본 발명은 노년황반변성의 진단, 치료 및 예방용 조성물 및 노년황반변성에 대한 진단 방법에 관한 것이다.
이미 우리나라뿐만 아니라 세계인들의 평균 수명은 80세를 넘어선 고령화 시대에 살고 있다. 우리나라의 경우 특히 급격한 평균수명의 증가와 출산율의 저하 등으로 인하여 세계에서 가장 빠른 속도로 고령화가 진행중이다.
평균 수명이 늘어난 만큼 건강한 삶을 지속하기 위해서는 노화와 외부환경에 의해 발병하는 질병의 치료가 매우 중요하다. 노령화에 의한 가장 흔하면서 대표적 질환이 눈과 관련된 질환이다. 노화에 의한 질환이라 여겨졌던 안과 질환은 변화된 환경적 요인에 의해 발병 연령이 낮아지고 있고 발병률 또한 높아지고 있다. 이미 서구에서 50세 이상 성인의 실명 원인 질환 중 1위인 노년황반변성은 우리나라에서도 급속히 발생율이 증가하고 있으며,유병율이 높다.
노년황반변성 (age-related macular degeneration, AMD)은 50대 이상의 성인에서, 망막과 망막색소상피, 맥락막혈관 (choriocapillaris)이 진행성, 다인자성으로 변성 (degeneration), 위축 (atrophy) 되는 질환이다. AMD는 이미 서구에서는 당뇨망막병증, 녹내장을 제치고 50세 이상 어른의 실명 원인 질환 중 1위를 차지할 만큼 중 요한 병이다. 미국에서는 현재 8백만 명 이상이 이 질병의 보다 흔한 형태인 비혈관성 또는 위축성 노년황반변성 (dry AMD)을 가진 것으로 보고 되었고, 매년 3백만 명 이상이 이 질병으로 시력을 잃고 있다. 우리나라에서도 식생활의 서구화와 환경요인, 평균수명의 연장에 따라 급속히 발생율이 증가하고 있다. 현재 65세 이상 인구 약 5백만 명의 5%인 25만명이 노년황반변성을 앓고 있는 것으로 추정된다. 이러한 노인성 질환은 의료비 부담이라는 경제적 문제 뿐만 아니라,“삶의 질”의 저하라는 커다란 사회적 문제로 부각될 가능성이 있다. 그러므로, 노년황반변성의 조기진단 및 효율적 치료를 위한 연구가 절실히 필요하다.
그러나, AMD의 발생기전은 다인자성이고, 부분적으로만 밝혀져 있다. 유전적 소인 이외에, 환경오염, (과도한)광선 노출, 산화 스트레스 등 유해 환경이 축적되어가는 것과의 연관성이 보고되었으나, 구체적인 메커니즘은 아직 완전히 알려지지 않았다.
현재 노년황반변성 환자의 치료 현황은 다음과 같다.
습성 노년 황반 변성인 맥락막 신생혈관 (choroidal neovascularization, CNV)의 치료는 크게 destructivetherapy (레이저광응고술, 광역학요법, 외과적수술)와 pathway-based therapy (항혈관내피세포성장인자억제제,anti-vascular endothelial growth factor)로 나뉘어진다.
그러나, 현재까지 상기의 어떤 방법도 심각한 시력 손실을 늦추거나 다소의 시력 개선을 가져올 뿐, 근본적인 치료가 되지는 못하고 있다. 특히, 이러한 치료들은 혈관 신생성 노년황반변성 (습성 노년황반변성, wet AMD,or neovascular AMD) 치료에 국한되고 있으며, AMD 환자의 70-80 %를 차지하는 비혈관 신생성 노년황반변성인위축성 변성 (건성 노년황반변성, dry AMD, geographic atrophy)의 치료는 거의 전무한 실정이다.
현재 습성 노년황반변성의 치료로 제한적인 시력 호전 효과가 입증된 것은 anti-VEGF의 반복적 유리체강내 주사가 유일하다. 이는 습성 노년황반변성의 맥락막 신생혈관 (choroidal neovascularization, CNV)의 혈관 생성 싸이토카인인 혈관내피세포성장인자 (vascular endothelial growth factor, VEGF)를 타겟팅하는 치료이다.그러나, anti-VEGF 치료를 받은 환자의 1/3에서만 제한적인 시력 호전이 입증되었고, 치료받은 환자의 1/6은 법적 실명상태로 진행한다. 또한, 정상성인 망막에 존재하는 VEGF를 지속적으로 억제함으로써 올 수 있는 망막 독성 등의 안전성 문제도 제기되고 있다. 이 치료는 유리체강내 약물의 짧은 반감기로 인하여 자주 반복 주사해야하고, 반복주사함에 따라 유리체출혈, 망막박리, 안내염 같은 합병증 발생 가능성이 증가한다. VEGF는 강력한 혈관확장제 역할을 하기 때문에 심장의 관상동맥의 이완과 혈액 순환을 유지시키는 역할을 한다. AMD 환자들이 고령이기 때문에 심혈관 질환의 위험이 높은 군들이므로 심각한 부작용 위험성도 있다. 따라서 VEGF 이외에 AMD 특이 분자 또는 단백질 마커를 찾아내고 이를 타겟팅하는 물질의 개발이 필요하다.

또 본 발명은 대상(subject)에서 노년황반변성에 대한 정보를 얻기 위한 방법으로서,(a) 대상으로부터의 생물학적 샘플에서 하나 이상의 바이오마커를 측정하는 단계로서, 여기서 하나 이상의 바이오마커는 상기 본 발명의 단백질 조성물 중에서 선택되는 단계; 및 (b) 측정치 또는 측정치들을 정상인과 비교하여 그 측정치가 정상인보다 높은 경우에 노년황반변성과 상호관련시키는 단계를 포함하는 노년황반변성에 대한 정보를 얻기 위한 방법을 제공한다.
