Abstract
CD3 작용제 및/또는 CD28 작용제를 함유하는 배양 배지에서 36시간 초과 48시간 미만 동안 T-세포를 활성화시키는 단계를 포함하는, T-세포 활성화 방법이 개시된다. 상기 활성화 방법에 의해 T-세포를 활성화시키는 단계; 활성화된 T-세포 내로 외래 유전자를 도입시키는 단계; 및 외래 유전자가 도입된 T-세포를 배양하는 단계를 포함하는, 유전자 변형 T-세포를 생산하는 방법이 개시된다. 상기 생산 방법에 의해 수득된 유전자 변형 T-세포를 포함하는 세포 집단이 개시된다. T-세포를 36시간 초과 48시간 미만 동안 활성화 단계에 적용시키는 데 사용되는, CD3 작용제 및/또는 CD28 작용제가 담지된 이동성 중합체 사슬의 매트릭스 또는 비드가 개시된다.
Description
본 발명은 T 세포 활성화 방법에 관한 것이다. 보다 구체적으로, 본 발명은 키메라 항원 수용체(본 명세서에서 "CAR"로 지칭함)를 발현하는 T 세포(본 명세서에서 "CAR-T 세포"로 지칭함)를 생산하는데 효과적인 T 세포 활성화 방법에 관한 것이다.
CAR-T 세포 요법은 암을 치료하는 방법 중 하나로 알려져 있다. CAR-T 세포는, 예를 들어 환자 자신으로부터 수집한 자가 T 세포를 사용하여 생산되지만; 높은 제조 비용 및 제조 실패의 위험은 사업 운영의 문제이다. 특히, 건강한 사람에 비해 CAR-T 세포 요법을 받는 암 환자는 항암제 등에 의한 치료로 인해 T 세포가 손상된 경우가 많기 때문에 환자 유래 T 세포로부터 CAR-T 세포의 생산 효율은 떨어지는 경향이 있다.
CAR-T 세포를 생산하기 위해서는 자극 물질(예를 들어, 항-CD3 항체 및 항-CD28 항체)을 사용하여 T 세포를 활성화하는 단계가 전형적으로 수행된다. 예를 들어, 특허문헌 1은 자극 물질이 부착된 이동성 중합체 사슬의 매트릭스(나노매트릭스)가 T 세포의 활성화를 위해 사용되는 것이 개시되어 있다.
일반적으로 T-세포 활성화 단계는 24시간, 48시간(2일), 또는 72시간(3일)과 같은 1일의 배수로 수행되어 왔다(비특허문헌 1 내지 7).

본 발명의 목적은 CAR-T 세포의 생산 효율을 향상시키기 위한 수단을 제공하는 것이다.
본 발명자들은 T-세포 활성화 단계에 주목하여 다수의 공여자로부터의 T 세포를 조사했다. 그 결과, T-세포 활성화 단계에서 활성화 기간을 특정 범위(예를 들어, 36시간 초과 48시간 미만)로 설정함으로써 CAR 발현 효율이 개선된다는 것을 발견했다.

본 명세서에서, 항목 1 등의 T-세포 활성화 방법, 항목 5 등의 유전자 변형 T 세포를 생산하는 방법 및 항목 6 등의 유전자 변형 T 세포를 포함하는 세포 집단은 각각 "본 발명의 활성화 방법", "본 발명의 생산 방법" 및 "본 발명의 세포 집단"이라고 지칭된다.
본 발명은 T 세포에 많은 부담을 주는 활성화 단계를 최적화할 수 있고, CAR-T 세포와 같은 유전자 변형 T 세포를 효율적으로 생산할 수 있기 때문에 생산 일수 감소로 인해 생산 비용이 감소될 수 있고, 유전자 변형 T 세포의 생산에서 생산 효율이 개선될 수 있다.

T 세포 활성화 방법
본 발명의 활성화 방법은 CD3 작용제 및/또는 CD28 작용제를 함유하는 배지에서 T 세포를 36시간 초과 48시간 미만 동안 활성화시키는 단계(이하 본 명세서에서 "활성화 단계"라고 지칭함)를 포함하는 T 세포 활성화 방법이다.
사용 시 목적과 용도에 따라 T 세포가 활성화되는 한, 임의의 "T 세포"가 사용될 수 있다. 예로는 αβ T 세포, γδ T 세포, CD8+ T 세포, CD4+ T 세포, 종양 침윤 T 세포, 기억 T 세포, 나이브 T 세포, NKT 세포 및 조절 T 세포를 포함한다.
