皮損傷を局所フルオレセインによる染色によって決定した(Fluor-I-Strip、Ayerst Laboratories、ペンシルベニア州フィラデルフィア)。角膜フルオレセイン染色をコバルトブルー光下、細隙灯生体顕微鏡で調べ、デジタルカメラで撮影した。角膜の色素染色は以下のように点数化した:点状染色なしの場合はスコア0;角膜の3分の1未満が染色された場合はスコア1;3分の2以下が染色された場合はスコア2;そして3分の2超が染色された場合はスコア3(Horwath-Winter J 2013)。
涙液産生をフェノールレッド含浸綿糸で測定した(ゾーンクイック;オアシス、カリフォルニア州グレンドラ)。マウスにおけるこの試験の妥当性は以前述のように実施した(Dursunら、2002)。糸を宝石細工人の鉗子で保持し、外側眼角に60秒間置いた。涙液産生は、涙液によって濡れて赤色になった糸のミリメートルで表される。
動物を安楽死させた後、眼を外科的に切除し、10%ホルマリンで固定し、パラフィン包埋し、5μmの切片に切断した。上下結膜の杯細胞を測定するために過ヨウ素酸シッフ試薬(PAS;シグマアルドリッチ)で切片を染色し、デジタルカメラを装備した顕微鏡で調べて撮影した。結膜中のPAS陽性杯細胞を、各眼から5つの切片で測定した。
全RNAを、TRIzol(Invitrogen)を使用して細胞から抽出し、RNアーゼフリーDNアーゼI(キアゲン、カリフォルニア州サンタクラリタ)で処理してゲノムDNAを取り出し、次にRNA精製キット(ダイナビーズ;Invitrogen)で精製した。cDNAの合成をスーパースクリプトIII(Invitrogen)によって実施した。定量的リアルタイムPCRをGeneAmp7700配列検出系(アプライドバイオシステムズ、カリフォルニア州フォスターシティ)で実施した。増幅は、3pmolのプライマー、段階希釈したRT産物およびSYBR Green PCRコア試薬(アプライドバイオシステムズ)を含有する総量40μlで実行した。特異的PCRプライマーの配列は、マウスTNF-α(受入番号:NM_013693)センス、5’-CTACCTTGTTGCCTCCTCTTT-3’(配列番号79)、アンチセンス、5’-GAGCAGAGGTTCAGTGATGTAG-3’(配列番号80);マウスIL-1β(受入番号:NM008361)センス、5’-GGTGTGTGACGTTCCCATTA-3’(配列番号81)、アンチセンス、5’-ATTGAGGTGGAGAGCTTTCAG-3’(配列番号82);マウスIL-6(受入番号:NM031168)センス、5’-GTCTGTAGCTCATTCTGCTCTG-3’(配列番号83)、アンチセンス、5’-GAAGGCAACTGGATGGAAGT-3’(配列番号84);マウスMCP-1(受入番号:NM011333)センス、5’-CTCGGACTGTGATGCCTTAAT-3’(配列番号85)、アンチセンス、5’-TGGATCCACACCTTGCATTTA-3’(配列番号86);ウサギMMP-9(受入番号:NM001082203)センス、5’-TGCGAGTTTCCGTTCATCTT-3’(配列番号87)、アンチセンス、5’-GTAGAGCTTGTCCTTGTCGTAG-3’(配列番号88)であった;ステップサイクルプログラムは、95℃で15秒間の変性、および62℃で1分間のアニーリングおよび伸長、合計40サイクルに設定した。全ての測定は三重反復で測定された。サイクル閾値(Ct)は、リアルタイムの蛍光発光がベースライン発光を上回る閾値に達するPCRサイクル数に対応し、GeneAmp7700SDSソフトウェアを用いて分析された。次に、目的のPCR産物および対照mRNA(GAPDH)のCt値を用いて試料間のmRNAの相対量を計算した。