본 발명의 일 구현예에 있어서, 상기 방법은 상기 단백질 바이오마커의 발현이 정상인과 비교하여 1.5배 이상인 경우에 노년황반변성과 상호관련시키는 것이 바람직하나 이에 한정되지 아니한다.즉 노년황반변성의 병태생리와 관련이 있을 것으로 강력히 추정되는 단백질의 경우 1.5배 이상 증가되지 않을지라도 최종 정량 분석을 위한 단백질 선택에 있어 고려 대상으로 하였다.
또 본 발명은 유전자 정보 번호(GI No.) 4504517의 Hspβ-1; 및 유전자 정보 번호(GI No.) 4504919의 사이토케라틴(cytokeratin) 8로 구성된 군으로부터 선택된 하나 이상의 단백질을 포함하는 노년황반변성 치료용 항-VEGF 치료제 반응 예측용 조성물을 제공한다.
서열번호 1 내지 서열번호 20에 기재된 펩타이드로 구성된 군으로부터 선택된 하나 이상의 펩타이드를 유효성분으로 포함하는 노년황반변성 진단용 키트를 제공한다.
본 발명에서 면역원성 단편이란 본 발명의 바이오 마커 단백질에 대한 항체에 의해 인식될 수 있는 하나 이상의 에피토프(epitope)를 가지고 있는 바이오마커 단백질의 단편을 의미한다.
본 발명의 다른 목적을 달성하기 위해, 본 발명은 본 발명의 바이오마커 단백질 또는 그 면역원성 단편에 특이적으로 결합하는 항체를 유효성분으로 포함하는 노년황반변성 질환의 진단제를 제공한다.
본 발명의 항체는 폴리클로날 항체일 수도 있으나, 모노클로날 항체인 것이 바람직하다.
폴리클로날 항체는 당업자에 알려진 종래방법에 따라 면역원인 바이오마커 단백질 또는 그 단편을 외부숙주에 주사함으로써 제조될 수 있다. 외부숙주는 마우스, 래트, 양, 토끼와 같은 포유동물을 포함한다. 면역원은 근내, 복강내 또는 피하주사방법으로 주사되며, 일반적으로 항원성을 증가시키기 위한 보조제(adjuvant)와 함께 투여된다. 외부숙주로부터 정기적으로 혈액을 채취하여 향상된 역가 및 항원에 대한 특이성을 보이는 혈청을 수거하거나 이로부터 항체를 분리 정제한다.
모노클로날 항체는 당업자에 알려진 융합에 의한 불멸화된 세포주 생성기술[Koeher and Milstein 1975,Nature,256:495)]에 의해 제조될 수 있다. 그 제조방법을 간단히 설명하면 다음과 같다. 먼저 순수한 단백질을 20 ㎍을 얻어서 Balb/C 쥐에 면역화를 시키거나 펩타이드를 합성하여 소혈청 알부민과 결합시켜 쥐에 면역화시킨다. 그 후에 쥐에서 분리된 항원-생산 임파구를 인간 또는 마우스의 미엘로마와 융합하여 불멸화된 하이브리도마를 생성하며, 엘라이져(ELISA)방법을 사용하여 원하는 모노클노날 항체를 생성하는 하이브리도마 세포만을 선택하여 증식한 후 배양물로부터 모노클로날 항체를 분리 정제한다.
본 발명의 다른 목적을 달성하기 위해, 본 발명은 개체의 체액에서 본 발명의 바이오마커 단백질 또는 그 면역원성 단편의 존재를 검출하는 단계를 포함하는, 개체의 노년황반변성에 대한 정보를 얻는 방법을 제공한다.
본 발명의 방법에서, 인간 혈액인 것을 특징으로 하며, 상기 검출 단계는 개체의 간의 혈액 용액으로부터 2차원(2-D) 전기영동으로 바이오마커 단백질 또는 그 면역원성 단편의 존재를 직접 검출하거나, 혈액을 본 발명의 항체와 접촉시켜 항원항체반응을 통해 바이오마커 단백질 또는 그 면역원성 단편의 존재를 간접적으로 확인하는 것을 포함한다.
항원항체반응으로서 현재 널리 알려진 immunoassay 법은 효소면역측정법(ELISA, Coated tube), 항체결합 magnetic particle을 tube에 결합시킨 다음 antigen-tracer와 난분해성 오염물질을 서로 경쟁적으로 반응시켜 효소반응을 유발시켜 정량하는 magnetic particle법, 항체결합 latex particle을 이용한 latex particle법 등이 있다.
본 발명의 다른 목적을 달성하기 위해, 본 발명은 본 발명의 바이오 마커 단백질 또는 그 면역원성 단편에 특이적으로 결합하는 항체를 유효성분으로 포함하는 노년황반변성의 진단 키트를 제공한다.
본 발명의 진단키트는 당업자에 알려진 종래의 제조방법에 의해 제조되며, 전형적으로 동결건조형태의 항체와 버퍼, 안정화제, 불활성 단백질 등을 포함한다. 상기 항체는 방사종, 형광원, 효소 등에 의해 표지화될 수 있다.
본 발명의 단일클론항체는 immunoassay 키트(ELISA, antibody coated tube test, lateral-flow test,potable biosensor)에 다양하게 이용될 수 있을 뿐만 아니라, 보다 높은 특이도와 민감도를 나타내는 항체의 개발을 통한 다양한 노년황반변성 검출 스펙트럼을 갖는 단백질칩 개발에도 이용될 수 있다.
본 발명의 다른 목적을 달성하기 위해, 본 발명은 노년황반변성 환자의 방수 (Aqueous humor)에 증감하는 군에서 선택된 단백질을 유효성분으로 포함하는 노년황반변성 질환의 치료 및 예방용 조성물을 제공한다.