T 세포는 일반적으로 혈액 및 골수액과 같은 체액; 비장, 흉선, 림프절 및 간과 같은 조직; 또는 원발성 종양, 전이성 종양 및 암성 복수와 같은 암성 조직에 침윤하는 면역 세포로부터 말초 혈액 단핵 세포(PBMC) 및 백혈구 성분채집술로서 수집될 수 있다. 본 발명에 사용되는 T 세포는 수집된 면역 세포의 집단으로부터 단리 및 정제된 특정 T 세포일 수 있거나, 또는 단리 없이 세포 집단(예를 들어, PBMC)에 함유된 T 세포일 수 있다. T 세포는 유도 만능 줄기 세포(iPS 세포), 배아 줄기 세포(ES 세포), 기타 줄기 또는 전구 세포를 적절한 조건 하에서 배양함으로써 T 세포로 분화 유도하여 수득된 것일 수 있다.
T 세포는 인간 또는 인간 외의 포유동물(비인간 포유동물)로부터 유래될 수 있다. 비인간 포유동물의 예로는 마우스, 랫트, 햄스터, 기니피그, 토끼, 개, 고양이, 돼지, 소, 말, 양 및 원숭이를 포함한다.
본 발명의 한 실시양태에서, 본 발명의 방법에 의해 활성화된 T 세포는 외인성 유전자를 도입시키기 위한 T 세포, 예를 들어 키메라 항원 수용체(CAR)를 발현하기 위해 외인성 유전자를 도입시키기 위한 T 세포이다. 이 실시양태에서, T 세포는 예를 들어 CD8+ T 세포 또는 CD4+ T 세포일 수 있고, 혈액으로부터 채취한 것, 예를 들어 말초 혈액 단핵 세포(PBMC)에 함유된 것, 또는 인간, 예를 들어 암 환자 또는 CAR-T 세포가 투여되어야 하는 인간으로부터 유래된 것일 수 있다.
"CD3 작용제" 및 "CD28 작용제"는 작용제로서 CD3 및 CD28(수용체)에 각각 결합하고 활성화를 위해 T 세포를 자극할 수 있는 물질이다. 전형적인 CD3 작용제는 항-CD3 항체이고, 전형적인 CD28 작용제는 항-CD28 항체이다. 유사한 효과를 가진 항체 이외의 물질(예를 들어, T-CEP(T-세포 확장 단백질(Matus 등, JCI Insight, 2020, vol. 5, issue 22, 1-14) 및 PHA(피토헤마글루티닌(Weng 등, J. Ethnopharmacol, 2002, 83(1-2):79-85))도 CD3 및 CD28 작용제로서 사용될 수 있다. 항-CD3 및 항-CD28 항체는 단클론 또는 다클론 항체일 수 있고, 바람직하게는 단클론 항체이다. 항-CD3 및 항-CD28 항체는 단편화될 수 있다. T-세포 활성화 단계에서 RetroNectin(상표)(재조합 인간 피브로넥틴 단편, Takara Bio Inc.)은 CD3 및/또는 CD28 작용제와 함께 사용될 수 있다.
CD3 작용제 및 CD28 작용제의 농도는 T 세포의 표면 분자를 자극하여 T 세포에 신호를 전달하여 활성화를 유도하기에 충분한 농도이면 특별히 제한되지 않는다. 예를 들어, 항-CD3 항체 및/또는 항-CD28 항체가 사용되는 경우, 각 항체의 최종 농도가 0.1 내지 20 μg/mL일 정도의 양으로 사용될 수 있다. 항-CD3 항체 및 항-CD28 항체를 담지하는 이동성 중합체 사슬의 매트릭스(예를 들어, TransACT)가 사용되는 경우, 각 항체의 최종 농도는 10 내지 300 ng/mL가 되도록 사용된다. 항-CD3 항체 대 항-CD28 항체의 몰비는 1:5 내지 5:1, 바람직하게는 1:2 내지 2:1일 수 있다.
본 발명의 활성화 방법은 CD3 작용제 및/또는 CD28 작용제를 함유하는 배지에서 T 세포를 36시간 초과 48시간 미만 동안 배양함으로써, T 세포를 활성화시킨다. "36시간 초과 48시간 미만"의 활성화 기간은, 예를 들면 37, 38, 39, 40, 41, 42, 43, 44, 45, 46, 47시간이다. 이러한 값(N 시간)은 "36시간 초과 48시간 미만"에 포함되는 본 발명을 실시하기 위한 새로운 활성화 기간 범위를 결정할 때, 하한치(N 시간 이상 또는 N 시간 초과) 및/또는 상한치(N 시간 이하 또는 N 시간 미만)로서 임의로 선택될 수 있다. 본 발명의 바람직한 실시양태에서 활성화 기간은 37 내지 44시간, 보다 바람직하게는 39 내지 44시간, 더욱 바람직하게는 39 내지 42시간, 특히 바람직하게는 39 내지 41시간이다.