酸化ストレス誘発性脂質過酸化を、角膜および結膜における4-ヒドロキシ-2-ノネナール(4HNE)の免疫組織化学的検出によって評価した。ホルマリン固定パラフィン包埋眼標本をキシレン中で脱パラフィンし、段階的な一連のエタノール濃度で再水和した。スライドを10%ヤギ血清で60分間ブロックし、次に4-HNEに対する一次抗体(1:100希釈)(ab46545、Abeam)とともに室温で4時間インキュベートした。続いてスライドを適切なペルオキシダーゼ標識ヤギ免疫グロブリン(1:500希釈;ケミコン、カリフォルニア州テメクラ)とともに20分間インキュベートし、次に色素原基質(3,3’-ジアミノベンジジン)とともに2分間インキュベートした後、ヘマトキシリンで対比染色した。
角膜縁幹細胞を6カ月齢のニュージーランド白ウサギから単離し、DMEM/F-12基礎培地に基づく細胞懸濁培養によって14日間連続培養して、前述のように(Hoら、2013)角膜のような上皮細胞分化を達成した。高浸透圧ストレスに起因するROSまたはMMP-9活性を誘導するために、90mM NaClの添加によって達成された高浸透圧培地(463mOsm)中で細胞を一晩インキュベートした。DMEM/F-12基礎培地(309mOsm)中で培養した細胞を陰性対照として使用した。高浸透圧ストレス誘発性ROSまたはMMP-9活性への予防効果を検出するために、NaClの処理の前に細胞を10μM PDSPで20時間前処理した。
MMP-9活性を検出するために、10μLの培地を用いて前述の方法(Liら、2004)のようにゼラチンザイモグラフィーを実施した。バンド強度を、モデルGS-700イメージングデンシトメーター(バイオ・ラッドラボラトリーズ、カリフォルニア州ハーキュリーズ)によるザイモグラフィーで評価し、Lab works4.0ソフトウェアを用いて分析した。
ROSによって酸化されると緑色蛍光化合物2’,7’-ジクロロフルオレセイン(DCF)を放出する2’,7’-ジクロロジヒドロフルオレセインジアセテート(H2DCFDA;モレキュラープローブス、オレゴン州ユージーン)を使用して、細胞内ROS生成をアッセイした。分光蛍光アッセイによってROSを検出するために、細胞をPBSで洗浄し、次にNP-40溶解緩衝液(10mMトリス-Cl、pH7.4、10mM NaCl、および0.5%NP-40)で溶解し、5μM H2DCFDAを含有するPBSとともに暗所で37℃で15分間インキュベートした。蛍光(励起、488nm;発光、520nm)を、Spectra MAX GEMINIリーダー(モレキュラーデバイス、米国カリフォルニア州サニーベール)で測定した。H2DCFDAを添加していない対照ウェルからのバックグラウンド蛍光を実験の測定値から減算した。
PDSPが乾燥ストレス(DS)誘発性眼表面欠損において治療効果を有するかどうかを決定するために、マウスを制御された環境チャンバ(CEC)に14日間収容して眼表面の崩壊を生じさせた。CECで14日後、本発明者らは最初の実験に蛍光スコアが2を超えるマウスを使用した(図1(A))。その後、同じ乾燥ストレスプロトコールを維持しながらさらに5日間、25~200μMのPDSP(3回/日)またはPDSPビヒクル(BSS中1%CMC)でドライアイを局所的に処置した。
杯細胞は、主に結膜円蓋の表層上皮に存在し、粘液涙液産生を担う。NS眼の過ヨウ素酸シッフ(PAS)染色は、杯細胞が結膜上皮で連続的な均一パターンを示すことを示した(図2A)。しかし、乾燥ストレスの14日後(0日目)、結膜のPAS染色は、杯細胞の数がNS群と比較して著しく減少したことを示した(58±3.1対38±3.2;図2B)。点眼薬中のPDSPまたはビヒクルで5日間処置すると、結膜杯細胞数はビヒクル処置対照と比較してPDSP処置した眼において有意に高かった、(46±3.8対37±1.1)。まとめると、PDSP処置は杯細胞の数を救った。
PDSPがDS誘発性角膜上皮破壊を抑制することができるかどうか調査するため、本発明者らは14日の乾燥ストレスプロトコールをマウスに適用し、これらのマウスを1日3回PDSPで局所的に処置した。14日後、角膜上皮の欠損をフルオレセイン色素染色によって評価し(図3)、この結果から角膜フルオレセイン染色スコアが、PDSP処置した眼と比較してビヒクル処置した眼において有意に高いことが明らかになった(1.7±0.3対0.7±0.2)。この結果は、PDSPが乾燥ストレスに対する眼表面の予防効果も示し得ることを示す。