본 발명의 약학적 조성물은 약제학적 분야에서 공지된 방법에 의해 약학적으로 허용되는 담체, 부형제, 희석제 등과 혼합하여 주사제 등의 제형으로 제조되어 정맥주사 등으로 투여될 수 있다. 본 발명에 따른 약학적 조성물의 투여량은 환자의 연령, 성별, 상태, 질환의 증상에 따라 적절히 선택될 수 있으며, 바람직하게는 성인기준으로 1일 0.001~100 ㎎의 단백질을 투여할 수 있다.
이하 본 발명을 설명한다.
AMD의 병인과 관련된 신규한 분비 바이오마커를 개발하기 위하여, 본 발명자들은 습성 AMD 환자의 방수(aqueous humor;(AH) ,ARPE-19 세포의 조건화된 배지(CM) 및 그 환자의 AH 및 CM의 엑소좀(exosome)의 전체 프로테오믹 프로파일링에서 동정된 단백질을 비교하기 위하여 통합적인 접근을 채택하였다.
Label-free, semi-quantitative, LC-ESI-MS/MS-기반 프로테오믹 프로파일링을 환자의 방수( aqueous humor;(AH) ,ARPE-19 세포의 조건화된 배지(CM) 및 그 환자의 AH 및 CM의 엑소좀(exosome)에 대해서 수행하였다. 또한, 액체 크로마토그래피 multiple reaction monitoring (LC-MRM) 분석을 환자 및 대조군 개체로부터 유래한 AH에 대해서 수행하였다. 본 LC-MRM 분석은 heat shock protein 70, cytokeratin 8, vitronectin, the 26S proteasome non-ATPase subunit 1, cathepsin D, retinol-binding protein 4 등이 및 15 환자의 AH에서 업레귤레이트되었고 이들 단백질들의 레벨은 ranibizumab의 안내(intravitreal) 주사 후에 감소되었다. 본 발명은 AMD 치료제을 위한 치료 타겟 및 바이오마커를 동정하였고 또 엑소좀의 형태로 인 비보에서 RPE 세포에 의하여 분비되는 AMD-관련 단백질들을 동정하기 위한 신규하고 효과적인 방법을 나타낸다.
이하 본 발명을 상세하게 설명한다.
전체 실험 전략
본 발명은 도 1에 기재된 것과 같이 네 단계로 수행되었다. 첫번째 단계로 본 발명자들은 두 AMD 환자 및 그들의 대조군 유래 AH의 전체 프로테옴에서 변화를 LC-ESI-MS/MS로 프로파일링하였다. 본 발명자들은 추가 분석을 위하여 대조군과 비교하여 환자에서 업레귤레이트된 단백질들을 선택하였다. 2 단계에서, 본발명자들은 산화적으로 스트레스준 ARPE-19 세포 유래 CM 전체 분비단백질체를 프로파일링하고 1단계에서 생성된 것과 이들 데이터를 비교하였다. 3 단계에서, 본 발명자들은 환자 AH 및 산화적으로 스트레스 준 ARPE-19 세포의 CM에서 엑소좀 단백질의 특성을 규명하였다. 4 단계에서, 본 발명자들은 둘 이상의 프로테오믹 프로파일에서 발견된 단백질들의 LC-MRM 런에 대한 트랜지션을 결정하고 runs and performed LC-MRM 분석을 수행하였다.

치료(0.5 mg ranibizumab의 안내 주사) 전과 후에서 두 AMD 환자 및 그들의 매칭된 대조군(표 1의 샘플 세트 1) 유래 네 AH 샘플들을 분석하였다. 본 발명자들은 두 환자의 AH에서 총 323 및 270 단백질들(도 2A) 및 그들의 각 매칭된 대조군에서 147 및 201 단백질들을 동정하였다. 180 단백질들은 두 환자 샘플 모두에서 일반적이고, 173 단백질들은 대조군 샘플 모두에서 일반적이다. 알부민은 AH에서 가장 풍부한 단백질이다. 알부민 농도는 환자와 대조군 사이에 유의적인 차이가 없었다.
습성 AMD 및 ranibizumab 치료에 대한 반응 관련된 AH 분비단백체에서 변화를 특성화하기 위하여 환자를 분석하고 PANTHER (http://www.pantherdb.org/)에서 기능적인 주석에 의하여 그룹화하였다. 표 2는 그들의 생물학적 과정 및 가설적인 기능에 따라 동정된 단백질들을 리스트한다. 스트레스, 산화적 스트레스, 시각, 노화, 에팝토시스 조절 및 세포 분화 관련 단백질 발현이 환자에서 증가하고 치료 후 감소하였다. 총 5 단백질들이 대조군과 비교하여 양 환자들에서 1.5-배 이상 업레귤레이트되었다(표 2). heat shock protein β-1 (Hspβ-1)의 발현은 대조군과 비교하여 환자 1에서 1.5-배 이상 업레귤레이트되었다. cathepsin D의 발현은 두 환자들(각각 2.8 및 2.7-배)에서 업레귤레이트되고 치료 후 감소하였다. 환자 2에서 cathepsin D 및 desmoplakin의 증가된 발현을 웨스턴 블럿 분석으로 확인하였다(도 2E).

RPE로부터 분비될 수 있는 환자 유래 AH의 단백질들을 결정하기 위하여, 본 발명자들은 LC-ESI-MS/MS로 수행된 ARPE-19 세포의 CM의 분비단백체 분석 및 여섯 AH 샘플 의 프로파일링 연구에서 동정된 단백질들의 비교 분석을 수행하였다.