활성화 단계는 CD3 작용제 및/또는 CD28 작용제를 함유하는 배지를 CD3 작용제 및 CD28 작용제가 없는 배지로 교체하거나, 또는 CD3 작용제 및/또는 CD28 작용제를 함유하는 배지를 희석함으로써 종료될 수 있다. 배지를 희석함으로써 활성화 단계를 종료하는 경우, 본 발명에서의 활성화 기간은 이러한 희석 배지에서 T 세포를 배양하는 데 필요한 시간을 포함하지 않는다. 예를 들어, TransACT를 이용한 활성화 단계가 배지를 희석함으로써 종료된다면, 배지를 70% 이하(바람직하게는 50% 이하)로 희석함으로써 활성화 단계를 종료할 수 있다.
본 발명의 바람직한 실시양태에서, 본 발명의 활성화 방법을 수행하기 위한 배양 기간은, 24, 48, 또는 72시간으로 배양 기간을 변경한 것을 제외하고는 본 발명과 동일한 조건 하에 배양이 수행되는 대조군의 CAR 발현 효율과 비교하여, 높은 CAR 발현 효율을 달성할 수 있는 배양 기간이다. CAR 발현의 효율을 위해, (a) 세포 집단 내 CAR-발현 세포의 백분율 또는 (b) 세포 집단 내 CAR-발현 세포의 수는 지표로 사용될 수 있다. 이러한 지표 (a) 및 (b) 중 적어도 하나가 이하에 기술된 단계 3(유전자 변형 T 세포의 생산 방법; 본 발명의 생산 방법의 배양 단계) 후의 시기(stage)에서 대조군보다 높은 경우, CAR 발현 효율이 증가된 것으로 평가된다.
본 발명의 활성화 방법(활성화 단계)에서, T 세포를 활성화하기 위한 배양 시간은 특정 범위로 설정되며; 그러나, 배양 방법, 배지 및 다른 배양 조건(온도, 분위기, 세포 밀도 등)과 같은 배양의 다른 실시양태는 공지되거나 일반적인 T-세포 활성화 방법에 따르거나, 또는 공지되거나 일반적인 T-세포 활성화 방법에 기초하여 본 발명에 맞게 적절하게 조정될 수 있다. 활성화 단계에서 파종 시의 세포 밀도는, 예를 들면 1.0×105 내지 1.0×107개 세포/mL, 바람직하게는 2.0×105 내지 6.0×106개 세포/mL이다.
활성화 단계의 배양 용기는 특별히 제한되지 않으며, 배양 규모에 따라 플레이트, 디쉬, 페트리 디쉬, 플라스크, 백, 병, 탱크(배양 탱크) 등으로부터 적절하게 선택될 수 있다. 예를 들어, CliniMACS Prodigy(상표)(Miltenyi Biotec)와 같은 폐쇄형 자동 배양 시스템이 사용될 수 있다.
활성화 단계에서 T 세포 배양에 사용되는 공지되거나 일반적인 배지가 사용될 수 있으며, 필수적인 성분이 배지에 적절히 보충될 수 있다.
활성화 단계에서 배지의 예로는 AIM V, X-VIVO-15, NeuroBasal, EGM2, TeSR, BME, BGJb, CMRL1066, Glasgow's MEM, Improved MEM Zinc Option, IMDM, 199 배지, Eagle's MEM, αMEM, DMEM, Ham, RPMI-1640, Fischer 배지 및 기타 T 세포 배양용으로 시판되는 제품(예컨대, CTSTM OpTmizerTM T-Cell Expansion Basal Medium(Thermo Fisher Scientific), CTSTM OpTmizerTM Pro Serum Free Medium(Thermo Fisher Scientific), CTSTM OpTmizerTM Pro (Thermo Fisher Scientific) 및 CTSTM OpTmizerTM T-Cell Expansion Supplement(Thermo Fisher Scientific))을 포함한다. 이들 배지는 단독으로 또는 2종 이상의 혼합물로 사용될 수 있다.

본 발명에서 "이동성 중합체 사슬의 매트릭스"(본 명세서에서 때로는 "이동성 매트릭스"라고도 함)의 정의 및 실시양태는 특허문헌 1(WO2014/048920)에 기재된 용어의 정의 및 실시양태와 동일하고, 특허문헌 1의 설명은 본 발명(본 명세서)에 참고적으로 사용될 수 있다. 본 발명의 한 실시양태에서, CD3 작용제 및/또는 CD28 작용제는 CD3 및 CD28 항체가 상보적인 올리고뉴클레오타이드를 통해 단일 가닥 DNA 스캐폴드에 접합되는 DNA-기반 T-세포 활성화제일 수 있다(Keskar 등, J Immunother, 2020, vol.43, No.8, 231-235).
유전자 변형 T 세포의 생산 방법
본 발명의 생산 방법은 다음 단계 1 내지 3을 포함하는, 유전자 변형 T 세포의 생산 방법이다:
단계 1: 본 발명의 활성화 방법에 의해 T 세포를 활성화하는 단계.
단계 2: 외인성 유전자를 활성화된 T 세포에 도입시키는 단계(본 명세서에서 "형질도입 단계"라고 함).