ヒトおよびマウスのPEDFの同じ領域がDED対象の治療で同程度に機能するかどうかを調査すること。ヒトPDSPおよびネズミPDSPの活性を、マウスにおける乾燥ストレス誘発性ドライアイモデルで調べた。ストレス誘発性ドライアイを有するマウスにおいて、ヒトおよびマウスの両方のPDSPが、乾燥ストレス(図11)ならびに涙液産生(図12)によって誘発される眼表面損傷を回復させることができることが示される。
実験動物の乾燥ストレス誘発ドライアイにおいて炎症が眼表面損傷を増加させ得ることが示唆されている(Luoら、2004;De Paivaら、2006)。炎症誘発性メディエーターの中で、TNF-αまたはインターロイキン-1(IL-1)遮断薬で前処置されたマウスにおいて乾燥ストレス誘発性ドライアイが改善されたことが報告された[Ji YW 2013;Okanobo A 2012]。図4に示されるように、マウスをCECに14日間収容した後(0日目に設定;未処置マウス)、IL-1β、TNF-α、IL-6およびMCP-1をはじめとする炎症誘発性メディエーターのmRNAレベルは、非ストレス(NS)環境に収容されたマウスと比較して、それぞれ3.9倍、2.8倍、2.6倍、および2.4倍有意にアップレギュレートされた。しかし、マウスにおける5日間の局所PDSP処置は、眼のIL-1β、TNF-α、IL-6およびMCP-1のmRNA発現を、ビヒクル処置群と比較してそれぞれ2.4倍、1.9倍、2.0倍および1.7倍の倍数で明らかに阻止した。総合すると、本発明者らの結果は、PDSPがDS誘発性眼炎症応答を軽減したことを示す。
活性酸素種(ROS)の過剰産生に起因する眼表面の酸化的損傷はドライアイの病因に関与している(Wakamatsuら、2013)。本発明者らは、PDSPが脂質過酸化関連膜損傷を4-HNE免疫組織化学染色を通じて抑制することができるかどうかを調査した。図5Aに示されるように、角膜上皮の4-HNE染色は、マウスをCECに14日間収容した後に、上皮翼細胞で核および/または核周囲での局在を示し、以前の報告と一致した(Nakamuraら、2007)。4-HNEについて陽性に染色された細胞の数は、NS、ビヒクルおよびPDSP群においてそれぞれ1.9±0.6、20.1±1.4、および5.3±1.0(図5B)であった、これはPDSPが脂質過酸化を防ぐことができることを示す。その上、結膜におけるDS誘発性脂質過酸化も4-HNE染色によって評価し、免疫組織化学染色結果は、4-HNEシグナルがPDSP処置を受けた眼よりもビヒクル処理した眼において明らかに強いことを示した、これはPDSPの抗酸化効果の知見をさらに裏付ける(図5C)。まとめると、PDSP処置は、眼表面のDS誘導性酸化的損傷を有意に抑制した。
涙液浸透圧の上昇は、ドライアイにおいて炎症および眼表面損傷を誘発する中心的な機構であると考えられている(Stahlら、2012)。ドライアイ診断のための現在のカットオフは、正常な眼の300~310mOsmと比較して316 mOsmである(Liuら、2009)。しかし、角膜上皮細胞の高浸透圧のインビトロ研究、ROSおよびMMP-9活性を誘導するために、細胞をより高いレベルの涙液浸透圧(350~500mOsm)に長期間曝すことが必要とされる(Li DQ 2004;Li J 2016)。
角膜上皮細胞における高浸透圧ストレス誘発性MMP-9発現へのPDSPの効果を調べるために、細胞をPDSPまたは対照ペプチド(ConP)で20時間前処理し、次に細胞を高浸透圧培地(463mOsM)にさらに24時間曝した。図7Aに示されるように、ContP/高浸透圧培地で処理した細胞におけるMMP-9のmRNAレベルは、等張性培地中で培養した細胞(未処理対照)と比較して、3倍有意にアップレギュレートされた。しかし、PDSP前処理は、MMP-9 mRNA発現を2倍の倍数で阻止した。