Paraquat를 습성 AMD의 세포 환경의 산화적 스트레스를 가상하여 ARPE-19 세포에 첨가하였다. 본 발명자들은 ARPE-19 세포의 CM에서 334 단백질들을 동정하였다(도 2B). 표 3은 그들의 생물학적 기능에 따라 대조군 배양과 비교하여 24시간 동안 400 μM paraquat에 노출된 배양에서 1.5 배 이상으로 엡레귤레이트된 25 단백질을 나타낸다. 산화적으로 스트레스 준 ARPE-19 세포에서 변화된 발현을 나타내는 334 분비단백체 중, 65 단백질은 하나 또는 두 환자의 AH에서 발견된다(표 4). 이들 단백질들은 actin, myosin 9, Hspβ-1, cytokeratin 9, 및 cathepsin D를 포함한다.

환자의 RPE에 의하여 분비된 단백질들의 추가적인 인증과 본 발명에 가장 적합할 수 있는 단백질들의 리스트를 좁히기 위하여 본 발명자들은 AH 및 CM에서 분비 vesicles를 분석하였다. 세포가 세포외 공간으로 분비하는 microvesicles 중에서 본 발명자들은 엑소좀을 초점으로 선택하였다. 엑소좀은 현재까지 환자의 AH에서 발견되지 않았다. 400 μM paraquat의 24시간 처리는 FACS 분석에 의하여 결정된 것과 같이 ARPE-19 세포에서 세포 사멸 또는 에팝토시스를 유도하지 않은 반면, 500 μM 초과 농도에서는 세포독성을 가졌다(데이터 도시 안함). 이 결과는 그 수집된 엑소좀 및 단백질들의 엑소좀 분비는 세포 사멸의 결론으로 생성된 것이 아니라는 것을 확인한다.
환자 AH 및 ARPE-19 세포 배양 CM 유래 엑소좀 펠렛은 실시예에 기재된 것과 같이 제조업자의 프로토콜에 따라 ExoQuickTM Exosome Precipitation Solution으로 얻었다. TEM은 환자 AH 및paraquat-처리된 ARPE-19 세포 배양 CM에서 엑소좀은 50-100 nm 직경의 균질 둥근 형 막 vesicles로 나타난다(도 3A). 엑소좀을 더 특성화하기 위하여 본 발명자들은 통상적인 엑소좀 마커 단백질들이 정제된 엑소좀 펠렛에 존재하는지를 조사하기 위하여 웨스턴 블럿 분석을 사용하였다. 가장 널리 사용된 마커들은 tetraspannins (CD9, CD63, CD81, CD82) 및 Hsp70을 포함한다. 환자 AH 엑소좀, ARPE-19 세포 CM 엑소좀 및 ARPE-19 세포 파쇄액 유래 동량의 단백질들을 동일한 젤레 로딩하였다. 본 발명자들은 환자 AH 및 ARPE-19 세포의 CM 유래 엑소좀에서 CD63을 검출하였다(도 3B). AH는 많은 양의 엑소좀을 포함하고 이것은 웨스턴 블럿 분석에 의하여 이들 분획에서 발견된 현저한 CD63 양에 의하여 반영된다. 본 발명자들은 cathepsin D의 레벨이 세포 파쇄액 또는 CM 유래 엑소좀과 비교하여 환자 AH 유래 엑소좀에서 높았다는 것을 주목하였다. 증가된 globular CD63의 발현이 대조군 세포와 비교하여 산화적으로 스트레스 준 ARPE-19 세포에서도 검출되었다(도 3C). 본 발명자들은 대조군 세포와 비교하여 paraquat-처리된 ARPE-19 세포의 CM에서 증가된 엑소좀의 총량이 증가하였다는 것을 알았다(데이터 도시 안함). 본 발명자들은 Tsg101(tumor susceptibility gene 101)와 함께, Hsp70, cytokeratin 8,및 26S proteasome non-ATPase subunit 1 (PSMD1)의 발현을 확인하였다(도 3D). 따라서 본 발명자들은 환자 AH에서 단백질들은 엑소좀을 통하여 RPE로부터 분비될 수 있다는 가정을 하였다. 본 발명자들은 AH 및 CM의 모든 엑소좀 단백질의 프로테오믹 프로파일링을 수행하여 이 가설을 더욱 조사하였다.

AH 내 일부 단백질이 엑소좀을 통하여 분비된다는 가능성을 파악하기 위하여 환자 AH 및 ARPE-19 세포 배양 CM에서 얻은 엑소좀 분비단백체를 label-free semi-quantitative 질량 분광기 시스템(LC-ESI-MS/MS)을 사용하여 분석하였다. 전체적으로, 본 발명자들은 환자 AH 및 ARPE-19 세포 배양 CM 유래 엑소좀에서 3회 반복실험에서 2회 이상 검출된 각각 145 및 220개 단백질들을 동정하였다(도 2C 및 2D). 이 단백질들은 annexin 계열(annexin A1, A2, A3, A4, A5), heat shock protein 계열(Hsp70, 90 alpha 및 beta), cytoskeletal 단백질들(cytokeratin 1, 5, 7, 8, 9, 18, 및 19), chaperone 단백질, proteasome-ubiquitin 경로 멤버, proteases 및 protease inhibitors, coagulation 및 complement cascades 단백질들 등을 포함한다. cytokeratin 8 이외에, cathepsin D, 26S proteasome non-ATPase subunit 1, 및 Hsp 70은 웨스턴 블럿 분석에 의한 AH 및/또는 CM 유래 엑소좀에서도 발견되었다(도 3B 및 3D), actin (smooth muscle), myosin 9, Hspβ-1, cytokeratin 9, vitronectin, 및 retinol-binding 단백질 4는 프로테오믹 분석에 의한 AH 또는 CM의 엑소좀에서도 발견되었다. 표 5는 대조군과 비교하여 ARPE-19 세포 CM 또는 환자 AH에서 1.5배 이상 업레귤레이트된 엑소좀 단백질들을 나타낸다. 환자 AH 및 환자 AH 유래 엑소좀에서 동정된 각각 363 및 145 단백질들 중에서, 78개 단백질들이 두 샘플 모두에서 나타났다(표 6). 이들 단백질들의 분자적 기능, 생물학적 과정, 세포 구성요소 및 경로 주석을 PANTHER 소프트웨어를 사용하여 분류하였다. 도 2F, 2G, 및 2H에서 나타난 것과 같이, 이들 단백질의 대부분은 대사 과정, 면역 시스템, 자극에 반응 또는 발생 과정에 관련된다.