단계 3: 외인성 유전자가 도입된 T 세포를 배양하는 단계(본 명세서에서 "배양 단계"라 함).
단계 1: 활성화 단계.
활성화 단계는 본 발명의 활성화 방법을 수행하여 T 세포를 활성화시키는 단계이다. 본 발명의 활성화 방법을 기술하는 전술한 사항은 활성화 단계의 세부 사항을 설명한다.
단계 2: 형질도입 단계
형질도입 단계는 활성화 단계에서 수득한 T 세포에 외인성 유전자를 도입시키는 단계이다.
"외인성 유전자"는 T 세포에서 원하는 단백질을 발현시키기 위해 외부로부터 도입되어야 하는 유전자이며, 생산하고자 하는 T 세포의 용도에 따라 적절히 선택될 수 있다. 하나 이상의 외인성 유전자가 사용될 수 있다.
본 발명의 한 실시양태에서, 외인성 유전자는 CAR을 발현하는 유전자를 포함할 수 있다. 본 발명의 한 실시양태에서, 외인성 유전자는 CAR을 발현하는 유전자, 사이토카인 및/또는 케모카인을 발현하는 유전자를 포함할 수 있다. 예를 들어, CAR 및 사이토카인과 케모카인의 특정 조합(바람직하게는 IL-7 및 CCL19)을 외인성 유전자를 사용하여 발현시킴으로써 T 세포의 항종양 활성은 향상될 수 있다. 다른 외인성 유전자로서, 항원 특이적 TCR(T-세포 수용체) 및 STAR 수용체(합성 TCR 및 항원 수용체)(Liu 등, Sci. Transl. Med. 2021, vol. 13, issue 586)가, 예를 들어 T 세포에 도입될 수 있다.
(1) CAR
일반적이거나 알려진 CAR과 마찬가지로, T 세포에 의해 발현되는 CAR은 기본적으로 (i) 암세포의 세포-표면 항원을 인식하는 표적 인식 부위(예를 들어, 단일 사슬 항체), (ii) 막관통 영역, 및 (iii) T 세포의 활성화를 유도하는 신호 전달 영역의 부위에 있는 펩타이드가 필요에 따라 스페이서를 통해 연결되도록 구성된다.
(i) 표적 인식 부위는 암세포의 세포-표면 항원을 인식하는 항체(예를 들어, 단일 사슬 항체)일 수 있다. 암세포의 세포-표면 항원을 인식하는 단일 사슬 항체 (i)는 전형적으로 항원에 특이적으로 결합하는 단클론 항체의 항원 결합 부위에서 유래된 경쇄 가변 영역과 중쇄 가변 영역, 및 이러한 영역을 연결하는 링커 펩타이드로 구성된 단일 사슬 가변 단편(scFv)이다. (i) 표적 인식 부위는 CAR이 암세포에 대해 항종양 효과를 특이적으로 나타낼 수 있는 한, 임의의 표적을 인식할 수 있다. 예를 들어, 암세포의 세포 표면 항원에 특이적으로 결합하는 항체의 불변 영역을 인식하는 항체(단일-사슬 항체)일 수 있다.

신호 전달 영역이 복수의 세포내 영역을 함유하는 경우, 세포내 영역은 2 내지 10개의 아미노산으로 이루어진 링커 펩타이드(올리고펩타이드 또는 폴리펩타이드)를 통해 서로 연결될 수 있다. 이러한 링커 펩타이드의 예는 글리신-세린 연속 서열로 이루어지는 펩타이드를 포함한다.

T 세포가 발현하는 전술한 CAR의 아미노산 서열은 T 세포의 용도, 전형적으로 암 치료용 의약의 활성 성분으로서의 기능에 따라 적절히 설정될 수 있다. 표적 인식 부위 (i)의 일례인 암세포의 세포 표면 항원에 대한 단일 사슬 항체의 아미노산 서열로서, 다양한 아미노산 서열이 알려져 있고, 이러한 아미노산 서열에 대한 정보는 본 발명에서도 사용될 수 있다. 대안적으로, 암세포의 원하는 세포 표면 항원에 대한 신규 항체는 통상적인 방법에 따라 생산될 수 있으며, 신규 항체(바람직하게는 중쇄 및 경쇄 가변 영역, 특히 CDR)의 아미노산 서열을 결정함으로써 수득되는 정보는 본 발명에 사용될 수 있다. 또한, 막관통 영역 (ii) 및 T-세포 활성화 신호 전달 영역(iii)의 아미노산 서열로는 인간 및 인간 이외의 포유동물 유래의 다양한 아미노산 서열이 알려져 있고, 이러한 아미노산 서열은 또한 NCBI(미국국립생물공학정보센터; http://www.ncbi.nlm.nih.gov/guide/) 및 UniProt(The Universal Protein Resource; https://www.uniprot.org)와 같은 데이터베이스에도 등록되어 있다. 따라서, 이러한 정보는 본 발명에도 사용될 수 있다. 특히, 표적 인식 부위 (i)가 암세포의 표면 항원에 결합하는 항체의 불변 영역을 인식하는 항체인 경우, 통상적인 방법에 따라 수득되거나 또는 생산될 수 있다.