단백질들의 생성
두 환자들의 AH의 전체 프로테옴 프로파일링에서 후보 단백질들을 대조군의 AH 유래 데이터와 비교하여 동정하였다(표 2). LC-MRM 용 타겟 단백질들은 두 가지 기준으로 선택되었다: 1) 단백질이 하기 세 프로파일 ARPE-19 세포 배양 CM, AH 유래 엑소좀, 또는 CM 유래 엑소좀 중에 하나에 존재하여야 함(표 7); 그리고 2)이들 단백질들이 LC-MRM 분석에서 용이하게 관찰될 수 있도록 MS 스캔에서 타겟 단백질의 펩타이드들이 자주 관찰되어야 함. 이런 기준에 기반하여, 습성 AMD를 가지는 환자 AH에 존재하고 환자의 RPE(또는 RPE를 포함한 망막) 유래된 것으로 간주되는 후보단백질들을 선택하고 quantitative LC-MRM 분석을 사용하여 AMD 환자 유래 AH 샘플에서 이들 단백질들의 레벨을 측정하였다. 그 선택된 단백질들은 desmoplakin; actin (aortic smooth muscle); myosin 9; Hsp70; Hspβ-1; cytokeratins 8, 9, and 14; vitronectin; 26S proteasome non-ATPase subunit 1 (PSMD1); cathepsin D; 및 retinol-binding protein 4 등이었다.

후보 단백질들을 LC-MRM 분석을 수행하였다. 우수한 MS 신호를 가지는 타겟 단백질들에 대한 독특한 트립신처리 펩타이드를 선택하는 것이 중요하다. 본 발명자들은 MRMPilot 소프트웨어(ABSCIEX, Foster City, CA)를 사용하여 주어진 타겟 단백질들에 대한 다중 트립신처리 펩타이드들을 선택하였다.
MRM transition을 표 8에 나타낸 것과 같이, 12 단백질의 34 펩타이드들에 대해서 최적화하였다. 그 선택된 펩타이드들을 QTRAP 5500 triple quadrupole/linear ion trap 질량 분광기를 사용하여 LC-MRM 실험에 의하여 조사하였다. 모든 12 단백질의 발현 레벨은 그들의 매칭된 대조군과 비교하여 환자 AH에서 상승하였다(도 4A). 그들 중에서, actin (aortic smooth muscle); myosin 9; Hspβ-1; cytokeratins 8, 9, and 14; vitronectin; 26S proteasome non-ATPase subunit 1 (PSMD1); cathepsin D; 및 retinol-binding protein 4는 t-test 분석에 의하여 결정된 것과 같이 적어도 한 트랜지션에서 대조군과 비교하여 2배 이상 업레귤레이트되었다. 바이오마커로서 이들 단백질들의 추가적인 평가는 케이스-컨트롤 연구에서 널리 사용되는 ROC(receiver operating characteristic) 곡선 분석을 사용하여 수행하였다. 본 발명자들은 12 단백질들의 각각으로부터 랜덤하게 한 transition을 선택하고 모두 세번의 인텐시티를 사용하여 ROC 분석을 수행하였다. ROC 곡선은 이들 단백질들을 대조군 개체로부터 AMD 환자들을 구별할 수 있다는 것을 보여준다. 가장 주목할만한 인디케이터 단백질은 AUC 0.668을 가지는 cathepsin D이다(도 4B).
그들의 통계적인 유의성을 포함하여 잠재적인 바이오마커의 유용성을 입증하는 것 이외에, 본 발명자들은 이들 단백질들 중 일부의 차별적인 발현이 AMD의 치료를 위한 ranibizumab에 대한 반응성과 관련이 있는지를 결정하는 것을 찾았다. 분비된 단백질들이 세포 기능에서 잠재적으로 관련되는지를 동정하고 anti-VEGF 치료-반응 표현형의 개발을 위하여 본 발명자들은 치료 전과 후 모두에서 OCT(optical coherence tomography)을 사용하여 sub- 및 intraretinal 부종을 반영하는 중심황반두께(central macular thickness,CMT) 를 평가하고 통계분석을 수행하여 특정 단백질의 레벨과 CMT 사이의 상관관계를 동정하였다. 분석된 12 단백질들 중에서 cytokeratin 8 및 Hspβ-1 발현과 치료제에 의한 더 나은 치료 반응 (OCT에서 측정한 CMT의 더 큰 감소로 대변됨) 사이에 가장 밀접한 상관관계를 가졌다(Spearman 상관관계:cytokeratin 8에 대해 coefficient 0.56, p-value 0.03, Hspβ-1에 대해 coefficient 0.53, p-value 0.04). 다변량 회귀 분석은 cytokeratin 8이 CMT의 더 큰 감소와 관련된다는 것을 나타내었고(estimate 7.80, error 4.72, p<0.05)(즉, 치료전, 대조군에 비해 증가한 cytokeratin8의 증가정도가 클수록 치료 반응이 좋음), 치료 반응을 예측하는데 이 단백질의 잠재적인 유용성이 강조되었다.

이하 비한정적인 실시예를 통하여 본 발명을 더욱 상세하게 설명한다. 단 하기 실시예는 본 발명을 예시하기 위한 의도로 기재된 것으로서 본 발명의 범위는 하기 실시예에 의하여 제한되는 것으로 해석되지 아니한다.
실시예
1:개체 및
AH
샘플 수집
AH 샘플을 2011년 9월 1일부터 2012년 10월 31일부터 건국대 병원 안과에서 26명 미치료된 습성 AMD 및 26 명 연령 및 성별 매치된 백내장(대조군) 환자로부터 수집하였다. 분석된 26 세트의 샘플은 대조군 유래 샘플, 치료(ranibizumab의 안내주사) 전 환자 유래 샘플, 및 첫번째 치료 1개월 후 환자로부터 얻은 샘플로 구성되었다. 26 명 환자들은 모두 본 발명에 참가 전에 습성 AMD에 대한 어떠한 치료도 받지 않았다. 다른 안과 질환(예를 들어 녹내장, 포도막염 또는 진행성 망막 질환), 비조절 전신 질환(예를 들어 당뇨 또는 관절염) 환자들, 또는 레이져 또는 안내 수술을 한 환자들은 배제하였다. 대조군 개체들은 시력 회복을 위한 노령 백내장 수술을 하였다. 본 발명자들은 환자의 나이와 대조군 개체의 나이 및 환자의 나이에 해당하는 각 개체에서 백내장 양을 매치시켰다. 대조군 개체들은 백내장 이외에 다른 안과 질환은 없었다. 환자 및 대조군들로부터 유래한 임상 데이터를 표 1에 요약하였다. 대조군 샘플들은 백내장 수술 직전에 얻었다. 습성 AMD 환자 유래 샘플은 0.5 mg ranibizumab의 첫번째 안내 주사 수행 전 및 주사 1개월 후에 얻었다. 두 세트의 샘플들(6 샘플)을 LC-ESI-MS/MS 분석(표 1에서 샘플 세트 1)에 의한 전체 단백질 프로파일링에 사용하였다. 9 세트의 샘플(27 샘플)을 AH 유래 엑소좀의 조제에 사용하였고, 추후 전체 단백질 프로파일링은 LC-ESI-MS/MS 분석(표 1에서 샘플 세트 2)에 의하여 수행되었다. AH 샘플 당 엑소좀의 양이 적기 때문에 본 발명자들은 치료 전 후 모두에서 대조군 및 AMD 환자들 각각에 9 모아진 샘플의 프로테오믹 프로파일링을 수행하였다. 결국, 15 세트의 샘플(45 샘플)은 LC-MRM (표 1에서 샘플 세트 3)에 의하여 분석되고 데이터를 15 세트에 대해 얻었다.
AH 유래 엑소좀의 웨스턴 블럿 분석 및 전자 현미경 조사를 위하여, 110 AH 샘플을 습성 AMD를 가지는 환자로부터 수집하였다. 이 샘플들은 안내 anti-VEGF 주사 전에 수집하였다. 그러나 이들 110 환자들은 ranibizumab 또는 bevacizumab의 안내 주사와 같은 그들의 질환에 대한 어떠한 타입의 치료를 일부 받았고; 일부 환자들은 녹내자 또는 당뇨와 같은 다른 질환을 가지고 일부 환자들은 레이져 치료 또는 다른 안내 수술을 전에 받았고, 그 환자들의 나이는 52에서 92세 범위이고 62 명 남자 48명 여자를 포함하였다.
모든 샘플 수집과 안내 주사는 표준 멸균 과정을 사용하여 수행되었고 AH 샘플은 30-게이지 바늘을 사용하여 전방 천자(anterior chamber paracentesis)에 의하여 얻었다. safe-lock microcentrifuge 튜브(1.5 ml) 내 AH 샘플 (100-150 ul)은 즉시 -80℃에서 동결하고 분석 때까지 저장하였다. 본 발명은 헬싱키 선언의 가이드라인을 준수하였고 고지된 문서화된 동의서를 모든 환자들 및 대조군 개체로부터 받았다. AH 수집 과정은 건국대 병원의 임상시험 심사위원회에 의하여 승인을 받았다.

ARPE
-19 세포 배양 및 그 세포 상등액(
CM
)의 분비단백체(
secretome
)
인간 망막 색소 상피(retinal pigment epithelial) ARPE-19 세포를 10% FBS 및 1% 항생제가 보충된 DMEM/F-12 에서 배양하였다. 약 5X106 세포를 각 100-mm 배양 디쉬에 플레이트하고 증식을 하게 37℃, 5% CO2 배양기에서 유지하였다. 세포 컨푸루언스가 ~90%에 도달할 때, 그 세포를 PBS에서 3회 세척한 후 혈청 결핍 조건 하에서 400 μM paraquat (Sigma)으로 37℃에서, 24시간 동안 처리하였다.
동시에, 전체 세포 파쇄액을 CM을 생산한 세포로부터 조제하였다; 이 파쇄액들은 후에 웨스턴 블럿 분석용 엑소좀과 병렬적으로 사용하였다. 총 100 ml의 세포 상등액(CM)을 수집하여서 480 xg 에서 10 분간 원심분리하고 죽은 세포 및 세포 찌꺼기를 제거하기 위하여 1,900 xg 에서 10 분간 원심분리하였다. 그 CM을 LC-ESI-MS/MS에 의한 분비단백체 분석용 Amicon®Ultracel-10K molecular weight cutoff 원심분리 여과 장치(Millipore)를 사용하여 ~ 1 ml까지 농축하였다.