(2) 사이토카인

(3) 케모카인

외인성 유전자를 T 세포에 도입시키기 위한 수단은 특별히 제한되지 않으며, 다양한 공지된 또는 일반적인 수단이 사용될 수 있다. 전형적으로, 외인성 유전자는 발현 벡터를 사용하여 T 세포에 도입되고, 발현된다. 발현 벡터는 선형 또는 환형일 수 있으며, 플라스미드, 바이러스 벡터 또는 트랜스포존 벡터와 같은 비-바이러스성 벡터일 수 있다.
발현 벡터를 T 세포에 도입시키기 위한 수단은 실시양태에 따라 적절하게 만들어질 수 있다. 예를 들어, 발현 벡터는 바이러스 감염법, 인산칼슘법, 리포펙션법, 마이크로인젝션법, 또는 전기천공법과 같은 공지의 방법에 의해 T 세포 내로 도입될 수 있다. 발현 벡터는 공지된 수단에 의해 또는 적절한 시판 키트를 사용하여(설명서에 따름) 각 방법에 사용하기에 적합한 형태로 생산될 수 있다. 형질도입 단계에서, 이러한 제제를 T 세포에 접촉시키고; 일반적으로 발현 벡터를 함유하는 제제가 첨가된 배지에서 T 세포를 배양할 수 있다.
본 발명의 한 실시양태에서, 발현 벡터는 바이러스 감염 방법에 의해 T 세포에 도입된다. 바이러스 벡터의 예로는 레트로바이러스 벡터, 렌티바이러스 벡터, 아데노바이러스 벡터 및 아데노-연관 바이러스 벡터를 포함한다. 레트로바이러스 벡터의 바람직한 예로는 pMSGV 벡터(Tamada K 등, ClinCancer Res 18:6436-6445(2002)), pMSCV 벡터(Takara Bio Inc.에서 생산), 및 SFG 벡터를 포함한다. 레트로바이러스 벡터가 사용되면, 벡터에 함유된 유전자는 숙주 세포(본 발명에서는 T 세포)의 게놈에 혼입되므로, 장기간 안정적으로 유전자를 발현시킬 수 있다. 이러한 바이러스 벡터를 사용하는 경우, 상응하는 시판 키트를 사용하여 원하는 외인성 유전자를 함유하는 벡터 및 각 바이러스의 패키징 벡터(플라스미드)를 숙주 세포에 형질감염시켜 재조합 바이러스를 생산한 다음, 수득한 재조합 바이러스를 T 세포에 감염시킬 수 있다. 시판 바이러스 벡터용 키트의 예로는 Retrovirus Packaging Kit Eco(Takara Bio Inc.에서 생산)를 포함한다. 숙주 세포의 예로는 GP2-293 세포(Takara Bio Inc.에서 생산), Plat-GP 세포(Cosmo Bio Co., Ltd.에서 생산), PG13 세포(ATCC CRL-10686), 및 PA317 세포(ATCC CRL-9078)를 포함한다.
복수의 외인성 유전자가 사용되는 경우, 모든 외인성 유전자는 하나의 발현 벡터에 함유될 수 있거나, 모든 외인성 유전자는 상이한 발현 벡터에 별도로 함유될 수도 있거나, 복수 개의 외인성 유전자 중 일부가 하나의 발현 벡터에 함유될 수 있고, 다른 유전자는 다른 발현 벡터에 별도로 함유될 수 있다. 하나의 발현 벡터가 복수의 외인성 유전자를 함유하는 경우, 해당 외인성 유전자가 상류측으로부터 하류측으로 배열되는 순서는 특별히 제한되지 않는다.
외인성 유전자는 전술한 바와 같은 CAR, 사이토카인 및 케모카인과 같은 T 세포에 의해 발현되는 원하는 단백질 또는 폴리펩타이드의 아미노산 서열을 암호화하는 염기 서열을 갖는 핵산(폴리뉴클레오타이드)으로 구성될 수 있다. 관련 기술분야의 기술자는 T 세포에서 원하는 단백질(폴리펩타이드)을 발현할 수 있는 발현 벡터를 설계하고 생산할 수 있을 것이다. 발현 벡터에 함유된 핵산은 DNA를 화학적으로 합성하여 생산할 수도 있고, 또는 cDNA로 생산(클로닝)할 수도 있다.

핵산 유도체는 핵산과 비교하여, 뉴클레아제 내성, 안정화, 상보적 가닥 핵산과의 친화성 증가, 세포 투과성의 향상 또는 가시화의 개선을 위해, 핵산에 또 다른 화학물질을 첨가함으로써 수득된 분자라면 임의의 분자일 수 있다. 구체적인 예로는 5'-폴리아민-부가물 유도체, 콜레스테롤-부가물 유도체, 스테로이드-부가물 유도체, 담즙산-부가물 유도체, 비타민-부가물 유도체, Cy5-부가물 유도체, Cy3-부가물 유도체, 6-FAM 부가물 유도체, 비오틴- 부가물 유도체 등을 포함한다.