분리 및 형태학적 및 생화학적 특성규명
엑소좀은 제조업자의 프로토콜에 따라서 ExoQuickTM Exosome Precipitation Solution (System Bioscience, SBI)을 사용하여 환자 AH 및 ARPE-19 세포 배양 CM으로부터 분리하였다. 요약하면, 세포 및 세포 찌꺼기를 제거하기 위하여 3000 xg에서 15 분간 원심분리한 후, ExoQuickTM 시약을 그 상등액에 첨가하고 잘 혼합한 후 그 혼합물을 4℃에서 오버나잇 저장하였다. 추후적으로, 그 ExoQuick/샘플 혼합물을 1500 xg 에서 30 분간 원심분리하였다. 원심분리 후, 그 엑소좀은 용기의 밑에서 엷은 yellow-white 펠렛으로 나타났다. 그 상등액을 뽑아내고 모든 미량의 용액을 잔류 ExoQuickTM 용액을 1500 xg 에서 5분간 원심분리하여 스핀다운한 후 제거하였다. 엑소좀 펠렛을 재부유하고 투과 전자현미경(TEM), 웨스턴 블럿 분석 및 LC-ESI-MS/MS에 사용하였다.
TEM을 위하여, 엑소좀을 직접적으로 Formvar-carbon-coated 400 mesh copper EM 그리드(PELCO® TED PELLA Inc.) 상에 흡착하고 상온에서 20분간 건조하였다. 그 견본을 신선하게 조제된 2.0% aqueous uranyl acetate (Fluka)으로 네가티브 염색을 하고, 건조하고 JEM-1100 투과 전자현미경을 사용하여 acceleration 전압 80 kV (JEOL, Japan)에서 촬영하였다. 엑소좀은 상대적으로 균질화된 크기(약 50-150 nm 직경) 둥근 막 vesicles로 정의하였다(Thery, C., Amigorena, S., Raposo, G., and Clayton, A. (2006) Isolation and characterization of exosomes from cell culture supernatants and biological fluids. Current protocols in cell biology Chapter 3, 3.22.1-3.22.29 ).
웨스턴 블럿 분석을 위하여, CM 및 환자의 AH 유래 엑소좀 펠렛을 protease 저해제 칵테일(Roche)를 포함한 PBS에 재부유하였다. 그 현탁액의 단백질 농도를 개량된 Bradford Assay (Bio-Rad Laboratories)에 의하여 결정하였다. 15 μg 단백질을 포함한 세포 조제물을 웰 당 로딩하였다. 그 막을 1시간 동안 5% 탈지분유로 블럭하고 4℃에서 하기 항체: anti-CD63 (Santa Cruz), anti-cathepsin D (Santa Cruz), anti-Hsp70 (BD Sciences), anti-cytokeratin 8 (Abcam), anti-26S proteasome non-ATPase subunit 1 (PSMD1) (LSBio), 또는 anti-Tsg101 (Abcam)로 오버나잇 배양하였다. Horseradish peroxidase-부착된 goat anti-rabbit 또는 anti-mouse IgG (Cell Signaling) 2차 항체를 사용하였다. chemiluminescence 기질(ECL Prime, Amersham)을 면역반응 단백질들을 육안화하기 위하여 사용하였다.
면역조직화학을 위하여, ARPE-19 세포를 상온에서 1시간 동안 4% paraformaldehyde로 고정화하고 10분 동안 0.2% Triton X-100으로 침투시켰다. 2% 소 혈청 알부민으로 블럭킹한 후, 그 고정화된 세포를 4℃에서 anti-CD63 (1/1000) 항체로 오버나잇 배양하였다. 그 후 그 배양액을 형광 부착된 2차 항체(Alexa Fluor 555 anti-rabbit IgG; 1:1000; Invitrogen)로 상온에서 2시간 처리하였다. 음성 대조군을 위하여, 배양액을 2차 항체로만 처리하였다. 그 mounted 슬라이드를 공초점 현미경(FV-1000 Spectral, Olympus)하에서 여기 파장 568 nm (x800)에서 관찰하였다.

분석, 데이터베이스 검색 및 인증
트립신 처리된 펩타이드들을 C18 레진 (5 μm, 200Å)으로 충진된 융합 실리카 microcapillary 컬럼(12 cm x 75μm) 상에 로딩하였다. LC 분리를 3-40% 용매 B (0.1% formic acid in 100% ACN) 선형구배로 유속 250 nl/min으로 60 분간 수행하였다. 그 컬럼을 직접적으로 nano-electrospray 이온 소스가 구비된 LTQ linear ion-trap 질량 분광기(Finnigan, CA)에 연결하였다. 그 전자스프레이 전압은 0.95 kV이고, MS에서 MS/MS로 스위칭 역치는 500이었다. MS/MS에 대한 표준화된 충돌 에너지들은 35%의 메인 RF (radio frequency amplitude)이었고, 활성화의 duration은 30 ms이었다. 스펙트럼은 데이터 의존 스캔 모드에서 얻었다. 각 풀 MS 스캔은 다섯개 가장 강한 피크의 MS/MS 스캔에 의하여 수반하였다. 동적 배제에 대한 반복 피크 카운트는 1이고, 그 리피트 duration은 30 s이었다. 동적 배제 duration은 180 s이었고, 배제 질량의 폭은 ±1.5 Da이었다. 동적 배제의 리스트 사이즈는 50이었다. 본 발명자들은 6 AH 샘플을 3회 반복 분석하고 추가 실험(LC-MRM)을 위하여 적어도 2회 반복 분석에서 동정된 단백질들을 선택하였다.
그 LC-ESI-MS/MS 스펙트럼을 National Center for Biotechnology Information (http://www.ncbi.nlm.nih.gov/) non-redundant 인간 단백질 데이터베이스를 검색하는 SEQUEST 써치 엔지을 가지는 BioWorksBrowserTM (version Rev. 3.3.1 SP1, Thermo Fisher Scientific Inc., CA)를 사용하여 분석하였다. 그 검색 조건은 다음과 같다: 트립신 효소 특이성, 2 이하 missed 절단, 절단체 이온에 대한 ±2 amu 질량 에러, 펩타이드 tolerance ±1 amu, methionine (+16 Da) 잔기의 산화 및 cysteine에 대한 carbamidomethylation (+57 Da)의 고정화된 변형. 펩타이드 인증을 위하여, 본 발명자들은 decoy 데이터베이스 전략을 적용하여 cross correlation (Xcorr) 값을 이용한 false-positive (FP) rates를 계산하였다. 높은 신뢰를 가지는 데이터를 얻기 위하여, 다중 펩타이드 히트를 가지는 단백질 인증을 0.1% FP rates 이하를 가지는 분류된 펩타이드들에 대해 수행하였다. 따라서 본 발명자들은 역치 컷오프(에러율 < 1%, 단일 펩타이드) 하에서 부정확한(reversed) 펩타이드 및 단백질 동정체를 제거하였다. 각 샘플에 대해서 3회 반복 실험을 수행하였다. 샘플 특이적인 단백질을 동정하기 위하여 label-free 단백질 정량법을 단백질 리스트의 3회 반복 쌍에 적용하였다. 샘플 특이적인 단백질을 그것의 평균 양 비율이 적어도 1.5 배이면 결정하였다.

15 명 환자 유래 AH 샘플(각 50 μg)을 HPLC 급 워터의 6 M urea 및 50 mM ammonium bicarbonate (pH 7.8)에 녹였다. 변성된 AH 단백질을 30 mM DTT로 2시간 동안 환원하고 알킬화를 위하여 암 조건하에서 1시간 동안 55 mM iodoacetamide 처리를 수반하였다. 알킬화된 AH 샘플을 시퀀싱 급 변형된 트립신(Promega, Madison, WI)에서 37℃에서 오버나잇 처리하였다. 포름산을 그 샘플에 첨가하여서 그 효소 반응을 중지시켰다. MRM 모드를 흥미있는 단백질의 특정 펩타이드 정량 분석용 nanospray ionization source를 구비한 QTRAP 5500 hybrid triple quadrupole/linear ion trap 질량 분광기(Applied Biosystems / MDS Sciex, Carlsbad, CA)에 대해서 수행하였다. 주어진 MRM Q1/Q3 이온 값(precursor / fragment ion pair)을 모니터하여 각 후보 단백질에 해당하는 특이적으로 타겟된 펩타이드를 선택하였다. 그 MRM 스캔을 1800-2100 V 범위에서 이온 스프레이 전압을 가지는 포지티브 모드에서 수행하였다. 그 MRM 모드 세팅은 다음과 같다: curtain gas 및 spray gas 각각 10 및 20 psi 그리고 충돌 가스는 유닛 resolution에 셋팅. DP(declustering potential)을 100 V에 셋팅. 그 질량 resolution은 advanced MS 파라미터를 사용하여 유닛으로 세팅하였다. correct LC-MRM를 위하여, enhanced product ion (EPI) 스캔에 의하여 선택된 펩타이드의 모니터링을 rolling collision energy 선택 및 100 카운트의 threshold switching으로 수행하였다. 포지티브 모드에서, 30의 product, 스캔 범위 100-1000 Da, 및 두 스캔을 사용하였다. advanced MS 탭에서, quadrupole resolution을 low로 세팅, 스캔 스피드는 10,000 amu/s이고, dynamic fill time을 선택하였다.
상기 본 발명의 데이터는 적어도 3회 독립적인 실험의 평균±S.D.로 나타내었다. 통계적 비교는 95% 신뢰도를 가지는 unpaired two-tailed Student's t- 테스트를 사용하여 만들었다. 유의성 레벨은 P<0.05에 세팅하였다.

- 대상(subject)에서 노년황반변성에 대한 정보를 얻기 위한 방법으로서,
(a) 대상으로부터의 생물학적 샘플에서 하나 이상의 바이오마커를 측정하는 단계로서, 여기서 하나 이상의 바이오마커는 상기 제1항에 기재된 단백질 조성물 중에서 선택되는 단계; 및
(b) 측정치 또는 측정치들을 정상인과 비교하여 그 측정치가 정상인보다 높은 경우에 노년황반변성과 상호관련시키는 단계를 포함하는 노년황반변성에 대한 정보를 얻기 위한 방법. - 제 3항에 있어서, 상기 방법은 상기 단백질 바이오마커의 발현이 정상인과 비교하여 1.5배 이상인 경우에 노년황반변성과 상호관련시키는 것을 특징으로 하는 노년황반변성에 대한 정보를 얻기 위한 방법.
- 대상(subject)에서 노년황반변성에 대한 정보를 얻기 위한 방법으로서,
(a) 대상으로부터의 생물학적 샘플에서 바이오마커를 측정하는 단계로서, 여기서 바이오마커는 상기 제2항에 기재된 단백질 조성물인 단계; 및
(b) 측정치 또는 측정치들을 정상인과 비교하여 그 측정치가 정상인보다 높은 경우에 노년황반변성과 상호관련시키는 단계를 포함하는 노년황반변성에 대한 정보를 얻기 위한 방법. - 제 5항에 있어서, 상기 방법은 상기 단백질 바이오마커의 발현이 정상인과 비교하여 1.5배 이상인 경우에 노년황반변성과 상호관련시키는 것을 특징으로 하는 노년황반변성에 대한 정보를 얻기 위한 방법.
- 유전자 정보 번호(GI No.) 4504517의 Hspβ-1; 및 유전자 정보 번호(GI No.) 4504919의 사이토케라틴(cytokeratin) 8로 구성된 군으로부터 선택된 하나 이상의 단백질을 포함하는 노년황반변성 치료용 항-VEGF 치료제 반응 예측용 조성물.
- 서열번호 1 내지 서열번호 20에 기재된 펩타이드로 구성된 군으로부터 선택된 하나 이상의 펩타이드를 유효성분으로 포함하는 노년황반변성 진단용 키트.