형질도입 단계에서 배양 용기, 배지, 첨가 성분 및 기타 배양 조건은 본 발명의 활성화 방법에서 활성화 단계에 대해 기술된 것과 동일할 수 있으며, 열거된 것 중에서 적절하게 선택할 수 있다. 하지만, 형질도입 단계에서의 배양 용기, 배지 및 첨가 성분은 활성화 단계에서와 반드시 동일할 필요는 없으며, 적절하게 변경될 수 있다.
단계 3: 배양 단계
배양 단계는 외인성 유전자가 도입된 T 세포를 배양하는 단계이다. 배양 단계에서의 배양 기간은 원하는 단백질이 T 세포에 의해 발현되기에 충분히 길다면 특별히 제한되지 않는다. 배양 기간은 예를 들면 20일, 바람직하게는 3 내지 7일이다.
발현 벡터가 기능성 유전자로서 약물 내성 유전자를 포함하는 경우, 발현 벡터가 도입된 T 세포를 선택하기 위해 배양 단계에서 사용된 약물 내성 유전자에 상응하는 약물이 배지에 첨가될 수 있고, 이에 따라 T 세포를 배양한다. 발현 벡터가 기능적 유전자로서 형광 단백질을 암호화하는 유전자(리포터 유전자)를 함유하는 경우, 발현 벡터가 도입된 T 세포는 배양 단계에서 또는 그 후에 형광현미경에 의해 관찰될 수 있거나, 또는 발현 벡터가 도입된 T 세포가 세포 분류기에 의해 분류될 수 있다.
배양 단계에서 배양 용기, 배지, 첨가 성분 및 기타 배양 조건은 본 발명의 활성화 방법에서 활성화 단계에 대해 기술된 것과 동일할 수 있고, 열거된 것 중에서 적절하게 선택될 수 있다. 그러나, 배양 단계에서 배양 용기, 배지 및 첨가 성분은 활성화 단계에서와 반드시 동일할 필요는 없으며, 적절히 변경될 수 있다.
본 발명의 생산 방법에 의해 수득되는 유전자 변형 T 세포를 함유하는 세포 집단의 적용은 특별히 제한되지 않는다. 세포 집단은 임의의 원하는 목적에 사용될 수 있다. 예를 들어, 세포 집단은 의약(세포 제제)을 생산하기 위해 사용될 수 있다.
의약(세포 제제)은 유전자 변형 T 세포를 함유하고, 필요에 따라 다른 성분을 추가로 함유할 수 있다. 관련 기술분야의 기술자라면 적용(치료할 질병, 투여 대상체 등) 및 투여 형태를 고려하여 유전자 변형 T 세포를 사용하여 이러한 의약을 적절하게 생산할 수 있을 것이다.

유전자 변형 T 세포 이외에 의약에 함유될 수 있는 성분의 예로는 약제학적으로 허용되는 첨가제, 보다 구체적으로는 식염수, 완충 식염수, 세포 배양 배지, 덱스트로스, 주사용수, 글리세롤, 에탄올, 안정제, 가용화제, 계면활성제, 완충액, 보존제, 등장제, 충전제 및 윤활제를 포함한다.
의약은 공지된 유전자 변형 T 세포(예컨대, CAR-T 세포)와 동일한 방법으로 암 치료를 필요로 하는 대상체(암 환자 및 암에 걸린 동물 등)에게 투여하여 사용될 수 있다. 투여 방법의 예로는 종양내, 정맥내, 동맥내, 근육내, 피하 및 복강내 주사를 포함한다.
의약에 함유된 유전자 변형 T 세포의 양은, 예를 들어 암의 유형, 위치 및 중증도, 치료할 대상체의 연령, 체중 및 병태 등을 고려하여 용도, 투여 형태, 의도된 치료 효과 등에 따라 적절하게 조정할 수 있다. 예를 들어, 의약은 CAR-T 세포가 단일 용량으로, 보통 1x104 내지 1x1010, 바람직하게는 1x105 내지 1x109, 더 바람직하게는 5x106 내지 5x108의 양으로 투여되도록 제형화될 수 있다.
의약의 투여 간격은 특별히 제한되지 않으며, 1회에 투여되는 본 발명의 T 세포의 양을 고려하여 적절히 조정할 수 있다. 의약은 독립적으로 투여될 수 있으며, 예를 들어, 1일 4회, 3회, 2회 또는 1회, 격일, 3일마다, 4일마다, 5일마다, 6일마다, 주 1회, 8일마다, 9일마다, 10일마다, 주 2회, 한 달에 1회 또는 한 달에 2회이다.

정의

실시예
(I) 재료 및 방법
(1) 배지
2.6% CTSTM OpTmizerTM T-Cell Expansion Supplement(Thermo Fisher Scientific), 1% L-글루타민(Thermo Fisher Scientific), 1% 스트렙토마이신 및 2% CTSTM Immune Cell SR(Thermo Fisher Scientific)을 CTSTM OpTmizerTM T-Cell Expansion Basal Medium(Thermo Fisher Scientific)에 첨가하여 기본 배지를 생산했다. MACS GMP IL-2(Miltenyi Biotec)를 기본 배지에 20 IU/mL 또는 40 IU/mL의 양으로 첨가하여 배양 배지를 형성시켰다.
(2) PBMC를 이용한 소규모 CAR-T 세포의 생산
인간 PBMC(4명의 공여자: 공여자 A, B, C 및 D)를 해동한 다음, 배양 배지에 1.0x106 세포/mL의 농도로 희석했다. 세포 현탁액 대 MACS GMP T-Cell TransACT(Miltenyi Biotec)의 비율이 17.5:1이도록 세포를 배양 플레이트에 파종하고, 24 내지 72시간 동안 배양했다(활성화 단계). 활성화된 세포를 원심분리하고 배지를 새로운 배양 배지로 교체한 후, Retronectin(Takara Bio Inc.) 및 레트로바이러스로 사전에 예비코팅된 배양 플레이트에 세포를 5.0x104 세포/cm2의 농도로 파종하고, 다음날까지 배양했다(유전자 도입 단계). 유전자 도입 단계에서 CAR 유전자(항-GPC3 scFv, CD8 힌지 영역, CD8 막관통 영역, CD28 세포내 영역, 4-1BB 세포내 영역 및 CD3ζ 세포내영역 포함), IL-7 유전자, 및 CCL19 유전자를 포함하고, 이들 유전자가 각각 자가-절단 펩타이드(2A 펩타이드)를 암호화하는 뉴클레오타이드 서열을 통해 연결되어 있는 외인성 유전자를 도입시켰다. 세포는 배양 배지로 세척한 후, 모든 세포를 배양 플레이트에서 배양 배지에 4 내지 6일 동안 배양했다.
(3) T 세포를 이용한 임상 규모의 CAR-T 세포의 생산
Leukopak(인간 백혈구 성분채집술 제품, HemaCare)(3명의 공여자: 공여자 E, F, G)을 해동한 후, CliniMACS Prodigy(Milteny Biotec) 내 CliniMACS CD4 GMP Microbeads(Miltenyi Biotec) 및 CliniMACS CD8 GMP Microbeads(Miltenyi Biotec)를 사용하여 T 세포를 단리했다. 단리된 T 세포는 2.0x106개 세포/mL 이하의 농도로 배양 배지에 희석했다. 세포는 현탁액 대 MACS GMP T-Cell TransACT(Miltenyi Biotec)의 비율이 17.5:1이도록 세포를 배양 백에 파종하고 36 내지 48시간 동안 배양했다(활성화 단계). 활성화된 세포는 LOVO Cell Processing System(Fresenius Kabi)을 사용하여 배양 배지에 희석하고, Retronectin(Takara Bio Inc.) 및 레트로바이러스로 예비코팅된 배양 백에 6.07x105 세포/cm2 이하의 농도로 파종하고, 다음날까지 배양했다(유전자 형질도입 단계). 유전자 형질도입 단계에서 CAR 유전자(항-GPC3 scFv, CD8 힌지 영역, CD8 막관통 영역, CD28 세포내 영역, 4-1BB 세포내 영역 및 CD3ζ 세포내 영역 포함), IL-7 유전자, 및 CCL19 유전자를 포함하고 이들 유전자가 각각 자가-절단 펩타이드(2A 펩타이드)를 암호화하는 뉴클레오타이드 서열을 통해 연결되어 있는 외인성 유전자를 도입시켰다. 세포를 배양병(G-REX, Wilson Wolf)에 2.2x106 세포/cm2 이하의 농도로 파종하여 3 내지 7일간 배양했다.
(4) 유세포분석
생산 후 샘플 내 CAR-발현 세포의 백분율 또는 CAR-발현 세포의 수는 Protein-L을 사용하여 샘플 내 CD45-발현 또는 CD3-발현 세포 중 CAR-발현 세포를 측정하여 계산했다. 활성화 단계 직후 T-세포 활성화 마커(CD25 및 CD69)를 발현하는 세포의 백분율은 샘플 내 CD3-발현 세포 또는 CD4 또는 CD8 발현 세포에서 이들 마커를 발현하는 세포를 측정함으로써 계산했다.
(5) 활성화에 사용된 TransACT 및 IL-2의 잔여량 분석
활성화 단계 후 배지에 남아있는 TransACT의 잔여량은 T Cell Activation Bioassays(Promega, J1621)를 사용하여 측정했다. IL-2의 잔여량은 Human IL-2 DuoSet ELISA(R&D systems, cat # DY202)를 사용하여 측정했다.
(II) 결과
도 1은 "(2) PBMC를 사용한 소규모 CAR-T 세포의 생산"에 따라 수득한 세포 집단(활성화 단계 24, 48, 또는 72 시간) 내 CAR-발현 세포의 백분율 및 CAR-발현 세포의 수를 유세포분석에 의해 측정한 결과를 보여준다. CAR 발현 세포의 백분율은 공여자-독립적인 방식으로 48시간째에 가장 높아졌다(도 1a). 또한 CAR-발현 세포의 수는 48시간째에 높아지는 경향이 있었다(도 1b).
도 2는 "(2) PBMC를 사용한 소규모 CAR-T 세포의 생산"에 따라 수득한 세포 집단(활성화 단계 40, 44, 또는 48 시간) 내 CAR-발현 세포의 백분율 및 CAR-발현 세포의 수를 유세포분석에 의해 측정한 결과를 보여준다. CAR 발현 세포의 백분율은 공여자-독립적인 방식으로 40시간째에 가장 높아졌다(도 2a). 또한 CAR-발현 세포의 수는 40시간째에 높아지는 경향이 있었다(도 2b).
도 3은 "(2) PBMC를 사용한 소규모 CAR-T 세포의 생산"에 따라 수득한 세포 집단(활성화 단계 37 내지 44 시간) 내 CAR-발현 세포의 백분율을 유세포분석에 의해 측정한 결과를 보여준다. CAR-T 세포는 37시간 이상 및 44시간 이하의 활성화 기간에 높은 효율로 생산되었다.
도 4는 "(2) PBMC를 사용한 소규모 CAR-T 세포의 생산"에 따라 수득한 세포 집단(활성화 단계 40, 44, 또는 48 시간) 내 활성화된 T 세포가 지표로서 활성화 마커(CD25 또는 CD69)를 사용하여 유세포분석으로 측정한 결과, 및 배양 상청액에 남아 있는 활성화에 사용된 TrasACT 및 IL-2의 양이 분석된 결과를 보여준다. CD25-발현 또는 CD69-발현 세포의 백분율은 활성화 단계 직후 활성화 기간(40, 44, 및 48 시간) 사이에 차이가 없었다(도 4a 및 4b). 활성화 단계 직후 배양 상청액에 남아 있는 활성화에 사용된 TransACT 및 IL-2의 양은 활성화 기간(40, 44, 및 48 시간) 사이에 차이가 없었다(도 4c 및 4d). 도 1 내지 3의 결과에 제시된 CAR-T 세포의 생산 효율은 배지 내 주요 성분 및 활성화 정도 외에 다른 인자로 인해 증가한 것으로 생각된다.
도 5는 실시예의 "(3) T 세포를 사용한 임상 규모의 CAR-T 세포의 생산"에 따라 수득한 세포 집단(활성화 단계 36, 40, 44, 또는 48 시간)에서 유세포분석에 의해 CAR-발현 세포의 백분율을 측정한 결과를 보여준다. CAR-발현 세포의 백분율은 공여자-독립적 방식으로 40시간째에 높아지는 경향이 있었다. CAR-발현 세포의 백분율은 36시간째에 감소했고, 36시간 동안의 활성화는 CAR-T 세포의 생산에 불충분했다.

- 제1항에 있어서, 상기 CD3 작용제 및 CD28 작용제가 각각 항-CD3 항체 및 항-CD28 항체인, 활성화 방법.
- 유전자 변형 T 세포의 생산 방법으로서,
제1항의 활성화 방법에 따라 T 세포를 활성화시키는 단계,
상기 활성화된 T 세포에 외인성 유전자를 도입시키는 단계, 및
상기 외인성 유전자가 도입된 상기 T 세포를 배양하는 단계
를 포함하는 생산 방법. - 제5항에 따른 생산 방법에 의해 수득된 유전자 변형 T 세포를 포함하는 세포 집단.
- 제5항에 있어서, 상기 외인성 유전자가 키메라 항원 수용체 유전자인, 생산 방법.
- CD3 작용제 및/또는 CD28 작용제를 담지하는 이동성 중합체 사슬의 매트릭스 또는 비드로서, 상기 매트릭스 또는 비드는 배지에 첨가되어 T 세포를 36시간 초과 48시간 미만 동안 활성화 단계에 적용시키는 데 사용되는, 매트릭스 또는 비드.
- 외인성 유전자를 도입시키기 위한 T-세포 활성화 키트로서,
담체 상에 담지된 CD3 작용제 및/또는 CD28 작용제, 및
36시간 초과 48시간 미만 동안 상기 CD3 작용제 및/또는 상기 CD28 작용제를포함하는 배지에서 T 세포의 활성화를 설명하는 매뉴얼
